    Notes: Summary Peripheral benign nerve sheath tumours are infrequent tumours and affect major nerve trunks. Some authors have indicated a high and prohibitive incidence of neurological injury in resection of these lesions. The authors describe their findings in a retrospective study comprising 119 patients with spontaneous benign nerve sheath tumours of the peripheral nervous system. Seventy-three patients had a schwannoma, 41 had neurofibroma and 5 had plexiform neurofibroma; 25 of the 119 patients suffered from neurofibromatosis. All schwannomas were excised completely and the outcome of patients was 41.0% improved, 6.8% worsened, 52.0% unchanged. Twenty-eight neurofibromas were excised completely and 13 subtotally; the outcome for patients was 19.5% improved, 19.5% worsened and 61% unchanged. All plexiform neurofibromas were removed subtotally and the outcome for patients was 20% improved and 80% unchanged. The best surgical results at average follow-up of 6 years were observed in the patients with schwannoma, the worst in those with plexiform neurofibroma. Our results demonstrated that it is often possible to remove schwannomas as well as neurofibromas with an acceptable risk of injury to the nerve.

    Description / Table of Contents: Sommario La presenza di numerose varianti nelle attività dei T linfociti nei pazienti con sclerosi multipla dimostra la partecipazione del timo in questa patologia. II meccanismo eziopatogenetico della SM è probabilmente une espressione di una reazione autoimmune scatenata da una infezione virale dovuta a uno o più virus, che tende a perpetuarsi determinando cosi il progredire della malattia. È stato provato che il timo mantiene la sua immunocompetenza anche nell'età adulta ed ha un suo ruolo nella patogenesi di alcune malattie autoimmuni. Nella esacerbazione della SM vi è une riduzione dei linfociti T soppressori e la risposta dei linfociti ai mitogeni è a sua volta diminuita. I risultati clinici della timectomia nella SM non sono uniformi e in complesso non conchusivi, ma ciò è probabilmente dovuto alla carenza di criteri selettivi dei pazienti da sottoporre ad intervento. Gli Autori pensano che lo studio morfologico e funzionale della biopsia del timo può offrire criteri appopriati per valutare l'opportunità della timectomia.
    Notes: Abstract The presence of numerous changes in T lymphocyte activity found in MS patients points to the involvement of the thymus in multiple sclerosis. The etiophathogenetic mechanism of MS is probably an autoimmune reaction that is triggered by a viral infection caused by one or more viruses and that perpetuates itself, causing the disease to progress. It has been proved that the thymus maintains its immunocompetence even in adulthood and that it has a role in the pathogenesis in several autoimmune diseases. In exacerbations of MS there is a decrease in T suppressor lymphocytes while histological and lymphocyte subset changes have been demonstrated in the thymus of MS patients. The lymphocyte response to mitogens is also depressed in MS. The clinical results of thymectomy in MS are not uniform and are on the whole inconclusive, probably through the lack of criteria of selection of patients for surgical treatment. We consider that the morphological and functional study of the thymus biopsy specimen should supply the appropriate criteria of suitability for surgical treatment.
