    Notes: Summary Intradural extramedullary glial tumours of the spinal cord are rare. We report four such tumours arising from the dorsal cord. Myelography and operative findings were almost similar to that of an intradural neurofibroma. Surgical removal had resulted in rewarding neurological recovery. One of them had a recurrence after six years and was re-explored. Anterolateral attachment near the root entry zone suggests its origin probably from the spinal cord with an exophytic growth.

    Notes: Summary Twenty two patients with traumatic basal ganglia haematoma were studied. The mean Glasgow Coma Score on admission was 7. 17 patients had sustained high acceleration/deceleration injuries. The location and size of haematoma did not correlate with prognosis. Outcome was poor in 7 patients, while 8 patients died.

    Notes: Summary Traumatic intraventricular haemorrhage (TVH) is rare. The clinical profile and prognosis in 16 patients with traumatic intraventricular haemorrhage after blunt head injury are evaluated. Majority of the patients (94%) had low Glasgow coma score (〈8) on admission and none had a lucid interval. CT showed haematoma adjacent to the foramen of Monroe in 5 patients and localised to the frontal horn or the body of the lateral ventricle in six. The mortality was 62.5% in this subgroup of patients with head injury. Our findings are compared to those of previous reports in the literature. The poor prognosis of cases with TVH reflects the severity of trauma and general brain damage and is probably not related only to the intraventricular bleeding.

    Notes: Summary Forty three patients of subarachnoid haemorrhage of unknown aetiology have been studied for their clinical presentation, rebleed rate, morbidity and mortality. The results have been compared with other similar studies. The present study and those of others indicate a very good prognosis in acute stage. The rebleed rate ranged between 0 and 7% over a period of two to three years. The mortality rate ranged between 0 and 5% during the same period of follow-up. Majority of the patients returned to their full working capacity. The benign nature of this subgroup as compared to the poor outcome of subarachnoid haemorrhage as a whole prompted us to call it “Benign subarachnoid haemorrhage”.

    Notes: Summary 123 cases of meningioma operated upon between 1975 and 1984 were reviewed. There were 13 cases of symptomatic tumour recurrence and 15 cases of tumour progression. Both, recurrence and progression of the tumour were seen twice as often in males. They were not related to any particular age group or site of origin. Recurrecne was more common in the angioblastic type of meningioma. In the majority, progression of the tumour was seen within two years of the initial operation, while recurrence of the tumour was distributed evenly over time.

    Notes: Abstract During that last 6 years, 100 children have been treated for brain abscess at the National Institute of Mental Health and Neurosciences (NIMHANS), Bangalore. Of these, 69 abscesses were otogenic. The treatment included aspiration of the abscess (84% of cases) and antibiotic therapy. CT scan as a diagnostic tool was available only for a few cases in the last year. In 16% of the cases the abscess was excised. The overall mortality was 21%. Of the surviving children, 76% recovered totally. Epilepsy as a sequel occurred in 21% of the cases. The fact that mortality continues to be high is attributed to the degree of impairment of consciousness of the children at the time of admission. Early detection and successful treatment of brain abscess in children still remain a clinical challenge.

    Description / Table of Contents: Résumé Les auteurs ont étudié 60 cas de cisternographies à la RISA avec/ou ventriculographies chez des enfants âgés de un mois à 15 ans, atteints de différents troubles de la circulation du LCR. Cette étude comprenait des cas d'hydrocéphalie et d'autres malformations congénitales du système nerveux central ainsi que des séquelles post-traumatiques et post-méningitiques. L'examen se révéla fiable, non seulement dans la différenciation entre hydrocéphalie compensée ou progressive mais également dans la détection d'anomalies associées de la circulation de LCR en cas de méningo-myélocèles d'encéphalocèles. Cette technique est excellente pour la localisation d'une fistule de LCR; elle peut également visualiser la taille et l'extension d'un hématome sous-dural ou d'un hydrome. La concentration pathologique et l'élimination tardive du radio-élément dans la région sous-jacente à la lacune crânienne étaient constantes dans tous les cas de lacunes post-traumatiques. Des blocages uniques ou multiples furent observés dans des cas de séquelles postméningitiques (tuberculose).
    Abstract: Zusammenfassung Bei 60 Kindern im Alter swischen 1 Monat und 15 Jahren wurde die RIHSA-Cisternographie und/oder Ventrikulographie durchgeführt. Es sollten damit unterschiedliche Hydrocephalus-Formen und andere angeborene Mißbildungen des Zentralnervensystems, auch posttraumatische und postmeningitische Veränderungen nachgewiesen werden. Diese Untersuchungsmethode zeigt sich besonders wertvoll zur Differenzierung der einzelnen Hydrocephalus-Arten zum Nachweis von Anomalien der Liquorzirkulation, besonders bei Meningomyelozelen und Encephalozelen. Auch können Liquorfisteln und subdurale Hämatome oder Hygrome damit exakt festgestellt werden. Pathologische Veränderungen finden sich auch bei allen Patienten mit wachsenden Frakturen. Bei postmeningitischen (tuberkulösen) Verklebungen konnte der Stopp lokalisiert werden.
    Notes: Summary Sixty children aged between one month and fifteen years were submitted to RIHSA cisternography and/or ventriculography for a variety of lesions affecting the CSF pathways. These included cases of hydrocephalus and other congenital malformations of the central nervous system, posttraumatic and post meningitic sequelae. The investigation has been found to be valuable not only in differentiating between arrested and progressive hydrocephalus but also in unmasking associated anomalies of CSF flow and pathways in cases of meningomyelocele and encephalocele. It is excellent for delineating a CSF fistula and can also be utilised to demonstrate the size and extent of a subdural hematoma or hygroma. Pathological concentration and delayed clearance of the isotope in the region underlying the skull defect was a consistent finding in all cases of growing fracture (craniocerebral erosion). Single or multiple sites of blocks were observed in cases of postmeningitic (tuberculous) sequelae.

    Notes: Abstract Brainstem auditory evoked responses (BAER) were studied in 20 children with clinically diagnosed and CT-confirmed congenital hydrocephalus before and after shunt surgery. Ninety-five percent of the children showed abnormal responses preoperatively. Prolonged wave V latency was the most common abnormality, followed by increased interwave latencies. Total absence of evoked responses was more common in children with communicating hydrocephalus. Following shunt surgery 50% of cases returned to normal and 20% showed a significant improvement. Abnormalities persisted in 10% of cases. BAER abnormalities referrable to caudal brainstem dysfunction recovered first, following CSF diversion. Study of BAER is useful for identifying physiological brainstem abnormalities in hydrocephalic children and promises to be a sensitive non-invasive diagnostic tool for the detection of “non-infective complications” of shunt surgery, if performed serially during follow up.
