    Notes: Abstract Monocytes from human immunodeficiency virus (HIV) patients have an increased heterogeneity of phenotype and function. In a study of 120 HIV patients we have demonstrated that they have normal monocyte differential counts but that with progression of the disease an increasing proportion of monocytes show phenotypic and functional evidence for activation or maturation. A proportion of the monocytes are larger, with increased expression of CD11b, HLA-DR, CD45 and CD16. Concomitantly there was increased expression of TNF-α, high constitutive synthesis of PGE2 and high plasma IL-6 levels. This suggested that there exist a more dynamic situation of recuitment, activation and maturation of peripheral blood monocytes driven by HIV infection which results in a broader phenotypic profile.

    Notes: Antibody activity in the major classes and IgG subclasses against antigens in factory humidifier water was quantified by enzyme immunoassay (EIA) in 88 subjects who were exposed at work to the output from these contaminated humidifiers. Those with work-related symptoms had significantly higher mean titres than those who were symptom free, although values overlapped. The individuals with the highest IgG antibody titres also had the highest titres of IgM and IgA antibody, and these parameters did not discriminate between those with and without symptoms any better than the IgG titre. This was also true for the IgG subclasses where activity was predominantly measured in IgG 1. Quantifying the IgG antibody allowed us to demonstrate a significant correlation with years of work exposure (P〈0.001). There was no significant association between antibody and cigarette smoking, as assessed by smoking history and confirmed objectively by serum cotinine levels. There was a significant correlation with total IgG level (P〈0.001) suggesting that a non-specific immune enhancement may accompany the specific response. The antibody titres were followed up to 3 years after modification of the humidification systems, and during this time symptoms resolved and the antilunK levels progressively fell to undetectable levels. The EIA was adapted to measure antigen at nanogram levels thus providing a rapid test for screening of humidifier water as a technique that may help identify the nature of the antigens involved.

    Notes: Pigeon bloom, a waxy powder which coats the feathers of racing pigeons, has been investigated as a potential antigen in pigeon fanciers' lung. Precipitins to a soluble bloom-extract were demonstrated in fifty-nine of ninety-seven people studied at two Scottish pigeon shows, whilst precipitins to pigeon serum were found in thirty-seven. A radioimmunoassay was developed to allow an estimation of the intensity of antibody response to this antigen in nineteen pigeon fanciers being investigated for pigeon fanciers' lung. The mean antibody level to bloom extract was significantly higher (P〉0.05) in those with acute symptoms compared to those with less severe symptoms. Pigeon bloom is a highly antigenic material and provides an alternative to pigeon droppings as a source for the inhalant antigens regularly encountered by those keeping pigeons.

    Notes: Background The importance of Th2-type lymphocyte function in asthmatic airway inflammation is well recognized, but less is known about the factors which regulate the function of these lymphocytes in asthma. The macrophage-derived cytokine, interleukin (IL)-15 has a number of T cell regulatory properties which might be of relevance to asthma and its treatment.Objective The aims were to identify and quantify the T cell regulatory cytokine IL-15 in induced sputum samples from asthmatic patients, in comparison with IL-13, and to relate the levels of these cytokines to treatment with inhaled steroids.Methods Induced sputum was collected from 16 asthmatics (eight steroid and eight non-steroid treated) and eight normal controls. IL-15 and IL-13 levels were measured by enzyme-linked immunoassay (ELISA) in sputum. IL-15 levels were also measured in sputum cell culture supernatants and localized to specific sputum cells by immuno-cytochemistry.Results IL-15 levels were increased and IL-13 levels were decreased in sputum fluid from steroid-treated compared with non-steroid-treated asthmatics. IL-15 was localized specifically to macrophages and the proportion of these cells expressing IL-15 correlated with sputum fluid IL-15 and IL-15 levels in cell culture supernatants, and all were higher in the steroid-treated asthmatics.Conclusion IL-15 and IL-13 production appears to be reciprocally regulated by steroid therapy in asthma patients. The steroid-associated increase in IL-15 may regulate a fundamental shift away from an inflammatory Th2-type environment in asthma and may be an essential component of the cytokine modulation underlying the therapeutic benefit of corticosteroids in this condition.

    Notes: There was a reduced prevalence of symptoms of Extrinsic Allergic Alveolitis (EAA) among the cigarette smokers in a survey of 102 volunteer pigeon breeders. These smokers had a significantly lower antibody response against the inhaled antigens associated with the disease; only one of twenty-three smokers (4.3%), but thirty-nine of sixty-five non-smokers (55.4%) had elevated serum IgG antibody levels, despite similar degrees of avian exposure in each group. The appearance of antibody in six of fourteen ex-smokers (42.9%) suggested that the apparent inhibitory effect of smoking on the antibody response was reversible. The smoking group had lower total serum IgG and IgA, higher serum IgD, and their total IgM and IgE levels were similar to the non-smokers.

    Notes: IgG antibody against avian antigens was measured by quantitative radioimmunoassay in serum samples obtained regularly from twenty pigeon fanciers over a 1-year period. A seasonal variation was seen in nine antibody-positive subjects; eight of whom had symptoms of pigeon breeder's disease (PBD), and a clear peak of antibody production occurred during late summer, corresponding with the period of maximum avian contact in the sporting season. All subjects with insignificant specific IgG levels were asymptomatic and displayed minimal changes throughout the year despite a similar exposure pattern to antigen for all individuals. Raised total IgG was a feature of six symptomatic subjects, two of whom had raised total IgA. Three of these six subjects had maximum hypergamma-globulinaemia coinciding with peak specific-antibody levels, but in general the total imrnunoglobulin levels tended to remain high throughout the year with only marginal fluctuations. The total immunoglobulin levels in the other individuals were within normal limits and displayed no remarkable changes during the year. The subjects with pigeon breeder's disease had a more active immune responsiveness to avian contact, and the association of the highest levels with periods of maximal contact with antigen may have an important bearing on the dynamic nature of this condition.

    Notes: Background Previous studies on allergy to feathers have not adddressed whether orgatiisms living on feathers (mites. lice, moulds) are a source of allergens.Objective To investigate whether feather mites produced allergens of clinical relevance to bird keepers.Methods We examined serum IgE responses of 96 pigeon breeders to an extract of feather mites from pigeons (predominantly Diplaegidia columbae). using Western blotting, specific IgE assay using AlaSTAT EIA and RAST inhibition.Results Feather mites are a major source of soluble proteins derived from feathers, accounting for up to 10% of the total weight of the feather. Forty-three sera had a negative score (0) for anti-feather mite IgE. 27 were weakly positive (1–2) and 26 had strongly positive scores (3–4). Fewer pigeon breeders with scores ± 3 were asymptomatic than those with negative scores (12 versus 40%). more had late onset symptoms (with or without early onset symptoms; 77% versus 44%) and had IgE antibody against house dust mite (89% versus 23%). Western blotting of eight sera against the extract of Diplaegidia columbae revealed 20 IgE-binding components ranging from 22 to 200 kDa. A high diversity of components was recognized by each serum: arithmetic mean 7 (range 2 14). RAST inhibition indicated feather mites had species-specific epitopes as well as ones that cross-reacted with Dermatophagoides pteronyssinus.Conclusion Strongly-positive AlaSTAT scores to pigeon leather mite were associated with allergic symptoms of late onset in pigeon breeders. We conclude that feather mites are a major source of clinically-relevant allergens for pigeon breeders.
