    Description / Table of Contents: Abstract We examined the efficacy of the vertical infraclavicular block for plexus brachialis anaesthesia using a nerve stimulator after introducing the method (VIP1) and after three years of clinical experience (VIP2). In two prospective studies we compared the results with each other as well as with the efficacy of the axillary block (AX). At VIP1, we found a complete analgesia in 88% of the patients, whereas in 9% a supplementation was needed. In group AX the results were significantly worse (complete: 70%, supplementation: 24%; p〈0.001). No increase of the rate of efficacy could be found when having some clinical experience with the VIP (VIP2: complete 87%, supplement: 11%). In general, the results of the VIP depended on the motoric answer to the nerve stimulation. There were no complications of the VIP such as nerve lesions or pneumothorax. The VIP using a nerve stimulator is a simple, reliable and uncomplicated method for plexus-brachialis-anaesthesia, which is easy to learn.
    Notes: Zusammenfassung Zur Plexus-brachialis-Anästhesie durch den vertikalen infraklavikulären Zugang (VIP) mittels Nervenstimulation wurde in zwei prospektiven Studien die Erfolgsrate bei Neueinführung des Verfahrens (VIP1) sowie nach dreijähriger klinischer Erfahrung geprüft (VIP2) und untereinander sowie mit der Erfolgsrate der axillären Blockadetechnik mittels Nervenstimulation (AX) verglichen. Untersucht wurden jeweils ca. 100 Patienten. Bei VIP1 zeigte sich ein kompletter Blockadeerfolg in 88%, eine Notwendigkeit zur Supplementierung in 9% bzw. zum Verfahrenswechsel in 3% der Fälle. Die Ergebnisse der axillären Plexusblockade waren signifikant schlechter (AX: komplett 70%, Supplementierung 24%; p〈0,001). Die Erfolgsrate des VIP ließ sich durch längere klinische Erfahrung nicht steigern (VIP2: komplett 87%; Supplementierung 11%; Verfahrenswechsel 2%). Der Blockadeerfolg war abhängig vom Ort der motorischen Antwort auf die Nervenstimulation. Komplikationen des VIP wie Nervenschädigung oder Pneumothorax wurden nicht beobachtet. Die vertikale infraklavikuläre Blockade mittels Nervenstimulation ist als einfache, leicht zu erlernende, zuverlässige und komplikationsarme Methode zur Plexus-brachialis-Anästhesie einzuschätzen.

    Description / Table of Contents: Abstract A 50 year old female patient received anaesthesia of the arm by the vertical infraclavicular blockade of the plexus brachialis (VIP). Postoperatively an ipsilateral pneumothorax occured complicated by pleural effusion and a contralateral bronchopneumonia, which resolved completely after treatment. The blockade of the plexus was performed correctly, failures in determining the correct point of needle insertion could be excluded. Therefore a pneumothorax has to be regarded as a specific complication of the VIP, which might occur despite correct technique, and requires that the patient be informed of this eventuality. Nevertheless, the VIP is an important method due to its high success rate concerning blockade of the musculocutaneous nerve and tolerance of tourniquet. The risk of a pneumothorax is about 0.2 to 0.7%.
    Notes: Zusammenfassung Bei einer 50-jährigen Patientin wurde im Rahmen einer elektiven Operation an der Hand die Anästhesie durch die Vertikale Infraklavikuläre Plexus-brachialis Blockade (VIP) nach Kilka durchgeführt. Postoperativ kam es zum Auftreten eines ipsilateralen Pneumothorax, welcher durch einen Pleuraerguss und eine kontralaterale Bronchopneumonie kompliziert wurde. Unter invasiver Behandlung der Patientin kam es bezüglich aller Komplikationen zur restitutio ad integrum. Die Durchführung der Anästhesie war lege artis entsprechend der Erstbeschreibung, Fehler bei der Bestimmung der Punktionsstelle konnten mit größtmöglicher Wahrscheinlichkeit ausgeschlossen werden. Ein Pneumothorax bei der Vertikalen Infraklavikulären Plexus-brachialis Blockade stellt auch bei korrekter Durchführung unter Vermeidung von beschriebenen Kardinalfehlern ein methodenspezifisches und somit aufklärungspflichtiges Risiko dar. Der VIP ist dennoch eine wichtige Ergänzung der herkömmlichen Methoden zur Blockade des Plexus brachialis, da er sich durch eine hohe Erfolgsrate speziell im Bereich des N. musculocutaneus und in der Toleranz eines Tourniquets auszeichnet. Nach den bisherigen Erfahrungen ist von einem Pneumothoraxrisiko von 0,2–0,7% auszugehen.

