    Description / Table of Contents: Conclusion The secretory pancreatic function is studied during the intravenous tolbutamide test in 30 normal persons and 13 diabetic patients. For the evaluation of the secretion rate certain measures were developed. These measures showed significant differences in both groups of the absolute values of amylase (P ≦ 0,27%), of amylase in percentage of secretion-volume and of absolute carboxypeptidase secretion (0,27〈P ≦ 5%). The results indicate that the excretory pancreatic response as tested by tolbutamide is better in normal individuals than in diabetics.
    Notes: Zusammenfassung Die Untersuchung der exkretorischen Pankreasfunktion während des Tolbutamidtestes bei 30 Stoffwechselgesunden und 13 Diabetikern ließ für die Maßzahlen der absoluten Diastasewerte signifikante Unterschiede (P ≦ 0,27%) zwischen beigesteigerte exkretorische Pankrasleistung nach Tolbutamid bei Gesunden gegenüber Diabetikern hin.

    Description / Table of Contents: Summary To obtain a better assessement of subclinical diabetics, the intravenous tolbutamid-test-curves of 59 diabetics and 80 nondiabetics were evaluated by different methods. Some new parameters for the discrimination of diabetics and nondiabetics were presented. A graphical procedure was recommended for determinating a practicable discriminatory parameter. This parameter contains all original information of the tolbutamide-curve and can be obtained in 1–2 minutes. It completely differentiates the two groups. Using the parameter in a great number of cases, 0,3% false positive and 0,2% false-negative results are statistically expected. The results of theT 3-tests in a groupe of 28 „big-baby-mothers“ and 16 relatives of diabetics demonstrated the indirect validity of the test for detecting prediabetics.
    Notes: Zusammenfassung Als Beitrag zum Problem der Entdeckung latent-diabetischer Zustände wurde die Trennschärfe des Tolbutamid-Tests bei verschiedenen Auswertungsmethoden an Hand der Kurven von 59 Diabetikern und 80 Stoffwechselgesunden untersucht. Neue Kriterien zur Unterscheidung stoffwechselgesunder und diabetischer Tolbutamid-Test-Kurven wurden dargestellt, darunter ein graphisches Verfahren zur Ermittlung eines Trennmaßes (T3). Dieses Trennmaß enthält alle Meßinformationen der Kurve und kann in etwa 1–2 min pro Fall ermittelt werden. Am vorliegenden Material ermöglicht es eine vollständige Trennung beider Gruppen. Unter Zugrundelegung der beobachteten Häufigkeitsverteilung läßt sich die Zahl der möglichen Fehleinstufungen mit 0,3% falsch-positiven und 0,2% falsch-negativen Resultaten schätzen. Die Anwendung des T3-Tests bei einem auf Prädiabetes suspekten Kollektiv von 28 „Big-Babymüttern“ und 16 Familienangehörigen von Diabetikern ergab indirekt einen Validitätsnachweis des Verfahrens zur Erkennung latent diabetischer Zustände.

    Description / Table of Contents: Summary Triglyceride content of 81 human liver biopsies, obtained at laparoscopy or by percutaneous needle puncture, was quantitatively measured by chemical means and expressed as a percentage of wet weight. Depending on the degree of fat infiltration, triglyceride concentration varied from 0,5–24%. The degree of fat infiltration was histologically estimated by the convential method of grading and by a morphometric point system basing on the Delesse principle, which states that the volume fractionV i of a componenti in the tissue can be estimated by measuring the area fractionA i of a random section occupied by transsections ofi; the results obtained by this method are expressed as percent. The chemically measured triglyceride content and histologically resp. stereologically estimated degree of fat infiltration were compared and correlated statistically. There was a significant correlation between triglyceride content and various histological and stereological estimates of fat infiltration; paraffin sections proved more reliable than frozen sections for estimating the degree of fat infiltration. The correlation between the chemical triglyceride content and the results of estimating by the point system is a hyperbolic one, when expressed in mathematical terms. This hyperbolic function becomes quite clear considering the fact that weight percent was compared with volume percent. By means of mathematical transformations the correlation between the results of the chemical and the stereological method could be brought into a linear system; so it was possible to calculate single, partial and multiple correlation coefficients. Furthermore the relation between the lipid fractions of blood plasma, body weight, some laboratory data and the triglyceride content of the biopsies were examined. The only significant correlation was found between body weight and fat infiltration of the liver. On the basis of the results obtained a formula was developed for calculating the chemical triglyceride content from the stereologically estimated values.
    Notes: Zusammenfassung Die zwischen dem chemisch bestimmten Triglyceridgehalt und dem stereologisch mit Hilfe des Punkt-Treff-Verfahrens geschätzten Verfettungsgrad in Leberpunktionszylindern bestehenden signifikanten Korrelationen werden eingehend geprüft, theoretisch begründet und in einer Formel mathematisch zusammengefaßt. Entwicklung einer Schätzformel, die es gestattet, aus den Punkt-Treff-Werten auf den tatsächlichen Triglyceridgehalt zu schließen.

