    Notes: In an attempt to create uniform nationwide guidelines for the management of all stages of endometrial carcinoma, and to limit the use of adjuvant radiation therapy in stage I disease to high-risk patients only, a protocol was developed by the Danish Endometrial Cancer group (DEMCA). From September 1986 through August 1988, 1214 women in Denmark with newly diagnosed carcinoma of the endometrium have been treated according to this protocol. This figure represents all endometrial carcinomas diagnosed in Denmark during this 2-year period. The primary treatment was total abdominal hysterectomy and bilateral salpingo-oophorectomy and no preoperative radiation therapy was delivered. In 1039 cases no macroscopic residual tumor and/or microscopic tumor tissue in the resection margins was found following surgery. Based on surgery and histopathology, these patients were classified as: P-stage I low-risk (grade 1 & 2 and 〈inlineGraphic alt="leqslant R: less-than-or-eq, slant" extraInfo="nonStandardEntity" href="urn:x-wiley:1048891X:IJG06010038:les" location="les.gif"/〉 50% myometrial invasion), P-stage I high-risk (grade 1 & 2 and〉 50% myometrial invasion, and grade 3), P-stage II and P-stage III (Group 1). Distribution was as follows: P-I low-risk 641 patients, P-I high-risk 235, P-II 105 and P-III (Group 1) 58 patients. No postoperative radiation therapy was given to P-I low-risk cases. P-I high-risk, P-II, and P-III (Group 1) cases received external radiation therapy. Recurrence rate at 68–92 months follow-up was 45/641 (7%) in P-I low-risk, 36/235 (15%) in P-I high-risk, 30/105 (29%) in P-II, and 27/58 (47%) in P-III (Group 1) cases. Fifteen of 17 vaginal recurrences in P-I low-risk cases were salvaged (mean observation time 61 months).

    Notes: Abstract. The study comprises 66 patients with stage III and IV serous ovarian carcinomas of whom 64 had CA-125 positive tumors.While the percentage of CA-125 positive cells and the staining intensity showed no correlation to patient survival, the pattern of CA-125 distribution, membranous versus cytoplasmic, was significantly correlated to survival. In accordance with other observations on switching of the antigen reaction from membrane to cytoplasm with increasing grade of atypia or malignancy, patients with membrane positive CA-125 tumors had a significantly better prognosis than did patients with CA-125 in the cytoplasm.Furthermore, there was excellent agreement between, on the one hand, the binding pattern of CA-125 and, on the other, the ploidy of the tumor cells and the HGI histopathologic grading index, both previously shown to be of prognostic significance.

    Notes: The prognostic value of the volume-weighted mean nuclear volume (MNV) was investigated retrospectively in 100 ovarian cancer patients with FIGO-stage IB–II (n = 51) and stage III–IV (n = 49) serous tumors. No association was demonstrated between the MNV and the survival or between the MNV and two of five additional parameters: age and amount of residual tumor. However, both in stage IB–II and stage III–IV carcinomas, the MNV increased significantly with increasing tumor grade and DNA-index. Furthermore, FIGO-stage IB–II patients had significantly smaller nuclei than patients with stage III–IV disease.

    Notes: Summary The DNA distribution has been investigated in biopsies from human bladder carcinomas by means of flow cytometry (FCM). The FCM analysis was based on a preparation method yielding a suspension of single cell nuclei. Biopsies from 45 patients were analysed. The results demonstrate a correlation between the degree of cellular anaplasia and the occurrence of cell populations with increased DNA content in the tumours. Cells with diploid DNA content constituted more than 90% of well-differentiated tumours and normal bladder mucosa. Cell populations with aneuploid DNA content occurred in almost all biopsies with cellular anaplasia estimated as grades II and III by microscopic examination. The results indicate that FCM analysis may be a valuable adjunct to histology. The possible prognostic significance of altered DNA distribution in bladder tumours is discussed.

    Notes: Abstract Aortoliac angiography has always been an integral part of the pretransplantation work-up of renal transplant candidates in Norway. The present study was undertaken to investigate the value of this routine. Based on the angiograms of approximately 1400 patients evaluated for renal transplantation during the 7-year period 1984–1991, 26 were found to have aortic and/or iliac atherosclerosis requiring pretransplant vascular reconstruction. Fifteen of the 26 patients had aneurysm of the abdominal aorta and 11 had extensive aortoiliac occlusive disease. A prosthetic graft was inserted in 25 patients and endarterectomy of the aortic bifurcation was performed in one. The cause of death was coronary heart disease in four of six patients who died before, and in one patient who died after, transplantation. Sixteen patients received a renal transplant while four patients are still on the waiting list. Fifteen of the recipients are alive, 14 with functioning renal transplants. The low yield of patients below 40 years of age requiring vascular reconstruction calls into question the routine use of angiographic investigation of renal transplant candidates below this age. However, we recommend this routine for the higher age groups because it often provides the surgeon performing the transplantation with valuable information. Aortoiliac reconstruction as preparation for renal transplantation is advocated when atherosclerosis of a degree that may preclude transplantation is found. Because of the high risk of myocardial infarction in these patients, one must be especially aware of coronary atherosclerosis when evaluating patients for this procedure.
