    Notes: Summary Spectrometric experiments performed, in November 1976, within the framework of the ‘Latitude Survey Mission’ on board the NASA Convair 990 from Ames Research Center are briefly deseribed. The results presented concern odd nitrogen molecules, HCl and water vapor. In terms of vertical column density, HNO3 is predominant over NO+NO2 at all latitudes higher than 40 degrees. A seasonal variation of NO2 abundance is observed, with larger values in the summer hemisphere at high latitude. The mean zenith column density of HCl above 11 km is 1.5×1015 mol.cm−2, with no evidence for any seasonal or climatic variation. Local number densities as high as 1.4×1010 mol.cm−3 for HNO3 and 5.4×1014 mol.cm−3 for water vapor have been measured during the same flight near 11 km.

    Notes: Abstract Twenty-nine patients received the cytotoxic radiosensitizing agent lonidamine before, during, and after cranial irradiation for brain metastases. One patient was ineligible (meningioma). In 28 eligible patients, median survival was 29 weeks (range, 2 to 〉 220 weeks). Nine patients (32%) survived 〉 1 year and 3 (11%) survived 〉 2 years. The major toxic effects of lonidamine were myalgias, nausea and vomiting, somnolence, and ototoxicity. There was no evidence that radiation skin toxicity or cerebral toxicity was increased by the addition of lonidamine. None of the patients experienced shrinkage of their extracerebral disease on lonidamine. Median survival duration in this study was at the upper limit of that reported in the literature for radiation alone, and the proportion of 2 year survivors was also higher than usual for radiation alone. Hence, further studies may be warranted.

    Notes: Abstract We studied the toxicity and efficacy of adding in sequence 4 resistance modulators to combination chemotherapy and radiotherapyin the treatment of glioblastoma multiforme and poor prognosis anaplastic astrocytomas. Patients received cisplatin plus mitomycin-C concurrently with and following 60 Gy of radiotherapyadministered over 6 weeks. Resistance modulators were added in sequence to chemotherapy in each cohort of 6 patients as follows: metronidazole + pentoxifylline (cohort 1); + dipyridamole(cohort 2); + beta carotene (cohort 3). Central nervous system toxicity (which ranged from drowsiness to seizures and lossof consciousness) was frequent. The incidence of gastrointestinal symptoms was substantial, but was usually mild to moderatein severity. Three of 11 patients evaluable for response achieved a partial remission with treatment. The median survival duration for all patients was 26 weeks from initial diagnosis. The study was terminated prematurely because of significant toxicity (in this study as well as in parallel concurrent studies of similar design in other tumor types) and apparent lack of benefit.

    Notes: Abstract Twenty-three adult patients with meningeal involvement by a variety of malignancies were treated with the intra-Ommaya combination of methotrexate, hydrocortisone, cytosine arabinoside, and thio TEPA. Whole brain irradiation was also administered to most patients who had not previously received it. Most patients demonstrated improvement of cerebrospinal fluid parameters, but only 50% of the patients experienced neurological improvement. Patients who did not receive cranial irradiation and performance status 4 patients were less likely to respond than were patients who did receive cranial irradiation as part of their treatment and patients who were performance status 0–3. Four patients developed possible and 2 patients developed probable or definite serious neurological complications. Another 4 patients developed less severe, reversible neurological toxicity. It is possible (but still uncertain) that this regimen is more toxic than other less intensive regimens, and further studies should be undertaken cautiously.

    Notes: Abstract We present a theoretical study of a modified version of the dimensioning problem for telephone networks where the network does not contain any existing capacity and where the available capital is severely limited, a situation which can occur in private networks as well as in the public networks of developing countries. In the latter, case, the budget constraint is generally of paramount importance. This has two effects. First, the grade of service constraint is replaced by a budget constraint. Another difference from the original problem arises because the network may be operating under overload most of the time. This paper examines some cases of overload for which the link ordering is maintained. The main result of this paper is the derivation of a set of equations similar to the classical optimality equations that lead to the ECCS method, but for the modified version of the problem. As a consequence, we expect that it will be possible to obtain a fast iterative heuristic algorithm similar to the one currently used to solve the classical problem. A number of other points of theoretical interest are also discussed, mainly with the objective of gaining insight into the structure of the modified problem.

