    Description / Table of Contents: Summary A new method for the measurement of renin in human plasma is described. The method is based on the introduction of the internationally available renin standard of the Medical Research Council (MRC) London, as a calibration system. Thus, some principal disadvantages of methods expressing results in renin reaction velocity (angiotensin generation rate) only are avoided. Both renins, unknown and standard, react with a sheep substrate preparation and are handled identically throughout the whole procedure including the angiotensin I radioimmunoassay (RIA). The plasma renin concentration (PRC) is given in 10−6 MRC-renin units (µU/ml). Results: the renin standard is free of angiotensin, angiotensinases, and angiotensinogen; it is stable on storage. Identical enzyme kinetics are shown for both renins. An interference between endogenous and exogenous substrate could be avoided. The potentially harmful influences of proteins from the enzyme incubation mixture on the RIA dose response curve are shown. The use of an angiotensin I calibration system could be omitted. Using a standard renin dilution from 250–0.9 µU/ml almost the full biological range is covered. When giving an unrestricted diet the preliminary normal values of PRC are 21.9±12.6 µU/ml in recumbent and 40.1±18.8 µU/ml in upright position (n=16,x±s, age 20–35 years). Earlier findings of age-dependancy of PRC were confirmed.
    Notes: Zusammenfassung Es wird eine neue Methode zur Messung von Renin in menschlichem Plasma beschrieben. Die Methode basiert auf der Einführung des Renin-Standards des Medical Research Council (MRC), London als Eichsystem und vermeidet damit die prinzipiellen Nachteile von Verfahren, die Renin nur als Angiotensinbildungsgeschwindigkeit erfassen. Prinzip der Methode ist es, Standard- und Plasma-Renin während der Enzymreaktion (mit Schafsubstrat) und während des Angiotensin I-Radioimmunoassays (RIA) identisch zu behandeln. Die Plasma-Renin-Konzentration (PRC) wird in 10−6 MRC-Renineinheiten (µU/ml) angegeben. Ergebnisse: Der Renin-Standard ist stabil und frei von Angiotensin, Angiotensinasen und Angiotensinogen. Er zeigt gleiche Enzymcharakteristika wie Plasma-Renin. Eine Interferenz von endo- und exogenem Substrat konnte vermieden werden. Mögliche, gravierende Störungen des RIA durch Proteine der Enzymreaktionsansätze wurden nachgewiesen. Eine Eichung über einen Angiotensin I-Standard kann vermieden werden. Bei Anwendung einer Standard-Renin-Verdünnung von 0,9–250 µU/ml wird nahezu der gesamte biologische Bereich erfaßt. Unter freier Kost betrugen die vorläufigen Normalwerte für 30–35jährige in Ruhe 21,9±12,6 µU/ml, nach Orthostase 40,1±18,8 µU/ml (x±s,n=16). Frühere Beobachtungen über eine Altersabhängigkeit der PRC wurden bestätigt.

    Description / Table of Contents: Summary Inactive renin in normal human plasma was activated in vitro either by cryoactivation (incubation at 0° C/−5° C up to 3 months) or acid-activation (dialysis to pH 3.0 for 48 h followed by dialysis to pH 7.5). Plasma-renin-concentration was similar after either activation procedure (96±50µU/ml vs 100±43µU/ml). There was no further significant acid-activable renin after cryoactivation. These data suggest that conversion of inactive to active renin by optimized procedures is complete. The term „total renin“ for activation results under these conditions seems to be valid.
    Notes: Zusammenfassung Inaktives Renin in Normalplasmen wurde durch Kälte- (Inkubation bei 0° C/−5°C bis zu drei Monaten) oder Säureeinwirkung (Dialyse gegen pH 3,0 über 48 h, Rückdialyse gegen pH 7,5 über 24 h) aktiviert. Es fand sich kein signifikanter Unterschied der Plasma-Renin-Konzentration (PRC) nach beiden Aktivierungsverfahren. Maximal kälteaktiviertes Renin zeigte keinen weiteren signifikanten Anstieg durch Säureaktivierung. Die Ergebnisse zeigen, daß eine vollständige Reninaktivierung durch Optimierung der Methodik erreicht werden kann. Der Begriff „total renin“ für die Ergebnisse der PRC-Bestimmung nach Aktivierung scheint unter den hier gegebenen Bedingungen berechtigt.

