    Description / Table of Contents: Abstract Oxygen radicals and oxygen radial mediators are thought to be important components in the development of acute lung injury, sepsis, and multiple organ failure. Injured patients, patients with pulmonary diseases, and multiple trauma patients also showed an elevated lipid peroxidation, indicating increased oxidant stress. N-Acetylcysteine (NAC) has been used as an antioxidant in a wide variety of experiments. NAC has been suggested to act by raising concentrations of cysteine, and hence glutathione, and by scavenging of oxidant species [1, 11, 17, 29]. The present study was designed to investigate whether the application of NAC in intubated patients has an effect on concentrations of reduced glutathione in plasma and bronchoalveolar lavage fluid (BAL) and on the lipid peroxidation products malondialdehyde and conjugated dienes. Because NAC has been widely used as a mucolytic drug for the treatment of lung diseases, the influence on tracheobronchial mucus was studied, too. Methods. In a randomized, double-blind, placebo-controlled study, a total of 38 long-term ventilated patients of a surgical intensive care unit were investigated. Patients were treated for 5 days with either 3 g NAC/day or placebo. The plasma concentration of reduced glutathione, malondialdehyde, and conjugated dienes were measured on admission and on the 3rd and 5th days of treatment [8, 34, 48]. Additionally, the numbers of tracheobronchial suctionings were registered and chest radiographs were evaluated. A fibre-bronchoscopy was performed on admission and on the 3rd day of treatment. The amount and viscidity of tracheobronchial secretions were examined semiquantitatively, and glutathione levels were measured in the unconcentrated BAL. The study was approved by the ethics committee of the University of Ulm. Results. The two groups were comparable with respect to age, sex, APACHE II score and diagnosis (Table 1). We found no significant differences in reduced glutathione levels in the plasma or in the BAL (Figs. 1, 2). Plasma concentrations of malondialdehyde were similar (Fig. 3). Only the levels of conjugated dienes were significantly higher on the 5th treatment day in the placebo group (Fig. 4). The organ function of the lung (FiO2, PEEP, PaO2), liver (SGOT, bilirubin), and kidney (creatinine) and coagulation parameters (PTT, prothrombin time, platelet count) were similar in the two groups during the time of investigation. We observed no clinically relevant differences in the tracheobronchial mucus (Table 2). Conclusion. The present data do not support routine use of NAC in ventilated patients, either as an antioxidant or as a mucolytic agent. Intravenous administration of 3 g NAC/day had no clinically relevant effect on glutathione levels, lipid peroxidation products, tracheobronchial mucus, and clinical condition.
    Notes: Zusammenfassung In einer prospektiven, randomisierten, plazebokontrollierten Doppelblindstudie wurde an insgesamt 38 langzeitbeatmeten Patienten einer operativen Intensivstation überprüft, ob eine routinemäßige intravenöse Applikation von N-Acetylcystein (NAC) sinnvoll ist. Hierzu erhielten 20 Patienten 3 g NAC/Tag kontinuierlich über eine Spritzenpumpe für fünf Tage, 18 Patienten erhielten Plazebo. Bei allen Patienten wurde am 1. Beatmungstag (Ausgangswerte) und am 3. und 5. Behandlungstag die Konzentration von Glutathion, Malondialdehyd und konjugierten Dienen im Plasma bestimmt. An diesen Untersuchungspunkten wurde weiterhin die Zahl der durchgeführten tracheobronchialen Absaugungen registriert und das Röntgenbild des Thorax beurteilt. Alle Patienten wurden am 1. und 3. Beatmungstag bronchoskopiert, wobei die Menge und Zähigkeit des Tracheobronchialsekrets semiquantitativ beurteilt wurde. Zusätzlich wurde eine bronchoalveoläre Lavage (BAL) durchgeführt und die Glutathionkonzentration in der BAL-Flüssigkeit gemessen. Die beiden Gruppen unterschieden sich weder hinsichtlich der Glutathionkonzentration im Plasma noch in der BAL. Auch die Malondialdehydkonzentrationen waren in beiden Gruppen gleich. Die konjugierten Diene waren am 5. Tag in der Plazebogruppe signifikant höher. Hinsichtlich der Funktion von Lunge, Leber, Niere und Gerinnung gab es im zeitlichen Verlauf zwischen den beiden Gruppen keine signifikanten Unterschiede. Klinisch relevante Veränderungen im tracheobronchialen Sekret traten ebenfalls nicht auf. Aufgrund der vorliegenden Ergebnisse ist eine routinemäßige Applikation von N-Acetylcystein bei langzeitbeatmeten Patienten nicht indiziert.

