    Description / Table of Contents: Summary On the basis of personal studies this report aims at directing the attention to the method and clinical value of determining the life-span of the thrombocytes and of determinations of the surface activity. Whereas determination of the thrombocyte life-span with Cr51-labelled platelets provides clinically reliable and reproducable results and is of utmost importance for the diagnosis of thrombocytopenias, surface activity determinations to determine the indication for splenectomy so far can only be considered in selected patients with chronic Werlhof's disease. With regard to thrombocytopenia associated with hypersplenism it has been possible to determine a new pathogenetic relationship by studies with Cr51-blood platelets.
    Notes: Zusammenfassung Mit dieser Arbeit sollte auf Grund eigener Untersuchungen die Aufmerksamkeit auf Methodik und klinische Wertigkeit der Thrombozytenlebensdauerbestimmung und der Oberflächenaktivitätsmessungen gelenkt werden. Während die Thrombozytenlebensdauerbestimmung mit51Cr-markierten Plättchen klinisch verläßlich und reproduzierbare Resultate liefert und für die Diagnostik von Thrombopenien größte Bedeutung hat, kommen Oberflächenaktivitätsmessungen zwecks Indikation zur Splenektomie vorerst nur bei ausgewählten Patienten mit chronischem M. Werlhof in Frage. Für die Thrombopenie bei Hypersplenismus hat sich durch Untersuchungen mit51Cr-markierten Blutplättchen ein neuartiger pathogenetischer Zusammenhang auffinden lassen.

    Description / Table of Contents: Summary In approximately 1/3 of all patients with vitiligo it was possible, with the aid of the Schilling test, to demonstrate a vitamin B12 absorption disorder which was produced by an Intrinsic-Factor-deficiency. The coincidence of vitiligo with pernicious anemia, the relationship of the B12-absorption disorder to the duration and severity of vitiligo, also permits, taking into consideration the melanin-antibodies which were first demonstrated in 1965 and the assumed immunological etiology of pernicious anemia, the assumption for an auto-immuno-genesis for vitiligo. The author discusses an immunological relationship with still unknown individual reactions between vitiligo, pernicious anemia and certain thyroid diseases.
    Notes: Zusammenfassung Bei etwa 1/3 aller Patienten mit Vitiligo konnte mit Hilfe des Schilling-Testes eine auf einen Intrinsic-Faktor-Mangel beruhende Vitamin-B12-Resorptionsstörung nachgewiesen werden. Die Koinzidenz der Vitiligo mit der Perniciosa, die Abhängigkeit der B12-Resorptionsstörung von Dauer und Schweregrad der Vitiligo läßt unter Berücksichtigung der 1965 erstmals nachgewiesenen Malanin-Antikörper und der vermuteten Immun-Genese der Perniciosa auch für die Vitiligo eine Auto-immun-Genese vermuten. Eine in ihren Einzelreaktionen noch unbekannte, immunologische Beziehung zwischen Vitiligo, Perniciosa und bestimmten Schilddrüsenerkrankungen wird diskutiert.

    Description / Table of Contents: Summary 20 patients with malignant diseases were given L-asparaginase (ASNase) intravenously and the resulting blood levels were determined. Two ASNases were used: ASNase A from E. coli and ASNase X, a chemically modified ASNase A. Both of them remained, in comparison with other therapeutics, very long in the blood. Following the first injection, the mean half-life time of elimination from the blood-plasma was 11 hours after application of A, and 24 hours after application of X, respectively. By repeated daily injections, treatment with X resulted in a rise of blood levels until the seventh day, whereas during treatment with A no accumulation of blood levels could be observed.
    Notes: Zusammenfassung Bei 20 Patienten mit malignen Erkrankungen wurden die Blutspiegelverläufe nach i.v. Injektion von L-Asparaginase (ASNase) untersucht. Zur Anwendung kamen die ASNase A aus E. coli und die ASNase X, eine chemisch modifizierte ASNase A. Beide ASNasen wiesen im Vergleich zu der Mehrzahl anderer Medikamente eine relativ hohe Verweildauer im Blutplasma auf. Nach der ersten Injektion betrug die Halbwertszeit der Elimination aus dem Blutplasma im Mittel für ASNase A etwa 11 Std. Bei wiederholter täglicher Behandlung stieg der Blutspiegel nach Gabe von X bis zum 7. Tage kontinuierlich an, während bei Behandlung mit A keine meßbare Blutspiegel-Kumulation eintrat.