    Notes: Abstract We consider here the problem of the optimal network design either for multi‐rate circuit switching or VC switching in an ATM network through the use of effective bandwidth models. We put the emphasis on the development of a fast and stable numerical algorithm for computing solutions of networks of up to 50 nodes. We show that the main difficulty is in the calculation of the gradient of the objective function in the presence of the Erlang fixed point system. We then propose an approximation for this calculation based on the power series of the inverse of a matrix. Next, we review explicit representation techniques for the matrices involved in this calculation and show that reductions in CPU time of factors as large as 20 can be obtained with practically no penalty in terms of memory requirement. We also show that the dimensioning problem is ill‐conditioned and that a simple scaling of the dimensioning variables is sufficient to speed up the convergence. As a final point, we use the algorithm to examine some features of the problem. We show that the objective function has no definite convexity property, that alternate routing is used mostly by wideband calls and that the grade of service constraints are generally binding only for these wideband calls.

    Notes: Abstract We conducted a Phase II study of PROMACE-MOPP and intrathecal (IT) therapy followed by cranial radiation in 7 patients (4 male, 3 females) with diffuse large cell lymphomas (including one T cell) involving the central nervous system (CNS). Median age was 47 years (range, 25–78). Median performance status was 2 (range, 2 to 3). Two patients had positive CSF cytology. No patients had prior chemotherapy or radiotherapy. Treatment consisted of PROMACE (cyclophosphamide 650 mg/m2, Adriamycin 25 mg/m2, etoposide 120 mg/m2 days 1 and 8, methotrexate (MTX) 1.5 g/m2 and folinic acid 50 mg/m2 (× 5) day 15, and prednisone 60 mg/m2 days 1–14) × 3–4 courses. MOPP consisted of mustargen 6 mg/m2 and vincristine 1.4 mg/m2 days 1 and 8, procarbazine 100 mg/m2 and prednisone 40 mg/m2 po days 1–14 × 3–4 courses. IT drugs were MTX 20 mg and hydrocortisone 20 mg day 1 and cytosine arabinoside 100 mg day 8, courses 2 to 6, or more frequently if CSF cytology was positive. Following MOPP, 4000 cGy whole brain radiation (XRT) and 2000 cGy boost was given. Response was evaluated before XRT Two patients declined XRT, 3 declined MOPP and 2 declined IT drugs. Two patients had extracerebral disease and 5 were primary CNS lymphomas. Response after PROMACE was CR: 3 patients; PR 2: stable 1. One patient, with extracerebral disease, experienced PR in the abdomen and CR by CT scan in the brain, but had persistent positive CSF cytology. This patient died from pneumocystis pneumonia 10 weeks after her last CSF cytology and 17 weeks after her diagnosis. After PROMACE +/- MOPP 6 patients experienced CR's. Median (range) survival was 100 (17–334) weeks, with 1 patient lost to follow up at 32 weeks. Toxicity included febrile neutropenia; 6 patients; pneumocystis pneumonia: 1 (fatal); thrombocytopenia; 5; stomatitis: 3; diarrhea; 2; nausea; 3. Conclusion: This regimen is active in the treatment of CNS lymphomas, although toxicity is substantial.

    Notes: Abstract A review of 43 patients with cerebral metastases, an unknown primary, and no other sites of metastases is presented. 27/43 (62.7%) had solitary metastases and 37.2% (16/43) had multiple metastases. Surgical treatment involved complete resection in 30.2%, subtotal resection in 37.2% and biopsy alone or no surgical procedure in the remainder. 39/43 patients underwent whole brain irradiation with the majority receiving 3000–4000 rads/10–20 fractions. Overall survival was 52% at six months and 20% at one year, and was significantly better in patients with solitary as opposed to multiple metastases (p 〈 0.03). A failure analysis including autopsy data demonstrates that (28/41) 68.3% of patients died of progressive intracranial disease without extracerebral metastases. Implications for treatment strategies are discussed.