    Description / Table of Contents: Abstract A high dose of local anaesthetic is necessary for the combined “3-in-1”/sciatic nerve block. Prilocaine is recommended for its low toxicity. However, in some patients prilocaine results in pronounced methaemoglobin formation due to toludine. Little has been known hitherto about the use of high-dose mepivacaine for the combined 3-1/sciatic nerve block. This study was undertaken to compare the use of 700 mg mepivacaine 1% and of 700 mg prilocaine 1%. Methods. The study was approved by the ethics committee of our hospital. Once their informed consent had been obtained in writing 3×20 patients (ASA 1–2) undergoing planned surgery on the foot or ankle joint were enrolled in the study. The patients were randomized to the following three groups on a double-blind basis: group 1,700 mg mepivacaine without epinephrine; group 2,700 mg mepivacaine with 0.2 mg epinephrine (1:350000); group 3,700 mg prilocaine 1%. Arterial blood samples for determination of local anaesthetic serum levels were collected over a 120-min period. We determined methaemoglobin and oxygen saturation before and 120 min after the blockade and continued these measurements for 6 h in group 3. At 15-min intervals, all patients were questioned about early signs of toxicity. The perioperative monitoring including blood pressure, ECG and pulse oximetry. Data were analysed using ANOVA and Student's t-test, P〈0.05 considered statistically significant. Results. The blocking efficacy did not differ among the groups (groups 1, 2, 3: 90%, 95%, 90%). The maximum mepivacaine serum level in group 1 was 3.91 μg/ml ±0.95 and 2.94 μg/,ml ±0.58 in group 2 (Fig. 2). Over the entire observation period the addition of epinephrine resulted in a significant reduction of the serum level (between 60.3% at t=15 min and 19.7% at t=120 min). In the prilocaine group the maximum serum level was 2.07 μg/ml ±0.56, significantly less than in either mepivacaine group. No patient showed signs or symptoms of local anaesthetic toxicity. In the prilocaine group there was wide variation in methaemoglobin formation among the patient, with a median of 10.1% (Fig. 3, Table 3). Three patients showed a maximum methaemoglobinemia between 16% and 17%. Five patients were still cyanotic after 6 h when they were transferred to the ward. The fractional SaO2 values amounted to 88% (median) with a minimum of 80.3%. Conclusion. Both mepivacaine 1% and prilocaine 1% are appropriate local anaesthetics for the combined 3-in-1/sciatic nerve block at a dose of 700 mg. There was no difference in the blocking efficacy. No patient showed clinical signs or symptoms of a local anaesthetic toxicity. Following prilocaine we are sometimes faced with high methaemoglobinemia, which may necessitate prolonged monitoring.
    Notes: Zusammenfassung In einer prospektiven, randomisierten Doppelblindstudie wurde bei 3mal 20 Patienten eine kombinierte „3-in-1“/Ischiadicus-Blockade durchgeführt. Wir verglichen den Blockadeerfolg, die arteriellen Serumspiegel und die Nebenwirkungen beim Einsatz von 700 mg Mepivacain 1% ohne (Gruppe I) und mit Adrenalin 1:350000 (Gruppe II) sowie 700 mg Prilocain 1% (Gruppe III). Der primäre Blockadeerfolg ohne supplementierende Analgetika war in allen drei Gruppen vergleichbar und lag zwischen 90 und 95%. Die maximalen Serumspiegel betrugen in Gruppe I 3,91±0,95 μg/ml nach 60 min und in Gruppe II 2,94±0,58 μg/ml nach 120 min. Zu allen Zeitpunkten führte Adrenalin zu einer signifikanten Reduktion der Mepivacain-Serumspiegel (p〈0,01). In der Prilocaingruppe waren die maximalen Serumspiegel mit 2,07±0,56 μg/ml signifikant niedriger als in beiden Mepivacaingruppen (p〈0,001) und traten nach 15 min auch signifikant früher auf. Bei keinem Patienten fanden sich Symptome oder klinische Zeichen einer Intoxikation durch zu hohe intravasale Lokalanästhetikumspiegel. Als einzige Nebenwirkung kam es in der Prilocaingruppe zu einer Methämoglobinbildung von im Median 10,1% 3 bis 4 h nach Injektionsende, wobei drei Patienten Werte zwischen 16 und 17% aufwiesen. Bei fünf Patienten dieser Gruppe war auch nach 6 h noch eine ausgeprägte periphere Zyanose nachweisbar. Sowohl Mepivacain 1% ohne und mit Adrenalin als auch Prilocain 1% eignen sich in einer Dosierung von 700 mg für die Durchführung der kombinierten „3-in-1“/Ischiadicus-Blockade, wobei Prilocain allerdings den Nachteil einer im Einzelfall klinisch relevanten Methämoglobinbildung aufweist.

    Description / Table of Contents: Abstract The object of this review is to demonstrate the non-nutritional importance of carbohydrates and fat as they represent the classic energy carriers in parenteral nutrition. Concerning the pathophysiological changes of organ metabolism and intermediary metabolism as well as the pharmacological function of this nutritive substrates it is necessary to adjust parenteral nutrition strategy to various clinical pictures. The major goals of parenteral applicated carbohydrates are to avoid hyperglycemia, to return the increased hepatic glucose production during trauma and sepsis back to normal, and to reduce protein catabolism. Realizing this goals the dosage of glucose infusion underlies close metabolic borders depending on the present disease. Because of favourable effects of hepatic glucose and protein metabolism, xylitol, a non-glucose polyol, represents a useful alternative energy source to glucose. The non-energetic nutrition therapy with fat consists of application of the essential fatty acids linolic and linloeic acid and considers the immunmodulatory effects of various fatty acids as precursors in the eicosanoid metabolism. As demonstrated at the organ systems of liver and lung this pharmacological effects of any lipid infusion might influence specific organ functions.
