    Description / Table of Contents: Abstract Capnometry, the noninvasive measurement of end-expiratory CO2 concentration (cCO2, vol%) or calculation of its respective partial pressure (pCO2; mmHg) is an established method. However, for prehospital settings, capnometry is still used very restrictively, mainly owing to the respective devices used. The prerequisite for their use is sufficient accuracy (±2 mmHg) and easy handling. Two special capnometers (STAT CAP, Nellcor: mainstream, semiquantitative estimation; Capnocheck 8200, BCI: sidestream, quantitative measurement, numeric display), developed recently for potential use in emergency medicine, are said to fit these criteria. Therefore, the objective of the present investigation was to assess the accuracy and precision of both devices, comparing methods under standardized in vitro (reference gases) and in vivo (intubated and ventilated patients) conditions. Methods. Both devices (“STAT CAP”: pCO2 range, light bars; “Capnocheck 8200”) were evaluated regarding the accuracy of pCO2 (Capnocheck) and the precision of the CO2 range (STAT CAP). Tests were performed with four dry gas mixtures (STPD) of defined composition and during ventilation of 20 intubated patients (BTPS). All measurements were compared with the alveolar gas monitor “AGM 1304” (Brüel & Kjaer, Denmark) as a reference method with a proven ±1 mmHg accuracy of pCO2 measurement. Results. The “Capnocheck” (BCI) presented an accuracy of the pregiven pCO2 of 0.7–1.4 mmHg (dry gas mixtures, STPD) and an overestimation of 0.2±4.1 mmHg (BTPS) during ventilation with pure oxygen; inaccuracy during ventilation with 70% N2O in O2 proved to be +1.2±1.7 mmHg (BTPS). Nellcor's “STAT CAP” failed to reach the target value in 10% of analyses, as shown by the respective segment bar of the display. Conclusion. Evaluation of the accuracy of capnometers must focus on the necessary pH2O correction and the possible effects exercised by O2 (and N2O) as well as the possible dependence on barometric pressure (if pCO2, mmHg, is the desired value). The “Capnocheck” showed an accuracy of more than 2 mmHg in dry gas mixtures as well as in humidified air. Concerning the practical use during constant artificial ventilation, the digital display and accuracy of the sidestream capnometer allow for reliable conclusions on patients' ventilation and circulation (CO2 elimination). The 90% accuracy of the segment bar display of Nellcor's “STAT CAP”, per se covering only a rather broad range of 20 mmHg, obviously does not provide more than a rough overview. Therefore, the STAT CAP cannot be recommended for prehospital capnometry in the field. However, both the accuracy of the BCI capnometer (Capnocheck) and its numeric display and easy handling strongly recommend this device also for clinical use.
    Notes: Zusammenfassung Ziel vorliegender Untersuchung war die Prüfung der CO 2 -Meßgenauigkeit bzw. -Meßbereichspräzision zweier neuer, potentiell für den Rettungsdienst konzipierter Kapnometer unterschiedlicher Funktionsweise im Rahmen eines Methodenvergleichs. Untersucht wurden das Hauptstrom- (STAT CAP, Nellcor: Balkensegmentanzeige, pCO 2 -Bereiche) und das Nebenstromkapnometer (Capnocheck 8200, BCI International: pCO 2 -Wertanzeige). Geprüft wurde mit vier Referenzgasen und an 20 beatmeten Patienten. Als Referenzmethode diente der Atemgasmonitor „AGM 1304“ (Brüel & Kjaer) mit einer pCO 2 -Meßgenauigkeit von ±1 mm Hg. Die Abweichungen des „Capnocheck“ vom Sollwert betrugen für die Prüfgase 0,7–1,4 mm Hg, bei Beatmung mit reinem O 2 0,2±4,1 mm Hg (unter N 2 O 1,2–1,7 mm Hg). Der „STAT CAP“ traf den vorgegebenen Sollwert nur in 90% mit dem richtigen Segment seiner Bereichsanzeige. Der „Capnocheck“ erreichte sowohl in trockenen Prüfgasen (STPD) als auch in wasserdampfgesättigter Ausatemluft (BTPS) eine pCO 2 -Meßgenauigkeit von ≤±2 mm Hg. Die Bereichsanzeige des „STAT CAP“ erlaubt aufgrund der Unschärfe des pCO 2 -Bereichs von 20 mm Hg sowie wegen des 10%igen Trefffehlers lediglich eine grobe Orientierung. Dieses Gerät (Preis ca. DM 2500) ist zur Verwendung als Kapnometer im eigentlichen Sinne untauglich, könnte jedoch zur „vor-Ort-Beurteilung“ der Tubuslage Bedeutung erlangen. Die Meßgenauigkeit und gute Handhabbarkeit des „Capnocheck“ empfehlen dieses Kapnometer (Preis ca. DM 4500) sowohl für den potentiellen Einsatz bei beatmeten Patienten im Rettungsdienst als auch für den innerklinischen Betrieb.

