    Notes: Abstract In a double-blind clinical study, electrocardiogram, blood pressure and systolic time intervals were measured in 40 depressive patients treated with either paroxetine (30 mg/day) or amitriptyline (150 mg/day) for 6 weeks. While amitriptyline significantly increased the heart rate, the QTc interval and the PEP/LVET ratio, paroxetine did not alter any of the cardiovascular parameters measured.

    Description / Table of Contents: Zusammenfassung Bei Magersüchtigen (n = 20) fand sich eine relativ enge Beziehung zwischen pathologischen Ergebnissen im DST und dem Grad der Gewichtsabnahme. Bulimische Verlaufsvarianten hoben sich diesbezüglich nicht von Syndromen ab, die die Kerngruppe der Anorexia nervosa bilden. Und auch die Krankheitsdauer war ohne Bedeutung. Diese Befunde werden als Hinweis auf sekundäre Regulationsstörungen im Bereich des hypothalamisch-hypophysäradrenokortikalen Systems gedentet, nicht aber als Beleg für enge verwandtschaftliche Beziehungen des Krankheitsbildes zur endogenen Depression. Wie von anderer Seite nachgewiesen wurde, wirkt sich auch bei ihr die Variable Gewichtsveränderung wesentlich auf die DST-Resultate aus.
    Notes: Summary In patients suffering from anorexia nervosa (n = 20) a significant correlation was found between pathological DST results and weight loss. In this respect, patients with bulimic symptomatology do not differ from the main anorectic group. In addition, the correlation was unaltered by the length of the disease. These results are interpreted as indicative of secondary regulatory disturbances of the hypothalamic-hypophyseal-adrenal axis, whereas they are inconsistent with a close connection between anorexia nervosa and endogenous depression. As demonstrated by other authors, weight change exerts an influence on DST results in endogenous depression as well.

    Notes: Summary The objective of this study was to identify clinically relevant predictors of short-term (2 years) outcome in schizophrenic patients and to gather these predictors into one simple prognosis model for the course of the disease. Ninety-nine schizophrenic outpatients classified according to DSM-III criteria were observed for 3 years with regard to their relapse (rehospitalization) frequency. Predictors of both a high and a low relapse risk permitting a prognostic assessment of the individual case were based on various sources: case history data, especially those relating to the previous course of the disease; cocial and therapeutic situation; psychopathological findings; emotional family atmosphere; and prognosis scales.

    Description / Table of Contents: Summary A report is given about a 57-year-old woman with a major depression, melancholic subtype. While under amitriptyline 75 mg/day no adverse effects were observed, immediately after increasing the dosage to 150 mg/day ”head spasms” with headache, profuse sweating and arterial hypertension appeared. The diagnosis of a pheochromcytoma was made and adrenalectomy was performed. This is the first case of unmasking a pheochromcytoma by amitriptyline; furthermore a connexion was found with the daily dosage of the antidepressant.
    Notes: Zusammenfassung Es wird über eine 57jährige melancholische Patientin berichtet, bei der nach einer Dosiserhöhung von Amitriptylin von 75 auf 150 mg/die „Kopfkrämpfe” mit heftigen Kopfschmerzen, Schweißausbrüchen und hypertonen Blutdruckwerten auftraten. Es wurde ein Phäochromozytom diagnostiziert und operativ entfernt. Dies ist die erste Fallbeschreibung einer Demaskierung eines Phäochromozytoms durch Amitriptylin; des weiteren konnte ein Zusammenhang mit der Dosis des Antidepressivums nachgewiesen werden.
