    Notes: Summary A newly developed colorimetric method, DNA enzyme immunoassay (DEIA), was applied to the detection of neuraminidase subtypes N1 and N2 of influenza A viruses. Reverse transcription and polymerase chain reaction with universal primers were used for genomic amplification of H1N1, H2N2, and H3N2 strains. Following amplification, an aliquot of the PCR product was hybridized to biotinylated DNA sequences (N1/N2 probes) immobilized on microtiter wells. The hybridization event was revealed by monoclonal antibodies to double stranded DNA in a standard ELISA reaction. The assay described here was able to distinguish accurately between the two neuraminidase subtypes of human influenza A viruses. It is a simple and rapid method facilitating the handling of a large number of samples and therefore seems to be easily applicable to diagnostic laboratories.

    Description / Table of Contents: Summary The occurrence of a new influenza virus subtype in 1997 in Hongkong caused only minor outbreaks due to the swift and determined actions of the institutions involved. However, this incident again made it clear that an influenza pandemic is still a pending threat. As a result the WHO presented a pandemic preparedness plan which is to facilitate the global measures taken to fight an imminent or ongoing pandemic. Strong efforts of influenza surveillance and the coordination of worldwide measures are essential to this plan. Furthermore the WHO suggested the developement and introduction of similar national preparedness plans.
    Notes: Zusammenfassung Nach dem Auftreten eines neuen Influenza A-Subtyps in Hongkong 1997 sind die Erkrankungsausbrüche in der Bevölkerung durch schnelles und konsequentes Handeln aller Beteiligten sehr begrenzt geblieben. Dennoch haben diese Ereignisse die Gefahr einer jederzeit drohenden Influenzapandemie deutlich vor Augen geführt. Als Konsequenz dieser Erfahrungen hat die WHO einen Bereitschaftsplan entwickelt, der dazu dienen soll, durch globale Surveillance neue Influenza-Subtypen frühzeitig zu entdecken und die nachfolgenden Maßnahmen zur Schadensbegrenzung bzw. Eindämmung einer Pandemie möglichst effektiv durchzuführen und weltweit zu koordinieren. Die WHO hat mit dem vorliegenden Plan den Aufruf verbunden, dementsprechende nationale Pläne zu entwickeln.

    Description / Table of Contents: Summary In 1998/99 the influenza season lasted the complete winter. While the early season hit particularly the southern parts of Germany, later on mainly the eastern and northern parts were affected. Over 2000 influenza viruses were isolated and identified as influenza A (H3N2) (67%) or influenza B (33%). All isolates met the expected virus variants which were part of the current vaccine. Altogether, the influenza activity was more intense than in the two previous years.
    Notes: Zusammenfassung Die letzte Influenza-Saison erstreckte sich über den gesamten Winter 1998/99. Zu Beginn war besonders der Süden Deutschlands betroffen, im weiteren Verlauf der Osten und Norden des Landes. Mehr als 2000 Influenzaviren wurden isoliert und als Influenza A(H3N2) (67%) und Influenza B (33%) identifiziert. Alle Viren entsprachen den erwarteten Virusvarianten, die als Komponenten in der Impfstoffkombination verwendet worden sind. Die diesjährige Influenzawelle war insgesamt stärker als in den beiden Jahren zuvor.

    Description / Table of Contents: Summary There are no unequivocal symptoms linked to Influenza and hence diagnosis only considering the clinical picture is uncertain. The predictive value of typical symptoms of Influenza depends largely on the incidence of the infection. Some studies and observations indicate a high predictive value when the epidemic situation is additionally considered. Concepts taking into account the epidemic situation of influenza can enhance the proportion of proper diagnoses and thereby the appropriate use of neuraminidase inhibitors, as supplement to vaccine prophylaxis.
    Notes: Zusammenfassung Die Influenza hat keine eindeutigen Symptome, und eine Diagnose allein anhand des klinischen Bildes ist unsicher. Der prädiktive Wert „typischer Symptome” ist von der Häufigkeit der entsprechenden Erkrankung abhängig. Einige Studien und Beobachtungen weisen auf einen hohen prädiktiven Wert „typischer” Influenza-Symptome bei sinnvoller Berücksichtigung der epidemiologischen Situation hin. Konzepte, die diese Zusammenhänge berücksichtigen, können einen sinnvollen Einsatz der Neuraminidase-Inhibitoren – als Ergänzung der Impfprophylaxe – ermöglichen.