    Description / Table of Contents: Abstract Objective: To ascertain whether there is a difference between total intravenous anaesthesia with propofol (P) and remifentanil (R) and inhalational anaesthesia with desflurane (D) and nitrous oxide (N) with regard to haemodynamic reactions, recovery profile and postoperative analgesic demand in patients scheduled for elective microsurgical vertebral disc resection. Methods: 50 patients (ASA I–II, 18–65 years) were randomly assigned to receive total intravenous anaesthesia with propofol and remifentanil or inhalational anaesthesia with desflurane and nitrous oxide. After standardised induction of anaesthesia in both groups (1 µg·kg−1 remifentanil, 1.5 mg·kg−1 propofol, 0.1 mg·kg−1 cisatracurium), anaesthesia was maintained in the D/N group with desflurane in 50% N2O. The patients of the P/R group received a constant infusion of 2 mg·kg−1·h−1 propofol and a constant infusion of 0.5 µg·kg−1·min−1 remifentanil, which was reduced after 15 min by 50%. The administration of desflurane and the infusion of the anaesthetics were adjusted to maintain a surgical depth of anaesthesia. At the end of surgery the anaesthetics were discontinued and early emergence from anaesthesia was assessed by measuring time to spontaneous ventilation (VT〉4 ml/kg), tracheal extubation, opening of the eyes and stating correct name and data of birth. The frequency of analgesics and total demand for analgesics were determined using patient-controlled analgesia and recorded for 2 h postoperatively. In addition the pain level of the patients was measured on a visual analogue scale and the incidence of postoperative shivering, nausea and vomiting was noted. Results: Patients anaesthetised with desflurane responded to tracheal intubation and skin incision with increasing blood pressure and showed higher heart rates than patients anaesthetised with propofol and remifentanil, but there were no other haemodynamic differences between the groups in response to surgical stimuli. There were significantly shorter times to spontaneous ventilation (3.2 vs. 6.3 min), extubation (3.8 vs. 9.5 min), eye opening (3.0 vs. 11.5 min) and giving name and date of birth (4.8 vs. 14.3 min) in patients anaesthetised with remifentanil and propofol than in those receiving desflurane and nitrous oxide. In addition, patients anaesthetised with remifentanil and propofol had a greater incidence of postoperative shivering. There were no significant differences between the two groups in the patients’ pain scores, analgesic demand and incidence of nausea and vomiting. Conclusion: Patients anaesthetised with propofol and remifentanil have significantly shorter emergence times than patients anaesthetised with desflurane and nitrous oxide. The low incidence of postoperative pain after microsurgical vertebral disc resections requires no large-scale analgesic therapy, even after total intravenous anaesthesia including remifentanil.
