    Notes: Summary Changes in distribution of elastin and elastin receptor were studied during maturation of the ductus arteriosus. In this vessel intimal thickening is a normal process. It starts with separation of endothelial cells from the internal elastic lamina by the formation of a wide subendothelial region, followed by an increase in number of radially orientated cells in the inner media and subendothelial region. For the first time the nature of these originally inner media cells could be definitely determined as being derived from smooth muscle cells. Areas rich in these modified smooth muscle cells contained less elastin as compared with regions rich in circularly orientated smooth muscle cells. The internal elastic lamina had disappeared underneath intimal cushions of the canine ductus arteriosus, whereas in the fetal human ductus arteriosus, splitting of the internal elastic lamina was observed. Elastin was not present underneath separated endothelial cells, which suggests that these cells do not synthesize elastin, at least not after separation. Both smooth muscle cells and endothelial cells demonstrated the presence of the elastin receptor. Changes in its distribution were not observed. The temporal and spatial association between the altering distribution of elastin and the absence of normal cushion formation in the persistent ductus arteriosus suggests a role of the elastin receptor in the process of intimal thickening.

    Notes: Summary The multicompartmental frog skin epidermis model proposed in a previous paper was applied to computer simulation studies on the kinetics of the wash-out process of Na* from frog skin. Both the kinetics of loading of the model membrane with Na* from the outside to reach steady-state conditions in all internal compartments, and of the wash-out process were followed. This was done for the case when two Na+ pumps were operative, or inoperative, simulating the inhibitory effect of ouabain on active Na+ transport in frog skin. The two pumps were characterized as transmembrane Na+ flow pumps, and internal Na+ maintenance pumps which contribute but little to net inward Na+ flux. The simulation results were in good agreement, both qualitatively and quantitatively, with data in the literature on the behavior of frog skin epidermis. This analysis gives support especially to the views held by Zerahn on location and size of the active Na+ transport pool in skin epithelium. Beyond this, however, this study clearly delineates the experimental conditions under which the estimation of the Na+ transport pool by the method of measuring the wash-out rate of Na* may be successful, and under which conditions this method will fail.

    Notes: Summary The operation of a seven-compartment model is described with respect to flows of Na+ within and across this system, simulating published results obtained on frog skin. The seven compartments represent: one outside and one inside solution compartment; the subcorneal space; the first reacting cell layer (1. RCL); the remaining cell compartment; the non-, or slowly exchangeable Na+ compartment; the extracellular space. Assuming reasonable volumes for the epidermal compartments and further chosing, by trial and error, appropriate rate constants, a set of seven simultaneous linear differential equations was solved by the application of the Continuous System Modeling Program (CSMP), using an IBM 1130 computer. Initial conditions for influx, backflux and net flux were taken which correspond to [Na+]0; [Na+] i =115mm. Print-out data were obtained at 0.5-min intervals for 30 min, when steady states were obtained in 13 models studied, varying certaink's thus simulating actions of chemical agents (hormones; drugs). Simulation was achieved with regard to rate of influx, backflux and net flux, steady-state time (30 min), and electrical potentials. In addition, this approach gave detailed information on Na+ pool sizes and their variations with changes ink's. These results are compared to published data on frog skin and good agreement between operation of skin epidermis and model was found.

    Notes: Summary The operation of the multicompartmental frog skin epidermal model 10E described in the preceding paper was tested to find out by computer simulation whether it responds to changes in [Na+] in the same manner as frog skin. In the range from 5 to 115mm [Na+]0, the rate of net Na+ flux across skin is known to increase. The results can be fitted to Michaelis-Menten's law of reaction kinetics, or, alternately, to Hoshiko's linear function, plotting fluxvs. log [Na+]0. Model 10E simulated the laboratory results on skin, provided that the rate coefficients at the site of entry of Na+ into the system were varied in exactly the same manner as they actually were found to vary in skin. In model studies, Na+ backflux (outflux) decreased with increasing [Na+]0, contrary to observations on skin. This discrepancy may be related to adaptive reactions in skins (decrease in permeability) when [Na+]0 is lowered, a feature that has not been modeled. It is known that the skin p.d. changes, mostly, by approximately 35 mV per decade change in [Na+]0. Model 10E gave very nearly the same result when the rate coefficients for entry of Na+ were changed as mentioned above (i.e., varied exactly as they were found to vary in skin). Skin and model 10E behaved similarly in that, at [Na+]0=[Na+] i =115mm, the extent to which labeling with Na* from the outside (12%) and from the inside (88%) is possible was the same. Model data are presented which show in which way the Na+ pools, [Na+] in the individual compartments, and intercompartmental fluxes changed with changing [Na+]0. Because of lack of experimental data on skin for comparison, these calculated results are purely hypothetical, but they are not unreasonable.

