    Description / Table of Contents: Zusammenfassung. Für Patienten mit chronischen Wunden fehlen in Deutschland standardisierte Therapiekonzepte in zentralen Behandlungszentren. Wir haben vor 6 Jahren ein solches Zentrum eingerichtet, wobei die lokalchirurgische Sanierung, feuchte Wundbehandlung und begleitende Therapie der Grunderkrankung im Mittelpunkt des standardisierten Behandlungskonzeptes steht. In dieser Wundsprechstunde wurde die lokale Wundbehandlung und die Koordination der einzelnen interdisziplinären Behandlungsschritte vorgenommen. Ein spezielles Wunddokumentationssystem diente zur Qualitätssicherung. Die Einbeziehung der Hausärzte und ambulanten Pflegedienste führte zu einer Erweiterung der Infrastruktur und ermöglichte eine kontrollierte Vernetzung der stationären und ambulanten Behandlung, wodurch 42 % der Patienten ausschließlich ambulant behandelt werden konnten. Nach den vorgestellten Daten konnte eine verbesserte Patientenversorgung erreicht werden: 69 % der Wunden, die zuvor 30 Monate erfolglos behandelt wurden, waren nach unserem Protokoll innerhalb von 12 Monaten abgeheilt. Die Analyse der Daten unterstreicht den Stellenwert der lokalchirurgischen Sanierung, wonach weder die Wundtiefe noch die Wundinfektion einen Einfluß auf die Heilungsraten hat, vermutlich durch die radikale Entfernung der Nekrosen. Nach den vorgestellten Daten rechtfertigen medizinische, soziale und wirtschaftliche Gründe die Einrichtung einer interdisziplinär organisierten Wundsprechstunde, die zwischenzeitlich in Deutschland Modellcharakter aufweist.
    Notes: Summary. In Germany there is no standardized wound care for patients with chronic wounds in specialized centers. We have established a wound care unit for the past 6 years. The principal concept of therapy was characterized by standardized local surgery, moist wound dressings and concomitant treatment of the underlying disease. We performed local therapy, coordinated the interdisciplinary treatment and developed a new wound documentation system for quality control. We established a close network, integrating general practitioners and home care organizations to realize a mainly outpatient treatment supported by short hospital therapy. Exclusive outpatient treatment was performed in 42 % of all patients. According to our prospective data, we achieved an improvement in wound care: 69 % of the wounds resistant to therapy for a mean of 30 months healed within 12 months after therapy according to our protocol. Our data strongly supported the importance of local surgery: neither wound depth nor wound infection had any influence on the healing rate, presumably due to radical excisional debridement of necrotic tissue. The presented data justify on medical and economic grounds the establishment of such wound care centers in Germany.

    Description / Table of Contents: Résumé  La présence de facteurs de croissance a été montrée dans la consolidation osseuse mais pas encore établie dans les cals par distraction. Nous avons étudié les cytokines durant la distraction chez 3 patients. Des examens immuno-histo-chimiques ont été faits à partir de biopsie pour étudier les facteurs TGF-β, IGF-I, récepteurs TGF-β II, récepteurs IGF et l’antigène nucléaire de prolifération (PCNA). Histologiquement, nous avons trouvé de l’os immature dans la zone médiane du cal et, dans la zone de remodelage une calcification avec os lamellaire. Les ostéoblastes en général et les cellules fibroblastes-like de la zone médiane ont été positives pour le facteur TGF-β et son récepteur. Il existe une corrélation entre la quantité de cellules positives et l’activité proliferative trouvée par la PCNA et par la densité radiologique. Le facteur IGF-I a été détecté ubiquitairement. En conclusion, les facteurs de croissance sont présents dans l’os en formation et en remodelage des cals par extension. La correlation entre l’activité proliferative cellulaire et la densité radiographique, souligne leur rôle dans le processus ostéogénique.
    Notes: Abstract  Although growth factors have been demonstrated during bone healing, their presence has not yet been confirmed in callus distraction. Therefore, in 3 patients we searched for cytokines during callus distraction. Bone biopsies were immuno-histochemically stained for TGF-β1, IGF-I, TGF-β type II receptor, IGF receptor, and proliferating cell nuclear antigen (PCNA). Histologically we found immature woven bone in the middle of the callus zone and increasing calcification and lamellar bone in the re-modelling zone. Osteoblasts and fibroblast-like cells in the middle zone, and osteoblasts in all zones stained for TGF-β and its receptor. The number of positive staining cells related to proliferous activity as assessed both by PCNA, and by bone density in radiographs. IGF-I could be detected everywhere. In conclusion, growth factors are present in bone formation and in areas of re-modelling during callotasis. Their relation to proliferous activity and radiographic density supports their involvement in osteogenesis.