    Notes: Zusammenfassung Fragestellung: Unterscheidet sich eine totale intravenöse Anästhesie mit Propofol (P) und Remifentanil (R) von einer Inhalationsanästhesie mit Desfluran (D) und Lachgas (L) bei lumbalen Bandscheibenoperationen hinsichtlich der Steuerbarkeit der Narkose, der Beeinflussung hämodynamischer Parameter, des Aufwachverhaltens und des postoperativen Analgetikabedarfs der Patienten? Methodik: 50 Patienten (ASA I–II, 18–65 Jahre) wurden randomisiert entweder einer P/R- oder D/L-Gruppe zugeteilt. Nach standardisierter Narkoseeinleitung (1 µg/kg Remifentanil, 1,5 mg/kg Propofol, 0,1 mg/kg Cisatracurium) wurde die Anästhesie in der D/L-Gruppe bedarfsadaptiert mit Desfluran in 50% N2O und in der P/R-Gruppe mit 2 mg/kg/h Propofol und 0,5 µg/kg/min Remifentanil aufrechterhalten, wobei die Remifentanildosis nach 15 min halbiert wurde. Am Operationsende unmittelbar vor der Umlagerung in die horizontale Rückenlage wurde die Zufuhr der Anästhetika abrupt unterbrochen und folgende Aufwachzeiten erfaßt: Eintritt Spontanatmung (VT〉4 ml/kg), Extubation, Augenöffnen, richtiges Benennen von Namen und Geburtsdatum und der Analgetikabedarf der ersten 2 postoperativen Stunden im Aufwachraum. Ergebnisse: Die Patienten der D/L-Gruppe reagierten auf den Intubationsreiz und die Hautinzision mit signifikanten Blutdruckanstiegen und zeigten signifikant höhere Herzfrequenzwerte, während ansonsten die hämodynamischen Parameter während des Narkoseverlaufs vergleichbar waren. Die Patienten der P/R-Gruppe erreichten signifikant früher eine stabile Spontanatmung (3,2 vs. 6,4 min), konnten früher extubiert werden (3,8 vs. 9,5 min), öffneten früher die Augen (3,0 vs. 11,5 min) und waren eher in der Lage, ihren Namen und Geburtsdatum zu benennen (4,8 vs. 14,3 min), wiesen aber auch signifikant häufiger Muskelzittern auf. Keine signifikanten Unterschiede fanden sich im Analgetikabedarf sowie in der Inzidenz von Übelkeit und Erbrechen. Schlußfolgerung: Die Patienten erwachen aus der TIVA mit Propofol/Remifentanil schneller als aus der Desfluran/N2O-Narkose und erreichen schneller ein höheres Vigilanzniveau, wobei die geringe Intensität postoperativer Wundschmerzen nach Bandscheibenoperationen kein aufwendiges Konzept zur postoperativen Analgesie erfordert.

    Description / Table of Contents: Abstract For many decades, heparins have been used successfully for prophylaxis and treatment of thromboembolic complications world-wide. Although heparin-induced thrombocytopenia (HIT Type II) is a well-known adverse effect of heparin therapy, thromboembolic complications during heparin therapy are rarely diagnosed exactly to be related to HIT. At present an immunologic etiology of HIT by generation of multimodal immune complexes against a neo antigen of heparin and platelet factor 4 is equivocally accepted. The incidence of HIT seems to be related to the type of heparin (unfractioned/low molecular weight) or other underlying risks such as peripheral occlusive vessel disease. Mortality and complications resulting from HIT is reported to be about 20–30% each. For diagnosis of HIT Type II, clinical observation and simultaneous laboratory testing are essential. Discontinuation of heparin is a simple and essential maneuvre, and anticoagulation has to be continued by alternative drugs. The heparinoid danaparoid-sodium and the thrombin inhibitor recombinant hirudin have been used successfully world-wide for treatment in many patients with HIT Type II including cardiopulmonary bypass surgery or renal replacement procedures. Furthermore, other therapeutical alternatives (e.g. immuneglobulins, prostaglandines) exist. Randomised controlled studies have to evaluate which drug has to be preferred in the future including risk/ benefit ratio. The need of supplementary surgical procedures (e.g. embolectomy) depends on the individual clinical status. The patients have to be informed in detail about their underlying disease and further deleterious consequences of re-exposition with heparin. HIT should be recorded in an emergency certificate and the national Committee on Drugs should be informed about this severe side effect of heparin therapy.