    Notes: Zusammenfassung Ziel dieser Arbeit ist es, die nicht-energetische Bedeutung der klassischen Energieträger Kohlenhydrate und Fette im Rahmen parenteraler Ernährungskonzepte darzustellen. Basierend auf der pharmakologischen Funktion dieser Nährsubstrate im Kontext der pathophysiologischen Veränderungen des Organ- und Intermediärstoffwechsels definiert sich eine Ernährungsstrategie, die versucht, eine differenzierte, auf einzelne Krankheitsbilder abgestimmte Nährstoffzufuhr durchzuführen. Kohlenhydratzufuhr: Dabei werden als wesentliche Ziele einer parenteralen Kohlenhydratgabe die Vermeidung von Hyperglykämie, die Normalisierung der nach Trauma und während Sepsis gesteigerten hepatischen Glukoseproduktion sowie die Reduktion der Proteinkatabolie genannt. Bei der Realisierung dieser Zielgrößen sind der Zufuhr von Glukose in Abhängigkeit vom vorliegenden Krankheitsbild und der verwendeten Dosierung metabolische Grenzen gesetzt. Der Zuckeraustauschstoff Xylit repräsentiert aufgrund günstiger Effekte auf den hepatischen Glukose- und Proteinstoffwechsel eine metabolisch sinnvolle Alternative zur alleinigen Verwendung von Glukose. Fettapplikation: Die nicht-energetische Ernährungstherapie mit Fetten beinhaltet einerseits die Zufuhr der essentiellen Fettsäuren Linolsäure und α-Linolensäure und berücksichtigt andererseits das immunmodulatorische Potential verschiedener Fettsäuren als Präkursoren im Eikosanoidstoffwechsel. Am Beispiel der Organsysteme Leber und Lunge wird aufgezeigt, daß wegen dieser pharmakologischen Bedeutung einzelner Fettsäuren jede Lipidinfusion prinzipiell einen Einfluß auf spezifische Organfunktionen nehmen kann.

    Description / Table of Contents: Abstract There are many closed-loop control systems for muscle relaxants reported, but only a few could cope with the introduction of the latest shorter acting neuromuscular blocking drugs. These new muscle relaxants such as mivacurium require a fast adapting closed-loop system for controlling an adequate infusion. Methods. After approval of the local ethics committee and having the patients’ informed consent a total number of 75 patients [ ASA I and II] were included in the study and assigned either to a training-, prediction-, prediction-/feedback- or a validationphase, as needed. Anaesthesia was induced and maintained with propofol in a TCI-mode with a plasma level of 3 to 5 µg/ml and 0.1 mg fentanyl boli as needed in all patients. In the last validation phase, having 20 patients, the prediction error and the error of the whole system was taken and analysed. Results. A closed-loop system using a neural network as a predictor could be established. In the final validation phase constisting of 20 patients the mean square prediction error was found to be 0.1%±0.2% [ mean±SD]. The mean square error of the whole system was 0.55%±0.59% [ mean±SD]. Conclusions. A closed-loop system for control of a mivacurium infusion could be established. The system proofed to be reliable for a closed-loop infusion of mivacurium in order to maintain a predefined degree of neuromuscular blockade of 95% during routine surgery. The performance of the described controller is comparable to all recent attempts and could therefore be useful for scientific studies. It should be futher validated and esthablished for other muscle relaxants, as well.
    Notes: Zusammenfassung In den vergangenen Jahren wurden unterschiedliche closed-loop-Systeme mit modell-prädiktivem Charakter zur Regelung von Muskelrelaxanzien beschrieben. Seit der klinischen Einführung von Mivacurium ist eine dank seiner kurzen Halbwertszeit für den Regelungsansatz sehr geeignete Substanz verfügbar, deren Dauerinfusion bisher nur von wenigen Systemen geregelt werden kann. Der Einsatz eines neuronalen Netzwerks als Prädiktor für eine solche Regelung soll in der vorliegenden Arbeit untersucht werden. Methodik. 75 Patienten, die sich einem abdominal chirurgischen Eingriff in Vollnarkose unterziehen mußten, wurden nach den Erfordernissen der Synthese des neuronalen Netzwerks der Trainings-, reinen Prädiktions-, der Prädiktions/Regelungs- und der Validierungsphase zugeteilt. Eine standardisierte Narkose wurde mit Propofol in einem TCI-Modus mit 3–5 µg/ml und 0,1 mg Fentanylboli aufrechterhalten. Die Regelung des Systems im Hinblick auf Prädiktions- bzw. gesamt Regelungsfehler wurde bei den 20 Patienten mit einem T1-Wert von 5% in der Validierungsphase statistisch untersucht. Ergebnisse. Es konnte eine Regelung mit einem neuronalen Netzwerk als Prädiktor zur Regelung einer Mivacuriuminfusion synthetisiert werden. Das Quadrat des gemittelten Prädiktionsfehlers über alle 20 Patienten der Validierungsphase lag bei 0,1% mit einer Standardabweichung von 0,2%. Die Abweichung der Sollgröße von der Istgröße wurde gemessen. Hier wurde der tatsächliche T1-Wert vom Ziel T1-Wert [ 5% bzw. 95%ige Blockade] berechnet. Der mittlere quadratische Fehler des gesamten Systems betrug hierbei 0,55%, die Standardabweichung 0,59%. Schlußfolgerung. Der Einsatz eines neuronalen Netzwerks erlaubt es, eine Mivacuriuminfusion exakt zu steuern und damit die neuromuskuläre Blockade über den Zeitraum einer Operation auf einem vorgegebenen Relaxationsniveau zu halten. Die Ergebnisse sind mit denen anderer Reglungsansätze vergleichbar, bei einer deutlich verbesserten Prädiktionsfunktion. Der Einsatz dieses Systems ist im Rahmen von wissenschaftlichen Studien, die eine konstante Relaxationstiefe erfordern, denkbar. Eine Ausweitung auf andere Muskelrelaxanzien wäre auch wünschenswert.

