    Notes: Summary Two cystadenomas occurring near the frenulum of the penis revealed a predominantly secretory pseudostratified columnar epithelium with PAS-reactive dome-shaped cytoplasmic protrusions at the luminal cellular parts. The content of secretory vacuoles was discharged into the cyst lumen by an exocytotic (eccrine) and ballooning type of extrusion. There were no indications of a real apocrine extrusion mechanism. No histogenetic derivation of these penile cystadenomas from apocrine sweat glands could be proved. An antennalike filamentous fuzzy coat on the luminal cytoplasmic membrane was most remarkable. Regionally, an epidermoid differentiation had developed. Morphologically, this process had begun just above the basal cell layer which had remained unchanged and led to the establishment of a stratified epithelium.Hyalin lamellarlike flattened cells at the luminal part displayed necrobiotic features and resembled periderm cells. There was no keratin pattern. Luminal cells of epidermoid differentiation still revealed a filamentous fuzzy coat which indicated that the undifferentiated cuboidal basal cells basically had a prospective secretory meaning and probably represented the target cells in the process of metaplasia.

    Notes: We describe two cases of livedo racemosa generalisata with cerebrovascular defects (Sneddon syndrome). The histology is characterized by a proliferation and migration of medial smooth muscle cells in ascending arterioles of the upper subcutis and deep dermis. Migrating smooth muscle cells with a high content of intermediate filaments colonize the sub-endothelial intimal space, with subsequent narrowing of the vessel lumen. Since the discoloration of the skin is provoked by a reactive dilatation of venules, the biopsy should be performed in the adjacent normal-looking skin, taking in the upper subcutis.

    Notes: Three cases of chronic intection with Borrelia burgdorferi are descrihed. The patients presented with nodular or discoid fibrosis, partly in conjunction with acrodermatitis chronica atrophicans (ACA). Juxta-articular fihrotic nodules may develop within a few months of the onset of ACA. Nodular, discoid morphoea-like, and widespread cutaneous fibroses in chronic Borrelia infection may be provoked by trauma, surgery or electromagnetic radiation. They respond well to antibiotic therapy. These lesions offer an in vivo model for studying the evolution of immunologically induced fibrosis.

    Notes: Juxta-articular fibrotic nodules in chronic Borrelia burgdorferi infection commonly regress rapidly under antibiotic therapy. They may therefore serve as a good invivo model for studying the development and regression of cutaneous fibrotic processes. As shown in a typical case of acrodermatitis chronica atrophicans, this spirochete-induced fibrosis in the upper subcutis of the elbow region is histologically characterized by broad hyalinized collagen tracts interspersed with prominent perivascular lymphocytes and plasma cells. These immune cells vanish completely after 5 days of antibiotic treatment, while fibroblasts discharge matrix vesicles and form elastic fibres.

    Description / Table of Contents: Zusammenfassung Bei 9 hautgesunden südafrikanischen schwarzen Freiwilligen im Alter von 1–72 Jahren wurde sonnenlichtexponierte und nichtexponierte Haut licht- und elektronenmikroskopisch untersucht unter besonderer Berücksichtigung altersabhängiger und aktinisch bedingter Veränderungen dermaler Strukturen. Während bei einer 72jährigen Frau eine ausgeprägte aktinische Elastose im oberen Corium der sonnenexponierten Haut licht- und elektronenoptisch nachweisbar war, fand sich bei einem 72jährigen Mann eine nur elektronenoptisch darstellbare geringe Elastose. Im Gegensatz dazu waren bei diesen beiden Fällen im unteren Corium der sonnenexponierten und in allen Schichten der nichtexponierten Haut nur altersentsprechende elastische Fasern vorhanden. Diese Beobachtungen stehen in Gegensatz zu der noch gängigen Auffassung, daß eine aktinische Elastose bei der kongoiden Rasse nicht auftritt. Das elastotische Material ist offensichtlich ein de-novo-synthetisiertes und sezerniertes pathologisches Produkt von (durch chronische UV-Bestrahlung) alterierten Fibroblasten.
    Notes: Summary In 9 otherwise dermatologically normal South African Black volunteers (1–72 years old), sunlight-exposed and non-exposed skin has been examined by light- and electronmicroscopy with special references to age-dependent and actinic alteration of dermal structures. Two 72 year old Blacks exhibited typical dermal elastosis: in 1 case to a marked extent and already detectable by lightmicroscopy, in the other case only to a mild degree. In contrast, only age-related elastic fibers were revealed in the lower dermis of sunlight-exposed skin and in all dermal layers of unexposed skin. These observations are contradictory to the general view that actinic elastosis does not occur in Blacks. The elastotic material is obviously a de-novo synthesized and secreted pathological product of chronically UV-altered fibroblasts.

