    Notes: Postsurgical infection risk is correlated with subcutaneous tissue oxygenation. Mild hypercapnia augments cutaneous perfusion. We tested the hypothesis that peripheral tissue oxygenation increases as a function of arterial PCO2 in surgical patients. Twenty patients were randomly assigned to intra-operative end tidal PCO2 of 3.99 (control) or 5.99 kPa (hypercapnia). All other anaesthetic management was per protocol. Tissue oxygen partial pressure, transcutaneous oxygen tension, cerebral oxygen saturation, and cardiac output were measured. Mean (SD) subcutaneous tissue oxygen tension was 8.39 (1.86) kPa in control and 11.84 (2.53) kPa hypercapnia patients (p = 0.014). Cerebral oxygen saturation was 55 (4)% for control vs. 68 (9)% for hypercapnia (p = 0.004). Neither cardiac index nor transcutaneous tissue oxygen tension differed significantly between the groups. Mild intra-operative hypercapnia increased subcutaneous and cerebral oxygenation. Increases in subcutaneous tissue oxygen partial pressure similar to those observed in patients assigned to hypercapnia are associated with substantial reductions in wound infection risk.

    Description / Table of Contents: Zusammenfassung Aus der Magenvorderwand großcurvaturseits im Bereich der Längsmuskelzüge präpariert man ein (10 cm × 3 mm) Muskelläppchen extramucös, ohne das Magenlumen zu eröffnen. Das Läppchen wird kardianahe gestielt, nach cranial umgeschlagen und unter manometrischer Kontrolle doppelt um die Kardia herumgeführt. Die Fixation des Läppchen erfolgt an der Magenhinterwand. So entsteht eine omegaförmige Zwinge, deren Zügel mit zunehmender Magenfüllung angezogen werden. Nachuntersuchungen bis zu 12 Monaten p.o. beweisen die Integrität und Funktionsfähigkeit der Omegaplastik.
    Notes: Summary In 25 mongrel dogs a 10 cm × 3 mm wide longitudinal muscle flap was freed extramucosally from the anterior surface of the greater gastric curvature without entering the gastric lumen. The flap pedicle was located near the cardia. The flap was retracted cranially and wrapped twice around the cardia under manometric control. It was fixed on the dorsal gastric surface. In this manner an omega-shaped sling was formed. This sling is pulled tight with increasing gastric tension. Follow-up examinations up to 12 months post-operatively proved that the omega-plasty was functioning correctly.

    Description / Table of Contents: Zusammenfassung Durch speziell gearbeitete Polyglycolsäuredübel (Braun, Melsungen) gelingt es tierexperimentell, frei transplantierte Bänder zu fixieren, abgescherte osteochondrale oder chondrale Fragmente zu befestigen und ausgesprengte, nicht tragende Knochenlamellen zu readaptieren. Die Implantate werden ohne entzündliche Reaktion angenommen, sind 4 Wochen stabil und werden innerhalb von 5 Monaten resorbiert. Indikationen könnten sich in der rekonstruktiven Unfall- und der Handchirurgie ergeben.
    Notes: Summary In cooperation with Dr. Braun (Melsungen) special dowels made of polyglycolic acid were developed. All operations were performed in the hind limbs of rabbits (n = 12) and dogs (n = 12). In 24 extremities the medial collateral ligament of the knee was removed and replaced by a tendon. In 12 extremities the cartilage or osteochondral fragment was shorn off and reattached by dowels. Follow-up studies showed good healing. No inflammation or foreign-body reaction was seen. These results lead us to assume that resorbable material can be applied in reconstructive surgery of the hand, joints, and bones.