    Description / Table of Contents: Abstract Sepsis and SIRS are characterised by increased hepatosplanchnic blood flow and oxygen transport due to sepsis-associated hypermetabolism with enhanced oxygen uptake. Regional hypermetabolism may be linked with a mismatch of oxygen availability and demand potentially resulting in a pathological splanchnic oxygen uptake/supply dependency. Splanchnic hypermetabolism has been hypothesised to be due to increased hepatic gluconeogenesis caused by accelerated glucose precursor uptake resulting from increased release from the peripheral tissues. This increased precursor efflux is triggered by cytokines. The response of splanchnic haemodynamics and oxygen kinetics, however, to therapeutic interventions does not necessarily parallel the different metabolic pathways. Therefore, understanding of both tissue perfusion and oxygenation as well as metabolism is pivotal for evaluating the effects of different therapeutic strategies in intensive care medicine.
    Notes: Zusamenfassung Sepsis und SIRS sind charakterisiert durch gesteigerten Blutfluss und Sauerstofftransport im Hepato-Splanchnikusgebiet, hervorgerufen durch eine Steigerung der Stoffwechselanforderungen mit dem Ergebnis einer erhöhten O2-Aufnahme. In Folge des Sepsis-induzierten Hypermetabolismus kann es zu einem Missverhältnis zwischen vorhandener Sauerstoffverfügbarkeit und aktuellem Sauerstoffbedarf kommen und daraus potentiell eine pathologische Abhängigkeit zwischen Sauerstoffverfügbarkeit und -bedarf resultieren. Als Ursache für den Hypermetabolismus wird unter anderem die gesteigerte hepatische Glukoneogenese diskutiert, die als Folge einer gesteigerten Aufnahmerate von vermehrt peripher freigesetzten Glukosepräkursoren angesehen wird. Dieser gesteigerte Fluss von Glukosepräkursoren wiederum ist durch reaktiv freigesetzte Zytokine mitbedingt. Bezüglich therapeutischer Interventionen muss beachtet werden, dass Effekte auf Hämodynamik und Sauerstofftransport im Splanchnikusgebiet der entsprechenden Stoffwechselantwort angepasst sein müssen, um eine zusätzliche Imbalanz zwischen regionalen Blutfluss, Sauerstoff- und Substratfluss einerseits und den regionalen Anforderungen andererseits zu vermeiden. Deshalb ist das Verständnis sowohl der Gewebeperfusion und -oxygenierung als auch der wesentlichen Stoffwechselaktivität unerlässlich, um Behandlungskonzepte einzuordnen und bewerten zu können.

    Description / Table of Contents: Abstract. Mucociliary clearance represents an important protective mechanism of the upper and lower respiratory tracts whereby inhaled particles and micro-organisms are removed from the tracheobronchial system. In intubated intensive care unit (ICU) patients, impaired ciliary function and mucus transport are associated with pulmonary complications [9]. Some authors have shown that theophylline increases mucus transport in healthy subjects and patients with chronic bronchitis [8, 16, 31, 36]. However, other investigators have found unconvincing or conflicting results [4, 20, 25]. Therefore, we studied the influence of theophylline on bronchial mucus transport velocity (BTV) in ventilated ICU patients. Methods. The study was approved by the ethics committee of our hospital. In a placebo-controlled double-blind study, the effect of therapeutic serum concentrations (10 – 20 µg/ml) of theophylline on BTV in ventilated patients in a surgical ICU was investigated. Group 1 (n=10) served as controls and received placebo. The patients of group 2 (n=10) were given intravenous infusions of 5 mg/kg body weight theophylline over 20 min, followed by 1 mg/kg per hour. Before administration of theophylline and 1 h after starting the infusion BTV, serum theophylline concentrations, and arterial blood gases were measured. Heart rate and blood pressure were registered every 5 min. In an additional study, the effect of theophylline on BTV at subtherapeutic serum concentrations (5 µg/ml) was investigated. BTV was measured with a small volume (0.05 – 0.08 ml) of albumin microspheres labeled with technetium-99m, which was deposited on the dorsal surface at the lower ends of the right and left main bronchi via a catheter placed in the inner channel of a fibre-optic bronchoscope. The movement of the microspheres towards the trachea was visualised and recorded using a scintillation camera [10]. Results. The theophylline treatment induced a significant improvement in BTV from 3.8 to 7.6 mm/min in the left main bronchus and from 0.5 to 5.4 mm/min in the right, while placebo was inactive. The increase in BTV, however, was associated with an increase in heart rate; in 3 patients the theophylline infusion had to be stopped because of severe tachycardia. Blood gases and blood pressures did not change. Theophylline at subtherapeutic concentrations had no effect on either BTV or heart rate. Conclusions. Theophylline at therapeutic concentrations improves bronchial mucus transport in intubated ICU patients. The increase in BTV may be associated with severe tachycardia, and therefore routine application cannot be recommended.
