    Notes: Abstract pCMB-Dextran T 10, a large, non-penetrating, SH-blocking reagent caused a reversible amplification of bradykinin sensitivity in the isolated rat uterus. High concentrations of this compound (1×10−3 M) can evoke spontaneous uterus contraction. pCMB-Dextran T 10 did not alter the biological activity of bradykinin. It is assumed that superficial membrane SH-groups are involved in the uterus contraction mechanism.

    Notes: Abstract. The calcium-binding extracellular matrix protein BM-40 was obtained as a mouse cDNA product from a stably transfected kidney cell clone. Electrophoresis and N-terminal sequence analysis demonstrated absence of the proteolytic processing previously observed for a mouse tumour-derived BM-40. Yet the two forms of BM-40 were very similar in their CD spectra, their calcium-dependent change in α helix content and their immunological epitopes. In surface plasmon resonance assays, recombinant mouse BM-40 showed distinct binding to the triple-helical domains of collagens I, II, III, IV and V with Kd=1–4 μM but no binding to collagen VI. These interactions were abolished in the presence of EDTA. Tissue-derived mouse BM-40, however, bound collagens I and IV with Kd=0.1–0.2 μM. Activation of collagen binding to give a similar Kd could be achieved for recombinant mouse BM-40 by treatment with the matrix metalloproteinase collagenase-3. The major cleavage site was located in helix C of the extracellular calcium-binding module of BM-40 and other less prominent cleavages occurred close to the N-terminus. The sensitive helix C site was just one residue away from that sensitive to endogenous tissue proteolysis, suggesting that cleavage could be a physiological mechanism to modulate collagen binding.

    Description / Table of Contents: Summary In 35 patients with Graves' ophthalmopathy (GO) thyroid function was tested by T3-RIA, T4-RIA, TBI, TRH-test, thyroid scanning, and determination of thyroid autoantibodies. Additional ultrasonography (A-scan), computed tomography (CT) of the orbit, and the determination of an exophthalmogenic serum activity in fish bioassay was performed. Typical alterations for GO were observed in 26 cases with ultrasonography. CT showed an enlargement of medial and/or lateral rectus muscles in 24 of 33 patients, and in 17 cases a region of high density in the apex of the muscle cone. The density of retrobulbar fat after i.v. injection of contrast medium did not differ significantly from that observed in normal men. Characteristic signs of GO were not detected in only 2 cases using both methods together. Exophthalmogenic serum activity was found in the IgG fraction of serum protein. The incidence rate was high (69%), but for diagnostic purpose the fish bioassay cannot be recommended.
    Notes: Zusammenfassung Bei 35 Patienten mit endokriner Ophthalmopathie (eO) wurde zu den Parametern der Schilddrüsenfunktion (T3, T4, TBI, TRH-Test), dem Szintigramm und der Bestimmung der Schilddrüsenantikörper ergänzend die Ultrasonographie (A-scan) und Computertomographie (CT) der Orbita, sowie der Nachweis exophthalmogener Serumaktivität im Fischbioassay durchgeführt. Charakteristische Sonogramme für eine eO fanden sich in 26 Fällen. Die CT ergab bei 24 von 33 Patienten die Verdickung der musculi recti mediales und/oder der musculi recti laterales, sowie bei 17 Patienten eine Verdichtung im Bereich der Orbitaspitze. Im retrobulbären Bindegewebe zeigte sich nach Kontrastmittelgabe keine signifikante Dichtezunahme. Mit beiden Verfahren zusammen waren nur bei 2 Patienten die Kriterien einer eO nicht erfüllt. Die exophthalmogene Serumaktivität wurde in der IgG-Fraktion im Fischbioassay nachgewiesen; die Trefferquote war mit 69% relativ hoch. Zur Diagnostik der eO kann jedoch der Fischbioassay nicht empfohlen werden.

