    Notes: Summary Existing methods for the treatment of chronic lymphoedema of the extremities by surgery, are briefly reviewed. The results of 20 years experience using the buried dermis flap are presented, as applied to 140 patients, affecting 102 lower limbs and 49 upper. The need for prolonged follow-up is stressed. In the lower limb 88 limbs affected by primary lymphoedema resulted in 51% good results and 32% significantly improved; the corresponding figures in 14 limbs with secondary lymphoedema were 57% and 14%. — In the upper limb, of 49 arms treated, good results were obtained in 61% and significant improvement in 18%. In all cases treated, recurrent attacks of cellulitis were reduced or abolished in the majority. Complications to operation are discussed; the commonest is localised skin flap necrosis, occuring in almost half the cases treated, but requiring formal reoperation and skin grafting in only a minority. — Evidence is submitted that the long-term results obtained, taken into consideration together with the cosmetic values and relative simplicity of the operation, makes it the procedure of choice in the surgical management of this condition.

    Notes: Abstract The addition of autoclaved mycelia of Aspergillus niger and the known phytopathogenic fungus Phytophtora cinnamomi to cultured cells of Cinchona ledgeriana Moens. caused a marked increase in the anthraquinone content of the plant cells. This finding in combination with the antimicrobial activity of the anthraquinones isolated from calli of Cinchona pubescens Vahl. led to the conclusion that anthraquinones are phytoalexins.

    Notes: White mica from the Liassic black shales and slates in Central Switzerland was analysed by transmission electron microscopy (TEM) and electron microprobe to determine its textural and compositional evolution during very low-grade prograde metamorphism. Samples were studied from the diagenetic zone, anchizone and epizone (T ≈100°–450 °C). Phyllosilicate minerals analysed include illite/smectite (I/S), phengite, muscovite, brammallite, paragonite, margarite and glauconite. Textural evolution primarily is towards larger, more defect-free grains with compositions that approach those of their respective end-members. The smectite-to-illite transformation reduced the amounts of the exchange components SiK−1Al−1, MgSiAl−2, and Fe3+Al−1. These trends continue to a lesser degree in the anchizone and epizone. Correlations between the proportion of smectite in I/S and the composition of I/S indicate that smectite layers may contain a high layer charge. Illite in I/S bears a compositional resemblance to macrocrystalline phengite in some samples, but is different in others. Paragonite first appears in the upper diagenetic zone or lower anchizone as an interlayer-deficient brammallite, and it may be mixed with muscovite on the nanometre scale. Owing to the small calculated structure factor for paragonite-muscovite superstructures, conventional X-ray powder diffraction cannot distinguish between mixed-layer structures and a homogeneous compositionally intermediate solid solutions. However, indirect TEM evidence shows that irregularly shaped domains of Na- and K-rich mica exist below 10 nm. Subsequent coarsening of domains at higher grades produced discrete paragonite grains at the margins of muscovite crystals or in laths parallel to the basal plane of the host muscovite. Margarite appears in the epizone and follows a textural evolution similar to paragonite in that mixtures of margarite, paragonite, and muscovite may initially occur on the nanometre scale. However, no evidence of interlayer-poor margarite has been found.

    Notes: Summary This paper describes a survey of British Community Mental Health Centres (CMHCs). CMHCs are by far the most visible manifestation of the implementation of community mental health care policies of the 1980s. While these centres have demonstrated achievements in terms of accessibility, co-ordination and responsiveness to clients, they have also been bedevilled by ambiguity. The vast majority of CMHCs aim to serve the needs of all forms of mental illness within their catchment area, yet in practice, they have frequently failed to meet the needs of people with long term severe disorders and concentrate instead on providing assessment and counselling services for neurotic and transient situational disorders. It appears that this is a function of an early failure of multidisciplinary teams to delineate boundaries and priorities. If the British CMHC movement sharpens its focus and combines this with a determination to address the complexities involved in both rationing and enhancing choice, it could begin to improve upon rather than simply repeat the history of similar developments in America.

    Notes: Abstract This paper describes methods by which strong vacuum-tight bonds between gold and alumina can be fabricated in an air atmosphere. Temperature was found to be a sensitive factor in bond formation; the higher the temperature, the stronger the bond, provided that the temperature remained below the melting point of gold. Intimate contact between the bonding gold and alumina surfaces during formation of the bond was also an important consideration. This was achieved by using an optically flat and polished ceramic surface and applying a low pressure of about 1 MPa to the bonding couple. Further enhancement of the contact can be achieved by depositing an additional thin layer of gold onto the ceramic surface by evaporation or by other means, and by thorough cleaning by high temperature heat treatment prior to bonding. Good bonding occurs in the range of 1 to 100 h. Bond strengths as high as 80 MPa were achieved.
