Summary Clinical trials with monoclonal antibodies directed against TNFα (anti-TNF mAbs) and soluble TNF receptor fusion proteins (sTNFR-IgGs) have demonstrated that systemic and synovial trapping of TNFα results in long lasting anti-inflammatory and anti-nociceptive effects in patients with rheumatoid arthritis. Clinical indices of inflammatory synovitis and laboratory parameters (CRP and ESR) respond to single and repeated administrations of anit-TNFα therapies in a dose-dependent fashion. Studies on the immuno-pharmacological profile in patients suggest evidence that TNFα trapping down-regulates the effector mechanisms involved in the immuno-inflammatory response in rheumatoid arthritis. Inhibition of PLA2- and COX-2-derived pathways of mediators of inflammation (prostanoids and leukotrienes) decreases signs and symptoms of inflammatory synovitis such as joint swelling, tenderness and pain. Down-regulating of the cytokine-inducible adhesion molecules ICAM-1, VCAM-1 and ELAM-1 in endothelial cells and synoviocytes results in a marked inhibition of trans-endothelial migration of inflammatory and immune cells. A decrease of cytokine-regulated metalloproteinase expression results in normalization of circulating MMP-1 and MMP-3 levels. The effect of TNFα neutralization on mechanisms of rheumatoid joint destruction has the long-term potential for preventing or decreasing the rate of erosive changes of cartilage and bone.

Zusammenfassung In kontrollierten klinischen Studien zur Wirksamkeit und Verträglichkeit von verschiedenen monoklonalen anti-TNFα-Antikörpern und sTNFR-IgG-Fusionsproteinen bei rheumatoider Arthritis wurde nachgewiesen, daß die systemische und synoviale Blockierung der TNFα-Aktivität eine tiefgreifende und langhaltende Wirkung auf die Symptome der Erkrankung hat. Die parenterale Verabreichung (i.v. oder s.c.) von Einzelgaben und wiederholte Applikationen über mehrere Wochen/Monate haben einen signifikanten dosis-abhängigen Effekt auf klinische Wirksamkeitsparameter (Gelenk-Schwellung und -Druckempfindlichkeit) sowie auf ESR und CRP. Aus Untersuchungen zur Rolle von TNFα und anderer pro-inflammatorischer Zytokine bei Synovitis und Gelenkdestruktion sowie zum immunpharmakologischen Profil von anti-TNFα-Therapien läßt sich schließen, daß durch Blockierung/Neutralsation von TNFα wichtige an der Pathogenese beteiligte Effektormechanismen moduliert werden können. Die Hemmung der durch pro-inflammatorische Zytokine induzierten Bildung pro-inflammatorischer Mediatoren vom Typ der Prostanoide und Leukotriene führt zur Verringerung von Schmerz, Schwellung und Druckempfindlichkeit. Durch Modulierung der zytokin-abhängigen Aktivität der Adhäsionsmoleküle ICAM-1 und ELAM-1 in Endothelzellen und Synoviozyten wird die trans-endothelialen Präsentation von Entzündungszellen und Immunzellen blockiert. Der signifikante Effekt von anti-TNF-Therapien auf die zur Matrixdegradation führende Freisetzung von Metalloproteinasen hat das Potential, destruktive Gelenkveränderungen im Verlaufe einer Langzeitbehandlung zu verlangsamen oder zu verhindern.
