    Notes: Abstract Our purpose was to determine the characteristics findings of recurrent hepatocellular carcinomas (HCCs) on sonography. We reviewed sonographic findings of 85 recurrent tumors in 57 patients with prior partial hepatectomy for HCCs. Of 57 patients, 47 (82.5%) had localized recurrence; one or two tumors in a segment (44 patients) and a recurrent tumor near the resected hepatic stump (3 patients). Of 85 tumors, 58 (68.2%) were less than 2 cm, and 82 of 85 tumors (96.5%) were less than 4 cm. A total of 49 tumors (57.6%) were seen as hypoechoic, and 20 tumors (23.5%) as hyperechoic. Posterior echo enhancement was seen in 36 tumors (42.4%), hypoechoic halo in 24 tumors (28.2%), lateral shadow in 21 tumors (24.7%), and mosaic pattern in 6 tumors (7.1%). Recurrent HCC can be detected at an early stage by sonography. Hypoechoic pattern and posterior echo enhancement are commonly seen. These findings suggest recurrent HCCs.

    Notes: Abstract A new technique of CT during arterial portography (CTAP) performed with a left brachial artery approach is presented. We used 4-F, 50-cm J-shaped sheath introducer with a straight dilator and angled 4-F catheter with side holes. Catheter insertion to the superior mesenteric artery (SMA) can be performed without any trouble by the help of a J-shaped long sheath introducer positioned in the aortic arch. In our experience, 10 min compression of the puncture site and 2 h observation after catheter removal brought no local complication. This method can be performed safely on outpatient basis.

    Notes: Experimental verification is made of a new polarization splitter which utilizes artificial anisotropic dielectrics. The splitter is composed of layers of periodically laminated SiO2/TiO2 thin films. The SiO2 and TiO2 films are alternately deposited by rf sputtering and reactive dc sputtering, respectively. The thickness of each layer is 50 nm, while the total number of the layers amounts to 2000. The measured polarization split angles are 5.7° (λ=0.63 μm) and 5.1° (λ=1.3 μm), being roughly the same as those predicted.

    Notes: A new spatial walk-off polarizer with a large polarization-splitting angle is fabricated. The polarizer, which utilizes artificial anisotropic dielectrics, is composed of layers of periodically laminated a-Si:H/SiO2 thin films. The thickness of each layer is 67 nm, while the total number of the layers amounts to 1440. The measured polarization-splitting angle is 15.5° at λ=1.55 μm, which is 2.7 times larger than that of existing polarizers.

    Notes: Abstract— Rats were subjected to cerebral compression ischaemia for 15min and were subsequently recirculated with blood for periods up to 3 h. In vivo incorporation of intravenously administered L-[1–14C]valine into total brain proteins was found to be severely inhibited (about 20% of controls) after 45 min of recirculation. After 3 h, protein synthesis had increased, the specific radioactivity of proteins then being about 40% of controls. The post-ischaemic inhibition of protein synthesis was accompanied by a breakdown in polyribosomes and a concomitant increase in ribosomal subunits. In vitro incorporation of L-[1–14C]phenylalanine by a postmitochondrial supernatant system derived from animals subjected to 15 min ischaemia and 15 min recirculation was also severely reduced and showed, in contrast to control animals, no response to the addition of a specific inhibitor of polypeptide chain initiation (Poly(I)). Together with the in vivo accumulation of ribosomal subunits this indicates a block in peptide chain initiation during the early stages of recirculation.Polyribosomes from animals subjected to 15 min ischaemia without recirculation showed a normal rate of in vitro protein synthesis which was inhibited by Poly(I) to a similar extent as polyribosomes from control animals. These results suggest that the post-ischaemic inhibition in chain initiation develops during the early stages of recirculation rather than during the ischaemic period itself.

    Notes: Rabeprazole sodium is a proton pump inhibitor.〈section xml:id="abs1-2"〉〈title type="main"〉Aim:To evaluate the efficacy and safety of 1-week triple therapy with rabeprazole, amoxycillin and clarithromycin for the eradication of Helicobacter pylori.〈section xml:id="abs1-3"〉〈title type="main"〉Methods:A total of 100 subjects with H. pylori were randomly divided into two groups of 1-week triple therapy with rabeprazole 10 mg b.d., amoxycillin 750 mg b.d. and either clarithromycin 200 mg b.d. (RAC400, n=50) or clarithromycin 400 mg b.d. (RAC800, n=50). Endoscopic examination with four biopsies (two specimens from the antrum and two from the gastric body) was performed. The status of H. pylori infection was determined using culture and histology (Giemsa stain) of the biopsy specimens. Sensitivity to clarithromycin was determined using the E-test: MIC 〉 8 g/mL was considered to be resistant, whereas MIC 〈 2 g/mL was considered to be sensitive. Cure was defined as no evidence of H. pylori infection 1 month after completion of treatment.〈section xml:id="abs1-4"〉〈title type="main"〉Results:There were no significant differences in the clinical characteristics of the two groups. Eradication rates (intention-to-treat and per protocol, respectively) were: RAC400: 86% (95% CI: 76–95%) and 89% (95% CI: 80–97%); RAC800: 94% (95% CI: 87–100%) and 97% (95% CI: 94–100%). There was no significant difference between the eradication rates of either regimen. Three subjects with failed eradication in the RAC400 group were all infected with a clarithromycin-resistant strain before beginning the therapy. Haemorrhagic colitis was the only severe adverse event, which was observed in one patient in the RAC800 group.〈section xml:id="abs1-5"〉〈title type="main"〉Conclusion:One-week triple therapy with rabeprazole, amoxycillin and low-dose clarithromycin is effective for the eradication of H. pylori infection.