    Description / Table of Contents: Abstract Incomplete anaesthesia is a major clinical problem both in single spinal and in single epidural anaesthesia. The clinical efficacy of epidural anaesthesia with augmentation (aEA) and combined epidural and spinal anesthesia (CSE) for cesarean section was investigated in a prospective randomized study on 45 patients. Methods: Anaesthesia extending up to Th5 was aimed for. Depending on the patient’s height, epidural anaesthesia was administered with a dose of 18–22 ml 0.5% bupivacaine and spinal anaesthesia with a dose of 11–15 mg 0.5% bupivacaine. Augmentation was carried out in all cases in epidural anaesthesia, initially with 7.5 ml 1% Lidocaine with epinephrine 1:400000, raised by 1.5 ml per missing segment. The epidural re-injection in CSE was carried out as necessary with 9.5–15 ml 1% lidocaine with epinephrine, depending on the height and difference from the segment Th5. Results: The extension of anaesthesia achieved in epidural anaesthesia after an initial dose of 101.8 mg bupivacaine and augmenting dose of 99 mg lidocaine reached the segment Th5. The primary spinal anaesthesia dose up to 15 mg corresponding to height led to a segmental extension to a maximum of Th3 under CSE. Augmentation was necessary in 13 patients; in 5 cases because of inadequate extent of anaesthesia and 8 cases because of pain resulting from premature reversion. The augmenting dose required was 13.9 ml. Readiness for operation was attained after 19.8 min (aEA) and after 10.5 min (CSE). No patient required analgesics before delivery. The additional analgesic requirement during operation was 63.6% (aEA) and 39.1% (CSE). Taking into account pain in the area of surgery, the requirement of analgesics was 50% (aEA) vs. 17.4% (CSE). Antiemetics were required in 18.2 (aEA) and in 65.2% (CSE). The systolic blood pressure fell by 17.7% (aEA) and in 30.3% (CSE). The minimum systolic pressure was observed after 13.4 min in aEA, and after 9.5 min in CSE. The APGAR score and the umbilical pH did not show any differences. General anaesthesia was not required in any case. A single epidural anaesthesia or a single spinal anaesthesia with isobaric 0.5% bupivacaine are not reliable as the sole, nonaugmentable anaesthesia. Isobaric spinal anaesthesia primarily achieves an appropriate extent of anaesthesia, but often also requires supplementary epidural reinjection for complete analgesia. An augmentation proves to be appropriate primarily with 0.5% bupivacaine in aEA. The operation can be commenced earlier with CSE.
    Notes: Zusammenfassung Inkomplette Anästhesien stellen sowohl bei einzeitiger Spinal- wie einzeitiger Epiduralanästhesie ein wesentliches klinisches Problem dar. Methodik: Die Wirksamkeit von Epiduralanästhesie mit Augmentation (aEA) (n=22) und kombinierter Spinal-Epiduralanästhesie (CSE) (n=23) zur Sectio caesarea wurde prospektiv und randomisiert an 45 Patienten untersucht. Angestrebt wurde eine Anästhesieausdehnung bis Th5. Jeweils abhängig von der Körpergröße wurde die Epiduralanästhesie mit Bupivacain 0,5% mit einer Dosis von 18–22 ml, die Spinalanästhesie mit einer Dosis von 11–15 mg Bupivacain 0,5% isobar durchgeführt. Eine Augmentation erfolgte bei der aEA in allen Fällen zuerst mit 7,5 ml Lidocain 1% (+Adrenalin 1:400000), erhöht um 1,5 ml je fehlendem Segment. Die Nachinjektion bei der CSE (Nadel-durch-Nadel-Technik) wurde bedarfsweise mit 9,5 ml– 15 ml Lidocain 1% (+Adrenalin 1:400000), abhängig von Körpergröße und Differenz zum Segment Th5 durchgeführt. Ergebnisse: Die Anästhesieausdehnung erreichte bei aEA nach einer Anfangsdosis von 101,8 mg Bupivacain und augmentierter Dosis von 99 mg (9,9 ml) Lidocain (7,5–18 ml) das Segment Th5. Die Spinalanästhesie führte unter CSE zu einer segmentalen Ausdehnung bis Th3. Bei 13 Patientinnen (56%) war eine Augmentation notwendig, in 5 Fällen wegen unzureichender Anästhesieausdehnung und in 8 Fällen wegen Schmerzen infolge vorzeitiger Rückbildung. Die Augmentation erforderte 13,9 ml/138,8 mg (9,5–23,5 ml). Operationsbereitschaft war nach 19,8 min (aEA) bzw. nach 10,5 min (CSE) erreicht (p〈0,0001). Vor Entbindung benötigte