    Description / Table of Contents: Zusammenfassung Bei 19 Patienten mit primärer Hyperurikämie, 6 mit sekundärer Hyperurikämie, 17 mit primärer Gicht und 30 Kontrollpersonen wird die Aufnahme von 2-C14 Harnsäure in die Erythrocyten gemessen. Es zeigt sich, daß die Erythrocyten von Gichtpatienten signifikant weniger Harnsäure aufnehmen als die von Kontroilpersonen. Dagegen unterscheiden sich die primären und sekundären Hyperurikämiepatienten nicht signifikant von den Kontrollen. Der Unterschied in der Aufnahme von Harnsäure bei Erythrocyten von Gichtpatienten und Kontrollen betrifft hauptsächlich den Anfangsteil der Aufnahmekinetik. Es wird die mögliche pathophysiologische Bedeutung der verminderten Harnsäureaufnahme für das Entstehen der Harnsäurepräcipitationen bei Gicht diskutiert und auf die Möglichkeit hingewiesen, den Harnsäure-Tracer-Aufnahmetest als diagnostisches Hilfsmittel für die Früherkennung der Gicht anzuwenden.
    Notes: Summary The uptake of 2-C14 urate by erythrocytes was measured in 19 patients with primary hyperuricemia, 6 patients with secondary hyperuricemia, 17 patients with primary gout and 30 controls. The uptake of urate in patients with primary gout was significantly lower than in the controls. In contrast no such difference could be observed in patients with primary and secondary hyperuricemia. The uptake of labeled urate by erythrocytes from gouty patients is especially diminished in the early phase of the uptake kinetics. The possible relevance of this finding for the pathogenesis of urate precipitation in gout is discussed. Further, we consider the application of the tracer urate uptake by erythrocytes as an aid in the early diagnosis of gout.

    Description / Table of Contents: Summary Ten patients with rheumatic diseases were given 50 mg prednisolone orally and intramuscularely. In order to test the different conditions of resorption, the eosinophils were counted and the plasma-17-hydrocorticoids were determined. After oral application we found a significant fall of eosinophils, reaching a maximum after 8 hours. The maximal rise of 17-HOCC was seen after 3 hours. Prednisolone-acetate given intramuscularely does not act on the eosinophile-cell-count and has only little influence on the 17-HOCC-level. 8 hours after both kinds of application the 17-HOCC-level appearently had reached the base-line again. The clinical value of both ways of application was discussed. Because of the delayed action of cristalline prednisolone-acetate, these results are not comparable with results found with water soluble preparations of cortisones.
    Notes: Zusammenfassung Zehn männliche Rheumakranke erhielten unter den Bedingungen eines AB-Versuches einmal 50 mg Prednisolon oral und einmal intramuskulär. Um die vermuteten unterschiedlichen Resorptionsbedingungen zu prüfen, wurde das Verhalten der eosinophilen Leukocyten und der 17-Hydroxycorticoide im Plasma verfolgt. Die orale Medikation führt zu einem signifikanten Eosinophilensturz, der nach 8 Std sein Maximum erreicht, und zu einem 17-Hydroxycorticoid-Anstieg mit einem Maximum nach 3 Std. Die intramuskuläre Verabreichung beeinflußt die Eosinophilen nicht und steigert die 17-HOCC nach 3 Std gering. Die HOCC-Spiegel liegen bei beiden Applikationsarten nach 8 Std wieder in Höhe des Ausgangswertes. Die Ursachen des verschiedenen Verhaltens werden diskutiert, Vor- und Nachteile für die klinische Anwendung besprochen. Da es sich bei dem intramuskulär verabreichten Prednisolonacetat um eine mikrokristalline Suspension mit langsamer Resorption handelt, lassen sich die Untersuchungsergebnisse nicht mit denen nach Verabreichung wasserlöslicher Prednisolonester vergleichen, die rascher resorbiert werden und klinisch kürzer wirken.

