    Description / Table of Contents: Zusammenfassung In Untersuchungen zum Einfluß peroraler Dusodril®-Gabe auf den cerebralen Gehalt an energiereichen Substanzen bei der weißen Maus konnten wir zeigen, daß entgegen den Angaben von Meynaudet al. unter Dusodril® keine Erhöhung der ATP-Gewebsgehalte gegenüber Kontrolltieren auftritt. Auch der Gehalt an Phosphokreatin, der Summe der Adeninnukleotide oder das Energiepotential werden durch Dusodril®-Gabe nicht erhöht. Die von Meynaudet al. angegebenen Differenzen deuten jedoch auf mögliche Einflüsse des Dusodril® auf den Hirnstoffwechsel der Maus in Ischämie hin.
    Notes: Summary In contrast to findings of Meynaud et al. we were able to demonstrate that no change occurs in cerebral ATP contents of white mice following peroral administration of Dusodril®. Also the cerebral content of creatine phosphate, the sum of adenine nucleotides and the energy charge potential remained unchanged. The “normal” ATP values published by Meynaudet al. which were used in advertisements of the Lipha-Pharm to prove cerebral actions of Dusodril® represent cerebral ATP values altered by vague periods of ischemia. So Dusodril® possibly acts on cerebral energy metabolism in ischemia but exerts no effect on normal cerebral energy values in the mouse.

    Description / Table of Contents: Zusammenfassung Es wurden die Mechanismen der Reninfreisetzung nach Furosemid und Ethacrynsäure beim Menschen untersucht. Geprüft wurde die Hypothese, ob akute Volumenverschiebungen innerhalb des Niederdrucksystems nach Furosemid zu einer neural vermittelten Reninstimulation führen. Sowohl Immersion in einem Wasserbad als auch Betablokkade reduzierte den Reninanstieg nach Furosemid nicht jedoch nach Ethacrynsäure. Es wird geschlossen, daß die akute Reninfreisetzung nach Furosemid, nicht jedoch nach Ethacrynsäure teilweise auf neuralen Reflexmechanismen — ausgehend von Volumenrezeptoren des Niederdrucksystems — beruht.
    Notes: Summary The mechanisms of renin release after furosemide (F) and ethacrynic acid (EA) in man were examined. We evaluated whether acute volume shifts within the low pressure system after F induce renin release via neural pathways. Immersion in a water-bath or beta-blockade reduced the increase of plasma renin concentration after F but not after EA. It is concluded that acute renin release after F but not after EA in man is partially due to neurally mediated reflexes originating from volume receptors in the cardiopulmonary area.

    Notes: Summary In 35 present women, undergoing tocolytic treatment with betamimetics had intensive monitoring by balloon-tipped catheter. After the administration of 2–4 μg/min of fenoterol the cardiac output (CO) increased from 8.2 l/min to over 11 l/min with a corresponding increase in mean pulmonary artery pressure (MPAP) from 11.7 mmHg to over 18 mmHg. The correlation coefficient between MPAP/CO was highly significant. In the 10 patients in whom we were able to monitor pulmonary capillary pressure (PCP), we observed no significant increase to suggest compromised cardiac performance. The calculated total resistance of the pulmonary vessels (TPR) decreased during the first 24h followed by a return to initial values. Our investigations suggest that the principal cardiovascular effect of betamime-tics in pregnant women is a volume-dependent increase in pressure in the pulmonary circulation. The simultaneous administration of betamethasone had no additional effects.

    Notes: Summary In search for new techniques of renal preservation, kidneys were persufflatedin situ with gaseous O2 at a pressure of 90–100 mm Hg via their vascular system (orthograde aerobic ischemia). The results on post-ischemic function and blood flow, and on the changes in the renal tissue levels of the adenine nucleotides were compared to those obtained from kidneys ischemically stored without additional measures (anaerobic ischemia). The kidneys were stored for 4 hrs at 6°C. A contralateral nephrectomy was performed before the preservation period. All dogs subjected to renal anaerobic ischemia survived. After orthograde aerobic ischemia, 3 out of 4 dogs died from uremia within 8 days, 1 animal survived. Although the aerobic ischemic preservation resulted in the maintenance of a metabolic status characteristic for aerobiosis, post-ischemic renal function recovered too slowly and insufficiently on the average. Renal blood flow was within normal ranges after anaerobic ischemia, it was reduced to about one third of this after orthograde aerobic ischemia.

    Notes: Summary Retrograde persufflation of ischemic kidneys of dogs via the renal vein with gaseous O2 was evaluated as a method of kidney preservation (retrograde aerobic ischemia) and compared to conventional ischemic storage without additional measures (anaerobic ischemia). Renal preservationin situ lasted 4 hrs at 6°C. A contralateral nephrectomy was performed before the preservation period. For retrograde aerobic ischemia the persufflation pressure was 30 mm Hg, the gas flowed off via opened capsular vessels. While dogs which had undergone renal anaerobic ischemia showed a marked temporary post-ischemic increase in renal blood flow and in the systemic blood pressure, dogs submitted to renal retrograde aerobic ischemia did not exhibit a storage induced loss of the renal vascular autoregulation. During and after retrograde aerobic ischemia the renal tissue levels of ATP, ADP, and AMP remained within normal ranges; ATP and the sum of adenine nucleotides decreased markedly during anaerobic ischemia. In both the groups a normal renal function was resumed again; but the recovery time of various parameters of renal function (clearance from inulin and PAH, urea excretion, serum concentrations of urea and creatinine) was shorter after retrograde aerobic ischemia than after anaerobic ischemia. Kidneys subjected to retrograde aerobic ischemia exhibited a better functional adaption to the contralateral nephrectomy during the 3 weeks of post-ischemic observation than kidneys stored in anaerobic ischemia.

    Notes: Summary The place of captopril (C) testing in the screening for renovascular hypertension is still controversial. Baseline and C-stimulated plasma renin concentrations (PRC) were measured in 113 hypertensives, who where referred for the exclusion of secondary hypertension. In addition intravenous digital subtraction angiography (DSA) and a renal scintigraphy were performed. When renal artery disease was revealed by DSA or renin was stimulated the renal arteries were visualized by direct arteriography (and treated by angioplasty if possible). 86 patients underwent each diagnostic test: 21% had renovascular hypertension. Unilateral renal artery stenosis (n=10) was detected by the captopril test (cutoff values: baseline〉40 µU/ml, after C〉180 µU/ml, sensitivity 100%). Bilateral renal artery stenosis (n=8) was missed when the disease was equally severe on either side (sensitivity 50%). The specificity of C testing was 82%, overall sensitivity (uni- and bilateral disease 78%, prevalence 21%, predictive value of the positive test 0.56, predictive value of the negative test 0.93). With i.v.-DSA the renal arteries were technically evaluable in 91% (82/92) of cases. The sensitivity for the detection of all renal artery stenoses was 79% (unilateral 100%, bilateral 40%, specificity 97%). The sensitivity of renal scintigraphy for the detection of unilateral renal artery stenosis was 50%, for the detection of bilateral renal artery stenoses 43%, specificity 81%. The present study demonstrates the usefulness of captopril for the detection of unilateral renal artery stenosis. Since bilateral renal artery disease is frequent and frequently missed by the test, overall and split renal function should be evaluated in addition. Both tests can easily be combined at one time when a captopril scintigraphy with renin determination is performed.
