Summary The factors which could disturb blood coagulation during prolonged cerebral operations have been investigated. There is no free plasmin neither in brain tissue nor in tumour tissue. An increased fibrinolysis is for this reason unlikely. In contrast a high level of the activators of Plasminogen are to be found both in brain tissue and especially in messenchymal tumours, but not in gliomas. An exception is the medulloblastoma which contains a large amount of activators. This observation supports the view that the medulloblastoma is a mesenchymal tumour (sarcoma). In all tumours large amounts of activators for the thromboplastic system occur. Because of this an exhaustion coagulopathy can be advanced. Trasylol® has been employed in higher dosage because as a more natural proteinase inhibitor it inhibits the activators of both systems.

Résumé Les facteurs qui peuvent perturber la coagulation sanguine pendant une opération cérébrale prolongée ont été étudiés. Il n'y a pas de plasmine libre ni dans le tissu cérébral ni dans le tissu tumoral. Une augmentation de la fibrinolyse est, pour cette raison, impossible. En contraste on trouve des activateurs du plasminogène dans le tissu cérébral et aussi dans les tumeurs mésenchymateuses, mais pas dans les gliomes. Une exception est représentée par les médulloblastomes qui contiennent une grande quantité d'activateurs. Cette constatation conduit à l'idée que le médulloblastome est une tumeur mésenchymateuse (sarcome). Dans toute tumeur une grande quantité d'activateurs pour le système thromboplastique survient. A cause de cela, les coagulopathies peuvent survenir. Le Trasylol® est encore utilisé en haut dosage parce comme un des plus naturels inhibiteurs de la protéinase, il inhibe les activateurs de deux systèmes.

Zusammenfassung Es wurde untersucht, welche Faktoren die Blutstillung während langdauernder Gehirnoperationen stören können. Weder Hirngewebe noch Tumorgewebe verfügen über freies Plasmin. Eine gesteigerte lokale Fibrinolyse ist deshalb unwahrscheinlich. Dagegen finden sich reichlich Aktivatoren des Plasminogens im Hirngewebe und besonders in mesenchymalen Tumoren, nicht aber in Gliomen. Eine Ausnahme bildet das Medulloblastom, das reichlich Aktivatoren hat. Dies spricht dafür, daß das Medulloblastom ein mesenchymaler Tumor (Sarkom) ist. In allen Tumoren finden sich reichlich Aktivatoren für das thromboplastische System. Dadurch kann eine Verbrauchskoagulopathie gefördert werden. Therapeutisch wird Trasylol® in hoher Dosierung empfohlen, weil es als natürlicher Proteinasenhemmkörper die Aktivatoren beider Systeme hemmt.
