    Notes: Background The standard assay for the clinical detection of contact allergy against nickel is the patch test. For research purposes, the in vitro lymphocyte transformation test (LTT) is mostly applied. However, the in vivo application of allergens includes the potential of sensitization, whereas the LTT demands 6 days of cell culture and radioactive labelling procedures.Objective The purpose of this study was to validate an enzyme-linked immunospot (ELISpot) assay to specifically detect cellular response against nickel sulphate in correlation with patch test, anamnestic data, and LTT results.Methods In probands with positive (n=60) and negative patch test results (n=19), cytokine ELISpot and LTT were performed in parallel using various concentrations of nickel sulphate, various numbers of responding peripheral blood mononuclear cells (PBMC), and various durations of cell cultures.Results Concentrations of 50 and 75 μM nickel sulphate were found to be optimal to stimulate in the ELISpot 4×105 and in the LTT 2×105 PBMC, respectively. An increase in ELISpot sensitivity was reached by pre-incubation with nickel sulphate for 24 h prior to transfer to ELISpot plates. In nickel-sensitive probands, an average precursor cell frequency of 19×105, 1.7×105, and 0.7×105 could be defined for IFN-γ, IL-2, and IL-4 producing PBMC, respectively. In non-sensitive probands IFN-γ producing cells were detectable, but with significantly lower frequency (2×105; P=0.004). The prior performance of patch tests had no significant effect on cytokine production or lymphocyte proliferation. Overall, the parameters patch test, anamnestic nickel allergy, ELISpot, and LTT results were positively correlated (P〈0.05).Conclusions Based on these experiments, the quantitative in vitro detection of cell-mediated reactions towards nickel as well as other heavy metal ions should be possible utilizing the above-described ELISpot assay.

    Notes: Summary In 13 patients with a lymphosarcoma of the stomach the diagnosis was obtained preoperatively in 7 cases by directed gastroscopic biopsy. The macroscopic claBification of Gütgemann and Schreiber is useful for prognosis of survival. Histopathologically the tumors were claBified as malignant non-Hodgkin lymphomas according to Lennert. A therapeutic schedule based on radical surgery, radiation, and combined chemotherapy is presented. Clinical staging and grade of malignancy of the lymphoma determine the therapeutic management.

    Description / Table of Contents: Zusammenfassung Die Xenotransplantation von 5 verschiedenen etablierten Tumorzellen auf die thymuslose Mäusemutante “nude” war erfolgreich. Zellen eines humanen Cervixcarcinoms (HeLa) und eines Burkitt-Lymphoms sowie zweier Polyoma-Virus induzierter Tumoren der Ratte und eines Spontantumors vom Hamster wurden untersucht. Während einer Beobachtungszeit von 21 bis 28 Tagen bildeten alle Zellen lokal progressiv wachsende Tumoren. Die kritische Zellzahl differierte je nach untersuchter Linie zwischen 104 und 106 Zellen. Das histologische Bild der Tumoren glich dem der Primärtumoren. Zwei etablierte Linien von normalen Kbroblasten (3-T-3) und Nierengewebe (CV-1) wuchsen dagegen nicht. Nach unseren Ergebnissen stellt die Xenotransplantation bei „nude“-Mäusen ein einfaches in-vivo Transplantationssystem zur Prüfung der Onkogenität von Gewebekulturzellen dar.
    Notes: Summary Successful xenotransplantation of 5 different established tumor cell-lines was performed in the congenital thymusless mouse mutant “nude”. The tumors were one human carcinoma of the cervix (HeLa), one Burkitt's lymphoma as well as two Polyoma virus-induced tumors from rats and one spontaneous tumor from hamster. During an observation period of 21–28 days all cells formed rapidly growing tumors at the injection site. The critical cell dose varied in the different lines between 104 and 106 cells. Histological appearance of the neoplasias resembled that of the primary tumors. Two established lines from normal flbroblasts (3-T-3) and kidney (CV-1) failed to grow. Our results indicate that xenotransplantation in “nude” mice may serve as a simple in-vivo assay for testing the oncogenicity of tissue culture cells.

    Notes: Summary A model of pretherapeutic classification of malignant melanoma is proposed in accordance to the TNM system. It differs principally from the melanoma classification issued by UICC in 1968 and withdrawn in 1974, in that a new category “R” is introduced for satellite and transit metastases, and that the T classification of the primary tumor contains not only the horizontal dimension, but the vertical elevation of the tumor above skin level. The new classification was tested in a retrospective study of 175 patients of the Hornheide Special Clinic. Clinical utility and relevance of the system could be documented. In the classification of primary tumor, the degree of elevation above skin level was found to correlate better with posttherapeutic prognosis of melanoma than the horizontal tumor diameter. The vertical dimension of a tumor is an indicator of its invasive potential and hence of the probability of metastatic spread. The pretherapeutic, purely clinical classification of melanoma cannot be meant to substitute histologic examination with its exact assessment of invasion etc. On the other hand, posttherapeutic definition of tumor levels by histology does not eliminate the necessity for correct pretherapeutic examination and macroscopic description of the tumor size in its three dimensions. Atraumatic assessment of tumor elevation with its implications plays an essential role in the planning of tumor therapy: In case of deep invasive melanoma, lymph node extirpation is indicated at the time of intraoperative examination of frozen sections of the primary. Experience has taught us that intraoperative examination may be hampered by considerable errors in determining the exact depth of an invasive growth, as frozen sections sometimes fail to give a clear picture of the largest tumor diameter. In such cases, clinical measuring of the tumor in its macroscopic appearance may serve as a rough guide for excision and histology.
