    Description / Table of Contents: Abstract Extensive Catabolism is a hallmark of patients with acute renal failure (ARF) complicating critical illnesses. Catabolism is due to dysregulation of protein metabolism as well as a consequence of deminished renal excretion and renal replacement therapy (RRT). Inadequate nutritional support predisposes patients to malnutrition and increased mortality risk. Since the catabolic rate varies widely in ARF patients and can not be predicted by clinical parameters, direct quantification of the protein catabolic rate should be performed regularly. The urea nitrogen appearance rate (UNA) is a valid and reproducible estimate of nitrogen balance in critically ill patients undergoing continuous RRT. Amino acid losses up to 50 g/day and protein losses up to 15 g/day occur during continuous RRT and need to be compensated for. In order to achieve neutral or positive nitrogen balance a nutritional regimen providing 1.5–2.0 g protein/kg/day and 25–35 kcal/ kg/day may be required. Since glutamine losses during continuous RRT may exceed 4 g/day, glutamine supplementation (0.3–0.5 g/kg/day) appears to be recommendable.
    Notes: Zusammenfassung Ein schwerer Katabolismus tritt häufig bei kritisch Kranken auf, bei denen sich zusätzlich zu Sepsis, Trauma oder SIRS ein akutes Nierenversagen entwickelt. Primär ausgelöst durch Störungen im Eiweiß-Stoffwechsel, kann der Katabolismus jedoch durch die verminderte renale Exkretionsrate und die Anwendung verschiedener Nierenersatzverfahren aggraviert werden. Eine unzureichende Deckung des Nährstoffbedarfs fördert die Entwicklung einer Malnutrition und steigert das Mortalitätsrisiko. Die tatsächliche Katabolierate kann klinisch nicht abgeschätzt werden, und sollte daher anhand der Harnstoff-Stickstoff-Erscheinungsrate (UNA) quantifiziert werden, deren Berechnung einfach und gut reproduzierbar ist. Unter der Verwendung kontinuierlicher Nierenersatzverfahren ist eine komplette, an die Bedürfnisse des katabolen Patienten individuell angepasste Ernährung möglich. Kontinuierliche Nierenersatzverfahren können zu einem bedeutsamen Verlust an Aminosäuren (bis 50 g/Tag) und Proteinen (bis 15 g/Tag) führen, der bei der Ernährungsstrategie berücksichtigt werden sollte. Eine hohe Eiweißzufuhr von 1,5 bis 2,0 g/kg/Tag sollte mit einer niedrig-normalen Energiezufuhr von 25-kcal/kg/Tag kombiniert werden. Wegen der mit kontinuierlichen Verfahren verbundenen Glutaminverluste (bis 〉 4 g/Tag) scheint eine Glutaminsubstitution (0,3–0,5 g/kg/Tag) empfehlenswert.

    Description / Table of Contents: Zusammenfassung 36 Patienten mit unterschiedlicher Nierenfunktion (GFR zwischen 0,5–142 ml/min) erhielten 500 ml 10%iges Dextran 40 (Rheomacrodex®). Dabei ergab sich ein relatives Verteilungsvolumen von 10,2±2,2 1/100 kg. Die Eliminationshalbwertzeiten von Dextran 40 stehen mit der Nierenfunktion (51 Cr-EDTA-Clearance) in einem Zusammenhang, der sich rechnerisch am besten durch eine Potenzfunktion darstellen läßt. Patienten mit normaler Nierenfunktion (GFR über 90 ml/min) haben eine Eliminationshalbwertzeit von 573±138 min und eine 12 Std-Recovery im Urin von 48%. Ein Glomerulusfiltrat unter 10 ml/min führt dagegen zur Verlängerung der Halbwertzeit auf das Fünffache (Dextran 40-HWZ=2591±1022 min) und zum Rückgang der 12 Std-Recovery auf 4%. Im Clearancebereich zwischen 30 und 142 ml/min bleiben Eliminationshalbwertzeit und Urinausscheidung im wesentlichen konstant. Dextran 40 kann deshalb bei diesen Patienten nach den bekannten Regeln dosiert werden. Bei glomerulären Filtrationswerten unter 30 ml/min ist jedoch mit einer gestörten Elimination zu rechnen. Aus der veränderten Dextran 40-Ausscheidung im Urin und aus den Eliminationshalbwertzeiten ergeben sich Dosierungsrichtlinien, die diskutiert und tabellarisch aufgeführt werden.
    Notes: Summary 36 patients with different renal function (GFR=0.5−142 ml/min) received 500 ml of 10% dextran 40 (Rheomacrodex®). The relative distribution volume proved to be 10.2±2.21/100 kg. Dextran 40 half life time and renal function are correlated mathematically as a power function. In patients with normal renal function (GFR〉90 ml/min) we determined a dextran 40 half life time of 573±138 min and a 12 hour urinary recovery of 48%. In contrast to that a glomerular filtration rate below 10 ml/min prolongs the half-life five times to a value of 2591±1022 min and reduces the urinary elimination to 4% per 12 hours. In patients with a clearance between 30 and 142 ml/min no essential changes of half life time and urinary elimination can be observed. To those patients dextran 40 can be applied in the common dosage. Contrary to that a glomerular filtration rate below 30 ml/min leads to a prolonged elimination and consequently a different dosage has to be applied.

