    Description / Table of Contents: Summary Normal urin contains, similar to plasma, procoagulant and fibrinolytic activator material (uroplastin and urokinase). Urokinase and uroplastin as well are probably produced by the kidney. Several investigators suggest that these enzymes, potentially active in coagulation and fibrinolytic mechanisms, may contribute to regulate the hemostatic balance of blood. The cause of the hemostatic defect in chronic uremia is not known. Some of the typical features of defective coagulation are: abnormal prothrombin consumption, variable decrease of plasma factors V, VII, IX and VIII, defective platelet function due to decrease and/or reduced availability of platelet factor 3 and 4 as well as decrease in platelet adhesiveness. These changes however are usually present in variable degrees. In cases undergoing chronic hemodialysis anticoagulant therapy may be necessary in order to counteract obliteration of arteriovenous shunts or regularily is used to prevent clotting within the extracorporeal circulation. Thrombosis in arteriovenous fistulae sometimes requires local fibrinolytic therapy. Besides these possible influences of the kidney on hemostatic mechanisms, the renal alterations as consequences of defective hemostasis are of major importance, such as hematuria in hemophilia and during anticoagulant therapy, thromboembolic complications in renal arteries and venes, as well as capillary thrombosis with consecutive renal cortical necrosis during the course of consumption-coagulopathy. Further clinical and experimental investigations are necessary to clarify the relation between coagulation and acute glomerulonephritis as well as between hemostasis and tolerance towards transplants.
    Notes: Zusammenfassung Der Harn des Gesunden enthält ähnlich wie Plasma ein gerinnungsförderndes (Uroplastin) und ein Fibrinolyse-aktivierendes (Urokinase) Prinzip. Mit großer Wahrscheinlichkeit werden sowohl Urokinase als auch Uroplastin in der Niere gebildet. Von mehreren Autoren wird die Ansicht vertreten, daß diese gerinnungsaktiven Enzyme Ausdruck einer physiologischen Teilregulation der plasmatischen Hämostase sein könnten. Die Ursache der Hämostasestörung bei chronischer Urämie ist nicht bekannt. Pathologischer Prothrombinverbrauch, die wechselnd ausgeprägte Verminderung der Faktoren VIII, IX, VII, V, eine Thrombocytenfunktionsstörung durch Reduktion und/oder herabgesetzte Freisetzung der Thrombocytenfaktoren 3 und 4, sowie die verminderte Plättchenadhäsivität stellen regelmäßige Veränderungen dar, deren Ausmaß allerdings uneinheitlich ist. Im Rahmen der chronischen Hämodialysebehandlung ist die Antikoagulantientherapie zur Verhinderung von Obliterationen arterio-venöser Verbindungen gelegentlich, zur Vermeidung von Gerinnselbildung im extrakorporalen System in der Regel erforderlich. Thrombosierte arterio-venöse Fisteln können die lokale Fibrinolysebehandlung notwendig machen. Vielfältiger als diese mittelbaren oder unmittelbaren Einflüsse der Niere auf die Hämostase sind renale Veränderungen als Folge von Gerinnungsstörungen. Hämophilie, Nierenblutung unter Antikoagulantientherapie, thrombembolische Komplikationen im Bereich der Nierenarterien und -venen sind hier zu nennen. Den letztgenannten Veränderungen sind arterioläre und capilläre Thrombenbildung im Verlauf der Verbrauchskoagulopathie mit konsckutiver Nierenrindennekrose gegenüber zu stellen.

    Description / Table of Contents: Zusammenfassung Ein fibrinoplastischer Effekt durch Dextran wurde von Laurell beschreiben. Ricketts wies die Präcipitation von Fibrinogen durch Dextran nach. Abilgaard konnte durch hochmolekulares Dextran sowohl Fibrinogen als auch Fibrin präcipitieren. Hier konnte gezeigt werden, daß bereits unter Einwirkung von niedermolekularem Dextran eine, verglichen mit dem Leerwert, deutliche Zunahme der optischen Dichte auftrat, die als Maß für die Bildung von polymeren Fibrinmonomer-Dextranaggregaten angesehen wird. Eine wesentliche Zunahme der Extinktion und somit der polymeren Aggregate konnte durch hochmolekulare Dextrane nicht erreicht werden. Hieraus ergeben sich Fragestellungen hinsichtlich der Verwendung von Dextranlösungen z. B. bei der Verbrauchskoagulopathie (Lasch), die mit dem Auftrcten von Fibrinmonomer verläuft.