    Notes: Zusammenfassung Heparine sind seit Jahrzehnten bewährte Pharmaka zur Prophylaxe und Therapie thromboembolischer Komplikationen. Eine oft unterschätzte gravierende Nebenwirkung der Heparintherapie ist die Thrombozytopenie (HIT Typ II), die als Folge einer Immunkomplexbildung gegen Heparin/ Plättchenfaktor 4-Komplexe selbst thromboembolische Komplikationen hervorrufen kann. Die Inzidenz dieser Komplikationen ist u.a. vom Typ des Heparins (unfraktioniert/ niedermolekular) und weiteren Grunderkrankungen des Patienten abhängig. Insbesondere aufgrund der hohen Mortalität und der Rate an Defektheilungen kommt dem rechtzeitigen Erkennen des Krankheitsbildes eine hohe Bedeutung zu. Die Diagnose der HIT Typ II erfolgt primär nach klinischen Gesichtspunkten und wird durch valide Testverfahren ergänzt. Wesentliche Elemente der Therapie sind das Absetzen aller Heparine und die Weiterführung der therapeutischen Antikoagulation mit alternativen Pharmaka. Hierzu stehen im wesentlichen mit dem Heparinoid Danaparoid-Natrium und dem Thrombininhibitor r-Hirudin zwei bewährte Substanzen zur Verfügung, die jedoch unterschiedliche Wirkprinzipien und damit auch ein spezifisches Wirkungs/Nebenwirkungsprofil aufweisen. Eine eindeutige Präferenz kann aufgrund der vorliegenden Daten nicht angegeben werden. Chirurgische Verfahren (z.B. Embolektomien) kommen zusätzlich in Betracht. Die Patienten müssen ausführlich über das Krankheitsbild aufgeklärt werden; die Dokumentation in einem Notfallausweis und die Meldung an die Arzneimittelkommission der Deutschen Ärzteschaft ist obligat.

    Description / Table of Contents: Die kontrollierte Hypotension ist eine pharmakologisch induzierte Blutdrucksenkung auf einen arteriellen Mitteldruck von 50–60 mm Hg beim anästhesierten und kontrolliert beatmeten Patienten, oft ergänzt durch Lagerungsmaßnahmen, die das Operationsgebiet über die Herzebene anheben. Wichtigstes Ziel der kontrollierten Hypotension ist die Verminderung intraoperativer Blutverluste und damit des Bedarfs an Transfusionsblut. Im günstigen Fall können durch die kontrollierte Blutdrucksenkung intraoperative Blutverluste um mehr als 50% vermindert werden; allerdings ist die Wirkung sehr variabel und individuell nicht vorhersehbar. Die Technik der kontrollierten Hypotension ist weitgehend vereinheitlicht worden und erfolgt primär mit rasch und kurz wirkenden Vasodilatatoren wie Nitroprussid, Nitroglyzerin oder Urapidil, ergänzt durch β-Blocker und/oder Inhalationsanästhetika wie Isofluran, um die Wirkung und Steuerbarkeit zu verbessern und die erforderliche Dosis zu reduzieren sowie unerwünschte Wirkungen (Reflextachykardie, Tachyphylaxie, Reboundhypertonie) zu vermeiden. Durch Lagerung des Operationsgebiets über die Herzebene kann die Durchblutung während der kontrollierten Hypotension regional weiter vermindert und der venöse Abfluß besser drainiert werden. Hauptgefahr der kontrollierten Hypotension ist die unzureichende Durchblutung der Organe, insbesondere von Herz und Gehirn mit nachfolgender Schädigung (Apoplex bzw. neurologische Ausfälle, Myokardinfarkt), weiterhin die Zunahme des intrakraniellen Blutvolumens bei neurochirurgischen Patienten. Wichtigste Kontraindikationen sind daher die koronare Herzkrankheit, die Hypertonie mit Arteriolosklerose der Hirngefäße, zerebrale Durchblutungsstörungen sowie der erhöhte intrakranielle Druck. Bei strikter Beachtung der Kontraindikationen, sorgfältiger Auswahl und Vorbereitung des Patienten, korrekter technischer Durchführung und essentiellem Monitoring ist die Komplikationsrate der kontrollierten Hypotension gering. Die Mortalität wird mit 0,02–0,06% angegeben.