    Description / Table of Contents: Zusammenfassung Das chronische Ulcus venosum zeichnet sich trotz konsequenter Lokalbehandlung und Kompressionstherapie sowie neuen Operationsverfahren durch häufige Therapieresistenz aus. Neue Kenntnisse zur Pathophysiologie der Wundheilung belegen tierexperimentell den stimulierenden Effekt verschiedener Faktoren wie TGFβ, PDGF, EGF u.a. Die Ergebnisse der ersten klinischen Studien mit autolog gewonnenen Wachstumsfaktorenkonzentrat PDWHF zeigen eine Beschleunigung der Wundheilung. In dieser prospektiven Studie soll die stimulierende Wirkung des autolog gewonnenen PDWHF auf die vollständige Epithelialisierung kleiner Ulzera (Gruppe A, 〈5000 mm2) und auf die Granulation großer Ulzera vor Meshgraft-Deckung (Gruppe B, 〉5000 mm2) geprüft werden. Einschlußkriterium waren die ausschließlich venöse Genese der Ulzera sowie die Heilungspersistenz über 6 Monate trotz adäquater Lokaltherapie. Ausschlußkriterien stellten AVK, Diabetes mellitus, akute Wundinfektion, Thrombozytopenie sowie Schwangerschaft dar. Es wurden 24 Patienten mit 36 Ulzera behandelt, in 33% bei postthrombotischem Syndrom, in den übrigen Fällen bei schwerer Perforansinsuffizienz. Bei 38% der Patienten bestanden die Ulzera schon länger als 10 Jahre, in 6 Fällen in Form von Gamaschenulzera. Nach durchschnittlich 14 Wochen konnten insgesamt 30 Ulzera (77%) zur Abheilung gebracht werden, was der Gesundung von 24 Patienten (82%) entspricht. In Gruppe A waren nach durchschnittlich 16 Wochen 75% der Ulzera abgeheilt, die Meshgraft-Deckung in Gruppe B war bei 90% der Patienten nach 13 Wochen erfolgreich. Im Vergleich zu anderen bekanntenTherapiemaßnahmen zeigt PDWHF einen außerordentlich hohen Anteil von Heilerfolgen, besonders die sonst so problematische Hautdeckung ist durch Vorbehandlung der Ulzera mit PDWHF in fast allen Fällen erfolgreich. Eine prospektive und kontrollierte Studie sollte dieses Behandlungskonzept an einem größeren Patientengut prüfen.
    Notes: Abstract Despite the availability of various topical agents and of new technics for surgical correction, venous stasis ulcers are still characterized by high recurrent rates. Experimental data from wound healing studies demonstrate stimulation of wound healing after topical application of various growth factors (TGFß, PDGF, EGF). The results of clinical studies suggest that topical use of an autologous platelet releasate (PDWHF) containing various growth factors accelerates healing. In this prospective study the stimulating effect of autologous PDWHF on epithelialization of small ulcers (group A, 〈5000 mm2) and granulation of large ulcers before mesh grafting (〉5000 mm2) will be demonstrated. Inclusion criteria were the venous aetiology of the ulcer and the failure of conventional therapy for 6 month. Exclusion criteria were arterial occlusive disease, diabetes mellitus, acute wound infection, thrombocytopenia and pregnancy. There were 24 patients with 36 ulcers, caused by postthrombotic syndrome in on-third of cases and in two-thirds by severe insufficiency of the perforating veins. The ulcer had been present for more than in 10 years in 38% of cases, while there were 6 circumferential ulcers. The overall ulcer healing rate was 77% after a mean of 14 weeks. In group A 78% of the patients were healed after a mean of 16 weeks. In group B the mesh graft procedure was successful in 90% of the patients after a mean of 13 weeks. Compared with other conventional therapy studies, we achieved a higher healing rate. PDWHF seems to create ideal granulation tissue for mesh graft, indicated by a high uptake of the skin grafts. These data should be checked in a randomized, prospective and placebo-controlled trial.