keine Patientin ein Analgetikum. Der zusätzliche Analgetikabedarf im weiteren Operationsverlauf betrug 63,6% (aEA) und 39,1% (CSE) (n.s.). Berücksichtigt man Schmerzen im Operationsgebiet betrug der Analgetikabedarf 50% (aEA) vs. 17,4% (CSE) (p〈0,05). Antiemetika wurden in 18,2% (aEA) und in 65,2% (CSE) benötigt (p〈0,0023). Der systolische Blutdruck fiel um 17,7% (aEA) bzw. 30,3% (CSE) ab (p=0,0054). Bei der aEA wurde das Minimum des systolischen Drucks nach 13,4 min, bei der CSE nach 9,6 min beobachtet. Die APGAR-Werte sowie der Nabelschnur pH zeigten keine Unterschiede. Eine Allgemeinanästhesie wurde in keinem Fall erforderlich. Schlußfolgerung: Eine einzeitige Epiduralanästhesie oder eine einzeitige isobare Spinalanästhesie mit Bupivacain 0,5% sind als alleinige, nicht augmentierbare Anästhesie nicht zuverlässig. Die isobare Spinalanästhesie erzielt primär eine angemessene Anästhesieausdehnung, benötigt jedoch auch häufig eine ergänzende epidurale Nachinjektion zur kompletten Analgesie. Eine Augmentation erweist sich bei der aEA mit Bupivacain 0,5% primär als zweckmäßig. Die CSE ermöglicht einen früheren Operationsbeginn.

    Description / Table of Contents: Zusammenfassung Bei 44 Patienten mit chronischer Niereninsuffizienz unterschiedlicher Äthiologie wurde das immunreaktive Serum-Myoglobin bestimmt und mit den Werten von Probanden mit normaler Nierenfunktion verglichen. Es fand sich — unabhängig von der Grunderkrankung — eine hoch signifikante lineare Korrelation zwischen dem Serum-Myoglobin und der Serum-Kreatinin-Konzentration. Bei Patienten mit Serum-Kreatinin-Werten über 550 µMol/1 (6,2 mg-%) lagen die Serum-Myoglobin Konzentrationen fast regelmäßig im pathologischen Bereich. Dies gilt auch für chronische Dialysepatienten. Die Befunde zeigen, daß bei Patienten mit höhergradiger, chronischer Niereninsuffizienz das Serum-Myoglobin nur mit Einschränkung zur Infarktdiagnostik geeignet ist.
    Notes: Summary In 44 patients with chronic renal failure of varied etiology serum immunoreactive myoglobin was measured and compared to values obtained in patients with normal renal function. Irrespective of the underlying disease a highly significant linear correlation was found between serum immunoreactive myoglobin and serum creatinine concentration. In patients with serum creatinine concentrations above 550 µmol/1 (6.2 mg%) serum myoglobin was as a rule elevated above the range found in the controls with normal renal function. This was also true in dialysis patients. These result demonstrate that serum myoglobin may only be used with restrictions in the diagnosis of myocardial infarction in patients suffering from advanced chronic renal failure.

    Notes: Summary A muscular syndrome has been described in patients on clofibrate and fenofibrate therapy. The present paper describes four patients with impaired renal function in whom symptoms and signs of skeletal muscle damage developed during treatment with another clofibrinic acid derivative, bezafibrate. The syndrome was characterized by variable degrees of muscular cramps and paresis, excessive elevation of muscle enzymes in serum, myoglobinemia and myoglobinuria. Transient deterioration of renal function was also common. All patients had been overdosed with bezafibrate with regard to their renal function. It is concluded that bezafibrate like other lipid lowering agents of the clofibrate type may induce muscle damage, at least if doses are not adjusted to renal function. Extreme caution is warranted when treating patients with renal impairment with bezafibrate and strict dose adjustment to kidney function is necessary to avoid muscle damage.

    Description / Table of Contents: Zusammenfassung Es wird ein Patient mit einem typischen “late onset” McArdle-Syndrom beschrieben. Während ischämischer Arbeit fand sich bei dem Patienten ein exzessiver Anstieg des Ammoniaks im Plasma. Dies dürfte auf einer Aktivierung der Myokinase/Myoadenylat-Deaminase-Reaktionen beruhen. Neben dem charakteristischerweise fehlenden Laktatanstieg unter ischämischer Arbeit scheint eine vermehrte Ammoniak-Freisetzung für das McArdle-Syndrom typisch zu sein.