    Notes: Summary Induced hypotension is defined as a reduction in mean arterial blood pressure to 50–60 mm Hg in normotensive subjects. The aim of induced hypotension is to decrease intraoperative blood loss, decrease the need for blood transfusions and improve operating conditions. Most studies indicate that induced hypotension can decrease intraoperative blood loss by 50% in many surgical procedures; however, some studies report that blood loss is not significantly reduced. Current methods of induced hypotension are based on the use of rapid and short-acting vasodilators as primary agents (nitroprusside, nitroglycerine, urapidil), supplemented by volatile anesthetics (isoflurane) and/or β-blockers (esmolol) to improve effect, reduce dosage and prevent side effects (reflex tachycardia, tachyphylaxis, rebound hypertension). Proper positioning of the patient and controlled ventilation aid in reducing blood loss. Major risks of induced hypotension are a reduction in blood flow (i.e. ischaemia) of vital organs (brain, myocardium) and elevation of intracranial pressure in neurosurgical patients. Thus, major contraindications of induced hypotension are severe coronary artery disease, hypertension combined with arteriosclerosis of cerebral vessels and increased intracranial pressure in patients with cerebral disease. Complications are rare in otherwise healthy patients, but may be higher in elderly patients and those with underlying organ dysfunction. Therefore, careful assessment and selection of patients, together with consideration of the potential complications, appropriate choice of drugs and invasive beatby-beat monitoring, are essential for the safe practice of induced hypotension.

    Description / Table of Contents: Abstract Objective: To determine the influence of gamma-hydroxy-butyrate (GHB) on spontaneous and lipopolysaccharide (LPS)-stimulated release of tumour necrosis factor-alpha (TNF), interleukin-1 β (IL-1β), interleukin-6 (IL-6) and interleukin-10 (IL-10) in whole blood from patients undergoing coronary artery bypass grafting (CABG) with extracorporeal circulation (ECC). In addition, the pharmacological modulation on lipopolysaccharide (LPS)-stimulated cytokine release by GHB (GHB-Na and GHB-ethanolamide) was characterized in a separate in vitro-assay. Methods: In a prospective, randomized, double-blinded study, 12 patients undergoing elective CABG were assigned to receive either saline (control) or GHB-Na (25 mg/kg as loading dose followed by 25 mg/kg/h) intraoperatively. Blood samples were obtained (A) preoperatively, (B) 20 min after ECC and (C) 24 h after ECC. Plasma levels (spontaneous release) as well as LPS-stimulated cytokine secretion were measured in a whole blood culture system ex vivo and correlated with mRNA-expression in peripheral blood mononuclear cells (PBMC). In addition, the dose-response characteristics of modulation of the cytokine response by GHB was studied in vitro in the same assay. Results: Plasma IL-6 and IL-10 levels were significantly elevated after CABG, while TNF and IL-1β were detectable only occasionally in both groups. Expression of all cytokines studied was significantly reduced upon ex vivo LPS-stimulation at time point B. Despite maintained expression of TNF and IL-1β m-RNA-transcripts upon ex vivo LPS-stimulation in patients treated with GHB, release of the cytokines in the supernatant was decreased to a similar degree as in the control group. Cytokine response upon LPS-stimulation was restored 24 h after CABG for the group mean, however, with substantial individual heterogenity. In vitro, pharmacological doses of GHB-Na (2 mg/ml) attenuated LPS-induced IL-1β release. However, application of the GHB-receptor antagonist NCS-382 caused a nearly complete cessation of IL-1β release in vitro (to 2,5% of control). GHB-ethanolamide (LK 544) did not influence the LPS-stimulated release of the cytokines studied. Conclusion: The results suggest a biphasic response of stimulated PBMC cytokine gene expression during CABG with an initial tolerance to LPS-stimulation shortly after termination of ECC. However, whether or not PBMC express functional GHB receptors remains unclear in light of contradictory effects of the different ligands. In spite of the ex vivo and in vitro results, application of GHB-Na in doses which are primarily based on its use as an anesthetic agent do not seem to modulate the release of the cytokines studied.