    Notes: Summary Laurell observed that dextran has a fibrinoplastic effect. Ricketts showed that mixing of fibrinogenin vitro resulted in the precipitation of fibrinogen. High molecular dextran had a precipitating effect on fibrinogen and fibrin (Abilgaard). It could be demonstrated that also low molecular dextran-, compared with 0.9% NaCl solution-, caused an important increase in optical density. This is an indicator of the fibrin-dextran aggregate generation. The importance of these facts may offer new aspects in the administration of dextran in cases of consumption coagulopathy (Lasch).

    Description / Table of Contents: Zusammenfassung Bei 12 Patienten mit unterschiedlichen Hyperlipoproteinämien wurden der Thrombocytenumsatz und verschiedene Parameter der Blutgerinnung untersucht und mit den entsprechenden Werten bei 5 klinisch gesunden Männern verglichen. Bei den Hyperlipoproteinämiepatienten lag die Fibrinogenkonzentration signifikant höher als bei den gesunden Vergleichspersonen. Die maximale Breite im TEG war ebenfalls signifikant höher, die Recovery der radioaktiv markierten Thrombocyten war signifikant geringer. Alle anderen untersuchten Gerinnungsgrößen zeigten beim Vergleich der Gesamtkollektive keine eindeutigen Unterschiede. 2 Patienten mit Typ-II+IV-Hyperlipoproteinämie+ wiesen einen gesteigerten Thrombocytenumsatz auf. 4 von 7 Patienten mit Typ-IV-Hyperlipoproteinämie und 2 von 2 Patienten mit Typ-V-Hyperlipoproteinämie zeigten ebenfalls einen erhöhten Thrombocytenumsatz. Während die Patienten der Hyperlipoproteinämien Typ II+IV, III oder IV nur dann einen erhöhten Thrombocytenumsatz hatten, wenn klinische Hinweise auf eine Arteriosklerose bestanden, war der Thrombocytenumsatz bei 2 Patienten mit Typ-V-Hyperlipoproteinämie erhöht, obgleich keine Hinweise auf eine Arteriosklerose bestanden. Einer dieser Patienten mit Typ-V-Hyperlipoproteinämie wurde während einer abdominellen Krise untersucht. Dabei fanden sich nicht nur die Zeichen eines gesteigerten Thrombocytenumsatzes, sondern auch Hinweise auf einen gesteigerten Umsatz der plasmatischen Gerinnungsfaktoren im Sinne einer beginnenden Verbrauchscoagulopathie.
    Notes: Summary Platelet turnover and various parameters of blood coagulation were studied in 12 patients with different types of hyperlipoproteinemia and compared with the corresponding values of 5 healthy male volunteers. Patients with hyperlipoproteinemia exhibited higher blood concentrations of fibrinogen than the healthy subjects. The maximal amplitude in the thrombelastogramm was found to be significantly higher in patients with hyperlipoproteinemia, while the recovery of radioactively labelled platelets was significantly lower. All other parameters of blood coagulation studied showed no difference in both groups. An increased turnover of platelets could be found in 2 patients with type II+IV hyperlipoproteinemia. 4 out of 7 patients with type IV hyperlipoproteinemia and 2 out of 2 patients with type V hyperlipoproteinemia showed increased platelet turnovers, too. In patients with type II+IV, III or IV hyperlipoproteinemia an increased turnover of platelets could only be demonstrated in the presence of some clinical indications of atherosclerosis. On the other hand, platelet turnover was increased in 2 patients with type V hyperlipoproteinemia, although no indication of atherosclerosis could be found. One of these patients with type V hyperlipoproteinemia was examined during an abdominal crisis. During this period, we not only found increased platelet turnover but also indications of an increased turnover of plasma coagulation factors which can be seen in the early phases of consumption coagulopathy.