    Notes: Summary A patient with typical features of late onset McArdle's disease is described. During forearm ischemic work test the patient exhibited an exaggerated increase in ammonia release, largely exceeding normal values. It is suggested, that this is due to an activation of the myokinase/myoadenylate deaminase pathway. Besides lack of lactate release increased ammonia release during ischemia may be a typical feature of McArdle's disease.

    Description / Table of Contents: Zusammenfassung Ein Radioimmunoassay zur Bestimmung von Serum-Myoglobin (SMb) wird vorgestellt. Bei 50 gesunden Probanden beträgt der Meßwertbereich 0–90 ng/ml. Serielle Bestimmungen an 10 Patienten mit akutem Myokardinfarkt beziehungsweise Angina pectoris (AP) zeigen, daß SMb bei Myokardinfarkt vor CK und CK-MB pathologische Werte erreicht (im Mittel 250±95 ng/ml bei stationärer Aufnahme 3,3±1,4 h nach AP-Beginn). Die gleichzeitig bestimmte NAC-aktivierte CK liegt zu diesem Zeitpunkt noch im Normbereich und erreicht pathologische Werte erst 6,2±1,9 h nach Schmerzbeginn. Der Peak des Serum-Myoglobins liegt mit 506±194 ng/ml 8,8±2,8 h, der der CK mit 905±475 mU/ml 20,0±7,8 h nach Anginabeginn. CK-MB und CK unterscheiden sich im zeitlichen Verlauf nur unwesentlich. Ein Patient mit ausgeprägter AP hat bei sonst unauffälligem Enzymmuster pathologisch erhöhte SMb-Werte. Methodische und klinische Ergebnisse werden diskutiert.
    Notes: Summary A radioimmunoassay was developed to determine serum myoglobin (SMb). 50 healthy persons showed values between 0 and 90 ng/ml. Serial tests of 10 patients following acute myocardial infarction or during angina pectoris (AP) indicated that SMb reached pathological values before CK and CK-MB (average 250±95 ng/ml at the time of hospitalisation which corresponds to 3.3±1.4 h after beginning of angina pectoris). At hospitalisation the simultaneously determined CK was within normal limits and reached pathological values only 6.2±1.9 h after the onset of angina. Maximum of SMb was 506±194 ng/ml occurring 8.8±2.8 h after the beginning of AP, maximum of CK was 905±475 mU/ml occurring 20.0±7.8 h after AP. CK-MB and CK differed only slightly in their time course. One patient with severe AP had pathologically increased SMb values whilst all other enzymes were completely normal. Methodical and clinical results are discussed.

    Description / Table of Contents: Zusammenfassung Die lokomotorische Aktivität der ostafrikanischen WinkerkrabbenUca urvillei undU. annulipes wurde mit Hilfe einer radioaktiven Markierung (Co-60-Draht) in einer Gezeitenanlage (Tidenhub 1 m, 12,5stündige Zyklen) über mehrere Wochen registriert. Der Lokomotionsrhythmus ließ sich an die künstlichen Gezeiten ankoppeln. Das tidale Muster von Substratvibrationen war ein besonders wirksamer Zeitgeber. Licht-Dunkel-Zyklen waren gleichfalls als Zeitgeber wirksam: im LD 6,25∶6,25 ergab sich ein tidaler, im LD 12∶12 ein diurnaler Lokomotionsrhythmus. Wurden gleichzeitig Gezeiten und LD 12∶12 geboten, so richteten sich die Tiere nach den Gezeiten. Nach dem Übergang von Gezeitenbedingungen in Konstantbedingungen (LL, DD) zeigte nur eines von 15 Tieren einen circadianen Lokomotionsrhythmus. Alle anderen hatten arhythmische Muster, in denen nur gelegentlich „circadiane” Fronten angedeutet waren. Auch eine Periodenanalyse ergab keine Hinweise auf circadiane oder circatidale Rhythmen. Um diese arhythmischen Muster zu analysieren, wurde ein neuer Ansatz gewählt: jeder Aktivitäts- und Ruheschub (A bzw. R) wurde als Einzelereignis betrachtet, ohne nach einer periodischen Folge zu suchen. Aktivitätsmuster in Konstantbedingungen: Die Häufigkeitsverteilungen der Dauer von Aktivitäts- und Ruheschüben konnten bei der Mehrzahl der Tiere durch Exponentialfunktionen wiedergegeben werden. Zwischen der Dauer aufeinanderfolgender Aktivitäts- und Ruheschübe bestand keine Korrelation. Daraus folgt, daß der einzelne Schub nach seinem Beginn zu allen folgenden Zeitpunkten mit einer bestimmten, gleichbleibenden Wahrscheinlichkeit beendet werden kann. Diese Gesetzmäßigkeit ist als stochastischer Prozeß zu beschreiben. Aus den ermittelten Wahrscheinlichkeitswerten für A und R ergeben sich für die Dauer von A+R, am häufigsten Werte die mit 9–12 Std in der Größenordnung des Gezeitenzyklus liegen. Aktivitätsmuster in Gezeitenbedingungen: Die tidalen Zeitgeber kontrollierten die Dauer der Ruheschübe (vermutlich über eine endogene Instanz); die Dauer der Aktivitätsschübe blieb dagegen bei vielen Tieren unbeeinflußt.