    Notes: Zusammenfassung Fragestellung: Die Untersuchung des Einflusses von Gamma-Hydroxy-Buttersäure (GHB) auf die spontane und durch Lipopolysaccharid (LPS) induzierte Freisetzung der Zytokine Tumor-Nekrose-Faktor alpha (TNF), Interleukin-1β (IL-1β), Interleukin-6 (IL-6) und Interleukin-10 (IL-10) bei aortokoronaren Bypassoperationen (ACB) unter Anwendung der extrakorporalen Zirkulation (EKZ) sowie die Charakterisierung des pharmakologischen Effekts von GHB in 2 Präparationen (GHB-Na und GHB-Ethanolamid) auf die Lipopolysaccharid (LPS)-induzierte Zytokinfreisetzung in vitro. Methodik: In einer prospektiven, randomisierten Doppelblindstudie wurden insgesamt 12 Patienten untersucht, die sich einer elektiven ACB unterzogen. Je 6 Patienten erhielten intraoperativ entweder NaCl 0,9% (Kontrollgruppe) oder GHB-Na (25 mg/kg/h nach einem initialen Bolus von 25 mg/kg). Zu insgesamt 3 Meßzeitpunkten (A=präoperativ, B=20 min nach EKZ, C=24 h postoperativ) erfolgte die Blutentnahme zur Zytokindiagnostik. Die Plasmakonzentrationen (spontane Freisetzung) und die unter LPS-Stimulation beobachtete Freisetzung der verschiedenen Zytokine zu den einzelnen Meßzeitpunkten wurden in einem Vollblutansatz ex vivo gemessen und mit der m-RNA-Expression in peripheren mononukleären Zellen (PBMC) korreliert. Weiterhin erfolgte in einem in vitro-Ansatz die Analyse des pharmakologischen Einflusses von GHB selbst (GHB-Na und GHB-Ethanolamid) auf die LPS-induzierte Zytokinfreisetzung. Ergebnisse: Die Plasmakonzentrationen von IL-6 und IL-10 waren nach Ende der EKZ in beiden Gruppen (Kontrolle und GHB) im Vergleich zum präoperativen Ausgangswert signifikant erhöht, während TNF und IL-1β nur vereinzelt nachweisbar waren. Am Ende der EKZ war zu diesem Meßzeitpunkt die durch LPS stimulierbare Freisetzung aller untersuchten Zytokine ex vivo signifikant gegenüber dem präoperativen Ausgangswert vermindert. Trotz besser erhaltener Stimulierbarkeit der Zytokin-m-RNA war auch bei den mit GHB behandelten Patienten die Ausschüttung der Zytokine ex vivo signifikant gehemmt. Am ersten postoperativen Tag war die stimulierbare Zytokinantwort im statistischen Mittel wieder hergestellt, wobei deutliche interindividuelle Unterschiede auftraten. In vitro (pharmakologischer Dosierungen) bewirkte GHB-Na (2 mg/ml) eine signifikante, selektive Verminderung der Freisetzung von IL-1β, während GHB-Ethanolamid keinerlei Veränderungen der Zytokinantwort bewirkte. Zusätzlich hemmte der kompetitive GHB-Rezeptorantagonist NCS-382 die monozytäre IL-1β-Antwort fast vollständig auf 2,5% des Ausgangswerts ohne NCS. Schlußfolgerung: Die Ergebnisse der Freisetzung verschiedener pro- und antiinflammatorischer Zytokine bei ACB zeigen einen biphasichen Verlauf. Initial kommt es ex vivo zu einer relativen Suppression mit partieller Toleranz gegenüber LPS-Stimulation, die am ersten postoperativen Tag weitgehend überwunden ist. Ob die pharmakologischen Effekte von GHB-Na und NCS-382 auf die IL-1β-Freisetzung monozytärer Zellen über spezifische GHB-Rezeptoren vermittelt sind, muß aufgrund der diskrepanten Ergebnisse offen bleiben. GHB-Na bewirkt in klinisch üblichen Dosierungen im Vergleich zur Kontrollgruppe keine statistisch nachweisbare Modulation der Zytokinfreisetzung.

