    Description / Table of Contents: Zusammenfassung Anhand der Messung von T4, T3 und Reverse-T3 im Serum von 365 Probanden, älter als 65 Jahre, wurde der Frage nachgegangen, ob die im Alter häufig anzutreffende Erniedrigung des Serum-T3 dem fortgeschrittenen Lebensalter oder den begleitenden Erkrankungen zuzuschreiben ist. Die bei sorgfältig ausgewählten gesunden Alten erhobenen Ergebnisse zeigen, daß 1) das freie und Gesamt-T3 mit dem Alter abfallen, aber noch im Normbereich des jüngeren Vergleichskollektivs bleiben, daß 2) die T3-Konzentrationen bei gesunden alten Männern früher und stärker erniedrigt sind, so daß für Personen jenseits des 75. Lebensjahres die obere Grenze des Normalbereiches um 10% für Frauen und um 20% niedriger für Männer anzusetzen ist, und daß 3) ein low T3-Syndrom, gekennzeichnet durch erniedrigtes T3 und erhöhtes Reverse-T3, bei gesunden älteren Probanden nicht auftritt. Kinetische Untersuchungen zeigten, daß die tägliche T4-Produktionsrate im Alter um 20 µg, die von T3 um 10 µg abnahmen, daß aber eine erhöhte metabolische Clearance von T3 nicht für die Erniedrigung der T3-Serumkonzentration verantwortlich zu machen ist. Kombinierte Stimulationsteste mit TSH und TRH ergaben eine normale funktionelle Reserve der Schilddrüse, T3 zu produzieren. Als erster Schritt in der Sequenz von funktionellen Änderungen, die zu einem Absinken der T3-Konzentrationen im Alter führen, muß eine verminderte TSH-Produktion angenommen werden, die eine Anpassung der verfügbaren Mengen an Schilddrüsenhormonen an eine im Alter verminderte Masse an metabolisch aktivem Gewebe darstellen könnte.
    Notes: Summary T4-, T3- and reverse-T3 concentrations were measured in the sera of 365 subjects beyond the age of 65 in order to evaluate if the decrease of serum T3 frequently observed in old age can be attributed to old age per se or to concomitant non-thyroidal disease. The results obtained from a carefully selected healthy group of elderly people show that 1) total and free T3 levels are lower in senescence but well within the range for euthyroidism in younger healthy controls; 2) the decrease of serum T3 is more pronounced and occurs earlier in healthy old males than in females, so that for subjects over the age of 75, the upper limit for euthyroidism has to be adjusted by 10% in women and by 20% in men; and 3) there is no low T3 syndrome characterized by decreased serum T3 and increased serum reverse T3, solely due to old age. Turnover kinetics have shown the daily production of T4 and T3 in old age to decrease by 20 µg and 10 µg, respectively, and an increased T3 metabolic clearance not to account for the reduction of serum T3 concentrations. Combined stimulation tests with TSH and TRH showed that the functional reserve of the thyroid gland to produce T3 is maintained in old age. The first step in the sequence of events may be seen in an impairment of TSH secretion leading to an adaptation of the amount of thyroid hormones to a reduced mass of metabolically active body tissue in old age.

    Description / Table of Contents: Summary A non-specific inhibitor in serum of normal human subjects and of rats has been investigated, affecting the two antibody immunoassay (method C ofHales andRandle) for insulin in serum and thus leading to abnormally high values for the serum hormone. This inhibitor is not detectable in all subjects and can be inactivated by heating the serum at 56°C for 40 min, by repeated freezing and thawing of the serum, by treatment of the serum with EDTA (0,001–0,01 m) or with heparin (〉20 U/0,1 ml). Our investigations indicate, that complement or a component of complement is the inhibitor of the two antibody method.
    Notes: Zusammenfassung Es wird über einen Hemmfaktor im Serum von Normalpersonen und Ratten berichtet, der die immunologische Doppelantikörper-Methode vonHales u.Randle (Methode C) zur Bestimmung von Insulin unspezifisch beeinflußt und zu fälschlich hohen Hormonspiegeln im Serum führt. Dieser Faktor ist nicht in allen Seren nachweisbar und läßt sich durch Erhitzen des Serums auf 56°C für 40 min, durch wiederholtes Einfrieren und Auftauen des Serums, durch Zugabe von EDTA (0,001–0,01 m) oder von Heparin (〉20 E/0,1 ml) zum Serum ausschalten. Die vorliegenden Untersuchungen lassen vermuten, daß es sich bei diesem unspezifischen Hemmfaktor um Komplement oder um eine Komplementkomponente handelt.