    Description / Table of Contents: Zusammenfassung Nach Entzug von 400 ml Vollblut und anschließender Infusion von 500 ml eines kolloidalen Plasmaersatzmittels wurde bei 40 Probanden die intravasale und renale Kolloidelimination untersucht. Die einzelnen kolloidalen Lösungen waren nach folgenden Zeiträumen nicht mehr im Intravasalraum nachweisbar: 10% Hydroxyäthylstärke 200/0,5 (Anthron-Methode) nach 6 Wochen, 10% Dextran 40 (Anthron-Methode) nach 2 Wochen, 6% Hydroxyäthylstärke 200/0,5 (Anthron-Methode) nach 4 Wochen und 5,5% Oxypolygelatine (Hydroxyprolin-Methode) nach-2 Tagen. Kolloidale Plasmaersatzmittel stellen polydisperse Lösungen mit unterschiedlichem Molekulargewicht und Hydroxyäthylierungsgrad und deshalb auch mit einer Vielzahl unterschiedlicher Eliminationskonstanten dar. Bei wiederholter Applikation verschiebt sich die intravasale Kolloidkonzentration zu Gunsten der schwer eliminierbaren Moleküle mit längerer Halbwertzeit. Die Elimination der klinisch eingesetzten Dextran 40- und Oxypolygelatine-Lösung ließ sich am besten mit einem offenen 2-Kompartiment-Modell beschreiben. Die Elimination der mittelmolekularen Hydroxyäthylstärke 200/0,5 war als Ausdruck ihrer größeren Heterogenität auch unter Annahme von drei Eliminationskonstanten nur näherungsweise zu charakterisieren. In den ersten 4 Tagen wurde die Hydroxyäthylstärke 200/0,5 im Vergleich zu Dextran 40 rascher eliminiert. Anschließend aber fand sich für ca. 3% der zugeführten Hydroxyäthylstärke 200/0,5 eine wesentlich langsamere Elimination aus dem Intravasalraum. Besonders schnell wurde Oxypolygelatine ausgeschieden. Dementsprechend fand sich für Oxypolygelatine die größte renale Clearance, die eine enge Beziehung zum Molekulargewicht zeigte. Andererseits bedeutet eine rasche Elimination gleichzeitig eine entsprechend geringere Volumenwirkung.
    Notes: Summary After withdrawal of 400 ml whole blood and subsequent infusion of 500 ml of a colloidal plasma substituent, the intravascular and renal colloid elimination was investigated in 40 test subjects. The individual colloidal solutions could no longer be demonstrated in the intravascular space after the following times: 10% hydroxyethyl starch 200/0.5 (anthrone method) after six weeks, 10% dextran 40 (anthrone method) after two weeks, 6% hydroxyethyl starch 200/0.5 (anthrone method) after four weeks and 5.5% oxypolygelatine (hydroxyproline method) after two days. Colloidal plasma substitutes are polydisperse solutions with various molecular weights and degree of hydroxyethylation and therefore, also have a large number of different elimination constants. With repeated application, the intravascular colloid concentration shifts in favour of the molecules with a longer half life which are difficult to eliminate. The elimination of the clinically employed dextran 40 and oxypolygelatine solution could be best described with an open two-compartment model. As a result of its greater heterogeneity, the elimination of the moderately high molecular weight hydroxyethyl starch 200/0.5 could only be characterized approximately even assuming three elimination constants. In the first four days, the hydroxyethyl starch 200/0.5 was more rapidly eliminated compared to dextran 40. However, subsequently a very much lower elimination from the intravascular space was found for about 3% of the administered hydroxyethyl starch 200/0.5. Oxypolygelatine was eliminated especially rapidly. Accordingly, the greatest renal clearance was found for oxypolygelatine, which showed a close relation to the molecular weight. On the other hand, a rapid elimination simultaneously is followed by a correspondingly lower volume effect.

    Notes: Summary Herpes simplex virus (HSV) has been associated with various neurological disorders. In contrast, HSV infection is very rarely found in acute polyneuroradiculitis. In this report, a patient is described with a severe course of Guillain-Barré syndrome (GBS). HSV IgM-specific antibodies and a rise of complement-fixation antibodies were detected. During the acute phase of neurologic syndrome, a nerve biopsy showed myelin damage and IgM deposits on the inner layer of the perineurium. Plasma exchange, in combination with immunosuppression, was successfully applied as a treatment in the relapsing course of GBS. Finally, after recovery, HSV-specific IgM antibodies disappeared.

