    Notes: Abstract Purpose To evaluate the imaging characteristics and the diagnostic value of both abdominal ultrasound and conventional abdominal x-ray in identifying ingested drug packages in “body packers.” Materials and Methods Twelve individuals were studied prospectively by abdominal ultrasound and by abdominal x-ray to rule out incorporated drug containers. Results Both abdominal ultrasound and abdominal x-ray correctly identified the 7 of 12 individuals who had ingested multiple drug packages. In 5 of the 12 individuals, ultrasound as well as abdominal x-ray were correctly unremarkable with regard to abdominal foreign bodies. Conclusions Abdominal ultrasound and abdominal x-ray both represent valuable diagnostic tools in the assessment of ingested drug packages. Abdominal x-ray is superior to ultrasound in differentiating ingested drug-filled condoms from “pseudocondoms.”

    Notes: Summary In previous studies we have shown that glycolysis is strongly inhibited in cultured calvaria cells treated with ethane-1-hydroxy-1, 1-diphosphonate (EHDP) or dichloromethanediphosphonate (Cl2MDP). This study shows that the energy charge of the adenylate pool, and the ATP and Pi content, were not changed by treatment with diphosphonates except for a slight decrease of ATP and Pi at 0.25 mM Cl2 MDP. The uptake of Pi was diminished by 50% and 20% in cells treated for 6 days with Cl2MDP or EHDP, respectively, but not when diphosphonates were present only during the uptake studies.

    Description / Table of Contents: Zusammenfassung Bei 7 Patienten (Durchschnittsalter 42 Jahre) mit Rhythmusstörungen verschiedener Form wurden mit Hilfe der Myokardszintigraphie Verteilungsstörungen der myokardialen Perfusion bei im Koronarogramm unauffälligen extramuralen Koronargefäßen nachgewiesen. Die Defekte waren zum Teil umschrieben, zum Teil diffus. Bei 2 Patienten mit einem Linksschenkelblock projizierte sich die Perfusionsverteilungsstörung auf den oberen Teil des Septum interventriculare. Ein Zusammenhang zwischen der Topographie des Perfusionsdefektes und der Form der elektrophysiologischen Abnormität erscheint bei diesen Patienten wahrscheinlich. Als Ätiologie der myokardialen Perfusionsstörung kann bei einem der Patienten ein Zustand nach Myokarditis mit narbiger Ausheilung angenommen werden. Bei den anderen 6 Patienten muß die Ursache der Perfusionsstörung offen bleiben. Das Vorliegen einer „small vessel disease“ muß diskutiert werden. Die Untersuchungen weisen auf einen Zusammenhang zwischen Störungen im Bereich der Mikrozirkulation und Arrhythmien bei jüngeren Patienten hin.
    Notes: Summary In 7 patients with arrhythmias of various origin the myocardial scintigram displayed either a diffuse or circumscript defect of the perfusion. The coronary arteriogram was normal in all patients. The localized defect of the perfusion in 2 patients was in the region of the upper part of the interventricular septum. Both had a left bundle brunch block. A correlation between the perfusion defect and the electrophysiological abnormality seems probable. The perfusion defect in one of the patients is most probably caused by a previous myocarditis followed by fibrous changes. In the other 6 patients the cause for the perfusion defect is not obvious. A history of myocarditis is missing. The presence of “small vessel disease” in those patients has however to be considered. Our results point to the relation between an abnormality of the microcirculation and arrhythmias in younger patients.

    Description / Table of Contents: Zusammenfassung Bei 19 Patienten mit obstruktivem Syndrom wurde mit Hilfe der Computerszintigraphie die Verteilung der Lungendurchblutung in den Lungenober- und Lungenunterfeldern bestimmt. Es zeigte sich, daß in signifikanter Korrelation mit zunehmendem Druck in der A. pulmonalis die Oberfclder vermehrt durchblutet werden. Eine Ausnahme bildeten Patienten mit ausgedehnten emphysematösen oder restriktiven Veränderungen. Die Computerszintigraphie bietet die Möglichkeit, durch ein den Patienten nicht belastendes Verfahren, wichtige Hinweise für das Vorliegen einer pulmonalen Hypertonie zu gewinnen.
    Notes: Summary The distribution of blood flow in the lungs was determined in 19 patients with chronic obstructive airways disease. It could be shown that the upper parts of the lungs tend to be more perfused with increasing pulmonary artery pressure. Exceptions are patients with massive emphysematous or restrictive alterations of lung parenchyma in the upper zones of the lungs. The computer scintigraphy provides with information concerning the presence or absence of pulmonary hypertension without great stress to the patient.

    Notes: Abstract Objective: To determine whether the quality of infiltrations in chest radiographs can accurately predict the histological extent of fibrotic change in patients with acute respiratory distress syndrome (ARDS). Design: Retrospective clinical investigation. Setting: Intensive care unit (ICU) of a university teaching hospital. Patients and methods: Of 47 patients treated with extracorporeal membrane oxygenation (ECMO) for severe ARDS over a 5-year period, 23 patients underwent open lung biopsy at thoracotomy for treatment, mostly of pneumothorax. Chest films obtained by portable chest roentgenography preceding the operation were reviewed retrospectively and compared to the histomorphological results of the lung specimen. Results: Chest radiographs displayed mixed alveolar-reticular opacification in 60.2 %, alveolar patterns in 22.9 % and reticular opacities in 10.5 %. In 0.4 % there were no infiltrates, 6 % could not be evaluated because of insufficient quality. There was no relevant difference between the right and left lungs. Subdividing patients into two groups according to the histological results of either absent or mild (1) or severe (2) lung fibrosis, we found an alveolar haziness in 12.3 % in group 1 compared with 28.2 % in group 2, while reticular characteristics were identified in 13 % and 11 %, respectively. Conclusions: The most common opacity in chest radiographs of patients with severe ARDS treated with ECMO is mixed alveolar-reticular opacification. Severe lung fibrosis is not positively correlated with a reticular radiographic pattern. ECMO does not lead to specific radiological changes in conventional radiograms, contrary to clinical findings that treatment with ECMO might induce pleural or pulmonic haemorrhage, especially in the earlier days when systemic heparinization had to be used instead of the heparin-coated tube-surfacing.
