Abstract Currently available chemotherapy for large-bowel cancer is unsatisfactory. Many drugs have some antitumor effects and 5-fluorouracil is the single most effective agent now available. Several safe methods of administering this drug have been developed, and 20–25% of good-risk patients treated to mild toxicity who have measurable components of disease will show an antitumor response. Controlled clinical studies have not demonstrated 5-fluorouracil to be effective in prolonging survival when used as an adjuvant to surgery. The recent development of rodent tumor models that can be used for the detection of more active agents, and the development of better pharmacologic and kinetic investigational techniques, should produce better regimens whose effectiveness can be demonstrated in well performed human clinical trials.
