    Notes: Summary A computerized tomographic (CT) scanner system for intraoperative imaging is presented. The system consists of the following: 1) CT scanner with a mobile gantry, 2) digitally controlled operating table with central processing unit (CPU) and encoder unit; the table can be controlled by the scanner computer as accurately as the scanner bed, and 3) exclusively designed head fixation devices. It allows us to scan the patient on the operating table in the operating room pre-operatively, intra-operatively and immediately after surgery.

    Notes: Summary  An intraoperative classification of intradural internal carotid artery (ICA) aneurysms not related to the arterial division but based on their operative presentation and clipping techniques is introduced. On the basis of the surgeon's view of the operative field via the pterional approach in 156 operated intradural ICA aneurysms in 143 patients, these aneurysms were classified according to their location in relation to the long axis of the ICA as either proximal, middle or distal in type and also according to their relation to the cross section of the ICA as either lateral, medial, ventral or dorsal in type. Numerically the largest in frequency is the middle type of aneurysm by axial location and the lateral type of aneurysms by cross sectional location. Eighty five percent of the lateral type aneurysms were at the arterial division. The majority of the large to giant aneurysms were of the ventral type and no dorsal type aneurysms were seen. About one third of the ICA aneurysms in this series were located free of the arterial division. All dorsal type aneurysms and most of the medial type aneurysms were not related to the arterial division.  Clipping techniques were classified into perpendicular and parallel clipping, as to the direction of the clip-blades in relation to the carotid axis. The parallel clipping was further classified into forward clipping, in which a clip was applied from the distal side of the ICA, and reversed clipping, in which a clip was applied from the proximal side of the ICA. Most of the aneurysms located at the arterial division required the perpendicular clipping and those free of the arterial division required the parallel clipping. Furthermore, the forward clipping was useful for proximal type aneurysms and the reversed clipping for distal type aneurysms. For the middle type aneurysms clipping was performed bidirectionally.  This classification includes all types of the ICA aneurysms located at any points along its long axis and on its cross section, and is useful for planning safe and exact clipping of the ICA aneurysms.

    Notes: Summary The microsurgical anatomy of the arteries of the pituitary stalk and gland as viewed from above was studied in 50 adult cadaveric hemispheres using the operating microscope. There were three types of vessels to the pituitary from above: the superior hypophyseal artery originating from the internal carotid artery, the infundibular artery from the posterior communicating artery, and the prechiasmal artery from the ophthalmic artery. The superior hypophyseal artery originated from the medial to posterior aspect of the internal carotid artery. The average number of vessels of the superior hypophyseal artery was 2.2 per hemisphere, and the diameter was 0.25 mm on average. The majority (76%) of superior hypophyseal arteries arose from the proximal half of the segment between the origins of the ophthalmic and posterior communicating arteries of the internal carotid artery. The infundibular artery originated mainly from the medial side (69%) of the posterior communicating artery. Its diameter was 0.22 mm, and number 0.23 per hemisphere. The number of prechiasmal arteries was 0.06 per hemisphere. As a result, there were on average 2.5 vessels per hemisphere, totally 5 per brain, with the average diameter 0.25 mm, supplying the pituitary stalk and gland from above. The clinical application of these anatomical data to the diagnosis and treatment of suprasellar tumours and carotidophthalmic aneurysms is discussed.

    Notes: Summary A newly developed head fixation for intraoperative computerized tomographic (IOCT) scanning is presented. The system is developed based on the head holder of multipurpose head frame and is made of two kinds of advanced engineering material; carbon fiber reinforced plastic for head holder and frames, polyamide-imide polymer for joints, screws, and head pin. Clinical tests including autoclaving and sterilization were performed and revealed all materials had sufficient strength for clinical use. This fixation system enables us to increase the efficacy of IOCT scanning during open-field neurosurgery.

    Notes: Summary Basal forebrain cells of foetal rats were transplanted into the subarachnoid space of adult rats harbouring a kainic acid-induced unilateral lesion in the nucleus basalis magnocellularis. Passive avoidance response tests were performed eight weeks after the transplantation, and the results were compared with those of lesioned but non-transplanted rats and of non-lesioned control rats. Although acquisition impairments did not improve, retention impairments were significantly ameliorated in the transplanted rats. Histologically, transplanted foetal neurons survived and grew very well over the cortical surface, and exhibited facilitated neuritic elongation on acetylcholinesterase staining. Choline acetyl-transferaseimmunoreactive neurons were found along the needle track as well as in the subarachnoid graft tissues. The results seem to indicate that not the re-innervation from the graft to the host cortex but the diffusional supply of neurotransmitters and/or their synthetic enzymes and neurotrophic factors were responsible for improvement of memory deficits. The subarachnoid space proved to be an adequate place for growth of transplanted neuronal and glial cells for reasons of ample supply of oxygen and nutrition and of low tissue pressure.

