    Description / Table of Contents: Zusammenfassung Eine Analyse der Literatur und eigene Befunde über die Häufigkeit von 23 HLA-Antigenen bei Patienten mit Hepatitis-Oberflächen-Antigen (HBsAg)-positiver und -negativer chronisch aktiver Hepatitis (CAH) und gesunden Trägern von HBsAg und Antikörpern gegen HBsAg (Anti-HBs) erlauben die folgenden Schlußfolgerungen: 1. Bei Patienten mit CAH und HBsAg-Persistenz findet sich eine normale Häufigkeit von HLA-B8 (und -A1). 2. Auch bei Patienten mit HBsAg-negativen Formen einer CAH ohne Autoimmunphänomene (AutoAk) ist die HLA-B8-Häufigkeit normal. 3. Nur Patienten mit HBsAg-negativer CAH mit AutoAk haben eine statistisch signifikant größere Häufigkeit von HLA-B8 (und -A1) (p 〈 0,01 nach Korrektion für die Zahl der getesteten HLA-Antigene und Patientengruppen). 4. Es bestehen keine signifikanten Unterschiede in der Häufigkeit der getesteten HLA-Antigene bei CAH-Patienten mit HBsAg-Persistenz und gesunden Trägern von HBsAg und Anti-HBs. Somit konnten keine Hinweise für HLA-assoziierte Faktoren für das unterschiedliche Verhalten gegen das Hepatitis-B-Virus gefunden werden.
    Notes: Summary New own data and a survey of published data concerning the frequencies of 23 HLA antigens in patients with HBsAg positive and negative chronic active hepatitis (CAH) and healthy carriers of Hepatitis-surface antigen (HBsAg) and high titers of antibodies to HBsAg (Anti-HBs) allow to conclude as follows: 1. Patients with CAH and persistence of HBsAg show a normal frequency of HLA-B8 2. There is no increased frequency of HLA-B8 in HBsAg negative CAH without autoimmune antibodies. 3. Only in patients with HBsAg negative CAH with autoimmune antibodies is the frequency of HLA-B8 is statistically significantly increased (p 〈 0.01 after correction for the number of antigens and different groups of patients compared). These are patients with the autoimmune form of CAH. 4. There exist no significant differences in the frequencies of the HLA antigens tested in patients with HBsAg positive CAH and healthy carriers of HBsAg and Anti-HBs. Thus no indications could be found for HLA-associated factors in the different behaviour to the hepatitis-B virus.

    Description / Table of Contents: Zusammenfassung Das Lebergewebseisen mit seinen Fraktionen Ferritin und Hämosiderin wurde in Lebern von 61 Patienten bestimmt, bei denen im Pathologischen Institut der Universität Marburg a. d. Lahn eine Autopsie durchgeführt wurde. Zusätzlich wurden histochemische Färbungen mit der Berliner-Blau- und Turnbull-Blau-Reaktion angefertigt und der Ausfall der Eisenreaktion auf einer Rangskala von 0 bis 4+ eingeteilt. In den Leberparenchymzellen war mit der Berliner-Blau-Reaktion in 23 von 61 Fällen Eisen nachweisbar, mit der Turnbull-Blau-Reaktion in 12 von 61 Fällen. In den Sternzellen war mit der Berliner-Blau-Reaktion in 18 von 61, mit der Turnbull-Blau-Reaktion in 12 von 61 Fällen Eisen festzustellen. Mit beiden Färbemethoden wurde eine signifikante Korrelation zum quantitativ bestimmten Eisengehalt gefunden. Das histochemisch bestimmte Gewebseisen war am besten zur Hämosiderinkonzentration des Lebergewebes korreliert, weniger gut zur Gesamteisenkonzentration. Der Eisengehalt der Sternzellen war weniger brauchbar zur Schätzung des Eisengehalts.
    Notes: Summary Liver tissue iron and its fractions ferritin and hemosiderin were measured in livers of 61 patients autopsied at the Institute of Pathology of the University of Marburg/Lahn. In addition, histochemical strains with Prussian blue and Turnbull blue reaction were performed and scored from 0–4+ according to the iron reaction. In 23 out of 61 cases, iron in liver parenchyma was detected by Prussian blue reaction and with the Turnbull reaction in 12 out of 61 cases. Iron was found in Kupffer cells in 18 out of 61 cases by means of the Prussian blue reaction and in 12 out of 61 cases by the Turnbull blue reaction. A significant correlation was observed between staining methods and the quantitatively assessed iron content. The tissue iron measured histochemically showed best correlation with the hemosiderin concentration of liver tissue, and less with the total iron concentration. A lesser correlation existed between Kupffer cell iron content and the biochemically determined iron concentration.