    Notes: Summary Three cases with renal angiomyolipoma in Bourneville's disease are reported. In two patients advanced renal insufficiency developed, one being treated by chronic intermittent hemodialysis. In the third patient rupture of angiomyolipoma with severe retroperitoneal bleeding occurred. In one patient, tumorous enlargement initially was diagnosed only in the right kidney and preceded angiomyolipoma of the left kidney for several years. In this patient nephrectomy was performed because malignancy was assumed before diagnosis of Bourneville's disease was established. Occasionally, histology may suggest sarcomatous changes. However, the absence of distant metastasis and the overall long survival of patients with renal angiomyolipoma in Bourneville's disease underline the benign character of the tumor. Treatment should be conservative because surgical intervention with loss of kidney parenchyma may enhance progression to end-stage renal failure.

    Description / Table of Contents: Zusammenfassung Bei Patienten mit chronischer alkoholischer Lebererkrankung kommt es zum Anstieg von Immunglobulin A im Serum mit überproportionaler Zunahme der polymeren Form sowie zum Auftreten von IgA-haltigen Immunkomplexen. Die Ursachen liegen einerseits in einer alkoholinduzierten intestinalen Permeabilitätssteigerung für Makromoleküle mit Stimulierung der lokalen IgA-Immunabwehr, andererseits in einer verminderten hepatischen Clearance von polymerem IgA. Für die Störung der hepatischen Elimination dürfte ein hepatozellulärer, von der sekretorischen Komponente unabhängiger Mechanismus verantwortlich sein. Bei annähernd der Hälfte der Patienten mit alkoholischer Leberzirrhose findet sich in den Glomeruli, vorwiegend mesangial, polymeres und monomeres IgA, z.T. in Form von Immunkomplexen. Diese IgA-Ablagerungen gehen nur selten mit einer Urinsymptomatik einher. Welche Faktoren dazu führen, daß glomeruläre IgA-Ablagerungen eine Glomerulonephritis zur Folge haben, ist nicht geklärt.
    Notes: Summary Patients with chronic alcoholic liver disease show an increase in serum IgA with a preferential elevation of polymeric IgA. Furthermore, immune complexes containing IgA can be demonstrated. These changes may be explained by an alcohol-induced increase in intestinal permeability to macromolecular antigens leading to an IgA response, and by a decreased hepatic clearance of polymeric IgA not dependent on a secretory component. The prevalence of glomerular IgA deposits amounts to 50% in patients with alcoholic cirrhosis examined at autopsy. Polymeric and monomeric IgA, partially as immune complexes can be detected. However, urinary symptoms very rarely occur. The factors leading to glomerulonephritis following IgA deposition are poorly understood.

    Description / Table of Contents: Zusammenfassung Blutrückstand, Clearance, Effektivität und Handhabung der HFAK Typ 4 werden im Routinebetrieb eines Dialysezentrums einmal a) bei nicht entgastem Dialysat und Heparinisierung am Anfang der Dialyse sowie b) bei entgastem Dialysat und einer systemischen Heparinisierung ca. 5 min vor Dialysebeginn geprüft. Bei Versuchsbedingung A werden im Mittel 17,9 ± 12,9 ml Blutrückstand gefunden. Das entspricht einem Volumen von ca. 18% des Blutkompartments der HFAK 4. Bei Versuchsbedingung B wurde ein mittlerer Blutrückstand von 6,3 ± 2,8 ml gefunden, was 6% des Blutkompartments ausmacht. Dementsprechend war die Effektivität bei Versuchsanordnung A um 12% geringer als bei B. Die Clearancewerte für Kreatinin lagen bei einem Blutdurchfluß von 200 ml/min bei 114 und für Harnstoff bei 146 ml/min. Die Handhabung im klinischen Routinebetrieb ist komplizierter als bei vergleichbaren Einmaldialysatoren. Bei sorgfältiger Einhaltung einer systemischen Heparinisierung vor Dialysebeginn und einer effektiven Dialysatentgasung bietet die neue HFAK 4 Vorteile gegenüber den herkömmlichen Einmaldialysatoren.
    Notes: Summary Residual blood volume, clearance and effectivity of the Hollow Fiber Artificial Kidney 4 (HFAK 4) was studied by labelling red blood cells by 51-Cr in 84 routine dialyses under two conditions: a) with not deaerated dialysat and start of heparinisation at the beginning of the dialysis b) with deaerated dialysat and systemic heparinisation 5 minutes before beginning of the dialysis. Under condition (a) we obtained a mean of 17.9 ± 12.9 ml residual blood in the HFAK 4. This corresponds to 18% of the whole priming volume. Under condition (b) we measured a residual blood volumen of 6.3 ± 2.8 ml, which is 6% of the priming volumen. Consecutively the effectivity under condition (a) was 12% less than in (b). The clearances of creatinin, BUN and uric acid were higher in comparison to other dialysers at the exception of the Meltec Multipoint (1 m2) Kiil.
