    Description / Table of Contents: Zusammenfassung An 6 normalen männlichen Freiwilligen im Alter von 20–26 Jahren wurden Plasmacortisol und -renin stündlich, Plasmaaldosteron, -angiotensinogen und -angiotensinasen alle 3 Std über jeweils 24 Std unter Kontrollbedingungen und anschließend unter Suppression der ACTH-Freisetzung durch Dexamethason gemessen. Die höchsten Cortisolspiegel fanden sich gegen 7 Uhr, die niedrigsten zwischen 21 und 1 Uhr. Die Gabe von Dexamethason führte zu konstant niedrigen Cortisolkonzentrationen. Aldosteron war unter Kontrollbedingungen und unter Dexamethason gegen 4 Uhr am höchsten und zeigte niedrigste Werte zwischen 16 und 22 Uhr. Zwischen den mittleren Aldosteronkonzentrationen entsprechender Zeitpunkte der Kontroll- und der Dexamethasonperiode bestanden keine signifikanten Unterschiede. Ähnlich dem Aldosteron zeigte das Plasmarenin Maximalwerte gegen 4 Uhr. Alle Mittelwerte entsprechender Zeitpunkte zwischen 7 und 23 Uhr und die jeweiligen 24 Std-Mittelwerte jedes einzelnen Probanden waren unter dem Einfluß von Dexamethason signifikant erhöht. Für die Existenz circadianer Rhythmen des Angiotensinogens und der Angiotensinasen konnte kein Anhalt gewonnen werden. Dexamethason bewirkte keine signifikanten Veränderungen dieser Parameter. Die Ergebnisse deuten darauf hin, daß die circadianen Rhythmen von Aldosteron und Renin miteinander vergleichbar, jedoch nicht exakt mit dem des Cortisols synchronisiert sind. Unter der Hemmung der ACTH-Freisetzung durch Dexamethason steigt die Reninaktivität an, der Aldosteronspiegel bleibt unverändert. Angiotensinogen und die Angiotensinasen, Parameter, die die aktive Konzentration des Angiotensin II beeinflussen können, scheinen an der Regulation des Aldosterons nicht beteiligt zu sein.
    Notes: Summary Plasma cortisol and renin were estimated in 1 h intervals, plasma aldosterone, angiotensinogen and angiotensinases in 3 h intervals over periods of 24 h in six normal volunteers (age 20–26) under control conditions and subsequently under suppression of ACTH released by dexamethasone. Highest cortisol levels were found around 7 a.m., minimum levels between 9 p.m. and 1 a.m. Dexamethasone reduced cortisol to constantly low concentrations. Aldosterone was highest around 4 a.m. under control conditions and under dexamethasone, and showed lowest concentrations between 4 and 10 p.m. There were no significant differences between mean aldosterone concentrations at corresponding time points of the control and the dexamethasone period. Similar to aldosterone, renin showed peak values around 4 a.m. All mean values at corresponding time points between 7 a.m. and 11 p.m. and the 24 hour mean values of each subject were significantly increased under the influence of dexamethasone. No evidence could be achieved for the existence of circadian rhythms of angiotensinogen and angiotensinases. Dexamethasone did not cause significant changes of these parameters. The data suggest that circadian rhythms of aldosterone and renin are similar to each other but not exactly synchronized to that of cortisol. When ACTH release is inhibited by dexamethasone renin increases to maintain normal aldosterone levels. Angiotensinogen and angiotensinases, parameters which may influence the active concentration of angiotensin II, do not seem to be involved in the regulation of aldosterone.

    Notes: Abstract Microalbuminura (MA) was determined in 127 children and adolescents (age 3–21 years) with type 1 (insulin-dependent) diabetes mellitus. Patients with clinical evidence of long-term complications or macroproteinuria were excluded. Urinary albumin excretion was measured in a nocturnal 12-h collection and correlated with the albumin/creatinine ratio of a urine sample freshly voided on the morning immediately following the collection. The patients were divided into group A (n=83, age 〈16 years, duration of diabetes 1–13 years, mean 4.4) and group B (n=44, age 〉16 years, duration of diabetes 1–19 years, mean 8.7) and compared with appropriate controls. MA above 15 μg/min was present in 11 of 83 (13.3%) patients in group A and in 7 of 44 (15.9%) in group B. In a repeat urine collection at least 3 months later elevated MA persisted in 1 of 11 (group A) and in 4 of 7 (group B) patients. There was no correlation between increased MA in a 12-h urine collection and the albumin/creatinine ratio in a subsequently voided urine sample. MA was not strictly dependent on age, sex, duration of diabetes, haemoglobin A1, mean arterial blood pressure, plasma creatinine, creatinine clearance or serum beta-2-microglobulin. Further systematic studies and careful follow up are necessary to appraise whether intermittent MA is indeed an early manifestation of incipient kidney disease in children with type 1 diabetes.