    Description / Table of Contents: Zusammenfassung Zur Induktion einer experimentellen chronisch aktiven Hepatitis (CAH) wurden Kaninchen mit humanem leberspezifischem Protein plus komplettem Freundschen Adjuvans langzeitimmunisiert. Nach 23wöchiger Versuchsdauer waren im Serum Antikörper gegen allogene hepatocelluläre Membranantigene mit Hilfe isolierter normaler Kaninchenhepatocyten bzw. in der passiven Hämagglutination mit leberspezifischem Membranprotein als Antigen nachweisbar. Gleichzeitig zeigten isolierte Hepatocyten dieser Tiere in vivo-fixiertes IgG an den Plasmamembranen in Form eines gemischt linear-granulären Fluoreszenzmusters. Mit der Raji-Zell-Technik wurden im Serum der Tiere Immunkomplexe gefunden. Eine chronisch aktive Hepatitis (CAH) hatte sich bei 3 von 16 Tieren entwickelt. Nach 44wöchiger Versuchsdauer bestand bei 10 von 15 Tieren und nach 78 Wochen bei 6 von 7 überlebenden Tieren eine CAH. Alle 7 überlebenden Tiere hatten im Serum Antikörper gegen Membranantigen isolierter Kaninchenhepatocyten und in vivo-fixiertes IgG an den hepatocellulären Plasmamembranen. Es wird angenommen, daß es sich um einen Autoantikörper gegen hepatocelluläres Membranantigen handelt. Darüber hinaus dürften in vivo an den Hepatocyten Immunkomplexe fixiert sein. Die pathogenetische Bedeutung dieser humoralen Immunphänomene ist ungeklärt.
    Notes: Summary For the induction of experimental chronic active hepatitis (CAH) in rabbits long term immunization was performed with human liver-specific protein plus complete Freunds adjuvans. Serum antibodies against allogeneic hepatocellular membrane antigens could be detected after 23 weeks using isolated normal rabbit hepatocytes or by the passive hemagglutination technique with liver-specific membrane protein as antigen. Isolated hepatocytes from these rabbits showed in vivo fixed IgG on their plasma membranes in a linear and granular fluorescence pattern. Furthermore immune complexes (Raji-cell test) were detectable in the sera of these animals. After 23 weeks 3 out of 16 animals had developed a chronic active hepatitis (CAH). After 44 weeks 10 of 15 animals and after 78 weeks 6 of 7 surviving animals had a CAH. All these 7 animals had serum antibodies against membrane antigens of isolated rabbit hepatocytes and in vivo fixed IgG on their hepatocellular plasma membranes. The results suggest the existence of autoantibodies against hepatocellular membrane antigens. In addition, immune complexes may be bound to the plasma membranes. The pathogenetic role of these humoral immune reactions has yet to be determined.

    Notes: Summary To date, three types of NANBH have been distinguished by epidemiological, clinical and experimental data. We examined the liver biopsies of three patients with an acute NANBH resembling hepatitis A from the infection route, incubation period and clinical course. The liver biopsies revealed lesions with a portal and periportal predominance, thus also exhibiting parallels with hepatitis A on the histopathological level.
