    Description / Table of Contents: Zusammenfassung Die Penetration von Erythromycin wurde in 108 Fällen von sekretorischer Otitis media untersucht. Erythromycin-Ethylsuccinat wurde oral in einer Dosierung von 30–60 mg/kg/Tag verabreicht. In unterschiedlichen Zeitabständen wurde Mittelohrsekret zur Untersuchung entnommen. Nach 26 Stunden hatte die Erythromycinkonzentration im Mittelohrsekret die Plasmakonzentration erreicht (1,1–1,2 mg/l) und blieb im steady state in diesem Bereich. Die Elimination vollzog sich langsam. Noch 14 Stunden nach Verabreichung der letzten Dosis nach zehntägiger Behandlung betrug die Erythromycinkonzentration im Mittelohrsekret 0,9 mg/l, die Plasmakonzentration dagegen nur noch 0,2 mg/l. Die Erythromycinkonzentration im Sekret übertraf die minimalen Hemmkonzentrationen für die meisten Atemwegspathogene. Erythromycin kann somit als geeignetes Antibiotikum zur Behandlung der Otitis media gelten. Da die Mucosa der Atemwege bei Entzündungen einheitlich reagiert, besitzt das Ergebnis der Penetrationsstudie zudem Gültigkeit für entzündliche Erkrankungen in jedem mit respiratorischem Epithel ausgekleideten Bereich.
    Notes: Summary The penetration of erythromycin was studied in 108 cases of secretory otitis media. Erythromycin ethylsuccinate was given orally in a dosage of 30–60 mg/kg/day and samples of middle ear effusion were obtained at different intervals. The concentration of erythromycin in the middle ear effusions reached the plasma level (1.1–1.2 mg/l) after 26 hours. The plateau levels were maintained during the steady state. Elimination was slow; 14 hours after the last dose of a ten-day treatment, the concentration of erythromycin in the middle ear effusions was still as high as 0.9 mg/l, compared to 0.2 mg/l in plasma. The concentration of erythromycin in the middle ear effusions surpassed the MICs of most respiratory pathogens. Erythromycin can thus be regarded as a suitable drug in the treatment of otitis media. Furthermore, since the respiratory mucosa reacts in a uniform manner to an inflammatory stimulus, the result of this penetration study is valid for inflammatory diseases in any area lined with respiratory epithelium.

    Description / Table of Contents: Zusammenfassung 72 Kinder ohne weitere Grundkrankheiten erhielten in verschiedenen Zeitabständen vor der Adenoidektomie Erythromycin-Ethylsuccinat in einer Dosierung von 30–60 mg/kg/Tag. Die Erythromycinkonzentration wurde in homogenisiertem Gewebe, in Vollblut und in Plasma bestimmt; damit war die Berechnung der blutfreien Gewebespiegel möglich. Die Gewebespiegel (1,5–2,0 mg/l) entsprachen den Plasmaspiegeln. Sechs Patienten mit chronischer Tonsillitis erhielten 1000 mg Erythromycin-Ethylsuccinat durch eine Magensonde; drei bis fünf Stunden später wurde kryochirurgisch die Tonsillektomie vorgenommen. Die ungewöhnliche Applikationsform wurde gewählt, um eine Oberflächenkontamination der Tonsillen zu vermeiden, die zu verfälschten Meßwerten führen könnte. Die Erythromycinkonzentration wurde bestimmt wie oben beschrieben. Die Konzentrationen im Tonsillengewebe entsprachen den gleichzeitig gemessenen Plasmaspiegeln. Es wurde gezeigt, daß Erythromycin in Rachenmandeln und Tonsillen penetriert, dabei werden Gewebespiegel ähnlich den simultan bestehenden Plasmaspiegeln erreicht. Daraus läßt sich ableiten, daß Erythromycin in lymphatisches Gewebe anderer Lokalisation ebensogut penetriert.
    Notes: Summary Erythromycin ethylsuccinate was given orally (30–60 mg/kg/day) to 72 otherwise healthy children for different intervals before adenoidectomy. The concentration of erythromycin was assessed in homogenized tissue, in whole blood and in plasma, making it possible to calculate blood-free tissue levels. The tissue levels (1.5–2.0 mg/l) were the same as the plasma levels. Six patients with chronic tonsillitis were given 1000 mg erythromycin ethylsuccinate via a gastric tube, and tonsillectomy à froid was performed three to five hours later. This unusual mode of administration was chosen to avoid tonsil surface contamination, which might cause artificially high values. The concentration of erythromycin was assessed as described above. The levels of erythromycin in the tonsil tissue were the same as the concomitant plasma levels. Erythromycin has been shown to penetrate into adenoid and tonsil tissue and to attain levels similar to the simultaneous plasma levels. It may be inferred from this study that erythromycin penetrates into other lymphatic localities to the same degree.