    Notes: Summary Ketoconazole (Nizoral), an oral broad spectrum antifungal agent, inhibits ergosterol synthesis in fungi and cholesterol synthesis in mammalian cells by inhibition of the 14-demethylation of lanosterol. After a blunted cortisol response to ACTH in normal men after ketoconazole has been shown by others we studied the influence of the antifungal agent on the cortisol secretion in a patient with a cortisol producing adrenal adenoma in vivo and in vitro. Repeated oral doses of ketoconazole (200 mg every 5 h over a period of 48 h) induced a reproducible clear cutt fall of serum cortisol levels under 2.5 µg/dl. The inhibitory effect on the cortisol secretion could be detected first 5 h after the first dose, 9 h after the last dose cortisol levels recovered. In addition the inhibitory effect of ketoconazole on cortisol secretion could be reproduced in vitro by incubating tissue slices of the excised adrenal tumor together with the antifungal agent in concentrations equivalent to therapeutic serum levels. These findings emphasize that patients with an autonomous cortisol production caused by an adrenal tumor are proned to dangerous hypoadrenalism if treated with ketoconazole.

    Notes: Summary The prognostic value of the determinations of autoantibodies in Graves' disease is still questionable. So far, the role of different assay procedures used has not been intensively investigated. We simultaneously applied two different techniques, a radioreceptor assay and a T3 releasing in vitro assay, in the follow-up of patients with Graves' disease to directly compare the course of the antibody activities determined by these assays and to find out a prognostic significance of the composition of the antibody spectrum present. The initial activities of thyroid stimulating antibodies (TSAb) and TSH-binding inhibiting immunoglobulins (TBII) were not significantly correlated in patients before treatment. During a 12-month antithyroid medication antibody titres showed a concordant course in the majority of patients. In 6 of 25 patients, however, a discordant behaviour was clearly documented including dose-response curves. At the end of treatment, the patients could be divided into three groups: group I included 5 patients positive for both TSAb and TBII, group II 6 patients positive for TBII and negative for TSAb and group III 14 patients negative for both of them. During the following survey of 18 months all patients of group I, 2 patients of group II and 6 patients of group III experienced a relapse of hyperthyroidism. In conclusion, TSAb and TBII activities dissociate in some patients during antithyroid drug therapy. For the individual patient, the disappearance of both TSAb and TBII was no certain indicator for a longstanding remission of Graves' hyperthyroidism. The persistence of TSAb seems to be more reliably associated with persisting or rapidly relapsing disease than the persistence of TBII.

    Description / Table of Contents: Zusammenfassung Intravenös in Dosen von 1,0–1,5 mg/kg Körpergewicht und min zugeführtes Adenosin-3′,5′-cyclisches Monophosphat (cAMP) ist in der Lage, die basale Magensäuresekretion von kardiovasculär gesunden Personen über die Infusionsdauer leicht, aber signifikant anzuregen. Die unter der Wirkung von cAMP erzielte Säuresekretion erweist sich gegenüber dem durch Pentagastrin stimulierten maximalen Säureausstoß als hochsignifikant geringer. Eine Steigerung der infundierten Dosis von cAMP zur möglichen Verstärkung des säurestimulierenden Effektes verbot sich wegen der damit verbundenen drastischen Nebenwirkungen. Während intravenöser Infusion von 0,5 mg/kg und min cAMP lassen sich keine wesentlichen Änderungen der Volumenrate und der Sekretionsraten von Wasserstoffionen und Chlorid im Vergleich zu den basalen Vergleichswerten nachweisen. Die Befunde stützen das Postulat, daß cAMP für die Säurebildung im Magen von großer Bedeutung ist.
    Notes: Summary The response of human gastric mucosa to intravenous infusion of adenosine-3′,5′-cyclic monophosphate (cAMP) was studied in 12 subjects with normal cardiovascular system. Administration in large doses of 1.0 to 1.5 mg/kg body weight and per minute of exogenous cAMP slightly but significantly stimulated hydrochloric acid secretion during the infusion period. The effect of cAMP however, was much smaller than that of pentagastrin in doses used to eliciting maximal acid secretion. The appearance of serious side effects of cAMP rules out an elevation of the quantity infused of the substance to produce a possible enhancement of the acid-stimulating action. Administered at a dose of 0.5 mg/kg per minute, cAMP does not significantly alter volume rate nor secretory rates of hydrogen ions and chloride compared with basal acid output. These findings support the theory that cAMP plays an important role in the production and secretion of gastric acid.