    Notes: Abstract Objective: To characterize the impact of abdominal aortic aneurysm repair (AAAR) on spontaneous as well as lipopolysaccharide (LPS)-induced gene expression of pro- and anti-inflammatory cytokines. Design: Prospective, controlled in vivo / ex vivo study. Setting: University hospital. Patients and interventions: Whole blood from 14 consecutive patients undergoing AAAR withdrawn prior to surgery (T1), at the end of ischemia (T2), 90 min after declamping (T3) and on the first postoperative day (T4) was cultured in the absence or presence of LPS. Five patients undergoing elective inguinal hernia repair served as controls. Measurements and results: While tumor necrosis factor (TNF), Interleukin (IL)-1 and IL-10 plasma concentrations did not increase significantly, IL-6 was elevated at each time point, as compared with T1. Despite the spontaneous release of trace amounts of IL-6, the ability of cultured whole blood to mount a cytokine response in vitro to LPS was impaired for all cytokines studied at T2 (TNF –62 %, IL-1 –51 %, IL-6 –20 %, IL-10 –51 %). The stimulated IL-6 response was restored early after declamping (T3: + 56 %) and enhanced 1 day after operation (T4: + 144 %). In contrast, stimulated TNF and IL-1 responses remained depressed at T3 (TNF –48 %, IL-1 –64 %) and T4 (TNF –40 %, IL-1 –24 %). A biphasic pattern was observed for IL-10 with initial depression at T3 (-51 %) and restoration at T4 ( + 40 %). Among the different cytokines monitored, only impaired TNF responsiveness at early reperfusion (T3) correlated with the postoperative course, as reflected by APACHE II. Cytokine response to LPS was maintained or even increased during and after surgery in the whole blood from patients undergoing hernia repair. Conclusions: Despite consistent development of clinical signs of systemic inflammatory response syndrome (SIRS) and spontaneous release of IL-6 abdominal aortic aneurysm repair produces a state of impaired pro-inflammatory cytokine response upon a subsequent in vitro Gram-negative stimulus. This early impairment of TNF responsiveness seems to correlate with an unfavorable postoperative course.

    Notes: This prospective, clinical trial evaluated the effects of short-term propofol administration on triglyceride levels and serum pancreatic enzymes in children undergoing sedation for magnetic resonance imaging. Laboratory parameters of 40 children, mean age (SD; range) 67 (66; 4–178) months undergoing short-term sedation were assessed before and 4 h after having received propofol. Mean (SD) propofol loading dose was 2.2 (1.1) mg.kg−1 followed by continuous propofol infusion of 6.9 (0.9) mg.kg−1.h−1. Serum lipase levels (p = 0.035) and serum triglyceride levels (p = 0.003) were raised significantly after propofol administration but remained within normal limits. No significant changes in serum pancreatic-amylase levels were seen (p = 0.127). In two (5%) children, pancreatic enzymes and in four (10%) children triglyceride levels were raised above normal limits; however, no child showed clinical symptoms of pancreatitis. We conclude that even short-term propofol administration with standard doses of propofol may have a significant effect on serum triglyceride and pancreatic enzyme levels in children.
