    Notes: Conclusion En raison de l'âge des sujets examinés, un certain nombre d'aspects pathologiques athéromateux et athéroscléreux ont été découverts (Fig. 17). D'autre part, les multiples sinuosités observées paraissent présentes précocement, l'allongement relatif ultérieur dû à la sénescence ne fait que les accentuer. Elles conditionnent cependant le type d'écoulement dans l'artère et favorisent les lésions pariétales, facteur important dans le processus complexe de l'athérosclérose. Il convient d'insister enfin sur un point particulier, la fragilité du siphon carotidien au-delà de 60 ans. Les modifications anatomiques observées augmentent les difficultés de progression et les risques du cathétérisme sélectif ainsi que de la chirurgie de ces vaisseaux.

    Description / Table of Contents: Résumé Les caractères des artères carotides internes sont précisés sur 50 préparations anatomiques de sujets âgés de 42 à 76 ans et sur des coupes frontales, sagittales et transversales. Les données recueillies sont confrontées aux observations rapportées dans la littérature en insistant sur: - la biométrie des vaisseaux (pour l'artère carotide interne droite, diamètre externe dans la région cervicale moyenne: 5±0.8 mm, longueur moyenne: 154,9±13,8 mm, il n'existe pas de variation significative entre les artères droites et gauches); - les variations de trajet avec sinuosités fixées relativement précocément et leur importance dans la pathologie artérielle; - les rapports principaux de la portion cervicale (Cl), de la portion pétreuse (C2), notamment avec les différentes parties de l'oreille et de la portion caverneuse (C3), avec l'hypophyse.
    Notes: Summary Anatomical characteristics of the internal carotid arteries are detailed in 50 anatomical preparations of frontal, sagittal and transversal sections from subjects aged between 42 and 76 years. The results obtained are compared with those reported in the literature and the following points are emphasized: Dimensions of the vessels (for the right internal carotid artery, the external diameter in the region of the neck was found to be 5±0.8 mm, the mean length 154.9±13.8 mm. There was no significant difference between left and right arteries). The variations in its course, with tortuousities at fixed points appearing relatively early in life, and their importance to arterial pathologies. The principal relationships of the cervical portion (C1), the petrous portion (C2), with the different parts of the ear and the cavernous portion of the artery (C3) in particular, and with the pituitary.

    Notes: Summary Over a period of 18 years the authors have collected together 230 cases of herniation cervical discs. Multifactor analysis of the results of the various therapies is given. Surgical decompression has been done, mostly using an anterior approach, and it effectively treats the radicular damage; it stabilizes and improves spinal cord deficits whether the herniation is spontaneous or traumatic, acute or chronic. Computed tomography facilitates its anatomical identification and the decision regarding the therapy to apply.

    Notes: Summary The authors report their experience with 150 cases of discectomies performed with interbody grafts using a copolymer (Biocompatible Orthopedic Polymer or BOP*). Made of N-Vinylpyrrolidone-Methylmethacrylate monomers, polyamide fibres and calcium gluconate. BOP is a biocompatible, biodegradable, osteoconductive matrix, easy to use and quite safe. These properties are quite well appreciated for cervical interbody grafting. After discectomy, stabilization of the spine is immediate, and fusion slowly occurs around and through the biopolymer within one year. The biomaterial avoids morbidity inherent in the harvesting of an autograft, as well as any limitations resulting from the use of allografts.

    Notes: Summary Between 1960 and 1 June 1985, 21 patients were operated for thoracic disc herniations. The advent of new operative techniques (with transverso-arthropediculectomy) has brought about new indications in the management of such disorders. These new indications are important because such protrusions may be abruptly complicate by compression of blood vessels, which may produce abrupt and irreversible neurological deficit. The progress in the radiological examinations allows a more precise study of the topography and density of the hernia. Furthermore, the use of a surgical microscope and microsurgical techniques allow a surgical treatment without preoperative medullary angiography.

    Description / Table of Contents: Résumé Cette étude de la différenciation anténatale du disque intervertébral humain repose sur l'examen de coupes histologiques de quarante huit colonnes vertébrales d'embryons et de foetus. Le disque primitif est composé de deux structures : la notochorde, qui présente une expansion progressive de son diamètre au sein du disque, et le disque périnotochordal, d'abord mésenchymateux puis fibrocartilagineux. Il n'a pas été mis en évidence de signe histologique témoignant d'une interaction entre les cellules notochordales et les cellules périnotochordales qui puisse expliquer l'expansion de la notochorde au sein des disques. Le rôle de la notochorde dans la différenciation cartilagineuse de la zone centrale est par contre probable.
    Notes: Summary To study the antenatal differentiation of the human intervertebral disc, the columns of forty eight embryos and fetuses were examined histologically. The primitive disc is composed of two structures: the notochord which shows a progressive expansion into the disc, and the fibrocartilaginous perinotochordal disc. No histological sign of interaction between notochordal and perinotochordal cells, which may explain the notochordal expansion into the discs, was seen. On the other hand, the notochordal intervention in the cartilaginous differentiation of the inner zone is probable.
