    Notes: Summary During a space mission flown in microgravity early changes in intraocular pressure due to the fluid shift effect in microgravity were analyzed with an automatic self-tonometry. In total, 13 pressure readings per eye were obtained during the mission. The first pressure readings 16 min after reaching microgravity revealed a 92% rise in intraocular pressure compared to the daytime-correlated baseline data on earth followed by an adaptation phase lasting for several hours. Diurnal pressure profiles during the mission showed no relevant difference in altitude and amplitude.

    Notes: Abstract Electronic dynamometry measurements, performed upon intraocular lens (IOL) haptics of prototype one-piece three-loop silicone lenses, accurately defined the relationships between elastic force and haptic displacement. Lens implantations in the capsular bag of dogs (loop span equal to capsular bag diameter, loops undeformed immediately after the operation) were evaluated macrophotographically 5–8 months postoperatively. The highly constant elastic property of silicon rubber permitted quantitative correlation of subsequent in vivo haptic displacement with the resultant force vectors responsible for tissue contraction. The lens optics were well centered in 17 (85%) and slightly offcenter in 3 (15%) of 20 implanted eyes. Of the 60 supporting loops, 28 could be visualized sufficiently well to permit reliable haptic measurement. Of these 28, 20 (71 %) were clearly displaced, ranging from 0.45 mm away from to 1.4 mm towards the lens' optic center. These extremes represented resultant vector forces of 0.20 and 1.23 mN respectively. Quantitative vector analysis permits better understanding of IOL-capsular interactions.

    Description / Table of Contents: Abstract Applanation tonometry, following Goldmann's hypothesis serves as the gold Standard in tonometry. However, even this approach principle does not always eliminate examiner's error, especially in defining the applanation surface. An automatic surface and force control system is used to increase measurement precision, thus increasing precision and speed of measurement. The instrument, already in use for the patient's self-tonometry, is now also available for the ophthalmologist. An additional coaxial observation system allows precise centering of the applanation surface onto the cornea. This allows new applications of applanation tonometry in daily routine.
    Notes: Zusammenfassung Applanationstonometer nach dem Goldmann-Prinzip gelten heute unbestritten als Referenzgeräte für die Bestimmung des intraokularen Druckes. Aber auch hier kann es, insbesondere bei der Ausmessung der Applanationsfläche, zu einem gewissen Untersucherfehler kommen. Unter Beibehalten der Goldmann-Hypothese für die Applanationstonometrie sollte deshalb die Automatisierung des gesamten Messablaufes erreicht werden. Dies gilt sowohl für die Ausmessung der Applanationsfläche wie auch für die Kontrolle der dazu benötigten Messkraft, dem eigentlichen Messparameter der Goldmann-Applanationstonometrie. Gleichzeitig sollte der eigentliche Messvorgang erheblich beschleunigt werden, was wiederum mögliche Messfehler verringert. Das angestrebte Prinzip wurde bereits erfolgreich bei dem automatischen Selbsttonometer für den Gebrauch durch den Patienten verwirklicht. Durch entsprechende Einfügung eines koaxialen Beobachtungsstrahlengangs kann nun auch der Augenarzt das Gerät für seine Messungen benutzen, von der größeren Präzision profitieren. Die dadurch gegebenen neuen Anwendungsmöglichkeiten, aber auch der technische Aufbau und die Kalibration werden dargestellt.

    Description / Table of Contents: In a randomised clinical study two new microprocessor-controlled tonometers working according to the Mackay-Marg principle (Tono-Pen and ProTon, two samples of each) and two established applanation tonometers [hand-held applanation tonometer, Draeger (HAT) and Goldmann tonometer] were compared with respect to accuracy, precision, examiner dependence and internal measurement differences (Tono-Pen I vs II, ProTon I vs II). The four tonometers were used by two examiners six times on each of 193 eyes of 100 patients of the university eye department. The same measuring procedure was performed on four cadaver eyes in situ, using a manometer system. The correlation coefficient between Tono-Pen and HAT and between ProTon and HAT was 0.9, with a high standard deviation, about ±3.1 mmHg. HAT gave the best results in repetitive measurements in the same eye (precision: r = 0.92, SD = ±1.41 mmHg). In applanation tonometers but not in Mackay-Marg tonometers measurement values depended significantly on the physician. There was a significantly higher standard deviation between Tono-Pen I and II than between ProTon I and II. Concerning in vitro calibration series on human bulbs (manometry), accuracy was high with each tonometer. The regression line of HAT is similar to that of ProTon. Tono-Pen overestimates the manometrically determined intraocular pressure. Because of their lack of precision the two new tonometers are not suitable for clinical use.
    Notes: In einer randomisierten klinischen Kalibrationsstudie wurden 2 neu-artige mikroprozessorgesteuerte Tonometer vom Mackay-Marg-Prinzip (Tono-Pen, ProTon; pro Tonometer je 2 Exemplare) und 2 herkömmliche Applanationstonometer (Handapplanationstonometer nach Draeger-, HAT-, Goldmann-Tonometer) von 2 Untersuchern an 193 Augen von 100 Patienten der Universitäts-Augenklinik sowie in vitro (Manometrie am Humanbulbus) angewandt. Die Meßergebnisse wurden hinsichtlich ihrer Reproduzierbarkeit, Genauigkeit, Untersucherabhängigkeit und geräteinterner meßtechnischer Differenzen statistisch ausgewertet. Die Korrelation der Tono-Pen- und ProTon-Meßwerte mit denen des HAT (Meßgenauigkeit) zeigt hohe Standardabweichungen (SD um ±3,1 mmHg) bei einem Korrelationskoeffizienten r um 0,9. Die Reproduzierbarkeit von Wiederholungsmessungen eines Tonometers an einem Auge ergibt beim HAT mit r = 0,92 und SD ± 1,41 mmHg das beste Resultat. Die Applanationstonometermeßwerte zeigen gegenüber denjenigen der Mackay-Marg-Tonometer eine statistisch signifikante Untersucherabhängigkeit. Beim internen Tonometervergleich liegt die Streuung zwischen den Tono-Pen-Geräten signifikant über derjenigen der ProTon-Tonometer. Die Reproduzierbarkeit von Einzelmessungen ist in vitro bei allen 4 Instrumenten hoch. Die Regressionsgeraden von HAT und ProTon sind ähnlich konfiguriert. Das Tono-Pen überschätzt den vorgegebenen Manometerdruck. Bei mangelnder Präzision sind die beiden neuartigen Tonometer Tono-Pen und ProTon in der vorliegenden Ausführung für den klinischen Gebrauch ungeeignet.

