    Description / Table of Contents: Zusammenfassung Es wird über 5jährige klinische Erfahrungen an Hand von 130 Keramik-Keramikhüft-endoprothesen berichtet. Die sehr guten bis befriedigenden Ergebnisse machen 84% aus. Über eine Reihe von Besonderheiten (Operationstechnik, Röntgenmorphologie der Grenzfläche um Keramikimplantate etc.) wird detailliert berichtet. Es werden verschiedene Komplikationen (Lockerungen: 2%, Keramikbrüche: 6%, Abriebphänomene) dargestellt, ramikbrüche: 6%, Abriebphänomene) dargestellt, wobei auch humanhistologische Befunde aus Kapselgewebe um Keramik-Keramikgelenke beschrieben werden. Das System hat sich in den Grundzügen bewährt, Änderungen, die die Prothese quo ad functionem sicherer machen, werden in einer 2. Publikation vorgestellt.
    Notes: Summary Clinical experience with 130 endoprostheses, entirely made of ceramic, having an average 5 year follow-up is reported. Excellent to sufficient results were seen in 84% of all cases. A number of particular matters of interest (operative technique, radiological morphology of the bone/ ceramic bone interface etc.) are discussed in detail. Complications (loosening of components: 2%; component fracture: 6%, wear phenomena etc.) are described and documented including histological observations from capsular tissue of total ceramic hip joints. The whole system has basically stood the test, during the implantation period, modifications to improve the function of the system will be described in another publication.

    Description / Table of Contents: Zusammenfassung Es wird über eine neue Implantationsmethode für die Hüftpfanne beim dysplastischen Acetabulum berichtet. Bei der intraacetabulären Hüftkopfplastik wird ein Teil des resezierten Hüftkopfes im lateralen Pfannenbett mit Schrauben verankert. Nach dieser knöchernen Rekonstruktion des Acetabulum kann eine kleine viereckige Keramikpfanne mit wenigen speziellen Instrumenten mühelos zementfrei eingesetzt werden. Nach ausführlicher Beschreibung des operativen Vorgehens werden die bisherigen klinischen Ergebnisse des Verfahrens an Hand von 11 eigenen Fällen mitgeteilt. Abschließend werden die operationstechnischen, biomechanischen und biologischen Vorteile dieser Implantationsmethode mit ähnlichen bereits andernorts angegebenen Methoden verglichen.
    Notes: Summary This report presents a new technique of socket implantation into the dysplastic acetabulum. A major part of the resected femoral head serves as the so-called intra acetabular femoral head graft and is screwed into the lateral part of the original acetabulum. By this technique it is possible to achieve a complete bony reconstruction of the dysplastic acetabulum into which a square shaped small ceramic socket can be easily implanted using few especially designed instruments. After the description of the operative technique this report presents the clinical results of this procedure based on 11 personal cases. Finally the technical, biomechanical and biological advantages of this implantation technique are discussed in view of other similar methods described recently.

    Description / Table of Contents: Zusammenfassung Es wird über experimentelle Untersuchungen zur unspezifischen Sarkomentstehung um Al2O3-Keramikplättchen und Abriebpulver bei der Ratte berichtet. Um solide und poröse Plättchen wurde eine Sarkomrate von 17,4 resp. 17.8% gefunden, um grob perforierte Scheiben lag die Sarkomrate bei 25,4%. Die gefundenen Werte liegen niedriger als vergleichbare Testergebnisse von Metallen und klinisch angewendeten Kunststoffen. Um pulverförmige Al2O3-Keramikimplantate sind keine Sarkome entstanden. Die Ergebnisse werden mit der reichhaltig vorhandenen Literatur des Fremdkörpersarkomes verglichen und ausführlich diskutiert. Es zeigt sich, daß allein die Ergebnisse pulverförmiger Implantate mit einiger Berechtigung prognostisch auch für die Klinik verwertet werden können. Das vorgelegte Resultat legt nahe, daß die Al2O3-Keramik allein beim Menschen höchstwahrscheinlich keine sarkogenetische Potenz besitzt.
    Notes: Summary This investigation presents experimental results on possible non-specific foreign body sarcoma induction by Al2O3-ceramic implants. Solid and porous discs, subcutaneously implanted in rats produce a sarcoma rate of 17.4 resp. 17.8 p.c. while roughly perforated discs initiate sarcomas at a rate of 25.4 p.c. These values were found to be lower than comparative values in the literature produced with different metals and medical grade plastics. Powdered alumina implants did not produce any sarcomas. Only this latter result can mainly be transferred into prognostic assumptions concerning the clinical application of Al2O3-ceramic. The results are discussed extensively in the lights of the existing literature, this indicating that the tested ceramic itself may not possess a significant sarcogenetic potency in humans.

    Description / Table of Contents: Zusammenfassung Die Ergebnisse der im ersten Aufsatz beschriebenen vollständigen und ausführlichen klinischen Analyse werden mit den beim Entwurf des Prothesensystems benutzten Kriterien verglichen. Es wird gezeigt, daß sich die meisten bewährt haben. Die Aspekte, bei deren die Erwartungen nicht erfüllt wurden, werden näher besprochen: Der dem Betrag nach kleine und biologisch tolerable, im wesentlichen den Einlaufvorgängen zugeschriebene Verschleiß wird durch erhebliche Materialverbesserungen weiter reduziert. Die wenigen Fälle starken Verschleißes sind stets zusammen mit klinisch exzeptionellen Situationen aufgetreten, deren Analyse zu Hinweisen zu ihrer Vermeidung führt. Die Material- und anderen Verbesserungen, die zur erheblichen Erweiterung der Sicherheit gegen Kugelbrüche durchgeführt wurden, werden im einzelnen beschrieben. Zusätzlich wird auf einige notwendige klinische Sicherheitsmaßnahmen hingewiesen.
    Notes: Summary The results of the complete and detailed clinical evaluation are compared with the original design criteria. It is shown that most of them stood the test. Those aspects which did not meet the expectations are discussed in detail: The occurrence of very little and biologically tolerable wear during the initial running-in period will be further reduced by a considerable improvement of the material properties. The few cases of severe wear had been associated with clinically exceptional situations, the analysis of which allows for suggestions to avoid such situations. The material improvements and all other steps taken to increase the safety margin considerably against fractures of the ceramic ball are described in detail. In addition, the attention is paid to some necessary clinical precautions.
