    Description / Table of Contents: Zusammenfassung An 11 Patienten an der Intensivbehandlungsstation als potentielle Nierenspender aufgenommen, wurden Kreislauf- und Nierenfunktion vor und während Dopamininfusion (175 µg/min) untersucht. Die Befunde der Kontrollperiode zeigten eine hochgradige Einschränkung der renalen Tubulusfunktion, die auf den Wegfall der zentralen Regulation hinweist. Während der Dopamininfusion zeigten die Kreislaufmessungen einen geringgradigen Anstieg von Blutdruck und Frequenz. Harnzeitvolumen und Nierendurchblutung stiegen ebenso wie die Natriumexkretion signifikant an, während die Inulin-Clearance nur eine geringe Zunahme aufwies. Die renale Funktionseinschränkung unter „Hirntod“-Bedingungen sowie der Einfluß von Dopamin auf Kreislauf und Nierenfunktion werden diskutiert. Die Ergebnisse weisen darauf hin, daß Dopamin bei der Vorbereitung des Nierenspenders für die Organentnahme einen günstigen Effekt besitzt.
    Notes: Summary Haemodynamics and renal function have been examined before and during i.v. infusion of dopamine (175 µg/min) in 11 patients admitted as potential kidney donors to an intensive care unit. The findings of the control period revealed an extreme decrease of renal tubular function which points to a cessation of central regulation. The cardiovascular system showed a slight increase of blood pressure and heart rate during dopamine. Urine flow and renal blood flow increased significantly as well as the excretion of sodium while inulin-clearance showed only a minimal rise. The functional renal impairment under conditions of brain death as well as the influence of dopamine on circulation and renal function are discussed. The results indicate that dopamine may exert a favourable effect in preparation of kidney donors for the withdrawal of the organs.

    Description / Table of Contents: Summary The fate of intrathecally administered homologous tetanus antitoxin (human tetanus hyperimmune globulin) was studied in 2 cases of severe tetanus with bioassay of antibody titers and determination of CSF and serum immunoglobulin concentrations. During the first 6 hours, the initially high antibody titers in CSF dropped steeply, and subsequently at a much slower rate over the following days. Although the appearance of antibodies in the serum inversely reflected their disappearance from the CSF, only less than 10% of the dose administered could be recovered in CSF and serum after 2 days. The possible causes for this observation are discussed. By intralumbal route, however, appreciable titers of tetanus antitoxin are attained in the CSF, with smaller doses of homologous antibodies than those generally applied intramuscularly and intravenously in tetanus treatment.
    Notes: Zusammenfassung Das Verhalten eines intralumbal applizierten homologen Tetanusantitoxins (Tetabulin 1000 IE) wurde bei zwei an manifestem Tetanus erkrankten Patienten geprüft. Die initial hohen Antitoxintiter-Werte im Liquor fielen innerhalb der ersten 6 Std rasch ab, wobei es gleichzeitig zu einem gegensinnigen Anstieg der Serumtiter-Werte kam. Nach 2 Tagen waren jedoch mehr als 90% der verabreichten Antikörperdosis weder im Liquor noch im Serum nachweisbar. Im Liquor konnten auch über längere Zeit (3 Tage) mit geringerer Dosis als über den Blutweg relativ hohe Titer-Werte erreicht werden.

    Notes: Abstract. A Photon Scanning Tunneling Microscope (PSTM) with probe–sample distance control by electron tunneling is used to probe the localized surface-plasmon fields of individual nanometric silver particles. As samples, conventional island films produced by thermal evaporation and regular particle arrays produced by an electron-beam-lithography-based technique, respectively, are used. In either case the strength and spatial localization of the surface-plasmon fields strongly depend on the excitation wavelength. The results are interpreted as different resonance frequencies of individual particles or of different sample areas. On regular arrays consisting of particles with a smallest diameter of 40 nm, the PSTM maps represent the plasmon field strength spatially resolved for individual particles.

    Notes: The optical response of regularly arranged noble metal wires with nanoscopic cross sections (nanowire gratings) strongly depends on the polarization direction of the incident light. We use silver and gold nanowire gratings produced by electron beam lithography to study this effect by optical extinction spectroscopy. For a polarization direction perpendicular to the wire axis, the excitation of a dipolar plasmon mode dominates the extinction spectrum. The spectral position of the plasmon resonance can be tuned by an appropriate choice of nanowire geometry and material. For a polarization direction parallel to the wire axis, the profile of the extinction spectrum varies mainly as a function of the grating constant. In particular, a transmission maximum for small grating constants is found. By combining the surface plasmon excitation and grating effect for orthogonal polarization directions, a spectrally selective polarizer with an extinction ratio of 26 is demonstrated. © 2001 American Institute of Physics.

    Notes: Addressing the fundamental question of miniaturization of light guiding and routing towards nanoscale optics, we study experimentally surface plasmon propagation in silver and gold thin films of finite widths in the micrometer range. Spatially confined excitation of surface plasmons is realized by a prism coupling arrangement involving an opaque aluminum screen for a distinct separation of excitation and propagation (measurement) region. The surface plasmon propagation length as a function of film widths is measured by detecting stray light due to surface plasmon scattering with a conventional optical microscope. © 2001 American Institute of Physics.

    Notes: We report on the experimental observation of multipolar plasmon excitations in lithographically designed elongated silver particles. In contrast to spheres, where the extinction bands of the respective multipolar plasmons overlap considerably to form a broad spectrum, spectrally well-separated extinction bands corresponding to plasmons of multipolar order up to n=6 are found. The results agree well with numerical simulations based on the Green's Dyadic method. © 2000 American Institute of Physics.

    Notes: Summary The author presents four cases with tracheostomies which were primarily placed unfavourably—in three cases inferior tracheostomy, in one case superior tracheostomy—to illustrate possible complications of classical tracheostomy. The necessity of very early surgico-plastic correction measures is emphasized, even in the presence of local infections.

    Notes: Abstract Based on experiences with 256 kidney transplant operations in 239 patients over a period of ten years (18. 6. 1965 to 15. 4. 1975) the role of the anaesthesiologist as it had developed in our institution has been described. Some problems with regard to the anaesthetic management of living donors were discussed. The care of patients who, due to their deteriorating cerebral function, are potential donors was also described. An outline of the own anaesthetic management of the recipient has been given and alternative methods were mentioned. Some special points were specifically stressed like the choice of relaxants, fluid replacement, blood transfusion, diuretic treatment starting already intraoperatively. Good intensive care of the potential donor is of great importance for the function of the transplanted organ. Very important is also the postoperative treatment of the recipient, which in our institution is performed in a teamwork between anaesthesiologist, surgeon and nephrologist. Finally, the functional results of our own series are briefly demonstrated.