    Notes: Zusammenfassung. In einer plazebokontrollierten Doppelblindstudie wurde der Einfluß von Theophyllin auf die bronchiale Transportgeschwindigkeit (BTG) von Intensivpatienten untersucht. Gruppe 1 (n=10) diente als Kontrollgruppe. Gruppe 2 (n=10) erhielt 5 mg/kg KG Theophyllin-Natriumglycinat als "loading dose" und anschließend 1 mg/kg KG/h. Vor Beginn der Theophyllinapplikation sowie eine Stunde nach Infusionsbeginn erfolgte die Bestimmung der BTG, des Theophyllinserumspiegels sowie der arteriellen Blutgase. Zur Bestimmung der BTG wurden 2 MBq Technetium-99 makroaggregiertes Albumin auf die Schleimhautoberfläche der distalen Hauptbronchen mittels Fiberbronchoskopie appliziert und die Wanderungsgeschwindigkeit mit einer Gammakamera erfaßt. Durch die Theophyllinapplikation kam es zu einem signifikanten Anstieg der BTG im linken Hauptbronchus von 3,8 auf 7,6 mm/min und im rechten von 0,5 auf 5,4 mm/min. Verbunden mit der Steigerung der BTG war allerdings ein unerwünschter Anstieg der Herzfrequenz, welcher bei drei Patienten zu einem Abbruch der Theophyllinapplikation führte. Blutgase und arterieller Blutdruck blieben unverändert. In einer weiteren Studie wurde bei zehn Patienten der Effekt einer niedrig dosierten Theophyllingabe (2 mg/kg KG, anschließend 0,5 mg/kg KG/h) untersucht. Diese Dosierung hatte weder einen Einfluß auf die BTG noch auf die Herzfrequenz. Aufgrund der vorliegenden Ergebnisse führt eine Theophyllinapplikation in therapeutischer Dosierung zu einer Verbesserung der mukoziliären Klärfunktion beatmeter Patienten. Ein genereller Einsatz kann jedoch nicht empfohlen werden, da dieser Effekt von einem unerwünschten Anstieg der Herzfrequenz begleitet sein kann.

    Description / Table of Contents: Abstract The administration of dry anaesthetic gases for ventilation leads to morphological changes of the tracheobronchial epithelium that may cause postoperative pulmonary complications. Therefore, additional humidification with a heat and moisture exchanger (HME) is suggested for ventilation during anaesthesia, particularly when using semi-open breathing systems. Recommendations concerning the use of a HME in the semi-closed system are controversial. There are no data in the literature as to whether a HME improves mucociliary transport under these conditions. We therefore studied bronchial mucus transport velocity (BTV) with and without the use of a HME in the semi-closed circle system in humans. Patients and methods. The study was approved by the ethics committee of our hospital. In a prospective, randomised trial a total of 22 patients undergoing major abdominal surgery were investigated. In all patients anaesthesia was induced and maintained with midazolam, fentanyl, and vecuronium. After intubation, a HME (BACT/VIRAL HME, Pharma Systems AB, Sweden) was inserted between the endotracheal tube and ventilation tubing in 11 patients; the other 11 were ventilated without a HME and served as controls. Ventilation was assisted with a fresh gas flow of 3 l in a semi-closed system (Dräger Sulla 808 V with an 8 ISO circle system and Ventilog 2 ventilator, Drägerwerk AG, Germany) and a 2:1 mixture of nitrous oxide and oxygen. The fresh gas passed through the soda lime canister. At the end of the operation BTV was measured with a small volume of albumin microspheres labeled with technetium Tc99m, which was deposited on the dorsal surface at the lower ends of the right and left main bronchi via a catheter placed in the inner channel of a fibre-optic bronchoscope [16]. Results. The two groups were comparable with regard to age, sex, preoperative lung function, duration of mechanical ventilation, and dose of anaesthetics (Table 1). There were no statistically significant differences in the BTVs (Fig. 1). Discussion. BTV does not improve with the use of a HME in the semi-closed circle system with a fresh gas flow of 3 l. With modern anaesthesia machines lower fresh gas flows should be administered, whereby the humidity and temperature of the inspired gases are further increased.
