    Notes: Abstract Digitonin extracts from retinas of the marine fish Porichthys notatus, from California and Puget Sound (USA), contain a single photopigment with maximum absorbance at 498 nm. In the presence of hydroxylamine the final product of bleaching absorbs maximally at 367 nm, indicating that the photopigment is based on retinal. Spectral sensitivity measurements performed on intact eyes and eye-cup preparations in darkadapted and selectively light-adapted states yield curves which suggest that the retina of P. notatus contains at least 3 different types of receptors. In the darkadapted state, the spectral sensitivity curve has a broad peak, with a maximum between 480 and 500 nm. In the blue and red light-adapted states, in addition to depressing the sensitivity, the shape of the sensitivity curve also changes. Under these conditions new peaks appear which suggest the presence of at least two different types of cones, with sensitivity maxima in the blue and the green regions of the spectrum, respectively. Comparison of the spectral sensitivity curves with the in vivo bioluminescence emission spectrum of the fish gives an especially good correspondence and suggests that the fish is able to perceive the bioluminescence of its own species.

    Notes: Abstract The compound eye of the galatheid crab Pleuroncodes planipes has been studied by means of visual pigment extractions and spectral sensitivity measurements under various conditions of adaptation. A photosensitive pigment with maximum absorption at 503 nm was identified. Its chromophore is retinal. The spectral sensitivity maximum is near 523 nm in the dark-adapted eye. Selective adaptation experiments suggest that the apical region of the eye has a homogeneous receptor population. The spectral sensitivity characteristics of P. planipes seem to be an adaptation to its mainly benthic life.

    Notes: Abstract The visual system of the midwater fish Stenobrachius leucopsarus (Myctophidae) has been studied by biochemical, anatomical, and electrophysiological methods. Partial bleaching analysis in the presence of hydroxylamine showed that the eye contains a single extractable photopigment, based on retinal and absorbing maximally at 492 nm. The photoreceptor population consists entirely of rods, approximately 5.0×105 rods per mm2 and 1.8×107 per retina. Visual sensitivity is enhanced by the lack of pigment granules in the pigment epithelium and the presence of a choroidal tapetum lucidum. Influenced by the high concentration of visual pigment, the spectral sensitivity has a broad plateau between 460 and 540 nm, which suggests that the eye retains high sensitivity to a diversity of bioluminescent stimuli and depth-attenuated solar light.

    Notes: Summary The spectral sensitivities of the four eyes ofNereis mediator (Polychaeta, Annelida) were measured from 400 nm to 600 nm by determining the reciprocal of the number of quanta required to elicit a constant electroretinogram of 350 μV.N. mediator is maximally sensitive at 480 nm, although it is very sensitive over a broad range from 400 nm to 540 nm (Fig. 4). Selective adaptation experiments (Fig. 5) and changes in the time characteristics of the electroretinogram under conditions of dark and green adaptation (Fig. 2) indicate the presence of a multi-receptor system.

    Notes: Summary The present study is a comparison of the pharmacokinetics of four local anaesthetics injected double blind in the right subdiaphragmatic area during outpatient laparoscopy performed under standard general anaesthesia in 28 young women. 80 ml of one of the following solutions was injected: Group A 0.5% plain lidocaine (n=7), Group B 0.5% lidocaine with 1/320.000 adrenaline (n=8), Group C 0.5% lidocaine with 1/800.000 adrenaline (n=7), and Group D 0.125% bupivacaine with 1/800.000 adrenaline (n=6). Blood samples were collected over 360 min from an iv catheter and serum concentrations were measured by gas chromatography. No adverse effects occurred in the study period. In Group A (plain lidocaine), Cmax was significantly higher and tmax significantly earlier than in Groups B and C (lidocaine with adrenaline). A toxic level was not found after either solution in any patient. The intraperitoneal use of doses of 400 mg lidocaine or 100 mg bupivacaine for perioperative analgesia was safe and solutions of lidocaine containing adrenaline appeared to pose even less risk than plain solutions.

