    Description / Table of Contents: Zusammenfassung Squashverletzungen wurden zweimal bundesweit erfaßt. Sie teilen sich in schlagbedingt (durch Schläger und Ball): 68,9% und nicht schlagbedingt: 31,1 %. Bei Schlagverletzungen ist zu 68,4 % der Kopf betroffen, ernst zu nehmen sind Augenverletzungen (direkte Balleinwirkung). Nicht schlagbedingt: Achillessehnenrisse (mit steigendem Lebensalter zunehmend), Gelenksverletzungen an Knie und Sprunggelenk (Bänder, Menisci) u. a. Anfänger verletzen sich häufiger als gute Spieler (4: 1), Männer mehr als Frauen (3: 1). Verletzungshäufigkeit im Vergleich: Squash 3,5/Tennis 3,0/Fuβball 26,8/Handball 27,0 (%/a).
    Notes: Summary Injuries in Squash have been collected twice in Germany. 68.9 % resulted from hits (ball, racket), 31.1 % were due to other reasons. Hit injuries affect the head in 68.4%, serious are eye injuries (direct ball blow). Other injuries: ruptures of the tendo Achillis (increasing with life age) and joint injuries to knee and ankles (ligaments, menisci). Beginners are hurt more often than good players (4:1), men more than women (3:1). Injury rates in comparison: Squash 3,5/Tennis 3.0/Soccer 26.8/Handball 27.0 (%/a). Reasons of injuries: lack of training and technique, unfairness, overeagerness.

    Description / Table of Contents: Zusammenfassung Die posteriore Schulterinstabilität stellt ein wesentlich schwierigeres diagnostisches und therapeutisches Problem dar als die anteriore. Zum einen verbergen sich dahinter ätiologisch und pathogenetisch grundsätzlich verschiedene Krankheitsbilder, die auch in Form von Mischbildern ineinander übergehen können. Zum anderen gibt es aufgrund der geringeren Inzidenz nur wenige und sehr heterogene retrospektive Studien bezüglich der therapeutischen Möglichkeiten. Grundsätzlich muß man traumatische und atraumatische Formen strikt voneinander trennen. Bei den rein traumatischen Formen handelt es sich um Luxationsimpressionsfrakturen des Humeruskopfes. Die Therapie dieser Luxationen ist abhängig von der Größe des Defekts, der Chronizität und dem funktionellen Anspruch des Patienten. Je nach Defektgröße kommt hier die geschlossene Reposition mit anschließender Ruhigstellung, die offene Reposition evtl. kombiniert mit Transposition des Tuberculum minus (McLaughlin), die retrograde Defektanhebung mit Spongiosaunterfütterung, die Drehosteotomie oder der endoprothetische Humeruskopfersatz in Frage. Die nicht traumatische rezidivierende hintere Instabilität ist primär konservativ zu behandeln. Muskelkoordination und Kräftigung der kopfzentrierenden und scapulaführenden Muskulatur nach Analyse des Luxationsmechanismus sind die Therapie der Wahl. Vor der Indikation zu operativen Maßnahmen ist das Ausschließen zugrundeliegender psychopathologischer Störungen essentiell. Bei den operativen Maßnahmen scheinen dorsale Kapselshiftoperationen den früher durchgeführten Knochenblockappositionen und Glenoidosteotomien überlegen zu sein. Die letztgenannten knöchernen Eingriffe sind dann indiziert, wenn als Luxationsursache eindeutig Störungen der Glenoidgeometrie in Frage kommen, sollten in der Regel aber auch mit Kapselshiftoperationen kombiniert werden.
    Notes: Summary The posterior instability of the shoulder is a more difficult diagnostic and therapeutic challenge than the anterior instability. There are many etiologies and causes of posterior instability. Most studies in the literature are retrospective and yield a great variation in therapeutic recommendations. Generally it has to be separated in traumatic and atraumatic instabilities. Most of the traumatic dislocations are impaction fractures of the humeral head against the dorsal glenoid. Therapy is depending on the size of the humeral defect, the duration of dislocation and the functional demand of the patient. Therapeutic possibilities are closed reduction and fixation with a cast, open reduction and the transfer of the lower tubercule (McLaughlin's procedure), lifting of the defect and supporting with cancellous bone, subcapital rotational osteotomy or arthroplasty. The therapy of choice for atraumatic instability is a individualized rehabilitation program with strengthening and balancing of rotatator cuff muscles and scapular stabilizers. Psychologic abnormalities and emotional problems have to be recognized prior to any operative procedure. These patients are no operative candidates. Operative treatment of choice is the posterior capsular shift adressing the causative redundancy or laxity of the postero-inferior capsule. Posterior bony procedures as glenoid osteotomy or bone block transfers are indicated, if the pathologic geometry of the glenoid is primarily responsible for posterior instability. It is strongly recommended to combine them with a capsular shift to adress the secondary capsular redundancy.