    Notes: Zusammenfassung In einer prospektiven randomisierten Studie wurde der Einfluß eines Wärme- und Feuchtigkeitsaustauschers (HME) auf die bronchiale Schleimtransportgeschwindigkeit während einer Narkosebeatmung im halbgeschlossenen Kreissystem untersucht. Bei 11 Patienten wurde sofort nach erfolgter Intubation ein HME (BACT/VIRAL HME, Pharma Systems AB, Schweden) zwischen Tubus und Beatmungsschläuchen plaziert, bei anderen 11 Patienten wurden die Beatmungsschläuche direkt am Tubus angeschlossen. Nach einer mehrstündigen Narkosebeatmung wurde die bronchiale Schleimtransportgeschwindigkeit (BTG) bestimmt. Zur Bestimmung der BTG wurden zwei MBq Technetium-99 makroaggregiertes Albumin auf die Schleimhautoberfläche der distalen Hauptbronchien mittels Fiberbronchoskopie appliziert und die Wanderungsgeschwindigkeit mit einer Gammakamera erfaßt. Die BTG war in der Filtergruppe sowohl im linken als auch im rechten Hauptbronchus gegenüber der Kontrollgruppe tendenziell höher (Medianwerte links 6,8 versus 5,8 mm/min; rechts 7,5 versus 6,4 mm/min), ein statistisch signikanter Unterschied ließ sich jedoch nicht nachweisen. Die vorliegende Studie zeigt, daß bei einer Narkosebeatmung im halbgeschlossenen System mit einer Frischgaseinleitung vor dem Atemkalk und einem Frischgasfluß von 3 l/min die bronchiale Schleimtransportgeschwindigkeit durch einen HME nicht verbessert wird.

    Description / Table of Contents: Abstract Pollution of work areas by volatile anaesthetics and nitrous oxide occurs during general anaesthesia. Short anaesthesia procedures are often carried out in operating theatres that are not equipped with air-conditioning systems. Methods of lovering exposure during short procedures, where mask anaesthesia is the usual procedure, are double masks and the laryngeal mask. The aim of our investigation was to determine the possibility of lowering the pollution of the environment to below national and international thresholds in a non-air-conditioned work area and to find out which method of anaesthesia is the most effective in environmental protection, i.e. which has the lowest leakage rate. Methods. In our investigation, at two points of the work area the mean enflurane [2-chloro-1-(difluoromethoxy)-1,1,2-trifluoro-ethane] concentrations were measured under daily routine conditions in a non-ventilated anaesthesiological work area by gas chromatography. Anaesthesia with single masks, double masks, laryngeal masks or endotracheal intubation was carried out. Results. No differences were found in the mean concentration of enflurane during the anaesthesia procedures. The leakage rates of endotracheal intubation anaesthesia were the lowest. Discussion. In unventilated work areas, it was not possible to lower the exposure of the personnel by changing the method of anaesthesia. The application of procedures like double or laryngeal masks does not avoid the need for installation of air-conditioning systems in all work areas were anaesthesia is performed.
    Notes: Zusammenfassung Während Inhalationsnarkosen kommt es zu Raumluftkontaminationen durch volatile Anästhetika und Lachgas. Belastungssenkende Verfahren für Narkosen, die als Maskennarkosen durchgeführt werden, sind Doppelmasken und Larynxmasken. Ziel unserer Untersuchung, die in einem nichtklimatisierten Eingriffsraum stattfand, war es, die verfahrensbedingte Leckage im Vergleich zur Intubationsnarkose zu quantifizieren. Gleichzeitig wurde geprüft, ob sich durch die Einführung von Larynx- oder Doppelmasken die MAK-Werte in nichtbelüfteten Arbeitsplätzen einhalten lassen. Es wurden Narkosen mit Einfachmasken, Doppelmasken, Larynxmasken und Intubationsnarkosen durchgeführt. Dabei bestimmten wir während jeder Narkose die mittlere Enflurankonzentration gleichzeitig an zwei Punkten im Arbeitsbereich des Anästhesisten. Aus den gemessenen Werten und den Narkosezeiten wurde die verfahrensbedingte Leckage berechnet. Es wurden keine Unterschiede der mittleren Narkosegasbelastung festgestellt. Die Narkosegaseemission war bei Intubationsnarkosen niedriger als bei allen anderen Verfahren. Belastungssenkende Effekte von Larynx- und Doppelmasken werden in nichtbelüfteten Räumen durch andere Expositionsquellen überlagert. Die Installation von suffizienten Klimaanlagen muß weiter für alle anästhesiologischen Arbeitsplätze gefordert werden.

