    Notes: Summary An autopsy case of mitochondrial encephalomyopathy with lactic acidosis and stroke-like episodes (MELAS) is reported. It presented with generalized muscle atrophy, stroke-like episodes, schizophrenia-like mental disorder and progressive dementia. Serum lactate and pyruvate levels were high. In the biopsied muscles, ragged-red fibers were observed by light microscopy and aggregation of abnormal mitochondria with paracrystaline formation by electron microscopy. The most characteristic neuropathological findings were infarct-like lesions widespread in the cerebral cortex. In addition, this case showed some unusual pathological features: (1) diffuse moderate fibrillary gliosis in the whole cerebral and cerebellar white matter, which might have been due to metabolic disturbances; (2) several focal lesions with demyelination and numerous spheroids in the pontocerebellar fibers; and (3) marked degeneration of the posterior columns and spinocerebellar tracts. Electron microscopic examination revealed that abnormal mitochondria were markedly aggregated in smooth muscle cells and endothelium of the cerebral and cerebellar blood vessels. These fine structural findings suggest a “mitochondrial angiopathy”.

    Notes: Abstract Background: Computed tomography during arterial portography (CTAP) under temporary balloon occlusion of the hepatic artery (BOHA-CTAP) was introduced to evaluate pseudolesions caused by portal venous impairments such as arterioportal shunt and tumor thrombus. Methods: BOHA-CTAP was performed in seven patients with hepatocellular carcinoma and correlated with clinical outcomes. For patients with wedge-shaped defects suggestive of pseudolesions, BOHA-CTAP was obtained by a 5-F balloon occlusion catheter into the proper hepatic artery through the second 5-F introducer inserted into the common femoral artery a few centimeters below the first 5-F sheath for CTAP. Results: Eight pseudolesions were determined clinically on follow-up CT, ultrasonography, or magnetic resonance imagings. On BOHA-CTAP, five of the eight pseudolesions were eliminated, and two were diminished in comparision with conventional CTAP. One wedge-shaped defect due to tumor thrombus in the portal vein did not show any change. Conclusion: BOHA-CTAP can reduce pseudolesions caused by portal venous impairments and enable the demarcation of the true tumors. RID="" ID="" 〈E5〉Correspondence to:〈/E5〉 J. Koizumi

    Notes: Abstract. We describe the imaging features in a patient with pseudomyxoma retroperitonei arising from a mucinous carcinoma of the appendix with spontaneous development of a skin fistula. The thick wall and septa characteristic of pseudomyxoma were best demonstrated on gadolinium-enhanced magnetic resonance imaging. This patient illustrates (a) the potential of pseudomyxoma to cause skin fistula and (b) the possibility that this intraabdominal process can first present as a dermatologic complication.

    Notes: Abstract The middle mesenteric artery, a third mesenteric artery arising from the aorta that principally feeds the transverse colon, is an extremely rare anomaly. We identified a middle mesenteric artery branching into the ileocolic artery and into the right, middle, and accessory middle colic arteries. It supplied the cecum and the entire ascending and transverse colon. This anomaly was detected with computed tomographic angiography.

    Notes: Summary The tuber cinereum of hypothalamus, cerebral cortex, cerebellar cortex and caudate nucleus of rabbits were examined under the electron microscope following intramuscular administration of chlorpromazine with special consideration of ultrastructural changes in amount and distribution of glycogen granules in their hypothalamus. In these regions, normal astrocytes and their processes contain glycogen granules diffusely scattered in the cytoplasm. In the neurons of the normal hypothalamus and cerebellar cortex, glycogen granules are seen in some presynaptic endings and distal parts of dendrites but not in the perikaryal cytoplasm. In the tuber cinereum of the hypothalamus, after chlorpromazine administration, abundant glycogen granules accumulate at the postsynaptic sites, especially in peripheric parts of dendrites, and clusters of glycogen granules appear in the perikaryal cytoplasm of the nerve cells. These findings are interpreted as an increase of glycogen in these cellular regions and the suggestion is made that chlorpromazine inhibits the glycolytic metabolism in the distal parts of dendrites, particularly at postsynaptic sites and in the perikarya of nerve cells of the hypothalamus.

    Notes: Summary Following administration of trifluoperazine (a psychotropic drug of the phenothiazine group), the brains of rabbits were examined by electron microscopy, with special reference to the glycogen changes in the globus pallidus, and were compared with the normal materials. After seven days' intramuscular injections of trifluoperazine (10 mg/kg/day), abundant glycogen granules were accumulated in dendrites of pallidum. Those animals showed extrapyramidal symptoms throughout the period. The administration of the drug might inhibit either neural activity or the glycolytic metabolism in dendrites of the pallidum.

    Notes: Summary Following administration of psychotropic drugs, the brains of rabbits were examined by electron microscopy, with special references to glycogen granules in the hippocampus, and were compared with the normal materials. After seven days' intramuscular injections of chlorpromazine (15 mg/kg/day) or trifluoperazine (10 mg/kg/day), glycogen granules appeared diffusely in the perikaryal cytoplasm of the pyramidal cells and small nerve cells and in the myelinated nerve fibers, and were accumulated in the dendrites of pyramidal cells and in the astrocytic cytoplasm. Those animals showed neuroleptic symptomes and extrapyramidal ones throughout the period. The administration of the drugs might inhibit the glycolytic metabolism in neurons and glial cells of the hippocampus.