    Description / Table of Contents: Summary Active renin was detected in 6 of 7 anephric patients (mean value: 0.72 ± 0.27 µU/ml,n=7; normals: 19.7 ± 2.4 µU/ml,n=10), using an assay method without intrinsic acid- or cryoactivation. Prorenin, measured as the difference between plasma renin concentration (PRC) before and after acid-activation in vitro, was present in considerable amounts in all patients (32.4 ± 3.5 µU/ml; normals: 80.7 ± 9.7 µU/ml). PRC after cryoactivation at −5°C during 1 week was insignificantly lower than after acid-activation in the patients. There was a linear correlation between PRC after either activation procedure (p 〈 0.01). Supine or upright posture did not influence active or inactive renin in the anephric subjects. Our studies show that low but significant levels of active renin and a considerable amount of prorenin are detectable in anephric patients. The data suggest that prorenin in normal subjects in a significant proportion is of extrarenal origin. Orthostasis has no effect on extrarenal active or inactive renin.
    Notes: Zusammenfassung Bei 6 von 7 anephrischen Patienten waren niedrige Reninspiegel im Plasma nachweisbar: 0,72 ± 0,27 µU/ml,n=7; (Normalpersonen: 19,7 ± 2,4 µU/ml,n=10). Prorenin, eine inaktive Reninvorstufe, die sich aus der Differenz der Plasma-Renin-Konzentration (PRC) vor und nach Säureaktivierung in vitro ergibt, war im Plasma aller anephrischen Patienten vorhanden: 32,4 ± 3,5 µU/ml (Normalplasmen: 80,7 ± 9,7 µU/ml). Nach Cryoaktivierung bei −5°C während 1 Woche war die PRC nur geringfügig weniger angestiegen als nach Säureaktivierung; zwischen beiden Aktivierungsverfahren bestand eine signifikante Korrelation (p 〈 0.01). Liegende Körperhaltung oder Orthostasebedingungen beeinflußten bei den anephrischen Patienten weder aktives noch inaktives Renin. Diese Untersuchungen zeigen, daß im Plasma anephrischer Personen niedrige, aber signifikante Reninspiegel und beträchtliche Prorenin-Konzentrationen vorhanden sind. Die Befunde lassen annehmen, daß Prorenin im Plasma von Gesunden zu einem wesentlichen Teil extrarenalen Ursprungs ist. Orthostase stimuliert offenbar nur renales Renin und hat keinen Effekt auf extrarenales Renin oder Prorenin.

    Notes: Summary A sensitive and specific radioimmunoassay for the measurement of arg-vasopressin (AVP) in human plasma is described. Recovery of added AVP from plasma was about 65–70%. An acetone extraction step was necessary to prevent unspecific blank effects. Sensitivity of the assay is 0.5 pg AVP/ml plasma. In normally hydrated subjects AVP-concentration ranged from 0.7 pg/ml to 5.8 pg/ml and showed a good correlation with plasma osmolality. In patients with complete diabetes insipidus (D.i.) AVP-values were below the sensitivity limit of the method and they were subnormal when D.i. was incomplete. There was no increase of AVP-concentration during fluid restriction in patients with complete or incomplete D.i. In subjects with psychogenic polydipsia AVP-values were normal and dehydration produced adequate rises of plasma AVP. In patients with SIADH (Schwartz-Bartter-syndrome) AVP-values were greatly enhanced (〉10 pg/ml) when correlated to plasma osmolality (〈170 mosmol/kg H2O).

    Description / Table of Contents: Zusammenfassung Bei Patienten mit einseitiger Nierenerkrankung und Hypertonie, vorwiegend einseitigen renovaskulären Erkrankungen, wurden der Nierenvenenreninquotient, die seitendifferente131Jod-Hippuran-Clearance und der Saralasin-Test untersucht und in ihrer Aussagefähigkeit verglichen: Dabei zeigte sich bei allen Patienten eine statistisch signifikante Zunahme des Nierenvenenreninquotienten mit zunehmendem Seitenunterschied des mittels131Jod-Hippuran-Clearance gemessenen renalen Plasmastroms. Der Nierenvenenreninquotient ist also nicht nur Ausdruck einer seitenunterschiedlichen Reninsekretion, sondern auch modifiziert durch die seitenunterschiedliche Durchblutung. Auch bei Angiotensin-unabhängigen Hypertonien bei einseitiger Nierenerkrankung ergaben sich bei starker Durchblutungsdifferenz beider Nieren Nierenvenenreninquotienten bis 2,0, ohne daß diese auf Hochdruckwirksamkeit hinwiesen. Bei geringerer Durchblutungsdifferenz beider Nieren zeigten dagegen schon niedrigere Nierenvenenreninquotienten eine einseitige Reninsekretion mit Hochdruckwirkung an. Nierenvenenreninquotienten im Grenzbereich zwischen 1,5 und 2,5 sollten deshalb nur unter gleichzeitiger Berücksichtigung der seitenunterschiedlichen Nierendurchblutung interpretiert werden. Patienten, bei denen auch unter Berücksichtigung der seitendifferenten Nierendurchblutung im Vergleich zur Angiotensin-unabhängigen Hypertonikergruppe erhöhte Nierenvenenreninquotienten gemessen wurden, zeigten im Saralasin-Infusions-Test ohne Ausnahme einen signifikanten Blutdruckabfall.
    Notes: Summary In patients with unilateral vascular kidney disease and hypertension, ratio of renal-vein-renin was compared with131I-Hippuric-acid clearance and change in blood pressure during Saralasininfusion. The ratio of renal-vein-renin was positively correlated with the ratio in renal plasma flow between the kidneys in all patients studied. The ratio of renins therefore is a result of two factors: The difference in renin secretion and the difference in blood flow in the two kidneys. In patients with angiotensin independent hypertension renin-ratios up to 2.0 were found without relevance to elevated blood pressure. When the difference in renal blood flow between both kidneys was small, even a slight difference in renal vein renin indicated hypertension related to increased renin secretion. Renin-ratios in the critical range between 1.5 and 2.5 should only be interpreted in respect to a similar ratio in renal blood flow.

