Die Gefäßbilder dieser Erkrankungen werden geschildert, der differentialdiagnostische Wert der Angiographie erörtert. AbschlieüsBend wird auf die Indikation zur Gefäßdarstellung bei entzündlichen Darmerkrankungen eingegangen.
    Notes: Summary Regional enterocolitis (Crohn's disease), ulcerative colitis, and segmental infarcts of the gut may be of similar appearance in barium investigations, but may be differentiated by angiography. The appearance of the blood vessels in these disorders is described, and the differential-diagnostic value of angiography is discussed. Finally, the indications for visualization of blood-vessels in inflammatory disorders of the gut are described.

    Notes: Abstract Radiographic contrast media (RCM) in clinical use cause unwanted allergic/pseudoallergic reactions of all grades of severity. They also induce histamine release from a variety of mast cell populations, the extent of the histamine release reaction depending on both the organ and species. In this study 3 RCM, which had been previously shown to be effective histamine releasing agents with canine liver cells, were investigated using anin vivo canine model based on the clinical situation. The dogs (n=36) were randomly allocated to one of 3 treatment groups and received a bolus injection (2 ml/kg body weight) of either Angiographin®, Hexabrix® or Telebrix®. Blood pressure was monitored continuously and blood sampling, for plasma histamine measurements, was performed before and 1, 5, 10, 20 and 30 min after RCM injection. All 3 RCM caused elevated plasma histamine levels in some animals: Angiographin® 9 of 12 dogs, 0.40 ng/ml, (0–1.9 ng/ml) median (range); Hexabrix® 11/12, 0.5 ng/ml (0–3.8 ng/ml); Telebrix® 7/12, 0.4 ng/ml (0–2.0 ng/ml). Cardiovascular reactions were observed in most animals. The hypotensive reactions occurred with a maximum 30 sec after RCM application and recovery was normally observed after 1–1.5 min. The response after Angiographin® or Telebrix® was significantly greater than after Hexabrix®. Hypertensive reactions occurred later (15 min (5–25 min)) and did not differ between the groups. All 3 agents tested were able to elicit histamine release and cardiovascular reactions. In comparison to histamine release occurring after intravenous, administration of other agents, such as hypnotics, the degree of histamine release was small. However, the frequency with which it was observed suggests that histamine acts as a contributory condition in adverse reactions to RCM. The possibility of either a localized histamine release (e.g. from liver cells) or complement activation leading to histamine release must be considered.

    Notes: Zusammenfassung Mangeldurchblutung der Magenschleimhaut bewirkt nach eigenen früheren tierexperimentellen Untersuchungen eine bedeutende Zunahme der Acidität. Es wurde vermutet, daß nicht die Minderdurchblutung, d. h. die Sauerstoffabnahme oder die CO2-Zunahme im Gewebe, für das Ansteigen der H-Ionenkonzentration direkt verantwortlich ist, sondern eine histaminartige Substanz, die durch CO2-Überschuß freigesetzt wird. In diesem Fall war ein Aciditätsanstieg im Magen auch dann zu erwarten, wenn die Durchblutung magenferner Körperteile, z. B. der Beine, eine gewisse Zeit (10 min) durch Unterbindung stark reduziert wird. Eine Zunahme der H-Ionenkonzentration im Magen wenige Minuten nach Lösung der Unterbindung fand sich bei fast allen Probanden. Die Vermittlerrolle des Histamins oder verwandter Substanzen bei der Säurestimulierung durch Hypoxämie scheint damit bewiesen.

    Description / Table of Contents: Zusammenfassung Von 1969 bis 1978 wurden an den Universitätskliniken in Marburg 68 Laparotomien zur Stadieneinteilung beim M. Hodgkin durchgeführt. Voraus gingen eine klinische Untersuchung, in 27 Fällen eine Lymphographie, in 58 Fällen eine Leber-Milz-Szintigraphie und in 18 Fällen eine Röntgenuntersuchung des Intestinaltraktes. Die Allgemeinsymptomatik der 68 Patienten korrelierte besser mit dem pathologischen als mit dem klinischen Stadium. Die Ultraschalluntersuchung ergab für den Milzbefall eine höhere Treffsicherheit (90%) als die Szintigraphie (74%); für den Leberbefall erwiesen sich Sonographie und Leberszintigraphie gleichwertig. Für die Erkrankung paraaortaler Lymphknoten sind Lymphographie und Ultraschalldiagnostik als einander ergänzend anzusehen. Unsere Ergebnisse werden mit den Mitteilungen der Literatur über abdominelle Sonographie (n=50), Leber-Milz-Szintigraphie (n=185) und Lymphographie (n=465) verglichen. Auf die Bedeutung prognostisch unterschiedlicher Gruppen z. B. im Stadium III wird hingewiesen.
    Notes: Summary Explorative laparotomies were carried out on 68 patients with Hodgkin's disease in the University Hospital of Marburg from 1969 through 1978. These laparotomies were preceded by clinical examination, abdominal sonography in 27 cases, lymphography in 55 cases, scintigraphy of liver and spleen in 58 cases, and radiographic examinations of the intestinal tract in 18 cases. Sonography revealed a greater accuracy (90%) for splenic involvement than scintigraphy (74%); the results of sonography and scintigraphy of the liver were comparable. For the detection of para-aortal lymphomas ultrasound and lymphography can be regarded as complementary methods. Our results are compared with findings in the literature on abdominal sonography (n=50), scintigraphy of liver and spleen (n=185), and lymphography (n=465) carried out before explorative laparotomy for lymphogranulomatosis. There was a better correlation for the systemic symptoms of the 68 patients with the clinical stage than with the pathologic stage. The importance of diagnosing prognostic different groups, e.g. in stage III, is stressed.

    Notes: Abstract Immediately after an infusion pyelography with diatri-iodobenzoate the GFR is reduced to 79.8%±14.9 of the initial value. This reduction of approximately 6 hours p.i. amounted to an average of 91.3%±15. The extent of the reduction is in inverse proportion to the degree of the renal insufficiency.