    Notes: Zusammenfassung Acht verschiedene Proben, wie sie als Ausgangsmaterial für Fertig-Trockensuppen Verwendung finden, wurden über 1 Jahr bei + 20°C und + 30°C gelagert. Die Proben waren Tomatenmehl, Karottenspäne, Zwiebelmehl, Fleischextrakt und Mono-Natrium-Glutaminat. Sie wurden z. T. als solche, in der Mehrzahl der Fälle jedoch als Gemische auf Veränderungen hin untersucht, wie sie mit zunehmender Lagerdauer bei folgenden Komponenten auftreten: Wasser, Glucose, Lysin, lutaminsäure, Aminostickstoff und Reduktone. Die ernährungsphysiologisch interessierende Minderung des Lysingehaltes erscheint bei den hier untersuchten Proben und Probenmischungen von geringer Bedeutung. Wesentlich sind jedoch die mit den hier beschriebenen Umsetzungen Hand in Hand gehenden, abträglichen Veränderungen hinsichtlich der Farbe des Geruches und des Geschmackes. Die organoleptische Prüfung von fertigen Tomatentrockensuppen in handelsüblich verschweißten Beuteln ergab, daß einjährige Lagerung bei + 20°C und insbesondere bei + 30°C solche Trockensuppen farblich, geruchlich und geschmacklich derart verändern kann, daß sie nicht mehr handelsfähig sind. Der sorgfältigen Verpackung zum Schutz gegen zusätzliche Wasseraufnahme bei der Lagerung ist besondere Aufmerksamkeit zu widmen.

    Description / Table of Contents: Zusammenfassung Die Diagnostikhilfe durch Computer ist von der automatischen Befundgewinnung zu unterscheiden. Die wichtigsten Verfahren einer Diagnostikhilfe durch Computer sind: a) Symptomenlistenvergleich (mittels Boolescher Algebra) b) Symptomengewichtung c) Wahrscheinlichkeitsansätze, wie z. B. das Bayessche Theorem oder die Diskriminanzanalyse. Symptomenlistenvergleiche berücksichtigen nicht die Sensibilität und Spezifität der Symptome. Objektive Gewichtungsverfahren setzen die Kenntnis dieser meist unbekannten Größen voraus. Bei Anwendung des Bayesschen Theorems ist die Schätzung der a priori-Wahrscheinlichkeit in mehrfacher Hinsicht problematisch. Vielfach wird zusätzlich Unabhängigkeit der Symptome vorausgesetzt, die oft nicht gegeben ist. Die lineare Diskriminanzanalyse ist an die Voraussetzungen normal verteilter Meßwerte und Gleichheit der Kovarianzmatrizen in den Vergleichsgruppen gebunden. Alle bisher üblichen Verfahren sind weiterhin durch folgende Fehlermöglichkeiten belastet: a) Nichterkennen von Krankheitskombinationen b) Ungenauigkeit der Befunderhebung c) Nichtberücksichtigung des zeitlichen Verlaufes, d. h. der Dynamik des Krankheitsprozesses. Es werden Möglichkeiten zur Ausschaltung dieser Fehlerquellen diskutiert. Durch die Anwendung von Computern in der Diagnostik werden Fragen herausgearbeitet, die für die Diagnostik ganz allgemein von Bedeutung sind (Entwicklung relevanter Kriterien zur Beurteilung der diagnostischen Effizienz, Frage der Definition von Krankheiten, Analyse der ärztlichen Entscheidungsstrategie etc.).
    Notes: Summary Help with diagnosis by the computer must be distinguished from the automatic acquisition of findings. The most important procedures in the help with diagnosis by the computer are: a) Comparison of lists of symptoms (by means of Boolean algebra) b) Weighting of symptoms c) Assessment of probability, e.g., by Bayes's theorem or by analysis of discriminants. Comparisons of symptom lists do not take into account the sensitivity and specificity of symptoms. Objective procedures of weighting presuppose knowledge of these values, which are usually unknown. When Bayes's theorem is used esti mation of the a priori probability is problematic on several counts. The independence of symptoms is often assumed, although this may not be the case at all. Linear analysis of discriminants assumes the normal distribution of the values and the equality of matrices of covariance in the control groups. All usual procedures are also subject to the following errors: a) Non-recognition of combinations of diseases b) Inexactitude of examination data c) Not taking into account the course of time, e.g., the dynamics of the pathological process. The prevention of these sources of errors in discussed. The use of computers for diagnostic purposes presents problems that are of general importance for diagnostic work (development of relevant criteria for the assessment of diagnostic efficiency, the question of definition of diseases, analysis of the strategy of medical decisions etc.).

    Description / Table of Contents: Zusammenfassung Chirurgische Datenbanken dienen prinzipiell in dreifacher Weise als Hilfe bei der chirurgischen Indikationsstellung: 1. Rasche Zur-Verfügung-Stellung der für eine Entscheidung relevanten auf den Einzelfall bezogenen Daten, 2. schnelle Präsentation der Erfahrungen, die in ähnlichen Entscheidungssituationen gesammelt wurden und 3. Bereitstellung der empirischen Grundlage für entscheidungstheoretische Ansätze der maschinellen Entscheidungshilfe. Das Institut für Medizinische Datenverarbeitung der GSF hat in Zusammenarbeit mit der Chir. Klinik der TUM und der Chir. Univ.-Klinik München eine Datenbank mit Informationen von über 60000 Patienten aufgebaut und die angeführten Hilfen z. T. bereits im routinemäßigen Einsatz realisiert. Zum Teil werden sie modellmäßig parallel zum konventionellen Vorgehen erprobt.
    Notes: Summary Surgical data banks are used in three main ways in the recognition of surgical indications,viz., a) patient data relevant to decision making are made available without loss of time; b) experience gained in situations involving similar decisions is promptly recalled; c) an empirical basis is provided for computerized decision making. The Institut für Medizinische Datenverarbeitung of GSF in co-operation with the Surgical clinic of TUM and of the University of Munich has built up a data bank containing information on more than 60000 patients, and some of the aids mentioned above are already in routine use. Some of the aids are being tested parallel to conventional procedures.
