    Description / Table of Contents: Summary Our experiments demonstrate little influence of the propriodeptive system on a quick voluntary movement. However, this influence becomes very intensive if a slow balancing contraction is initiated. It seems likely that the central impulses have the ability to eliminate or to put into action the proprioceptive input, according to the actual requirements.
    Notes: Zusammenfassung und Schlußfolgerung Unsere Ergebnisse lassen schließen, daß die rasche Willkürbewegung in ihrem Ablauf nur gering, langsam balancierende Bewegungen dagegen besonders stark durch den peripheren Regelkreis beeinflußt werden. Offenbar besteht vom zentralen Bewegungsentwurf her die Möglichkeit, proprioceptive Einflüsse je nach Bedarf auszuschalten oder zum Tragen kommen zu lassen.

    Notes: Abstract We describe two patients with upper medullary infarctions showing ipsilateral facial weakness and relative sparing of the upper facial muscles. Electrophysiological follow-up using transcranial magnetic stimulation of the motor cortex in combination with stimulation of the peripheral facial nerve disclosed a supranuclear (corticofacial) tract lesion in one patient and a partial nuclear/infranuclear intra-axial facial nerve lesion in another.

    Notes: Abstract Phasic inspiratory genioglossus activity prevents pharyngeal airway collapse in healthy subjects during sleep and is diminished or absent in obstructive sleep apnoea syndrome (OSAS), thus leading to pharyngeal obstruction. Case reports of OSAS after pontomedullary lesions indicate that impaired inspiratory genioglossal activity may result from brainstem lesions. We therefore investigated brainstem functions in 18 awake patients with OSAS using brainstem auditory evoked potentials, blink reflex, masseter reflex, masseter inhibitory reflex (in 11 of 18 patients), magnetic evoked potentials of the tongue and electrooculography with vestibular testing. Fifteen of 18 patients showed no electrophysiological abnormalities. One patient had a left pontine and two patients a bilateral pontomesencephalic lesion, although a causal connection with OSAS was not conclusively confirmed. Our results do not support the assumption of a relevant structural brainstem lesion in OSAS patients with normal neurological findings.

    Description / Table of Contents: Summary Due to the results of an interdisciplinary study on patients with vertigo as the chief symptom and on the background of psychodynamic theories concerning anxiety disorders the term of phobic postural vertigo (Brandt & Dieterich 1986) is discussed. It becomes obvious that phobic postural vertigo is a generalizing term which encompasses different forms of psychogenic vertigo. The authors plead for a more differentiated diagnosis and subgroup oriented classification of vertigo caused by psychiatric disorders.
    Notes: Zusammenfassung Aufgrund der Ergebnisse einer mehrjährigen interdisziplinären empirischen Untersuchung an Patienten mit der Leitsymptomatik „Schwindel” zum Thema „Psychosomatische Aspekte des Schwindels” und auf dem Hintergrund psychodynamischer Theorien zu Angsterkrankungen wird der von Brandt und Dieterich 1986 eingeführte Begriff des „phobischen Schwankschwindels” diskutiert. Deutlich wird, daß es sich dabei um eine nicht differenzierende Beschreibung handelt, die unterschiedliche somatoforme (psychogene und psychosomatische) Schwindelzustände umfaßt. Die Autoren plädieren für eine nosologische Subgruppen berücksichtigende Klassifikation psychogen bedingter Schwindelzustände.

    Description / Table of Contents: Zusammenfassung Nach dem Versuch der Reinigung von Kesselwagen, in denen eine wäßrige Dispersion von Vinylidenchlorid-Copolymeren transportiert worden war, erkranken zwei Männer an persistenten Hirnnervenstörungen, die vorwiegend den N. trigeminus, in leichterer Form auch die Nn. occipitales, auriculares und cutis colli, femer die Kaumuskulatur, Augenmuskeln und den Hypoglossus betreffen. Als Ursache werden Monochloracetylen und/oder Dichloracetylen ermittelt, die als hochtoxische gasförmige Verunreinigungen im Ausgangsprodukt Vinylidenchlorid (1,1-Dichloräthylen) vorkommen, oder die sich aus Verunreinigungen (Tetrachloräthan, Trichloräthylen) bei der Fertigung oder Lagerung des Produktes gebildet haben können. Die Bildungsmöglichkeiten sowie früher berichtete Erkrankungen durch verunreinigtes Trichloräthylen (verwendet als Narkosemittel, Analgeticum und Lösemittel) werden diskutiert.
    Notes: Summary Two workers developed persistent cranial nerve disorders after attempting to clean out tank cars in which an aqueous dispersion of vinylidene chloride copolymeres had been transported. Principally involved was the trigeminal nerve; to a lesser degree the occipital auricular and cervical cutaneous nerves as well as the muscles of mastication, the eye muscles and the hypoglossus were affected. The cause of the poisoning was identified to be monochloroacetylene and or dichloroacetylene which are present as highly toxic gaseous contaminants in the initial product vinylidene chloride (1,1-dichloroethylene) or developed from contaminants (tetrachloroethane, trichloroethylene) during the production or storage of the product. The mechanisms of formation of these gases are discussed, along with previously reported cases of neuropathy resulting from impure trichloroethylene used as an anesthetic, analgesic or solvent.

    Notes: Zusammenfassung 15 Patienten mit einer Multiplen Sklerose wurden elektroneurographisch untersucht. Es stellte sich heraus, daß die Patienten mit einer akuten Verschlechterung des Krankheitsbildes entgegen unserer Erwartung eine leichte Verzögerung der Leitgeschwindigkeit in den peripheren Nerven hatten. Die pathologische Leitverlangsamung ging zurück, sobald in hoher Dosierung Cortison gegeben wurde. Unsere Befunde sprechen dafür, daß der akute Schub einer Multiplen Sklerose eine „Allgemeinerkrankung“ darstellt, die nicht nur zu Veränderungen im Zentralnervensystem, sondern auch in den peripheren Nerven führen kann. Bei alten, „ausgebrannten“ Fällen ließen sich entsprechende Befunde nicht erheben.