    Description / Table of Contents: Summary The Central Malignant Melanoma Registry of the German Dermatological Society was founded in 1983 and has meanwhile developed into a major continuously updated multicentre project. Up to June 1994, 19250 reports of cutaneous melanoma had been received, from 41 departments of dermatology in the former Federal Republic of Germany, from 14 departments in the former German Democratic Republic, from 2 departments in Austria, and from 1 department in Switzerland. Analysis of the data revealed some epidemiological trends over time during the years 1983 to 1993. (1) During the last 10 years the percentage of male patients has steadily increased, from an average of 38% in the year 1983 to 46% in 1993. (2) Early diagnosis of malignant melanoma improved during the period of time investigated. The percentage of diagnoses of primary tumour alone increased between 1983 and 1993. The mean tumour thickness (Breslow) decreased in the West Germany from 1.8 mm to 1.3 mm and in East Germany from 2.5 mm to 1.7 mm. The proportion of nodular melanoma decreased correspondingly from 29% to 14% in the former Federal Republic of Germany and from 40.6% to 22.6% in the former Germany Democratic Republic. During the years 1990 and 1993, 64% of melanoma patients with the primary tumour alone were operated on in two consecutive sessions in the former Federal Republic of Germany and 34.2% of those in the former Germany Democratic Republic. During this period 73.7% of all melanoma patients were operated on under local anaesthesia. In recent years surgical operations were more often performed in two consecutive sessions, mostly under local anaesthesia and with decreasing safety margins, in keeping with the decrease in tumour thickness. The present analysis shows that the Central Malignant Melanoma Registry is an important instrument for investigating trends in clinical epidemiology and treatment of malignant melanoma in the German-speaking countries.
    Notes: Zusammenfassung Das Zentralregister Malignes Melanom der Deutschen Dermatologischen Gesellschaft hat seine Arbeit 1983 aufgenommen und sich seitdem zu einem kontinuierlich fortgeführten multizentrischen Projekt entwickelt. Insgesamt wurden bis Juni 1994 die Daten von 19.250 Patienten aus 41 Kliniken der alten Bundesländer, 14 Kliniken der neuen Bundesländer, zwei Kliniken aus Österreich und einer Klinik aus der Schweiz gemeldet. Die Auswertung der Daten läßt mehrere epidemiologische Trends im Zeitraum von 1983–1993 erkennen. a) Es fand sich eine kontinuierliche Zunahme des Anteils der Männer an den Melanom-Patienten, der im Mittel von 38% im Jahre 1983 auf 46% im Jahre 1993 anstieg. b) Die Früherkennung maligner Melanome wurde im untersuchten Zeitraum verbessert. Die Diagnosen im Stadium des Primärtumors stiegen im untersuchten Zeitraum an. Die mittlere Tumordicke nach Breslow nahm in den alten Bundesländern von 1,8 mm auf 1,3 mm und in den neuen Bundesländern von 2,5 mm auf 1,7 mm ab. Entsprechend verringerte sich der Anteil nodulärer Melanome in den alten Bundesländern im Mittel von 29,3% auf 14,1% und in den neuen Bundesländern von 40,6% auf 22,6%. In den Jahren 1990–1993 wurden in den alten Bundesländern 64% und in den neuen Bundesländern 34,2% aller primären malignen Melanome zweizeitig operiert. Insgesamt wurden 73,7% der Patienten in Lokalanästhesie operiert. In den letzten Jahren wurde vermehrt in Lokalanästhesie und zweizeitig operiert, wobei entsprechend der Abnahme der Tumordicke die endgültige Versorgung häufiger mit kleinerem Sicherheitsabstand erfolgte. Die vorliegende Analyse zeigt, daß das Zentralregister Malignes Melanom ein wichtiges Instrument zur Untersuchung von Entwicklungen der klinischen Epidemiologie und der Therapie des malignen Melanoms im deutschsprachigen Raum ist.

    Description / Table of Contents: Summary Frehsly isolated keratinocytes were used for the treatment of non-healing leg ulcers (21 cases). For application, keratinocytes were suspended in fibrin (Tissucol Duo S). One or more applications of keratinocytes initiated granulation and reepithelialization of nearly all leg ulcers (20 cases). The method is compared with the established therapy entailing transplantation of allogenic keratinocyte sheets onto leg ulcers. Our method shows the advantages: autologous cells that are immediately and easily available, freedom from pain, and short-time immobilization (48 h) after application.
    Notes: Zusammenfassung Schlecht heilende Ulcera crurum wurden mit autologen Keratinozyten behandelt (21 Fälle), welche aus patienteneigenen Haarfollikeln isoliert und kurz inkubiert waren. Ihre Applikation erfolgte in Fibrinkleber (Tissucol Duo S). Die ein- oder mehrmalige Applikation von Keratinozyten führte zur Induktion von Granulation und Reepithelisierung bei fast allen Patienten (20 Fälle). Diese Methodik wird verglichen mit der bisher bevorzugten Transplantation von vorwiegend allogenen angezüchteten Keratinozyten-Sheets auf Ulcera. Die wesentlichen Vorteile unserer Methode sind autologe Zellen, die sofort verfügbar sind sowie Schmerzfreiheit und kurze Liegezeit nach der Applikation.