    Description / Table of Contents: Summary Studies concerning the course of PB127 I and of free thyroxine (FT4; AFT4) during treatment with methylmercaptoimidazole have been carried out in 21 patients suffering from hyperthyroidism (diffuse type). Half-concentration-time data (T50%) were obtained from frequent single measurements while MMT was administered. PB127 I and AFT4 proved to be homologous parameters in their resp. relation to pretreatment BMR levels and PB131 I values. Under MMI treatment, T50%-AFT4 (6.5d) was considerably less than T50%-PB127 I (9.6), which is most probably due to an increase of thyroxine binding capacity.
    Notes: Zusammenfassung Bei 21 Patienten mit einer diffusen dekompensierten Hyperthyreose wurden vor und unter Therapie mit Methylmercaptoimidazol das proteingebundene Jod127 und das freie Thyroxin neben dem proteingebundenen Jod131 (nur vor Therapie) und dem Grundumsatz gemessen. Aus den Einzelwerten im Therapieverlauf wurden die Halbwertszeiten des proteingebundenen Jod127 und des freien Thyroxins bestimmt. Nach statistischen Kriterien erwiesen sich proteingebundenes Jod127 und freies Thyroxin in ihrer Beziehung zur Grundumsatzhöhe und in der Abhängigkeit ihrer Halbwertszeiten vom intrathyreoidalen Jodumsatz als homologe Parameter. Die Halbwertszeit des freien Thyroxins ist jedoch kürzer als die des gesamten Thyroxins. Der größere Gradient des Abfalls des freien Thyroxins ist im wesentlichen durch eine Zunahme der Gesamtbindungsvalenz der Transportproteine unter Therapie bedingt.

    Notes: Summary Often the diagnosis “light chain nephropathy” is not made before renal failure has developed. In the past these patients had a poor prognosis. Special features of this disease and improvement of renal function by plasma exchange therapy are shown in a case report. A 51-year-old man admitted to hospital with renal failure, proteinuria and hypercalcaemia was treated by Ca-free dialysis and membrane plasma separation (MPS). Renal biopsy studied by light and electron microscopy showed mainly tubular protein casts. It is assumed that this nephropathy is caused by a direct toxic effect of the kappa light chains on function and structure of tubuli. Proteinuria was markedly lowered and renal failure was made partly reversible by prompt and effective MPS. Thus, MPS may improve the prognosis of this stage of disease.

    Description / Table of Contents: Zusammenfassung 11 Patienten mit schwerer, dekompensierter, diffuser Hyperthyreose wurden kombiniert mit Favistan und Endojodin behandelt. Die Halbwertszeiten sowohl des gebundenen als auch des freien Hormons (5,7 bzw. 3,4 Tage) waren unter kombinierter Therapie signifikant kürzer als unter alleiniger Favistan-Gabe (9,3 bzw. 6,8 Tage). Zwischen der Höhe des Thyroxin-Ausgangswertes im Serum vor Therapie und der Zeit bis zum Erreichen des Euthyreose-Bereiches bestanden enge lineare Beziehungen. Da im Mittel der Ausgangswert des freien Thyroxins wesentlich stärker erhöht war als der des Cesamt-T4, erreicht trotz kürzerer Halbwertszeit die Konzentration des stoffwechselaktiven freien Hormons den Euthyreose-Bereich um 2–3 Tage später als die des Gesamt-Thyroxins. Bei Beachtung dieser Verhältnisse und Kenntnis der Ausgangswerte läßt sich die Mindestdauer der kombinierten Therapie abschätzen. Der gleichfalls bestimmte Verlauf der Jodkonzentrationen im Serum erlaubt Rückschlüsse auf die erforderliche Joddosis.
    Notes: Summary Under combined treatment with methylmercaptoimidazole and iodide, T/2 of total and free serum thyroxine (5.7. and 3.4 days) in 11 patients with severe, decompensated, diffuse thyrotoxicosis were considerably less than with MMI-therapy alone (9.3. and 6.8. days). A close linear correlation was observed between starting levels of thyroxine and time to reach normal T4 values; thus, estimates with regard to minimum time requirement of MMI-iodidetherapy are made possible.