Der genetische Moleküldefekt hat keine manifeste hämorrhagische Diathese zur Folge, sondern zeigt sich lediglich in diskreten Veränderungen der gerinnungsanalytischen Parameter (verlängerte Thrombin-, Reptilase- und Thrombinkoagulasezeit; erniedrigter Wert des thrombingerinnbaren Proteins im Vergleich zum Ergebnis der immunologischen Fibrinogenbestimmung; verzögerter Verlauf der Fibrinpolymerisation). Darüberhinaus werden Klinik und Diagnostik der Dysfibrinogenämie zusammenfassend dargestellt.
    Notes: Summary A new case of dysfibrinogenemia is reported which shows no signs of a haemorrhagic diathesis (dysfibrinogenemia Gießen III). The abnormal fibrinogen was detected by only slight but characteristic alterations of some parameter of the coagulation analysis (prolonged clotting times after addition of thrombin, Reptilase and thrombin coagulase; low fibrinogen concentration determined by methods based on clot formation in comparison to the immunological fibrinogen determination; delayed fibrin polymerization). In addition, clinical features and diagnosis of dysfibrinogenemia are described in general.

    Notes: Summary Hemodialysis is associated with alterations in myocardial contractility, but duration and precise determinants responsible for these changes are unknown. We investigated the effect of several variables, established to influence left ventricular (LV) contractility, which normally changed during dialysis: the plasma concentrations of ionizied calcium, potassium, bicarbonate, and magnesium and the removal of uremic toxins. The influence of three different isovolemic bicarbonatedialysis procedures in 16 patients with normal (group 1) and hypertrophied myocardium (group 2) was assessed by echocardiography prior to and up to 44 h following each dialysis. During the first procedure, ionized calcium and potassium concentration decreased, but LV performance remained unchanged in both groups. The second procedure with increased ionized calcium and decreased potassium concentration resulted in an improvement of mean circumferential fiber shortening (VCF from 1.15 to 1.56 circ/s (P〈0.001) in group 1 and from 1.05 to 1.16 circ/s (P〈0.05) in group 2. The positive inotropic effect declined gradually up to 12 h (group 1) and 2.5 h (group 2) respectively. In the third procedure when ionized calcium was increased and potassium concentration remained unchanged contractility did not improve. Removal of uremic toxins, decrease in magnesium, and increase in bicarbonate concentrations were comparable during each procedure. These results suggest that the ionized calcium to potassium ratio is the important determinant of dialysis-related augmentation in LV contractility. In LV hypertrophy the expected contractile response is diminished indicating a depressed inotropic state.

    Description / Table of Contents: Summary A report is given about the clotting analysis of 53 dogs, in all of whom an experimental pancreatitis was produced. A part of them was treated by intravenous or intraarterial application of Trasylol, another part received low molecular weight dextran intraarterially. During the first hours after operation a definite hypercoagulability of the blood was found. The clotting analysis reminded at the early phase of a consumptive clotting defect (Verbrauchskoagulopathie), which in some animals developed with the full picture. Those dogs given Trasylol or Rheomacrodex did not show this hypercoagulability. A vicious circle of the following criteria is discussed in the pathogenesis of acute pancreatitis: Trypsin-and Elastase-effect, Thromboplastin and intravascular clotting because of hypocirculation and shock. They may participate with different accentuation in the individual case and determine its outcome. The therapeutical effect of Trasylol is thought to be due to its anticoagulative effect on the one side, to its beneficial influence on the general circulatory situation on the other side. This is evidenced by the good effect low molecular weight dextran exerts on the coagulation by means of improving the microcirculation.