    Notes: Summary The East African fiddler crabsUca urvillei andU. annulipes were exposed to artificial tides (cycle 12.5 hrs, tidal range of 1 m). Locomotor activity patterns were recorded by means of a radioactive marker (60Co). The activity patterns could be synchronized by the artificial tides. Vibrations of the substratum connected with the tides were one of the effective Zeitgebers. Light-dark cycles also acted as Zeitgebers. An LD of 6.25∶ 6.25 initiated a tidal activity pattern, an LD of 12∶12 a diurnal one. When the crabs were exposed simultaneously with artificial tides and an LD of 12∶12 their activity was distinctly correlated with the tides. In constant conditions following tidal treatment only one of 15 individuals showed a circadian rhythm. All other animals showed arhythmic activity patterns. Though “circadian” fronts could occasionally be seen on visual inspection, there was no evidence for either circadian or circatidal rhythms when the data were arranged by a periodogram method. Therefore, a new approach was used to analyze these arhythmic patterns, measuring the duration of each activity burst (A) and of each resting period (R) separately. Activity patterns in constant conditions: The class frequency of both A and R followed exponential functions. No significant correlation was found between the duration of A and the following R. Thus each event (A or R) is terminated by a distinct probability. This principle can be described as a stochastic process. The probability values found for A and R are such that the resulting duration of A+R is usually in the range of the tidal cycle. Activity patterns in tidal conditions: There is evidence that the tidal zeitgebers control the duration of the resting period only and that the duration of activity is usually independent of the Zeitgebers.

    Notes: Summary L-Dopa supplemented by a peripheral decarboxylase inhibitor is considered the most potent therapeutic regimen prolonging active life in Parkinsonian patients. The long-term benefit of therapy is limited by adverse effects, such as dyskinesia and on-off phenomena, which can be mitigated by the concomitant administration of dopamine agonists, such as bromocriptine. In order to quantify the beneficial impact of early combination therapy, a controlled clinical trial (PRADO:PRA vi-del1 +DOpa) in patients with early Parkinson's disease was carried out, whereby L-Dopa monotherapy (in a fixed combination with benserazide (DoBe) was being compared with the same combination plus bromocriptine (DoBeBro). Patients were recruited and treated by 101 practising neurologists in the Federal Republic of Germany and in Hungary. 'Twenty seven clinical university centers cross-checked the patients at regular intervals. The trial started with 3 months of DoBe monotherapy (median dose of 375 mg L-Dopa for both randomized groups) followed by gradual substitution of DoBe by bromocriptine over 3 months in one of the groups (250 mg L-Dopa/10 mg bromocriptine). The target medication was maintained from study months 6 to 54. Parkinsonian symptoms were classified according to the Webster rating scale, the Hoehn and Yahr scale and the Zung Self-Rating Depression Scale. Adverse events and life status were checked at regular intervals. Special emphasis was given to motor performance tests. 587 patients (302 in the DoBe group and 285 in the DoBeBro group) were available for intention-to-treat analysis. Both groups were homogeneous at baseline in all observed parameters. DoBe and DoBeBro proved equi-effective in terms of antiparkinsonian activity after the substitution phase (P II) had been completed. In September 1991, after a median observation period on target medication of 38.4 months in the DoBe group and 40.1 months in the DoBeBro group, 18 versus 8 deaths had been registered. The Logrank test as well as analysis using the Cox model, both adjusted for age and sex, showedP-values of 0.018 and 0.021, respectively. The mortality risk associated with L-Dopa therapy was reduced by more than 50 % by its combination with bromocriptine. The study was terminated due to this difference in mortality. The causes of death were classified by the treating physicians and consultants. At the time of study termination 152 patients in the DoBe group and 121 in the DoBeBro group had already discontinued study medication. Of those further 26 patients had died by the date of the final evaluation, 15 on DoBe and 11 on DoBeBro. The results imply that combination therapy with bromocriptine should be preferred over L-Dopa monotherapy from the very beginning.
