    Notes: . Fine structural studies of a specialized vesicle system associated with the endoplasmic reticulum (ER) of exo-erythrocytic Plasmodium berghei suggest that this system may be the equivalent of a Golgi apparatus. Patches of ER, randomly distributed in the cytoplasm of developing parasites, are formed of smooth and ribosome-studded cisternae intermingled with each other. The vesicle systems are located between as well as at the edges of ER aggregates and appear to be in different stages of budding from the cisternae. Prolonged osmication reveals distinct staining of the nuclear envelope and ER of the parasites as well as part of the Golgi apparatus of the hepatocytes. However, the small vesicles associated with the parasite's ER are unstained, as are the coated vesicles in the Golgi region of the liver cell. These sites in the parasite cytoplasm seem comparable to the concave surface of the Golgi apparatus in liver cells. The pinched-off vesicles fuse with others to form the prominent peripheral vacuolization characteristic of the nearly mature exo-erythrocytic form. The formation of these peripheral vacuoles and their subsequent fusion with the parasite membrane may be an exocytosis mechanism supplying the rapidly expanding parasite with new plasma membrane material.

    Notes: Summary A recently developed sandwich ELISA, which detectsAspergillus galactomannan, was tested retrospectively in serial serum samples from an allogeneic bone marrow transplant recipient with proven invasive aspergillosis (patient 1) and another with suspected disease (patient 2). Galactomannan was detected in the serum 4 and 28 days, respectively, before pulmonary infiltrates suggestive of fungal infection first became apparent on the chest X-ray.Aspergillus was detected by ELISA and PCR in BAL fluid samples from both patients, and in CSF from patient 1. The diagnosis was confirmed at autopsy for patient 1 by histopathology and the recovery ofAspergillus fumigatus from the lung and brain. Furthermore, in both patients the course of the antigen titer in the serum during antifungal treatment corresponded with the clinical outcome. These results confirm that the sandwich ELISA appears to be useful for the early diagnosis of invasive aspergillosis. The value of the test for monitoring the response to antifungal treatment remains to be established in prospective trials.

Eine Beteiligung der Augen an einer Infektion ohne vorhergehendes Augenleiden ist selten beschrieben. Die vorliegende Arbeit beschreibt zwei Fälle von Gruppe G Streptokokken-Endokarditis, die sich klinisch mit einer endogenen Endophthalmitis präsentierten. Lokale und systemische Gabe von Antibiotika führte zur Defekt-Heilung mit Verminderung des Sehvermögens.
    Notes: Summary Group G streptococci can cause serious infections in patients with predisposing factors. Involvement of the eye has rarely been reported in patients without ocular history. Two cases of group G streptococcal endocarditis which presented with an endogenous endophthalmitis are reported. Topical and systemic antimicrobial therapy resulted in recovery, but visual outcome was poor.

    Description / Table of Contents: Zusammenfassung Das selten vorkommende Pneumoperikard, definiert als Luftansammlung im Herzbeutel, wird meistens durch ein Trauma verursacht. Der Krankheitsverlauf eines mit Induktionschemotherapie wegen akuter myeloischer Leukämie behandelten Patienten wird beschrieben. Auf der Basis einer pulmonalen Aspergillusinfektion entwickelte sich eine Perikarditis und ein Pneumoperikard. Zwar konnte der Patient erfolgreich mit antimykotischer Therapie behandelt werden und erholte sich von seiner Granulozytopenie, verstarb jedoch im Verlauf der nachfolgenden Chemotherapie. Im weiteren werden aus der Literatur die Fälle von zwei anderen Patienten mit einem mit pulmonaler Aspergillose assoziierten Pneumoperikard erläutert.
    Notes: Summary Pneumopericardium, the presence of air in the pericardial space, is a rare disorder that is usually caused by trauma. We describe a patient given induction chemotherapy for acute myelogenous leukemia who developed pulmonary aspergillosis that resulted in pericarditis and pneumopericardium. He responded to antifungal treatment and recovered from granulocytopenia, but died early during the next course of chemotherapy. Two other reported cases of pneumopericardium associated with pulmonary aspergillosis are summarized.