    Notes: Abstract Modern microsystem application is concentrated on the development of microstructured sensors and actuators in integrated devices. Examples of that approach are microoptical components for endoscopic surgery, sensors and micropumps in integrated liquid analysers for pollution measurements. Often microsystems replace normal systems like commercially available piston pumps by silicon made liquid pumps in the hope of reducing costs but there is a lack of finding new applications. In contrast our approach of combining special developed microcompartments constructed with transparent membranes or microsieves and microdosing systems based on the ink jet principle together with optical devices like CCD cameras allow to overcome a real bottleneck in the new field of evolutionary biotechnology, the problem of screening a huge number of samples at a reasonable price. The goal of this approach is to design new molecules like enzymes by means of Darwinian evolution i.e. mutation and selection. In applying the selection principle to self amplifying entities like cell populations, viruses or self-replicating molecules under controlled selection pressures there is an absolute need to process large numbers of these entities in parallel. Preferentially this can be done in arrays of different microstructured compartments. The selection process is designed that it leads to products which are optimized in regard of specific applications. Within the scope of this approach chemical products are deterministically synthesized in spatially adressable compartments by multihead microdrop systems.

    Notes: Abstract This paper is focused on the development of plastic nano titer plates for applications in high throughput screening (HTS). For screening systems with integrated confocal microscopes plastic chips have been fabricated by injection molding and injection compression molding which contain micro wells with volumes of 0.9 μl and 1.4 μl and bottom plates with thicknesses of 120 μm and 200 μm. In addition, plastic chips with through holes have been joined with 160 μm glass plates by an adhesive printing process. First fluorescence correlation spectroscopy (FCS) measurements show that the plastic plates with glass bottoms are qualified as screening grade FCS nano titer plates.

    Notes: Abstract. Since the introduction of open magnets and short-bore closed magnets, and the availability of fast imaging sequences, MR imaging has become a tool for guidance and control of percutaneous nonvascular and vascular procedures. In general, percutaneous biopsies or drainages require no specific hardware or software modifications. For lesion localization and control of the needle track, simple fast imaging sequences such as fast T1-weighted gradient-echo techniques or fast single-shot T2-weighted spin-echo sequences are sufficient and can be applied depending on the best tissue-to-lesion contrast. Typical scan times range from 1 to 3 s. Different types of biopsy needles are commercially available, allowing sampling of sufficient amounts of tissue. For drainage procedures non-ferromagnetic materials, such as nitinol wires, should be preferred to minimize image distortion by metallic artifacts. Indications for percutaneous biopsies include subdiaphragmatic liver lesions, lesions poorly visible on ultrasound or contrast-enhanced computed tomography, and lesions of the bone marrow characterized by an unspecific bone marrow edema. For percutaneous drainages, subdiaphragmatic lesions appear to be a good indication. With some experience the procedure time is not longer than that under CT or US guidance.

    Notes: Abstract A new method of detecting vesicoureteric reflux (VUR) is presented, utilizing ultrasound as the imaging modality. During the filling phase of a gas cystogram and during micturition, gas bubbles can be seen in the renal pelvis in cases of reflux. In 107 pediatric patients, sonographic reflux studies were performed and compared with radiographic voiding cystourethrograms. The accuracy of the sonographic reflux study was 93.4%, the sensitivity of grade III– IV was 100%, and the specificity was 95.6%. Hence sonographic reflux studies with air have been incorporated into our routine. Since that time, the number of voiding cystourethrograms has decreased by over 80%.

    Notes: Abstract A newborn infant exhibiting seizures and spastic tetraparesis at the age of 1 week was shown to excrete excessive quantities of sulphite, taurine, S-sulphocysteine and thiosulphate, characteristic of sulphite oxidase deficiency. In addition, increased renal excretion of xanthine and hypoxanthine combined with a low serum and urinary uric acid was consistent with xanthine dehydrogenase deficiency. Both deficiencies could be established at the enzyme level. The primary defect giving rise to the combined abnormalities is the absence of a molybdenum cofactor, a molybdenum-containing pterin being an essential component of both enzymes. The patient developed a severe neurological syndrome, brain atrophy and lens dislocation and died at the age of 22 months. Attempts at treatment, such as oral administration of ammonium molybdate, sodium sulphate, d-penicillamine, 2-mercaptoethane sulphonic acid, pyridoxine and thiamine did not influence the clinical course.

    Notes: Abstract The mathematical expression of the signal to noise ratio in fluorescence fluctuation experiments is derived for arbitrary sample profiles and for any mechanism of translational motion, and experimentally verified. The signal to noise ratio depends on the mean count rate per particle per dwell time, the mean number of particles per sample volume, time characteristics of the correlation function, sample profile characteristics, and the data collection time. Statistical accuracy of the third order moment of fluorescence intensity fluctuations is also studied. The optimum concentration for the third order moment analysis is about one particle per sample volume.