Die Strukturaufklärung ergab Identität mit HbM Saskatoon = β63 His → Tyr. Bei keinem der untersuchten Familienangehörigen war eine Blutfarbstoffanomalie vorhanden, so daß es sich um eine neue Mutation handelt. HbM Erlangen weist eine normale Sauerstofftransportfunktion auf. Es ist kombiniert mit einer kompensierten Hämolyse, die wahrscheinlich durch eine erhöhte Instabilität des abnorm strukturierten Hämoglobins verursacht wird. Gleichzeitig fand sich eine verminderte Aktivität der NADH-Methämoglobinreduktase.
    Notes: Abstract Report on a further hemoglobin M variant, discovered in a boy with congenital cyanosis. Chemical analysis of hemoglobin structure revealed indentity with HbM Saskatoon (= β63 His → Tyr). The abnormal hemoglobin is characterized by normal oxygen transport function. The observed slight constant hemolysis is thought to be due to increased instability of the abnormal hemoglobin molecule. In addition to the hemoglobin anomaly, the activity of NADH-methemoglobin reductase was reduced by approximately 40% of normal. None of the parents presented with an abnormal hemoglobin, indicating that the occurrence of hemoglobin M Erlangen is most likely the result of a new mutation.

    Notes: Abstract Nineteen out of 83 children with asymptomatic haematuria were classified as having IgA glomerulonephritis, characterized by diffuse mesangial deposition of IgA in the absence of systematic disease. Different histological patterns were observed: i.e., minor glomerular lesions (5 cases), focal and segmental proliferative glomerulonephritis (8 cases) and diffuse proliferative glomerulonephritis (6 cases). Recurrent gross haematuria is the clinical hallmark of the disease. Proteinuria was present in 14 children and exceeded 1 g/m2/day in 3 patients. Clinico-pathological correlations showed a close relation between the degree of proteinuria and the histological lesions. Serum IgA levels were elevated in 3 children. Glomerular filtration rate remained above 80 ml/min/1.73 m2 in a 1 to 9 year follow-up.

    Notes: Coupled arrays of vertical-cavity surface-emitting lasers were realized by patterning the reflectivity of the top-distributed Bragg reflector using a phase-matching layer and a metal grid. For improved current injection and better heat dissipation the devices were selectively oxidized. Continuous-wave room-temperature operation of these arrays has been achieved at 960 nm. Near- and far-field measurements confirm the phase-coupling in a typical out-of-phase mode pattern. Analysis of the differential efficiency reveals a strong decrease when the number of lasing pixels and hence the mesa size is increased. © 2000 American Institute of Physics.

    Notes: Background : Postprandial intragastric acidity is not uniform. Postprandial proximal gastric acid pockets have been described in the present study.Aim : To determine the effects of rabeprazole on regional intragastric acidity and proximal acid pockets.Methods : Ten normal subjects underwent two 8-day oral dosing regimens with placebo or rabeprazole 20 mg each morning in a randomized, double-blind protocol. Oesophago-gastric pH monitoring was performed on days 1 and 8.Results : Rabeprazole increased fasting and postprandial gastric pH to above 4 in each area of the stomach on days 1 and 8. With placebo, acid pockets were identified at the cardia/gastro-oesophageal junction in 62 and 50 of 150 pull-throughs on days 1 and 8, respectively. Acid pockets were detected postprandially 3.1 ± 0.2–5.8 ± 0.1 cm below the proximal border of the lower oesophageal sphincter with a mean pH drop from 4.6 ± 0.1 to 1.5 ± 0.1. Rabeprazole decreased the number of acid pockets to 30 and 27 on days 1 and 8, respectively. Rabeprazole also decreased their length and magnitude of the pH drop.Conclusions : Rabeprazole increased intragastric pH on day 1 and 8 and maintained an elevated pH during and after meals. Postprandial acid pockets, identified in the region of the cardia/gastro-oesophageal junction area postprandially, were decreased in number, length and magnitude by rabeprazole.