    Description / Table of Contents: Zusammenfassung Erhöhte intraerythrozytäre Natriumkonzentration sowie ein beschleunigter Einstrom von Na22 in die Erythrozyten von Patienten mit essentieller Hypertonie hatten zu der Vermutung geführt, daß dieser Erkrankung eine Transportstörung für Natrium an der Zellmembran zugrunde liegen könnte. Wir fanden bei 27 Patienten mit essentieller Hypertonie eine signifikante Erhöhung der durch Strophantin hemmbaren Na-K-ATP'ase an Erythrozytenghosts im Vergleich zu 32 gesunden Kontrollpersonen. Die Na-K-insensible Mg-APT'ase was dagegen bei beiden Gruppen gleich. Dieser Befund ist mit einer Aktivierung des membrangebundenen Natriumtransportes an Erythrozyten von essentiellen Hypertonikern vereinbar.
    Notes: Summary Increased sodium concentration and high influx of Na22 are reported in erythrocytes of patients with essential hypertension. It was speculated, that these findings are due to a disturbed transport for sodium across red cell membranes. We found a significantly increased activity of the ouabainsensitive Na-K-ATP'ase in red cell ghosts of 27 patients with essential hypertension compaired with 32 normotensive controls. There existed no difference in Mg-ATP'aseactivity between the two groups. These findings suggest an increased activity of the Na-pump in red cell membranes of patients with essential hypertension.

    Description / Table of Contents: Summary In patients with grade I and II essential hypertension studied during sodium loading (Na+ excretion above 175 meq·d−1) we found a bimodal behaviour of aldosterone excretion and could distinguish two groups of patients: In the major part of essential hypertensives sodium loading led to a suppression of aldosterone excretion below 6 µg·d−1, which is the highest control value during sodium loading, with an average of 2.7±1.4 (SD) µg·d−1. Aldosterone excretion in a second group of patients was not suppressible below 6 µg·d−1 despite forced sodium loading; it resulted in an average value of 10.0±3.0 (SD) µg·d−1. During sodium deprivation or free sodium intake, aldosterone excretion in the first group of patients followed exactly the behaviour of normotensive controls, while in the second group of essential hypertensives the correlation of aldosterone excretion and log. Na excretion or log. Na+/K+ ratio in 24 h urine (r=−0.59) was far below the control value ofr=−0.87. Serum potassium concentration during sodium loading was significantly (p〈0.001) lower (3.81±0.44 meq·l−1) in the essential hypertensives with non-suppressible aldosterone excretion compared to those with suppressible aldosterone excretion (4.26±0.37 meq·l−1). The blood pressure response to treatment with 200 mg spironolactone·d−1 was better (p〈0.05) in patients with non-suppressible aldosterone excretion compared to the essential hypertensives with normal aldosterone regulation. The plasma renin activity of both groups of patients was not significantly different, however, a tendency prevailed towards lower PRA-values in the patient group with non-suppressible aldosterone excretion during sodium loading.
    Notes: Zusammenfassung Bei Patienten mit essentieller Hypertonie Grad I und II fand sich unter Salzbelastungsbedingungen (Natrium-Exkretion über 175 mval·d−1) eine bimodale Verteilung der Aldosteron-Exkretion, aufgrund welcher zwei Patientengruppen unterschieden wurden: Beim größeren Teil der Patienten mit essentieller Hypertonie führte die Salzbelastung zu einer Suppression der Aldosteron-Exkretion unter 6 µ·d−1, dem höchsten Kontrollwert unter Salzbelastung, im Durchschnitt zu 2,7±1,4 (SD) µg·d−1. Trotz ausgeprägter Salzbelastung über 6 Tage war die Aldosteron-Exkretion bei einer zweiten Gruppe nicht unter 6 µg·d−1 supprimierbar mit einer durchschnittlichen Aldosteron-Exkretion von 10,0±3,0 (SD) µg·d−1. Unter Salzentzug, freier Salzzufuhr oder Salzbelastung war die Aldosteron-Exkretion der ersten Gruppe der der Kontrollpersonen vergleichbar, während bei der zweiten Gruppe der essentiellen Hypertoniker die Korrelation zwischen Aldosteron-Exkretion und Log Natrium-Exkretion oder Log Natrium/Kalium-Quotient im 24 h-Urin (r=−0,59) deutlich unter der der Kontrollpersonen (r=−0.87) lag. Die Serum-Kalium-Konzentration unter Salzbelastung der zweiten Hypertonikergruppe mit nicht supprimierbarer Aldosteron-Exkretion war signifikant (p〈0,001) niedriger (3,81±0,44 mval·l−1) als bei den übrigen Hypertonikern mit supprimierbarer Aldosteron-Exkretion (4,26±0,37 mval·l−1). Die Blutdrucksenkung unter 200 mg Spironolacton täglich war bei den Patienten mit nicht supprimierbarer Aldosteron-Exkretion signifikant (p〈0,05) besser als beim Rest der Patienten. Die Plasmareninaktivität beider Gruppen war noch nicht signifikant unterschiedlich, zeigt jedoch eine Tendenz zu niedrigeren Werten in der Gruppe mit nicht supprimierbarer Aldosteron-Exkretion.