    Notes: Zusammenfassung Es wird über die Gerinnungsanalysen von 53 Hunden mit experimentell erzeugter Pankreatitis berichtet, die zum Teil einer Behandlung mit Trasylol (intravenös und intraarteriell) und niedermolekularem Dextran (intraarteriell) unterworfen wurden. Es zeigte sich in den ersten Stunden nach erfolgter Operation eine deutliche Hypercoagulabilität des Blutes. Das gerinnungsanalytische Bild erinnerte an die Frühphase einer Verbrauchscoagulopathie, die bei einigen Tieren zur vollständigen Entwicklung gelangte. Unter Trasylol- und Rheomacrodexbehandlung wurde die Hypercoagulabilität nicht beobachtet. Zur Pathogenese der Gerinnungsstörung bei der akuten Pankreatitis wird ein Circulus vitiosus unterschiedlicher Akzentuierung von Trypsin- und Elastaseeffekt, Thromboplastin und vasculärer Gerinnung infolge Hypozirkulation und Schock diskutiert. Die therapeutische Wirkung des Trasylols wird einerseits auf seinen anticoagulativen Effekt, andererseits auf die Beeinflussung der allgemeinen Kreislaufsituation zurückgeführt. Zur Bestätigung der letzten Annahme wird der ebenfalls positive Effekt des mikrozikulationsfördernden Rheomacrodex auf die Gerinnungsverhältnisse angeführt.

    Description / Table of Contents: Zusammenfassung Das primäre Hypoventilationssyndrom ist eine bei Normalgewichtigen sehr selten beobachteteAtemregulationsstörung. Man kann es unter der Bezeichnung„Ondine's Curse“-Syndrome dem funktionell verwandten, aber ausschließlich bei extrem Adipösen vorkommendenPickwick-Syndrom zur Seite stellen. Anhand eines eigenen Falles mit primärem Hypoventilationssyndrom werden Pathogenese und klinische Symptomatik des bis heute ätiologisch unklaren Krankheitsbildes dargestellt. Die an diesem Patienten durchgeführten Untersuchungen stellen das primäre Hypoventilationssyndrom als einsellenes Naturmodell vor, an dem sich die Funktionsweise der peripheren Chemoreceptoren, die Pathogenese des pulmonalen Hochdrucks und der Wirkungsmechanismus von Atemanaleptika in idealer Weise am Menschen studieren läßt.
    Notes: Summary Primary hypoventilation syndrome is an extremely rare type of impairment of respiratory control. Termed “Ondine's Curse Syndrome”, it may be opposed to the more frequent “Pickwick Syndrome” which is found only in very obese patients. Giving a case report as an example, pathogenesis and clinical symptomatology of primary hypoventilation syndrome are discussed in detail. It is demonstrated that this syndrome may serve in an ideal way as a natural model for studying the peripheral chemoreceptor function, the pathogenesis of pulmonary hypertension and the therapeutic mechanism of respiratory stimulants.

    Description / Table of Contents: Zusammenfassung Es wird über eine 19 Jahre alte Patientin mit einer fakultativen hämorrhagischen Diathese berichtet, als deren Ursache eine kongenitale Hypodysfibrinogenämie anzunehmen ist. Hinweise liefern u.a. eine deutlich verlängerte Thrombin- und Reptilasezeit sowie Thromboplastin und partielle Thromboplastinzeit. Eine gesteigerte Fibrinolyse liegt nicht vor. Die immunologisch bestimmte Fibrinogenkonzentration liegt unter dem Normalbereich, so daß neben einer Störung der Fibrinogen-Fibrinumwandlung auf der Stufe der Fibrinmonomeraggregation eine Hypofibrinogenämie vorliegt. Die Streptokinase induzierte Fibrinogenolyse von Fibrinogen Gießen II Plasma zeigt im Vergleich zu Normalplasma eine inkomplette Fibrinogenolyse. Fibrinogen Gießen II unterscheidet sich als kongenitale Hypodysfibrinogenämie mit einer verzögerten Gerinnung und einer unvollständigen Fibrinogenolyse von den bisher beschriebenen pathologischen Fibrinogenen.
