Summary 1. Before starting the MRL-41 treatment of 47 female patients without natural ovulation the excretion of the pituitary gonadotropins was quantitatively determined. The achieved HPG values were compared with the different reactions to therapy, and the HPG excretion proved to give indications for the prognosis of treatment. In patients suffering from anovulatory menstrual disorders with normal excretion of gonadotropins, a stimulation of the ovarian function was observed during MRL-41 treatment in 80% of the cases. If menstrual disorders are accompanied by an diminished or augmented HPG excretion, the prognosis is distinctly less favorable than in cases where the excretion is within normal range. 2. In 3 cases with secondary amenorrhoea of long duration the HPG excretion was ascertained continuously and for an extended period of time during the MRL-41 treatment. In 2 of these cases a true ovulation was most probably induced. After a slight initial increase the HPG excretion showed — during the further observation — the same course as in ovulatory cycles of normal women. At the time of ovulation the typical peak of HPG secretion was observed. It is supposed that in these MRL-41 acted as an impulse which introduced the endogenic regulation of the ovulatory cycle. In another case with secondary hypogonadotropic amenorrhoea a continuous increase of gonadotropin excretion was seen during the long-term MRL-41 treatment. In this case it was not possible to induce a complete ovulatory cycle. The results indicate that at least two different modes of reaction of pituitary gonadotropins are possible under MRL-41 application. 3. According to the results so far available, a stimulation of the gonadotropic hormones of the pituitary is assumed during the action of MRL-41. However, the primary site of action of MRL-41 remains unknown. The response to therapy seems to depend on the functional initial situation of the hypothalamus-pituitary-ovarian system.

Zusammenfassung 1. Die Ausscheidung der hypophysären Gesamtgonadotropine wurde mit der Maus-Uterus-Gewicht-Methode bei 47 Patientinnen ohne natuürliche Ovulation vor Beginn der Behandlung mit MRL-41 quantitativ bestimmt. Die ermittelten HPG-Werte wurden mit den verschiedenartigen Reaktionen auf die Behandlung verglichen. Es zeigte sich, daß die Ausscheidung von HPG Hinweise für die Prognose der Behandlung gibt. Bei Regelstörungen ohne natürliche Ovulation mit normaler Gonadotropinausscheidung kam es während der Behandlung mit MRL-41 in 80% der Fälle zu einer Stimulierung der Ovarialfunktion. Bei Regelstörungen mit vermehrter oder verminderter Ausscheidung von HPG ist die Prognose deutlich ungünstiger als bei Fällen mit einer Ausscheidung im Normalbereich. 2. In drei Fällen mit langfristiger sekundärer Amenorrhoe wurde die HPG-Ausscheidung während der Behandlung mit MRL-41 fortlaufend über einen längeren Zeitraum ermittelt. In zwei von diesen Fällen wurden mit großer Wahrscheinlichkeit echte Ovulationen induziert. Nach anfänglich leichter Vermehrung der Ausscheidung von HPG zeigte die Ausscheidung im weiteren Cyclus den gleichen Verlauf wie bei ovulatorischen Cyclen gesunder Frauen. Dabei wurde der typische Ausscheidungsgipfel von HPG um die Zeit der Ovulation beobachtet. Es wird angenommen, daß MRL-41 bei diesen Fällen im Sinne eines Anstoßeffektes wirkte, welcher die endogene Autoregulation des ovulatorischen Cyclus in Gang setzte. In einem weiteren Fall wurde bei einer hypogonadotropen sekundären Amenorrhoe ein kontinuierlicher Anstieg der Gonadotropinausscheidung während der langfristigen Behandlung mit MRL-41 beobachtet. Ein vollwertiger ovulatorischer Cyclus konnte in diesem Falle nicht induziert werden. Die Ergebnisse zeigen, daß zumindest zwei unterschiedliche Reaktionsweisen der hypophysären Gonadotropine auf MRL-41 möglich sind. 3. Aufgrund der bisher vorliegenden Befunde wird angenommen, daß eine Stimulierung der gonadotropen Hormone der Hypophyse im Verlauf der Wirkungsweise des MRL-41 eine Rolle spielt. Der primäre Angriffspunkt von MRL-41 bleibt damit jedoch unbekannt. Die Reaktion auf die Behandlung scheint von der funktionellen Ausgangslage des Zwischenhirn-Hypophysen-Ovar-Systems abhängig zu sein.