    Notes: Zusammenfassung Gesamtgonadotropinbestimmungen mit der Kaolin-Aceton-Methode vonLoraine undBrown und dem Mäuse-Uterus-Test nachLevin undTyndale ergaben bei fünf nulliparen Frauen in jedem Einzelcyclus ein signifikantes Maximum der Gonadotropinausscheidung in dem 48 Std-Sammelharn, der dem Basaltemperaturanstieg unmittelbar vorausging. Die Höchstwerte betrugen im Mittel 17 (9,7–31,6) HMG/E, der Cyclusdurchschnitt 8 (5–17) HMG/E. Sämtliche Untersuchungsergebnisse konnten statistisch gesichert werden. Mit dem Verfahren nachBrown wurde die Oestrogenausscheidung in acht weiteren Cyclen bestimmt. Die Höchstwerte lagen jeweils am letzten oder vorletzten Tag des Temperaturtiefs und damit zum selben Zeitpunkt wie das Gonadotropinmaximum. Es besteht demnach eine zeitliche Korrelation zwischen dem Verhalten der Basaltemperatur und dem Zeitpunkt der höchsten Gonadotropinaktivität bzw. des höchsten Oestrogengehaltes im Harn während eines biphasischen Cyclus.

    Description / Table of Contents: Zusammenfassung Bei Verdacht auf gelockerte und infizierte Totalendoprothesen des Hüftgelenkes erweitert die Arthrographie die diagnostischen Möglichkeiten. Sie kann eine Prothesenlockerung mit großer Genauigkeit nachweisen und beim Vorliegen einer schleichenden Infektion die Bestimmung der Erreger und eine gezielte antibiotische Therapie ermöglichen. Da eine Prothesenlockerung oft schon durch eine genaue klinische und radiologische Untersuchung erkannt werden kann, sollte die Arthrographie nur bei unklaren Befunden Anwendung finden.
    Notes: Summary In questionable cases of loosening of the prosthesis or low-grade infection after total hip replacement arthrography may be useful in the investigation of these complications. It is able to prove with high accuracy if the prosthesis is loose and if there is a low-grade infection. In such cases bacteriological identification by culture makes possible a highly specificated therapy with antibiotics. As a loosening of the prosthesis often may be recognized only by means of exact clinical and radiological examination arthrography should be used only in doubtful cases.

Zunächst wird nur eine Hüfte operiert, um den Oberschenkel der anderen Seite als Hebel bei der Wirbelsäulenaufrichtung verwenden zu können. Erst nach erfolgter Wirbelsäulenaufrichtung wird die zweite Hüfte operiert. Auf die Vorbedingungen, Lagerung, Höhe, Technik und Nachbehandlung der Eingriffe wird näher eingegangen.
    Notes: Summary In cases of ankylosing spondylitis with severe kyphosis of the spine and flexion contractures of the hip joints, a combination of total hip replacement and corrective osteotomy of the spine seems to be indicated. It is advisable to perform the vertebrotomy after the operation of only one hip, in order to use the leg of the other side as a lever for the corrective plaster of paris cast. Only when the desired correction of the spine is achieved, the second hip should be operated. Indication for operation, operative procedures and post operative treatment are discussed.

Die besondere Form des Radiusköpfchens mit seiner charakteristischen Ausladung, bedingt durch eine Diameter-Differenz des Köpfchens und Schrägstellung der Gelenkfläche der Fovea capitis ist mitbestimmend für den Subluxationsmechanismus, die gefundenen Meßwerte sind jedoch bei Kindern und Erwachsenen identisch.
    Notes: Summary This paper reports possible causes of the disease called “pronatio dolorosa”. Radial heads of adults, children, and unborn children have been analyzed by means of measuring their dimensions and by studies of anatomical specimen. A marked valgus position of the elbow joint in children combined with the laxity of the ligamentous apparatus and the interosseous membrane is considered to the cause of the “pronatio dolorosa”. The tendency of the radial head to subluxate is caused mainly by his characteristical chape but the data found are identical for children and adults.