    Notes: Summary A new case of facultative hemorrhagic diathesis is described in a 19 year old femal and the coagulatory anomaly examined. The experiments show that the coagulatory disturbance should be ascribed to a defective aggregation of fibrin monomers associated with hypofibrinogenemia. Some of its characteristics are the markedly prolonged thrombin-, reptilase-time as well prolonged prothrombin and partial thromboplastin time. The plasma fibrinogen concentration measured by the immunologic method is reduced. The streptokinase induced digestion of fibrinogen Gießen II plasma occurred at a slower rate than normal plasma with persistence of the large fibrinogen degradation products. Fibrinogen Gießen II appears to be a congenital hypodysfibrinogenemia with abnormality of fibrinogen resulting in delayed coagulation and a retarded fibrinogenolysis rate.

    Description / Table of Contents: Summary In 82 dogs, both male and female, acute experimental pancreatitis was produced according to the procedure published byElliott, Williams andZollinger. Along with microscopic, histochemical and electron microscopic investigations, changes of enzyme and electrolyte concentrations as well as of the activity of proteinases were studied in order to pursue the pathways of pancreatic disorders. Besides, the examinations included studies on the effects of pancreatitis on the blood clotting mechanism and on changes of plasma volumes. It appears that, after an initial secretory obstruction and enlargement of the isthmic epithelium (“Isthmus blockade”), dilatation of acini and reflux of enzymes at the site of their production can be regarded as the most important lesions. Electron microscopic studies showed an irritation of the basal endoplasmatic reticulum at the beginning of the cellular lesion. In the further course, parapedesis through the cellular membranes can be observed, a phenomenon, the sequelae of which may be paralysis of preterminal or terminal vessels. Stimulation of functioning acinar cells obviously is followed by an “energetic” insufficiency, if ductal obstruction is maintained. When isthmus blockade continues, the initially reversible cellular lesion becomes irreparable, prodominantly because of vascular paralysis and consecutive tissue hypoxia. Enzyme displacement and local hypoxia form a circulus vitiosus which results in a tryptic necrosis. Chymotrypsin, trypsinogen, and trypsin could not be proved. Proteinases which were found effective at a neutral pH, however, appear to be responsible for the enzymatic auto-digestion of the pancreas.
    Notes: Zusammenfassung Bei 82 Hunden beiderlei Geschlechts wurde eine akute Pankreatitis in der Technik nachElliott, Williams undZollinger erzeugt. Pathologisch-anatomische, histochemische und elektronenmikroskopische Untersuchungen wurden den Befunden der Fermententgleisung, der Transmineralisation und Proteinasenaktivität gegenübergestellt, um die „Zündschnur“ des pathischen Geschehens zu verfolgen. Hiernach resultiert neben der „Sekretplombe“ eine Verquellung der Isthmusepithelien — eine „Isthmusblockade“. Die Folge ist eine Acinusdilatation und Enzym- wie Sekretrückstauung im Bereich der Produktionsstätte. Submikroskopisch läßt sich zeigen, daß am Beginn der Zellschädigung eine Irritation des basalen endoplasmatischen Reticulum besteht, in deren weiterem Verlaufe es zum Phänomen der Parapedese über die Zellbasis kommt, deren Folge die Paralyse der präterminalen und terminalen Strombahn sein kann. Die unter Sekretionsreiz gegen Sekretstauung arbeitenden Zellen geraten offenbar in eine „energetische“ Insuffizienz. Der zunächst noch reversible Zellschaden wird bei anhaltender Isthmusblockade durch Paralyse der capillären Strombahn und konsekutive Hypoxie dekompensierbar. Hypoxie und Fermententgleisung potenzieren einander und bilden einen Circulus vitiosus, dessen Folge die tryptische Nekrose ist. Chymotrypsin, Trypsinogen bzw. Trypsin waren nicht bestimmbar. Dagegen wurden im neutralen Bereiche wirksame Proteinasen nachgewiesen, die für die fermentative Autodigestion verantwortlich gemacht werden können. Weiterhin wird die Wirkung der Pankreatitis auf Blutgerinnung, Serumelektrolyte, Serumfermente und Plasmavolumen diskutiert.