    Description / Table of Contents: Summary So far medical requirements for aviators have been defined by national licensing authorities. For a long while ophthalmological problems, insufficiencies in vision, refraction or color vision, have been responsible for approximately 50 % of rejection of applicants for medical reasons. New international regulations proposed as well by ICAO as JAA try to lead to a new balance in these requirements. In general, ophthalmological requirements now allow for lower standards, specifically in refraction, but also in color vision. Also new examination methods are admitted. German and French authorities have announced their opposition being concerned about flight safety for the future.
    Notes: Zusammenfassung Die bisher national gültigen Richtlinien für die Tauglichkeit des Luftfahrtpersonals haben in der Vergangenheit alljährlich zu einer gewissen Ausschussquote bei den Antragstellern geführt – daran war das Sehorgan im weiteren Sinne traditionell jeweils mit etwa 50% als Ausschlusskriterium beteiligt. Die seit 1994 von der International Civil Aviation Organization (ICAO), Montreal, überarbeiteten und vorgelegten internationalen Richtlinien wie auch die im vergangenen Jahr von der Joint Aviation Authority (JAA) für die Europäischen Luftfahrtnationen vorgeschlagenen neuen Tauglichkeitsrichtlinien sollen im Laufe der kommenden zwei Jahre in gültiges deutsches Recht umgesetzt werden: Dabei geht es vor allem um die zulässigen Refraktionsgrenzwerte und andere Prüfungsmethoden für die Farbtauglichkeit. Von deutscher wie französischer Seite werden allerdings Bedenken gegen die z. T. zu weit gehenden Erleichterungen, ihre mögliche Auswirkung auf die Flugsicherheit geltend gemacht.

    Notes: Abstract In 159 eyes with a spherical correction varying from +14.0 to −27.0 diopters, the axial length and thickness of posterior ocular coats were measured ultrasonographically. As proven in experimental studies, retina, choroid, and sclera were resolved in a considerable percentage of cases by combining A- and B-mode techniques of modern ultrasonographic equipment. The presumed close inverse correlation between the axial length of the eye and ocular coat dimensions was confirmed by statistical analysis. These values were used to calculate the approximative volume of the posterior half of the ocular coats. There was evidence for a constant tissue volume regardless of the difference in surface and axial length parameters between hyperopic and myopic eyes. Results are discussed and compared with theories of axial growth under physiological and pathological conditions.

    Description / Table of Contents: Summary Aim of our study was to examine the clinical applicability of a suction pump in use for cataract extraction as dynamometer. The advantages of the new pump are: High efficiency and easier employment. Linear curves for this instrument were obtained with the applanation tonometer. So this method is suitable for clinical use.
    Notes: Zusammenfassung Ziel unserer Untersuchung war es, die klinische Brauchbarkeit eines seit Jahren verwendeten Gerätes, der Saugpumpe zur Kataraktextraktion mit dem Erysophak, für die Unterdruckdynamometrie zu prüfen. Die Vorteile der neuen Pumpe sind ihre größere Leistungsfähigkeit und leichtere Handhabung. Mit Hilfe des Applanationstonometers wurden Eichkurven für dies neue Gerät aufgestellt. Ihr linearer Verlauf läßt eine einfache und hinreichend zuverlässige klinische Verwendung zu.

    Notes: Abstract In an experimental study porcine corneas were trephined using different trephining procedures. For every series of trephinations, the initial cell count of the donor cornea was found to play a role in the resulting damage induced by the cutting procedure. Pretreatment of corneas and modifications of trephining conditions did not change the negative correlations between cell count and damage due to trephining.

    Notes: Abstract ⊎ Background: In glaucoma surgery, e.g. cyclodialysis ab interno, to date no viscous substances have been available that stay in the cyclodialysis cleft long enough to prevent scarring. We examined the qualities of two different viscoelastic substances that stay longer in the place of instillation than hyaluronic acid. ⊎ Methods: We performed cyclodialysis ab interno in 12 eyes of six owl monkeys. The right eyes were treated with the gel with the higher molecular weight, the left eyes using the gel with the lower molecular weight. The baseline data of intraocular pressure and outflow facility were compared to the values measured postoperatively up to the 140th day. Histological examination of the eyes followed. ⊎ Results: There was no statistically significant difference between the two substances. The mean intraocular pressure decrease was 7.9 (±2.9) mmHg in the right eyes and 4.8 (±2.7) mmHg in the left eyes. The histological examination showed that the gel had remained in the cyclodialysis cleft without signs of inflammation. In contrast to other viscoelastic substances studied previously, the substances in this study performed their space-retaining function without decreasing outflow facility. ⊎ Conclusion: Both substances are suitable means of improving the rate of success of cyclodialysis ab interno, as they remain where they are instilled long enough to prevent wound scarring in the cyclodialysis cleft.