    Description / Table of Contents: Abstract Background: Droperidol even in low doses such as 0,5 mg to 1,25 mg can increase postoperative anxiety and state of tension. The aim of this study was to determine whether these side effects occur frequently following low-dose droperidol and to see whether these are dose related. Methods: 184 female in- and outpatients ASA grade 1 and 2 undergoing gynaecological laparoscopy were recruited to this prospective, double-blind study. General anaesthesia was standardized (induction with thiopentone, fentanyl 2 µg/kg and vecuronium 0,1 mg/kg, tracheal intubation, maintainance with enflurane in N2O/O2). Patients were randomly allocated to receive saline (n=45), 0,625 mg (n=46), 1,25 mg (n=47) or 2,5 mg (n=46) droperidol i.v. 10 minutes before the end of surgery. 1, 3, 6, and 24 hours postoperatively, the patients’ anxiety, state of tension and overall mood was evaluated using two psychological questionnaires which had been tested for the perioperative period (Erlanger anxiety and tension-scale / BSKE-EWL-test). Sedation was evaluated by the staff of the recovery room. In addition, postoperative nausea and vomiting (PONV) was assessed using a 100 mm visual analogue scale and by counting the episodes of retching or vomiting. PONV was then rated over the whole observation period as none, mild, moderate or severe using a fixed scoring algorithm. Statistical analysis was performed using the ANOVA and the chi2-test. Results: The patients did not differ with regard to biometric data, duration of surgery and anaesthesia. The postoperative scores for anxiety, state of tension and overall mood were not different between the groups at any observation time (Fig. 1: anxiety and tension: P=0,5687; figure 2: overall mood: P=0,0647). Quality of sleep in the first night after surgery was the same in all groups (Table 2 and 3). Sedation was not significantly different (Table 4; P=0,0704). Furthermore, duration of stay in the recovery room did not differ (P=0,4353). On the other hand, three patients from the 2,5 mg droperidol group had to stay unexpectedly on the ward overnight, because they had been too much sedated to be discharged at home. This was not the case with any patient from the other groups. Compared to placebo, PONV over the whole 24 h observation period was significantly reduced by droperidol (Fig. 3; P=0,0338): completely free from PONV: placebo: 41,3%, 0,625 mg droperidol: 67,4%, 1,25 mg droperidol: 53,2%, 2,5 mg droperidol: 71,7%. Also the severity of PONV was reduced. Conclusion: In gynaecological laparoscopy under general anaesthesia with tracheal intubation, we recommend droperidol 0,625 mg in the prevention of PONV, as it reduces PONV as well as 2,5 mg with no severe sedation in this dosage. Psychological side effects did not occur more frequently after droperidol compared to placebo in any of the investigated dosages.
    Notes: Zusammenfassung In vorliegender Studie wurden das Ausmaß postoperativer Angst, innerer Anspannung und allgemeiner Befindlichkeitsstörungen nach intraoperativ verabreichtem Droperidol und eine mögliche Dosisabhängigkeit dieser Nebenwirkungen untersucht. Zu diesem Zweck wurden erstmals zwei evaluierte Selbstbeurteilungsfragebögen eingesetzt. 184 ASA I und II Patientinnen, die sich einer gynäkologischen Laparoskopie in standardisierter Intubationsnarkose unterzogen, wurden in diese randomisierte, doppelblinde und plazebokontrollierte Untersuchung aufgenommen. Zehn Minuten vor Narkoseende erhielten die Patientinnen i.v. Kochsalz, 0,625 mg, 1,25 mg oder 2,5 mg Droperidol. Angst und allgemeines Befinden sowie Übelkeit und Erbrechen in der postoperativen Phase (PONV) wurden 1, 3, 6 und 24 h postoperativ registriert. Die Werte für Angst und allgemeines Befinden zeigten zu keinem Untersuchungszeitpunkt Gruppenunterschiede. Die Schlafqualität in der ersten postoperativen Nacht war in allen Gruppen gleich. Verglichen mit Plazebo wurde PONV durch Droperidol in jeder Dosierung gesenkt (p=0,0338): komplett frei von PONV: Plazebo: 41,3%, Droperidol 0,625 mg: 67,4%, 1,25 mg: 53,2%, 2,5 mg: 71,7%. Bei gynäkologischen Laparoskopien in Intubationsnarkose empfehlen wir zur Prophylaxe von PONV 0,625 mg Droperidol, da es ebensogut antiemetisch wirksam ist wie 2,5 mg. Psychische Nebenwirkungen wie Angst oder andere Befindlichkeitsstörungen treten in keiner der untersuchten Droperidoldosierungen vermehrt auf.