    Notes: Summary The surgical outcome of 221 cases with multiple intracranial aneurysms operated upon during the years 1988 to 1994 were reviewed. The patients were classified into three groups according to the locations of the aneurysms; group 1: multiple aneurysms located unilaterally in the anterior circulation only (147 cases); group 2: multiple aneurysms located bilaterally in the anterior circulation only (44 cases) and group 3: multiple aneurysms located in both anterior and posterior circulation or in the posterior circulation alone (30 cases). In 132 cases of group 1 (89.8%) all aneurysms were treated in one-stage operations. Twenty-eight patients from group 2 (63.6%) received partial treatment, where only the ruptured or the symptomatic aneurysms were treated. In 12 other cases from group 2 (27.3%) all multiple aneurysms were treated in two-stage operations. In group 3 patients, one-stage operations were performed in 18 cases (60%), while 9 patients (30%) received partial treatment only. Of the 221 multiple aneurysm cases, 162 (73.3%) presented with manifestations of subarachnoid haemorrhage (SAH). The remaining 59 multiple aneurysms cases (26.7%) presented with manifestations other than SAH (unruptured aneurysms). In the postoperative follow-up, of the 221 multiple aneurysms cases, 113 (51.1%) were free of neurological deficit (excellent), 48 cases (21.7%) were capable of leading an independent life (good), 32 cases (14.5%) were not independent and needed to be assisted (fair), and 28 patients (12.7%) died. These results were comparable to the results of patients with single aneurysms operated on during the same period. Based on our results, we recommend that whenever possible all multiple aneurysms should be treated in one-stage operations. In unruptured multiple aneurysm cases surgical management is the recommended treatment. In poor grade SAH patients or unruptured multiple aneurysms in old patients, two-stage operations or partial treatment of only the ruptured or the symptomatic aneurysms may be adopted.

    Notes: Summary Aneurysms arising from the dorsal wall of the internal carotid artery are rare. The authors surgically treated twenty dorsal internal carotid artery aneurysms. Pre-operative angiographic findings were reviewed and classified into three types. Eleven aneurysms projecting superiorly on the lateral angiogram were found to be adherent to the base of the frontal lobe by the pterional approach. Five aneurysms which had been superimposed with the internal carotid artery on the lateral angiogram were found adhered to the medial surface of the temporal lobe. Four aneurysms not seen on the angiogram had no adhesion. Two aneurysms, which had not been seen on the initial angiograms, were visualized on the angiograms taken during the period of vasospasm. This type of aneurysms can be the source of a subarachnoid haemorrhage of unknown origin and requires repeated examinations. Premature rupture occured intra-operatively in five cases and postoperative bleeding was encountered in two. Clipping technique is discussed from the viewpoint of preventing intra- and postoperative rupture.

    Notes: Summary A newly developed brain spatula with transparent tip for brain retraction is introduced. The high-molecular polymer plastic material is used only for the tip of the new spatula whilst rest of the spatula is made up of the ordinary malleable metal. The transparent nature of the spatula tip helps us in observing the retracted brain vessels and cranial nerves in continuity with the main operating area. The extent of distortion as a result of the retraction can be directly observed assisting in prevention of an inadvertent injury. The new brain spatula with transparent tip is helpful for microneurosurgery under high magnification. Various shapes and sizes of the spatula can be used.

    Notes: Summary An immunofluorescence histochemical study of vasoactive intestinal polypeptide (VIP) was made in the major rat cerebral arteries of the whole mount preparation. A comparison was made between the distribution of VIP-immunoreactive and cholinergic nerves. An abundant number of VIP-containing nerves were observed in the internal carotid, anterior cerebral, middle cerebral and basilar artery. VIP and cholinergic nerves were unaffected by bilateral superior cervical ganglionectomy. The density and distribution of VIP-immunoreactive fibers was essentially the same as that of the cholinergic fibers of the rat cerebral vasculature. It is suggested that, as was previously demonstrated in other peripheral organs, VIP coexists within cholinergic neurons of the rat cerebral arteries.