    Description / Table of Contents: Zusammenfassung ObwohlCandida für gewöhnlich nicht im Urin vorkommt, läßt der Nachweis nicht sicher auf eine Harnwegsinfektion schließen. Andererseits kann der Nachweis von Hefen im Urin das erste Symptom einer systemischen Mykose sein. Trotz der bestehenden Probleme ist eine akkurate Diagnose, insbesondere bei Risikopatienten, von besonderer Bedeutung, da aszendierende Infektionen zur systemischen Erkrankung führen können. Amphotericin-B-Blasenspülung ist eine gebräuchliche Therapieform bei durch Pilzen verursachten Harnwegsinfektionen, obwohl keine spezifischen Richtlinien für diese Anwendung bestehen. Fluconazol erscheint aufgrund seiner Pharmakokinetik ideal für den Einsatz bei Harnwegsinfektionen. Die Ergebnisse von 5 Fallberichten und 99 Patienten aus verschiedenen, kleineren Studien, sind in diesem Review zusammengefaßt. Anhand der Ergebnisse dieser Studien erscheint der Einsatz von Fluconazol bei unkomplizierten und kompliziertenCandida-Harnwegsinfektionen gerechtfertigt.
    Notes: Summary The presence ofCandida in the urine is not considered normal but does not necessarily indicate urinary tract infection. On the other hand, yeasts in urine cultures might be the first symptom of systemic fungal infections. Despite the difficulties that exist, establishment of an accurate diagnosis is important especially in high risk patients because ascending infections may lead to disseminated disease. Amphotericin B bladder irrigation is a common mode of therapy for fungal urinary tract infection, although no specific guidelines exist as to the use of the procedure. The pharmacokinetic parameters posessed by the triazole antifungal agent fluconazole make it a candidate for treating fungal urinary tract infections. Five case reports and 99 patients reported in several small studies were reviewed. As evident from these reports, fluconazole appears to be of value in the treatment of both uncomplicated and complicated fungal urinary tract infections.

    Notes: Summary Enzyme histochemical methods were performed on sporozoite infected liver tissue of rats in order to gain insight into the nutrition and metabolism of exoerythrocytic forms of Plasmodium berghei. The following enzymes were demonstrated in the hepatocytic stages of the parasites, obtained 41 and 48 h after inoculation of sporozoites: acid phosphatase, cytochrome oxidase, NADH-tetrazolium reductase, succinate dehydrogenase, NAD+ and NADP+ dependent isocitrate dehydrogenase, NADP+-dependent malate dehydrogenase, lactate dehydrogenases, 6-phosphogluconate dehydrogenase and glucose-6-phosphate dehydrogenases and α-glycerol-phosphate dehydrogenase. The results suggest that a conventional Embden-Meyerhoff pathway, pentose phosphate pathway and Krebs' citric acid cycle may in part be present in these exoerythrocytic parasites. Alkaline phosphatase, nucleoside polyphosphatase, 5′nucleotidase. glucose-6-phosphatase, α-glucan phosphorylase, NAD+ dependent malate dehydrogenase, amino-peptidase M and non-specific esterases were not detected by our techniques in the parasite. The enzyme distribution of this intrahepatocytic malaria parasite revealed by histochemistry is compared with the enzyme distribution in the other phases of the parasite's life cycle.

    Notes: Abstract  The interval from the time of admission to the emergency room until the administration of antibiotics in patients presenting with a serious infectious disease was analysed. Fifty patients presumptively diagnosed in the emergency room as having a serious infection (respiratory tract, urinary tract, erysipelas, fever with neutropenia or bacteremia) needing immediate empirical antibiotic treatment were enrolled in the study. A median interval from time of admission to administration of antibiotics of 5 hours was determined (range 0.6–13.3 h). The interval was significantly shorter in patients admitted at night than in patients admitted during office hours (3.7 vs. 6.0 h, P〈0.05). There was no difference with respect to the presenting features, body temperature, laboratory values at presentation or number of cultures performed. In 41 of the 50 patients blood samples were taken for culture. More than 80% of the patients received an antibiotic chosen in accordance with hospital guidelines. The analysis revealed that the median delay of 5 hours before patients received their initial dose of antibiotic depended on several factors. Attempts to provide optimal antimicrobial therapy should thus concentrate not only on the correct choice and dosage of a drug but also on prompt institution of therapy.

    Notes: Abstract The aim of this study was to identify retrospectively trends in fungal bloodstream infections in The Netherlands in the period from 1987 to 1995. Results of over 395,000 blood cultures from five Dutch university hospitals were evaluated. Overall, there were more than 12 million patient days of care during the nine-year study period. The rate of candidemia doubled in the study period, reaching an incidence of 0.71 episodes per 10,000 patient days in 1995. The general increase in candidemia was paralleled by an increase in non-Candida albicans bloodstream infections, mainly due toCandida glabrata. However, more than 60% of the infections were caused byCandida albicans. Fluconazoleresistant species such asCandida krusei did not emerge during the study period. The increasing rate of candidemia found in Dutch university hospitals is similar to the trend observed in the USA, but the rate is lower and the increase is less pronounced.