Geprüft wurde die Hypothese, ob akute Volumenverschiebungen innerhalb des Niederdrucksystems nach Furosemid zu einer neural vermittelten Reninstimulation führen. Sowohl Immersion in einem Wasserbad als auch Betablokkade reduzierte den Reninanstieg nach Furosemid nicht jedoch nach Ethacrynsäure. Es wird geschlossen, daß die akute Reninfreisetzung nach Furosemid, nicht jedoch nach Ethacrynsäure teilweise auf neuralen Reflexmechanismen — ausgehend von Volumenrezeptoren des Niederdrucksystems — beruht.
    Notes: Summary The mechanisms of renin release after furosemide (F) and ethacrynic acid (EA) in man were examined. We evaluated whether acute volume shifts within the low pressure system after F induce renin release via neural pathways. Immersion in a water-bath or beta-blockade reduced the increase of plasma renin concentration after F but not after EA. It is concluded that acute renin release after F but not after EA in man is partially due to neurally mediated reflexes originating from volume receptors in the cardiopulmonary area.

    Notes: Summary We measured basal and dexamethasone-suppressed plasma ACTH in 246 patients with bronchogenic carcinoma (105 with small-cell carcinoma); in 138 of these patients (67 with small-cell carcinoma) basal and pentagastrin-stimulated serum calcitonin was also determined. In addition, in a subgroup of 120 patients (58 with small-cell carcinoma) plasma ADH with reference to plasma osmolality was also assayed. Non-suppressible plasma ACTH was found in 45% of patients with small-cell carcinoma but only in isolated cases of large-cell carcinoma, adenocarcinoma, and squamous-cell carcinoma. Serum calcitonin was increased in 28% of patients with small-cell carcinoma but only in few patients with other tumor types. Stimulation of calcitonin by pentagastrin was ineffective. Plasma ADH was inappropriately high in 47% of patients with small-cell carcinoma. Strikingly high also was the incidence of increased ADH concentrations in patients with large-cell (40%), adenocarcinoma (46%) and squamous-cell carcinoma (29%). By measuring plasma ACTH after dexamethasone suppression and ADH with reference to osmolality, the sensitivity of these tumor markers in detecting pathological hormone secretion is markedly increased. In small-cell carcinoma the simultaneous measurement of ACTH, ADH, and calcitonin gives a high yield of positive results (74%), indicating that this set of tumor markers is a promising aid in diagnosis and therapy control.

    Notes: Summary Diabetic patients under multiple injection insulin therapy (i.e., intensified insulin therapy, IIT) usually start this treatment during hospitalization. We report here on the logistics, efficacy, and safety of IIT, started in outpatients. Over 8 months, 52 type I and type II diabetics were followed up whose insulin regimens consecutively had been changed from conventional therapy to IIT. Two different IIT strategies were compared: free mixtures of regular and intermediate (12 hrs)-acting insulin versus the basal and prandial insulin treatment with preprandial injections of regular insulin, and ultralente (24 hrs-acting) or intermediate insulin for the basal demand. After 8 months HbA1 levels had decreased from 10.6%±2.4% to 8.0%±1.3% (means±SD). There was no difference between the two regimens with respect to metabolic control; but type II patients maintained the lowered HbA1 levels better than type I patients. Only two patients were hospitalized during the follow-up time because of severe hypoglycemia. An increase of body weight due to the diet liberalization during IIT became a problem in one-third of the patients. Our results suggest that outpatient initiation of IIT is safe and efficacious with respect to near-normoglycemic control. Weight control may become a problem in IIT patients.
