Summary 1. Erythrocyte survival was studied with radiochromium in 27 consecutive patients with liver disease: 17 subjects with acute viral hepatitis, 5 cases with acute exacerbations of chronic hepatitis and, for comparison, 5 subjects with cirrhosis of the liver. Results of red cell survival studies were compared with the level of hemoglobin, reticulocyte counts and the course of the ailment. 2. Increased hemolysis of mild to moderate degree was demonstrated in 9 out of 17 patients with acute viral hepatitis. In each instance, however, the hemolytic process was compensated. Minimal reticulocytosis was commonly found. Only one patient had a markedly enhanced red cell destruction rate (T 1/2 of 15 days). The hemolytic episodes in acute hepatitis are of brief duration and remain usually unrecognized. Their pathogenesis is discussed. 3. In 4 out of 5 patients with acute exacerbations of chronic hepatitis and in all 5 patients with cirrhosis a moderate to severe degree of hemolysis was present. Splenomegaly was commonly found in these cases and surface counts over the splenic area revealed increased Cr51 radioactivity due to splenic hypersequestration of erythrocytes. 4. The present study supports the concept that manifest hemolytic anemia may occur in progressive chronic hepatitis as well as in cirrhosis, while acute viral hepatitis is frequently associated with a hemolytic episode of short duration that remains clinically unrecognized.

Zusammenfassung 1. Die Erythrocyten-Überlebensdauer von 27 nicht ausgewählten Patienten mit Leberkrankheiten wurde mit der Radiochrom-Methode untersucht: 17 Fälle mit akuter Hepatitis, 5 Fälle mit akuten Schüben einer vorbestehenden chronischen Hepatitis und, zum Vergleich, 5 Fälle mit Cirrhose. Die Ergebnisse wurden mit dem klinischen Verlauf und den Hämoglobin- und Reticulocyten-Werten verglichen. 2. Bei 9 der 17 Fälle von akuter Hepatitis war ein leichter bis mittelschwerer Grad von Hämolyse nachweisbar; nur in einem Fall war die Hämolyse beträchtlich (T 1/2 von 15 Tagen). Bei allen Patienten war der hämolytische Prozeß kompensiert. Meistens bestand eine leichte Reticulocytose. Die hämolytische Episode bei akuter Hepatitis ist von kurzer Dauer und verläuft klinisch fast immer inapperzept. Die unklare Pathogenese wird diskutiert. 3. Bei 4 der 5 Fälle von akutem Schub einer chronischen Hepatitis und bei allen 5 Cirrhosen lag eine mittelschwere bis schwere Hämolyse vor. Bei diesen Fällen bestand eine Splenomegalie und die Cr51-Oberflächenmessungen ergaben zum Teil erhöhte Radioaktivität über der Milz als Ausdruck der splenogenen Hypersequestrierung der Erythrocyten. 4. Aus dieser Untersuchung geht hervor, daß bei floriden chronischen Hepatitisformen und progressiven Cirrhosen eine manifeste hämolytische Anämie und bei den meisten Fällen von akuter Hepatitis ein nur kurzdauerndes, klinisch nicht manifestes hämolytisches Syndrom vorliegt.