    Description / Table of Contents: Abstract Objective. Randomised controlled trials investigating the efficacy of dexamethasone alone or in combination with other antiemetics to prevent postoperative nausea and vomiting (PONV) were included in a meta-analysis to estimate the relative efficacy of these treatments. Methods. Studies were systematically searched using Medline, EMBASE, the Cochrane-Library, and by manual screening the reference lists and current issues of locally available anaesthesia journals. Studies identified were divided into four different groups. For each subgroup an independent analysis was performed: 1. Dexamethasone vs. placebo, 2. Dexamethasone+other antiemetic vs. other antiemetic alone, 3. Dexamethasone+other antiemetic vs. dexamethasone alone, 4. Dexamethasone vs. other antiemetics. The main end point in each study was defined as complete absence of nausea, retching, and vomiting after prophylactic antiemetic treatment. The pooled odds-ratios, the relative risk (RR) and the numbers-needed-to-treat (NNT) with their corresponding 95%-confidence intervals (given in parentheses) were calculated using a random effects model. Results. A total of 26 studies with 2561 patients were analysed. 1. As a sole antiemetic agent dexamethasone is superior to placebo to prevent PONV (RR: 0,49 (0,15–0,42); NNT: 3,4 (2,5–5,3)). 2. When dexamethasone and an other antiemetic (e.g. a 5-HT3-antagonist) are combined this drug combination is significantly more effective than the single antiemetic without dexamethasone (RR: 0,60 (0,46–0,78); NNT: 7,3 (5,7–10,2)). 3. A similar result was obtained when the dexamethasone combination was compared with dexamethasone alone. The combination is statistically superior (RR: 0,16 (0,08–0,32); NNT: 3,2 (2,2–6,3)). 4. Dexamethasone was usually compared with 5-HT3-antagonist and to a less extends also with dopamine antagonists. Summarising these studies, there was no significant difference concerning effectiveness (RR: 1,35 (0,99–1,85); NNT: 10,6 (5,6–92,6)). Conclusion. Dexamethasone has antiemetics effects that are superior to placebo treatment and are comparable with conventional antiemetic agents (e.g. 5-HT3-antagonist, dopamine antagonists). The drug is especially useful in combination with other antiemetics and increases the efficacy of the antiemetic partner drug.
    Notes: Zusammenfassung Fragestellung. Ziel der Untersuchung war die metaanalytische Aufarbeitung sämtlicher verfügbarer randomisierter, kontrollierter Studien, in denen Dexamethason allein oder in Kombination mit anderen Antiemetika zur Prophylaxe von Übelkeit und Erbrechen in der postoperativen Phase (PONV) eingesetzt wurde. Methodik. Es wurde eine systematische Literatursuche in den Literaturdatenbanken Medline, EMBASE und in der Cochrane-Library durchgeführt und Literaturverzeichnisse von Übersichtsarbeiten und aktuelle Ausgaben lokal verfügbarer Anästhesiejournale manuell durchsucht. Die Studien, die mit diesen Suchstrategien identifiziert worden waren, wurden in die vier folgenden Gruppen eingeteilt, die jeweils getrennt einer Metaanalyse unterzogen wurden: 1. Dexamethason vs. Plazebo, 2. Dexamethason+Partnerantiemetikum vs. Partnerantiemetikum allein, 3. Dexamethason+Partnerantiemetikum vs. Dexamethason allein, 4. Dexamethason vs. andere Antiemetika. Als Hauptzielgröße wurde jeweils der Anteil der Patienten extrahiert, der nach einer antiemetischen Prophylaxe während des postoperativen Beobachtungszeitraums komplett frei von Übelkeit und Erbrechen geblieben war. Daraus wurde die odds-ratio (OR), das relative Risiko (RR) und die “numbers-needed-to-treat” (NNT) jeweils mit den dazu gehörigen 95%-Konfidenzintervallen (jeweils in Klammern aufgeführt) mit einem Modell der zufälligen Effekte (random effects model) berechnet. Ergebnisse. Insgesamt wurden 26 Studien mit insgesamt 2561 Patienten in die Analysen aufgenommen. 1. Als Einzelsubstanz ist Dexamethason zur Prophylaxe von PONV einer Plazebobehandlung deutlich überlegen (RR: 0,49 (0,15–0,42); NNT: 3,4 (2,5–5,3)). 2. Wird Dexamethason zusammen mit einem anderen Antiemetikum (z. B. einem 5-HT3-Antagonisten) kombiniert, so erweist sich die Dexamethasonkombination der Einzelsubstanz als überlegen (RR: 0,60 (0,46–0,78); NNT: 7,3 (5,7–10,2)). 3. Dasselbe gilt, wenn die Dexamethasonkombination mit Dexamethason allein verglichen wird. Auch hier schneidet die Kombination deutlich besser ab (RR: 0,16 (0,08–0,32); NNT: 3,2 (2,2–6,3)). 4. Dexamethason wurde bisher v. a. mit 5-HT3-Antagonisten, seltener mit Dopaminantagonisten direkt verglichen. Bei diesen Vergleichen schneidet Dexamethason nicht schlechter ab als die Vergleichsantiemetika (RR: 1,35 (0,99–1,85); NNT: 10,6 (5,6–92,6)). Schlussfolgerungen. Dexamethason weist einen antiemetischen Effekt auf, der Plazebo überlegen und mit dem etablierter Antiemetika (5-HT3-Antagonisten / Dopaminantagonisten) vergleichbar ist. Besonders effektiv ist die Substanz, wenn sie in Kombination mit anderen Antiemetika gegeben wird. In diesem Fall verstärkt sie signifikant die antiemetische Wirkung der jeweiligen Partnersubstanz.