    Description / Table of Contents: Zusammenfassung. Die Acne inversa – Hidradenitis suppurativa – ist gekennzeichnet durch rezidivierende abscedierende und vernarbende Entzündungen der intertriginösen Haut. Zu den kausalen Therapieansätzen zählen die systemische Retinoidtherapie, die an der Follikelhyperkeratose angreift und die radikale Excision der betroffenen Hautareale. Methode: Die Langzeitergebnisse der radikalen Operation (Excision der betroffenen Haut mit 1-cm-Sicherheitsabstand zur klinisch gesunden Haut unter Mitnahme der Subcutis) bewerteten wir anhand von 12 Patienten (8 Männer, 4 Frauen), die von 1991–1999 in unserer Klinik operiert wurden (20 Operationen axillär und 5 genitofemoral). Die Erkrankungsdauer bis zur ersten radikalen Operation betrug durchschnittlich 11,75 (3–20) Jahre. Sieben Patienten erhielten präoperativ Isotretinoin per os über mindestens 2 Monate. Die Nachbeobachtungszeit betrug durchschnittlich 4,2 Jahre (5 Monate bis 9 Jahre). Ergebnisse: Neun Patienten blieben rezidivfrei, 3 Patienten entwickelten neue Abscesse in operierten und/oder zuvor nicht betroffenen Regionen. Mit dem kosmetischen Ergebnis waren 9 der 12 Patienten zufrieden. Bewegungseinschränkungen traten nicht auf. Zusammenfassung: Durch konservative Therapiemaßnahmen allein ist die Acne inversa meist nicht heilbar. Die systemische Therapie mit Isotretinoin führt zu einer Verminderung der Krankheitsaktivität und ist damit zur Vorbereitung der Operation geeignet. Nur die radikale Excision der betroffenen Hautareale im Gesunden unter Mitnahme der gesamten Fistelgänge und Abscesse, gegebenenfalls mit nachfolgender Spalthauttransplantation, führt zur Rezidivfreiheit. Für 11 der 12 Patienten stellte die mehrzeitige chirurgische Therapie das Ende eines langen Leidenswegs dar.
    Notes: Abstract. Acne inversa (synonyms are hidradenitis suppurativa, pyodermia fistulans sinifica) is a chronic inflammatory disorder of the terminal hair follicles of the intertrigines. Abscesses and fistules develop mostly in both axillae, in the anogenital region, and under the breasts. The disease is common, but the right diagnosis is often missed. Local incision gives only a short relief of pain. The therapy of choice is wide local excision. We report on 12 patients with acne inversa who were surgically treated from 1991–1999 in our department. Surgical results and patient satisfaction were assessed on an average of 4.2 years. In all, 20 excisions in the axillae and 5 excisions anogenital were done. Seven patients were treated preoperatively with isotretinoin over at least 2 months. All patients could move their limbs without trouble. The cosmetic result was good in nine patients and three expressed dissatisfaction. Nine patients did not develop new lesions in the treated area. Three patients had new inflammations in both treated and previously uninvolved and therefore not resected regions. Wide local excision gives very good functional and cosmetic results. It usually heate long-lasting disease for nearly disabled patients. Isotretinoin reduces the disease activity and allows radical surgical treatment.

    Notes: Summary The capillaries in cherry haemangiomas show perivascular hyalinized sheaths. In order to clarify the nature of this sheath material, the extracellular matrix of cherry haemangiomas from 20 normal volunteers (age range 30–64 years) was investigated using immunohistochemical and electronmicroscopical methods. Antibodies against collagen types III, IV and VI and laminin were used. Hyaluronic acid was visualized using the hyaluronic acid binding region of the cartilage proteoglycan as ligand. Electronmicroscopically, the sheaths contained multilaminated basement membrane-like material, collagen fibres 20–25 nm thick with a periodicity of 67 nm and broad-banded aggregates with a periodicity of 100 nm (zebra bodies or fibrous long-spacing fibres). Immunohistochemically, type IV collagen was stained throughout the whole sheath material. Staining for laminin was more confined to the endothelial side of the sheath. Intense staining for type III collagen and hyaluronic acid was found in the connective tissue of the subpapillary layer and between the cherry haemangioma capillaries. Much weaker staining for type III collagen and no staining for hyaluronic acid were found invariably in an area 4–10 Μm thick directly around the capillaries. Both sheath material and intercapillary connective tissue of the haemangiomas showed pronounced staining for collagen type VI. Immunogold staining revealed that type VI collagen was localized to microfibrils 5–6 nm thick and to the broad-banded aggregates with 100 nm periodicity. These findings further underline the assumption that the broad-banded aggregates consist of type VI collagen.

    Notes: Summary A strong correlation between capillary fenestrations and the demonstration of carbonic anhydrase (CA) has previously been shown. In the present histochemical study we sought to determine whether CA could serve as a marker for fenestrated capillaries in psoriasis. In normal human skin capillary staining for CA was found only in the fenestrated capillaries of the perifollicular and periglandular plexus. In psoriatic skin lesions, however, the intrapapillary capillaries also reacted for CA. From ultrastructural investigations it is known that these capillaries are fenestrated. Our findings have shown that there is a strong correlation between fenestrated and CA-positive capillaries in normal human skin as well as in the lesional skin of psoriatics. Therefore, the demonstration of CA activity may serve as a specific and sensitive marker for fenestrated capillaries in psoriasis using an uncomplicated method, which makes it possible to detect numerous fenestrated capillaries in a single histological section.
