    Notes: The relationship between the risk of prostate cancer and dietary intake of energy, fat, vitamin A, and other nutrients was investigated in a case-control study conducted in Montreal (Quebec), Canada. French Canadians aged 35 to 84 years with a recent, histologically confirmed diagnosis of adenocarcinoma of the prostate were identified through the admission offices of five major francophone teaching-hospitals in Montreal from 1989 to 1993. Population-based controls matched for age (±five years), language, and place of residence were selected by a modified random-digit dialing method. The study included 232 cases and 231 controls. Information on dietary intake was collected by means of a quantitative dietary history. No association was evident between energy intake and the risk of prostate cancer. In contrast, there was some evidence of an inverse association with intake of total fat, animal fat, monounsaturated fat, and particularly saturated fat (odds ratio = 0.69, 95 percent confidence interval = 0.40–1.18, P=0.05), while a nonsignificant positive association was found with polyunsaturated fat. In addition, high intake of retinol and vegetable protein (highest cf lowest quartile) was associated with reduced risk, but was not statistically significant. No associations were established between intake of other nutrients and risk. These patterns persisted after adjustment for a number of potential confounding factors.

    Notes: Abstract The current approach to screening for hepatitis C and non-A, non-B, non-C hepatitis in French blood transfusion centers involves a combination of a transaminase assay and tests for antibodies to hepatitis B core antigen (anti-HBc) and antibodies to hepatitis C virus (anti-HCV). A decision-analysis model was used to assess the cost-effectiveness ratio of this approach compared to the former approach, which included only transaminase and anti-HBc screening. Cost data were collected by a questionnaire sent to 26 centers throughout France. The average costs of diagnostic kits, equipment, staff, and administration were calculated. Estimates of prevalence and sensitivity values came from the medical literature. The cost-effectiveness ratio was expressed in French francs per infected donor detected. A sensitivity analysis of the variables in the model was performed to estimate the validity of the cost-effectiveness ratio. For 100,000 donations the incremental cost of the current approach reached FrF 2,566,111 (about US $ 500,000), with a marginal effectiveness of 180 donations detected. The sensitivity analysis showed the effect of prevalence on the incremental cost-effectiveness ratio. Transfusion centers may change their screening approach in areas of high or low prevalence of hepatitis C in France.

    Notes: Abstract Background: The prognostic implications of p53 accumulation, bcl-2immunoreactivity and tumour proliferative fraction in ovarian carcinomas arestill debated. Patients and methods: One hundred twelve ovarian carcinomas wereimmunostained for p53 protein, for bcl-2 and for the cell cycle-associatedKi-67 antigen. The immunostaining results were correlated with conventionalclinico-pathological variables, response to induction chemotherapy, andpatient survival. Results: p53 accumulation and bcl-2 immunoreactivity in more than10% of neoplastic cells were detected in 61 (54.5%) and 42(37.5%) cases, respectively. A positive correlation between p53accumulation and high (more than 30% neoplastic cells) MIB1 labellingindex (r = 0.235; P = 0.015) was ascertained, whereas no significantassociation was found between bcl-2 immunoreactivity and p53 accumulation orMIB1 labelling index. Both p53 accumulation and MIB1 immunoreactivitycorrelated significantly with a reduced overall survival, but the associationwas lost in multivariate analysis. However, patients with tumourssimultaneously showing p53 accumulation and MIB1 labelling index higher than30% had significantly reduced overall survivals, in both univariate andmultivariate analyses. Conclusion: The simultaneous evaluation of p53 accumulation and MIB1labelling index has independent prognostic implications in common epithelialmalignancies of the ovary, irrespective of the disease stage.
