    Notes: Summary Ultrastructural concentric laminations have previously been thought to be specific to oligodendroglioma. However, these structures were also recognized in fibrillary astrocytomas, a mixed glioma and a glioblastoma. These laminations continued or closely related to attenuated processes or cytoplasm of astrocytic tumor cells. In addition, some lamellae contained glial filaments. It is considered that the concentric laminations are derived from attenuated astrocytic processes and have no relationship with myelin.

    Notes: Summary The histogenesis of stromal cells in capillary hemangioblastoma has been the subject of debate. The light and electron microscopic studies of hemangioblastomas presented here showed pericytic and leiomyoblastic features in stromal cells. Cells cultured by the monolayer method showed similar features to those of the original tumors. Immunohistochemical studies for glial fibrillary acidic protein and factor VIII/von Willebrand factor indicated that stromal cells were antigenically distinct from astrocytes and endothelial cells. These findings suggest that stromal cells are closely related to pericytes and smooth muscle cells, and support Rhodin's speculation that pericytes serve as a precursor to smooth muscle cells.

    Notes: Summary  Whether and when unruptured aneurysms or aneurysmal rests following incomplete surgery result in subsequent bleeding are major concerns for neurosurgeons. By calculating the annual growth rate of aneurysms in long-term follow-up angiography (partly supplemented with MR-imaging and/or MR-angiography), we attempted to determine the surgical indications for these aneurysms. Long-term follow-up angiography ranging from one to 20 years was carried out on five patients whose ruptured aneurysms had been incompletely occluded, six patients with multiple aneurysms, of which the ruptured ones had been completely oblitered at operation and the small unruptured aneurysms, missed or misdiagnosed, and eight patients with unruptured aneurysms which were asymptomatic or symptomatic. The correlation of annual aneurysm growth rate to subsequent bleeding was investigated. Four out of six fast-growing aneurysms with high annual growth rates (more than 8% increase per year) resulted in subsequent bleeding whereas none of the fourteen slow-growing aneurysms with low annual growth rates (less than 8% increase per year) led to bleeding (Fisher's exact test; p〈0.01). Hypertension, patient age and aneurysmal location showed no significant correlation to the annual growth rate (Fisher's exact test; p〉0.05). This study suggests that aneurysmal rests after incomplete surgery and missed or misdiagnosed multiple or incidental aneurysms which are fast growing have a high possibility of subsequent bleeding and should be operated on as soon as possible.

    Notes: Summary Immunohistochemical studies for erythropoietin were carried out in six capillary haemangioblastomas, three of which were also studied by electron microscopy. The immunohistochemical studies showed that positively stained cells were scattered in the vicinity of capillaries, and that neither endothelial cells nor stromal cells were stained. In their morphology and distribution, the positively stained cells were identical to mast cells as observed by electron microscopy. In one case, erythropoietin was demonstrated in the cyst fluid of the tumour. These findings suggest that mast cells with abundant secreting granules in haemangioblastomas are capable of producing erythropoietin.

    Notes: Summary An approach which improves on the conventional suboccipital craniectomy was used to explore a jugular foramen, a hypoglossal neurinoma, and a tentorial meningioma. A postero-medial mastoidectomy supplemented by the conventional suboccipital craniectomy made it possible to expose the entire sigmoid sinus. Subsequent medial retraction of the exposed sigmoid sinus and continuous dura mater and the extradural removal of petrous bone around the jugular foramen exposed the whole extracranial portion of the tumours. The tumours were totally removed by this approach in which the extradural route corresponded to the extracranial portion of the tumours and the suboccipital route to the intracranial portion. Facial nerve and hearing disturbances, which are frequent complications of the previous approaches, did not occur in our cases. Providing adequate exposure with simple surgical procedures and showing no adverse postoperative sequelae, this approach is most suitable for surgery upon jugular foramen tumours with extracranial extension.

    Notes: Summary A long-term follow-up study covering between one and twenty-two years after surgery was carried out on 177 patients with direct operations for cerebral aneurysms. Rebleeding was detected in five out of 18 patients with incomplete operations; one patient within 6 months and four patients within 5 to 8 yeras after surgery. The rebleeding rate of incompletely operated patients in the first decade was similar to that of untreated patients with ruptured aneurysms. This suggests that wrapping, coating and incomplete clipping procedures provide some protection against rebleeding in the first 6 months but little in the period extending beyond 5 years after surgery. Showing two types of newly-formed aneurysms after complete surgery and enlarged residual aneurysms after incomplete surgery, the possibility of rebleeding from these aneurysms is discussed in the present report.

    Notes: Summary Intraoperative ultrasound diagnosis through a burr-hole was performed in 22 cases using a 5 MHz electronic sector-scanning transducer. The pathology along with the landmarks, such as ventricles and the interhemispheric fissure, were visualized. In two cases, a second burr-hole was placed nearby, and aspiration of the fluid was accomplished under real-time ultrasonic monitoring using the first hole as an acoustic window. Our results suggest a possibility of ultrasound-guided stereotactic surgery of the brain through twin burr-holes.

    Notes: Abstract Ultrastructurally, the antigenicity of major pituitary hormones in secretory granules was quantitatively investigated in five growth hormone (GH)-secreting adenomas, five prolactin (PRL)-secreting adenomas and eight clinically non-functioning (CN-F) adenomas. Sparsely granulated cells with a few or several small secretory granules (60–100 nm) exhibiting little or only weak antigenicity of various biochemically unrelated hormones were commonly observed in CN-F adenomas and occasionally in GH- and PRL-secreting adenomas. GH- or PRL-secreting adenomas consisted of many densely granulated cells with medium-sized (200–250 nm) or large (over 250 nm) secretory granules and a few or several sparsely granulated cells with small secretory granules. The densely granulated cells showed intense GH or PRL antigenicity and slight to moderate antigenicity for other hormones in large secretory granules and little or only weak antigenicity for various hormones including GH or PRL in small secretory granules. Their secretory granules larger than 160 nm or 140 nm significantly exhibited intense GH or PRL antigenicity (Fisher’s exact test; P 〈 0.05 and 〈 0.01, respectively). Two CN-F adenomas showed sparsely and densely granulated cells as well as intermediate cells. The densely granulated cells closely resembled GH-secreting cells. The intermediate cells simultaneously included small and medium-sized or large secretory granules exhibiting little/slight and intense GH-antigenicity, respectively. This study indicates that sparsely granulated cells of different categories showing slight antigenicity for various hormones, antigenically share the same origin, and that their hormonality, single or multiple, may be selectively activated in the developmental course of secretory granules.