    Notes: Objective To investigate the factors influencing the choice of treatment for ectopic pregnancy and to compare the subsequent fertility rates of radical, conservative-surgical or medical treatments.Design Population-based study of 835 ectopic pregnancies registered between 1992 and 1996.Setting Auvergne Ectopic Pregnancy Register (France).Sample Four hundred and seventy-six women with tubal ectopic pregnancy who were not using contraception at the time of conception. Subsequent fertility was studied for the 291 women who attempted to conceive again.Methods Comparison of reproductive outcomes according to ectopic pregnancy treatment. Cumulative fertility curves were calculated by the Kaplan-Meier estimator and compared by log rank test for univariate analysis and by Cox regression to take into account confounding variables.Main outcome measure Recurrence and fertility rates after ectopic pregnancy.Results The first treatment given was ‘radical’ for 178 women (37%), ‘conservative-surgical’ for 262 (55%), and ‘medical’ for 35 (8%). The treatment failed in 1% for radical treatment, 5% for conservative-surgical treatment, and 36% for medical treatment. The two-year cumulative rate of recurrence was 27% with no significant difference between treatments. For women with previous infertility factors (in particular diseased contralateral tube), the treatments differed significantly, with the rate of intrauterine pregnancy lower for radical treatment and higher for medical treatment than for conservative-surgical treatment. For women with no infertility factor, there was no significant difference between treatments.Conclusions These results should be confirmed in a controlled trial. The results of this study provide the elements necessary to plan such a trial.

    Notes: Objective To evaluate pregnancy after heart or heart-lung transplantation in France.Design A questionnaire survey of all French centres performing heart and heart-lung transplants provided an exhaustive registry on pregnancy after transplantation.Methods A questionnaire was sent to 36 centres. Anonymous reports on transplantation and subsequent pregnancies between 1984 and 1996 were analysed.Results Of 1290 heart and 120 heart-lung transplantation performed during the study period, 10 pregnancies (seven after heart transplantation and three after heart-lung transplantation) were reported in nine women who were delivered of 11 infants. High blood pressure complicated nine pregnancies and severe pre-eclampsia occurred in two. One woman developed Kaposi sarcoma of the cervix. Delivery at 35 weeks gestation (27–39) was by caesarean section in 50% cases. Mean birthweight was 1990 g (700–2880g) and 50% of the infants had a birthweight 〈 tenth centile. One infant developed cardiomyopathy as a condition inherited from her mother, and congenital hepatitis B was diagnosed in another.Conclusions Pregnancy after heart and heart-lung transplantation is feasible and can be successful, but this should be planned when cardiac and respiratory functions are stable. Adequate obstetrical and genetic counselling should be provided.

    Notes: Objective To evaluate the efficacy and tolerance of mifepristone in women undergoing induction of labour at term after previous caesarean section.Design A prospective double blind placebo controlled trial.Subjects Thirty-two women at term (after 37.5 weeks' amenorrhea) who had had a previous caesarean delivery with a low transverse uterine incision. All women had a clear clinical indication for induction of labour with unfavourable cervical conditions (Bishop's score 〈 4). They were randomised to receive either 200 mg of mifepristone or placebo on days one and two of a four-day observation period.Results Thirteen women entered spontaneous labour: 11 were treated with mifepristone and two were in the control group (P 〈 0.01). Thirteen women, still with an unfavourable cervix on day four needed cervical ripening with vaginal tablets of prostaglandins. Of these, four had received mifepristone and nine the placebo. Mean oxytocin requirements were lower in the mifepristone group (P 〈 0.01) and the mean time interval between day one and start of labour was also significantly shorter in this group. Mode of delivery and neonatal outcome were similar in both groups.Conclusions Induction of labour is facilitated in term women with prior caesarean section by the use of mifepristone. This induction agent appears safe and useful with no adverse events on the fetus or mother.