    Description / Table of Contents: Zusammenfassung Untersuchungen mit51Cr-markierten Blutplättchen haben ergeben, daß die Thrombopenie bei hämatologischem Hyperspleniesyndrom (Osteomyelofibrose, Lymphoretikulose und Felty-Syndrom) durch eine vermehrte Speicherung von Blutplättchen in der vergrößerten Milz hervorgerufen wird und nicht durch die früher postulierte „splenomegale Markhemmung“. Dafür konnten folgende Beweise erbracht werden: 1. Eine praktisch normale Thrombocytenlebensdauer. 2. ein gegenüber Normalpersonen und gegenüber Splenektomierten deutlich erniedrigter Recovery-Wert 3. ein isolierter Radioaktivitätsanstieg über der Milz 4. ein positiver Adrenalinstimulationstest. Als neuartiger Befund konnte bei je einem Patienten mit Lymphoretikulose und Osteomyelofibrose zusätzlich gezeigt werden, daß die pathogenetischen Faktoren für eine Thrombopenieentstehung (primäre Bildungsstörung, beschleunigter Abbau, Kombination von primärer Bildungsstörung und beschleunigtem Abbau, vermehrte Speicherung) ergänzt werden müssen durch die Kombination von beschleunigtem Abbau und vermehrter Speicherung.
    Notes: Summary Labeling of platelets with51Cr has shown that in the presence of „hypersplenic“ thrombocytopenia (osteomyelofibrosis, lymphoreticulosis and Felty-syndrome) the thrombocytopenia is causen by pooling of platelets in the enlarged spleen and not by “splenomegalic bone-marrow depression”. This pathogenetic pattern could be demonstrated by the following results: 1. nearly normal survival of platelets 2. decreased recovery values in case of “hypersplenic” thrombocytopenia as compared with normal or increased recovery values in case of normal volunteers or in case of patients with splenectomy. 3. an isolated increase of radioactivity over the spleen. 4. a positive adrenalin stimulation test. A hitherto unknown pathogenesis of thrombocytopenia was demonstrated in one patient with lymphoreticulosis and in one patient with osteomyelofibrosis. In these patients, thrombocytopenia was caused by the combination of increased destruction and pooling of platelets in the enlarged spleen.

    Description / Table of Contents: Summary Red blood cell survival was determined in 35 patients with aortic and mitral valvular disease and in 2 patients with isthmus stenosis. In 17 of these patients red blood cell survival was reduced. Distribution and frequence of haemolysis was in favour of aortic and mitral stenosis whereas in case of aortic insufficiency only 3 out of 14 patients had a shortened red cell survival time. Haemolysis was always compensated. A mechanism of mechanical haemolysis due to the degree of the transvalvular pressure gradient and due to increased intracardiac turbulence is suggested as a cause for the shortened red cell survival time. From preliminary studies it is suggested that these factors induce cavitation with release of high kinetic energy on red cells.
    Notes: Zusammenfassung Hämatologisch-hämodynamische Untersuchungen bei 37 Patienten mit unterschiedlichen Herzklappenfehlern haben ergeben, daß bei 17 dieser Patienten (45%) eine Verkürzung der Erythrocytenlebensdauer vorlag. Die Verteilung der Hämolysehäufigkeit der einzelnen Herzklappenerkrankungen ergab eine Bevorzugung der reinen Klappenstenosen (9 von 12 Patienten), während bei Aortenklappeninsuffizienz lediglich bei 3 von 14 Patienten eine Hämolyse nachgewiesen werden konnte. Die Hämolyse war in jedem Falle kompensiert und rein mechanisch bedingt, wobei als Ursache das Ausmaß des transvalvulären Druckgradienten sowie Art und Ausmaß der Strömungsturbulenz diskutiert werden. Die Möglichkeit der durch diese Faktoren ausgelösten Kavitation mit konsekutiver Freisetzung erheblicher kinetischer Energie auf die Erythrocyten wird diskutiert.
