Die primäre Gelenkknorpelverletzungen kommt als Kontusion, Impression oder osteochondrale Fraktur vor. Daraus können Chondromalacie, Chondropathia patellae und traumatische Osteochondrosis dissecans entstehen. Die Latenzzeit der Beschwerden lag zwischen 1 und 2 Jahren. Experimentell wurden Heilung und Transplantation verfolgt. Operatives Vorgehen waren Abtragung, Herdausräumung, Spongiosa- oder Knorpeltransplantation, Ventralisation. Ergebnisse und Fälle werden beschrieben.
    Notes: Summary In our follow-up study on 52 patients after approximately 21 months, the lesions most frequently observed were lesions of articular cartilage of the knee. Primary injury to articular cartilage takes the form of contusion, impression or osteochondral fracture, from which chondromalacia patellae and traumatic osteochondrosis dissecans can develop. Latency of the symptoms was 1 to 2 years. We observed transplantation and the healing process under experimental conditions. The surgical procedure consisted in shaving down of the cartilage, excision of the osteochondral lesion, transplantation of cancellous bone or articular cartilage grafts, and ventral transposition of the tibial tubercle. Results are discussed.

    Description / Table of Contents: Zusammenfassung Bei 112 Patienten wurden 62 Knorpelglättungen, 45 Ventralisationen nach Bandi und 5 primäre Patellektomien durchgeführt. 103 Patienten wurden durchschnittlich 1,5 Jahre nach Operation nachuntersucht. 80 % fühlten sich gebessert. Objektiv ließen sich 67 sehr gute und gute, 31 mäßige und 5 schlechte Ergebnisse feststellen. Die Analyse der unbefriedigenden Ergebnisse bestätigte mehrfach eine überforderung der jeweiligen Operationsmethoden. Im Falle beginnender femoropatellarer Arthrose empfiehlt es sich, die primäre Patellektomie zu erwägen.
    Notes: Summary Abrasion of cartilage was performed in 62 patients, ventral tilting after Bandi in 45, and primary patellectomy in 5. Of these 112 patients, 103 were examined at follow-up clinics, on average $$1{\raise0.5ex\hbox{$\scriptstyle 1$}\kern-0.1em/\kern-0.15em\lower0.25ex\hbox{$\scriptstyle 2$}}$$ years after the operation; 80 percent felt the operation had been beneficial. The authors found the results were good or very good in 67 cases, fair in 31, and poor in 5. In the cases with fair and poor results the surgical operation performed seems to have been inadequate. Primary patellectomy should be considered in cases of femoropatellar arthrosis.

    Description / Table of Contents: Zusammenfassung Die Behandlungsergebnisse von 33 schwersttraumatisierten kindlichen Extremitäten rechtfertigen fast immer einen operativen Erhaltungsversuch. Als limitierender Faktor erweist sich die Überschreitung der Ischämiezeit. Typische Komplikationen wie Infekt, Compartment-Syndrom und trophische Störungen bei Gefäß-/Nervenläsionen beeinflussen die Prognose. Defektfrakturen können infolge großer reparativer Potenzen bei Kindern unter Längenerhaltung stabil versorgt und sekundär mit autologer Spongiosa aufgebaut werden. Der Hautverschluß muß grundsätzlich spannungsfrei erfolgen.
    Notes: Summary The results of the treatment of 33 severely injured limbs of children justify aiming at surgical preservation. A limiting factor is the period of ischaemia. The prognosis is adversely affected by typical complications such as compartment syndrome and damage to vessels and nerves. Fractures with bone defects are stabilised under preservation of the length and built up with cancellous bone graft. Wound closure without tension is fundamental.

    Description / Table of Contents: Zusammenfassung Die Osteosynthese von Frakturen mehrerer langer Röhrenknochen bei Schwerverletzten Patienten erfordert eine differenzierte Indikationsstellung. Primäre Operationen dürfen nicht dazu führen, daß die vitalen Funktionen beeinträchtigt werden. Zu den Eingriffen erster Priorität zählen nur die zum Erhalt der Extremität notwendigen. Der Verlauf bei 109 Patienten mit 280 Frakturen wird analysiert. Wie 44 gute Ergebnisse bei 70 nachuntersuchten Patienten zeigen, kann eine adäquate Therapie den Einfluß der Verletzungsschwere auf die Prognose reduzieren.
    Notes: Summary Operative fracture treatment of several long bones of severely injured patients requires a differentiated indication. Interference with vital functions by primary operation has to be avoided. Only limb-saving surgery ranks among the procedures of first priority. The course of 109 patients with 280 fractures of long bones is analyzed. Follow-up of 70 patients with 44 good results demonstrates that the severity of the trauma can prognostically be reduced by adequate therapy.
