    Notes: Abstract Thrombocypenic purpura was noted in a 3-year-old Japanese girl with an active varicella infection. The administration of intravenous gamma globulin (IVG) drastically improved bleeding manifestations as well as the active varicella infection. We believe that IVG should be the first choice of therapy for thrombocytopenic purpura associated with active infections.

    Notes: Abstract Two-dimensional colour Doppler echocardiography was performed on a 1-month-old male infant with criss-cross heart, double outlet right ventricle, ventricular septum defect and pulmonary stenosis. Complex structural abnormalities were suspected after two-dimensional echocardiography (2-D echo) and confirmed by colour Doppler and magnetic resonance imaging (MRI). We stress that the blood streams in the ventricular inflow tracts revealed by colour Doppler and the spatial relationships of the cardiac segments disclosed by MRI are essential to make an accurate non-invasive diagnosis of this complex malformation.

    Notes: Abstract We report here our findings in two Japanese siblings who experienced recurrent bacterial and viral infections since early infancy. Recent symptoms included diarrhoea, conjunctivitis, rashes, headache, sore throat, joint pain, vomiting and vertigo, all similar to those seen in toxic shock syndrome, except for shock. These symptoms improved following gammaglobulin treatment.Staphylococcus aureus with coagulase type IV was continuously isolated from nasal smears producing toxic shock syndrome toxin-1 (TSST-1). Serum antibodies did not or only poorly responded to TSST-1, diphtheria toxoid, varicella virus and rubella virus, whereas total and subclass levels of serum immunoglobulin and in vitro DNA synthesis of lymphocytes stimulated by TSST-1,Staph. aureus, varicella vaccine and mitogens were normal. In the family, ten other members in three generations (five males: five females) including the mother had similar clinical symptoms. Thus, the disease may be inherited in an autosomal dominant fashion.

    Notes: A preliminary study was undertaken to provide clinical evidence to support the hypothesis that: “Migraine with aura, migraine without aura and aura alone are the same condition, which differ in degree rather than pathophysiology.” At the City of London Migraine Clinic, 50 patients consecutively attending the clinic with a past or present history of migraine with aura were questioned. Of the 50 patients questioned 36 (70%) had a combination of migraine with aura, migraine without aura andor aura alone; i.e. 70% had had more than one type of migraine attack. The duration, severity and frequency of attacks did not differ between migraine with and migraine without aura. Conclusion-the results support the hypothesis that migraine with and migraine without aura, and aura alone are not separate conditions, because: (1) most patients suffer from more than one type of migraine attack; (2) there are no significant differences in the characteristics of the migraine attacks in the different groups; (3) there are no significant differences in the characteristics of the subjects.

    Notes: Purpose - To analyze effectively magnetic shielding effects by shields with fine structure. Design/methodology/approach - Simplification of the fine structure makes it possible to analyze them efficiently. The authors have introduced a homogenization method to estimate effective permeability of magnetic composite structure for the static field. The homogenization method is applied to the analysis of magnetic shields composed of steel plates and steel rods against DC power lines to test its feasibility. Findings - The properties of the magnetic shielding are analyzed by using the homogenization method. The errors of the magnetic fields increase in case of very few layers. Originality/value - The simplification of the magnetic shields with fine structure by using the homogenization method makes it possible to analyze efficiently magnetic shielding effects, although the accuracy becomes worse in case of very few layers.

    Notes: The Preisach and Jiles models for hysteresis are applied to reconstruct BH loops from measurement data for constructional steel St 37. The distribution function for the Preisach model is determined from all the available, 18, measured BH loops starting from the initial curve. The five unknown parameters in Jiles model are determined by the simulated annealing method to minimize the distance between the largest measured BH loop and the corresponding computed loop. Although Jiles model gives differences from the measured BH loops for low applied fields, it provides results fitted well to the largest measured loop for which the parameters are optimized. The Preisach model gives good fitting over a wide range of the applied field.

    Notes: This paper describes a boundary element analysis of magnetic shieldings for electron microscopes. Since the thickness of the shielding layer is considerably small compared with its overall size, numerical analysis of electromagnetic fields inside the layer leads to an ill-conditioned matrix. This problem can be overcome by analytical evaluation of the interior electromagnetic field, which yields the impedance boundary condition (IBC) valid for static and eddy current fields, which expresses the relationship between the electromagnetic fields on both surfaces of the layer. In this paper the magnetic fields around a shielding layer are analyzed by the boundary element method under the IBC on the shielding layer. Two-dimensional and axisymmetric magnetic fields are analyzed to evaluate the shielding efficiency of shielding immersed in an ac magnetic field. It is shown that magnetic disturbances can be reduced to less than one-hundredth inside a shielding consisting of double shielding layers.
